Abstract:To the Editor: Increasing awareness of polyomavirus BK (BKV) as a cause of graft dysfunction has stimulated an attempt to develop animal models of this disease. A study recently published in the journal investigated how several clinically relevant variables affect the magnitude of kidney injury in this model (1). No viral antigens were demonstrated in renal tubules. It is stated that immunocompetent animals do not show acute rejection in this model. Our laboratory has performed mouse kidney transplantation in the same donor–recipient combination and obtained significantly different results, albeit using different conditions. Recovery of the donor kidney from C3H/HeJ (H-2k) male mice of age 8–10 weeks was done using a standard procedure. However, instead of the right flank, the donor organ was implanted in the neck of C57BL/6 (H-2b) mice using nonsuture microsurgery (2). Mouse polyomavirus (MPyV) infection was achieved by subcutaneous injection of 1 × 109 viral genomic equivalents. A potential advantage of a subcutaneous graft is that it is accessible to real-time imaging by multiphoton intravital microscopy for study of capillary blood flow, renal filtration, leukocyte adhesion, microvascular leakage and cell death (3). Histologic examination of the allograft was conducted in six infected and nine uninfected animals. Renal tubules did not show expression of large T antigen (using anti-Polyoma virus early antigen antibody, Abcam, Cambridge, MA, USA) or viral DNA (using two biotinalylated probes directed against MPyV LTA) (nt numbers 941–990 and 1121–1170; accession number NC_001515). However, quantitative PCR confirmed infection of the kidney with an intrarenal viral load of 2.18 × 106± 3.1 × 105 per mg soft tissue at day 7. All allograft but not the native kidneys showed severe interstitial lymphocytic infiltrates with tubulitis and peritubular capillaritis. Intimal arteritis with or without infarction seen in 5/6 infected and 9/9 noninfected animals examined on postoperative days 10–28 (Figure 1). Immunohistochemistry using anti-C4 antibody (Abcam) showed diffuse C4 in peritubular capillaries. Histologic findings in an allograft kidney from a mouse at the time of peak viremia, 7 days postinfection. There is severe interstitial mononuclear inflammation (A) with intimal arteritis (B). Diffuse C4d deposition in the peritubular capillaries (C) indicates that antibody mediated rejection is also contributing to the graft injury. No MPyV T-antigen could be documented in the renal tubules (data not shown) using an immunohistochemical staining procedure that worked successfully on a virus infected 3T3 cell line used as a positive control (D). In conclusion, severe T cell rejection, antibody mediated injury and intimal arteritis can confound interpretation of experiments in the mouse model of polyomavirus infection. Inclusion of immunosuppressionin this model may reduce allogeneic injury, accentuate viral cytopathic effects and result in a model more closely aligned with human BK virus nephropathy.
