Abstract:Objectives The purpose of this cone beam computed tomography (CBCT) study was to describe facial alveolar bone alterations and gray value (GV) changes around implants in aesthetic anterior maxilla with simultaneous guided bone regeneration (GBR) and to investigate potential influence of factors related to vertical alveolar bone stability, such as particle sizes of the deproteinized bovine bone mineral (DBBM) used. Materials and Methods A retrospective study design was adopted. Four facio-palatal cross-sectional CBCT images of 166 implants from 124 patients who had maxillary anterior implants were obtained. Measurements of the vertical facial alveolar bone level (VFBL), horizontal facial alveolar bone thickness (HFBT), and GV were identified of 1-3 years. Then, correlation coefficients for those parameters were calculated. Finally, linear mixed models were established to investigate potential factors influencing vertical alveolar bone resorption. Results Facial alveolar bone underwent fast bone modeling and remodeling during the first 6 months, with decreases of 1.00 +/- 1.19 and 0.74 +/- 0.86 mm in VFBL and HFBT in implant shoulder, respectively, and there was positive and significant correlation between the alterations in VFBL and HFBT1 (r(s) = .516, p = .000). Linear mixed models identified particle size of DBBM as a critical factor associated with vertical bone resorption (p = .000). The GV gradually increased during the follow-up period. Conclusions Rapid and unavoidable peri-implant bone resorption usually happened during the first 6 months after implant placement. GBR is a predictable treatment for maxillary anterior implants, since GV has an increasing trend. Large bone particles of DBBM help maintain vertical alveolar bone stability.
The dimension and morphology of alveolar bone at maxillary anterior teeth in periodontitis: a retrospective analysis—using CBCT

Abstract:Abstract The morphology of the alveolar bone at the maxillary anterior teeth in periodontitis patients was evaluated by cone-beam computed tomography (CBCT) to investigate the distribution of alveolar defects and provide guidance for clinical practice. Ninety periodontitis patients and 30 periodontally healthy individuals were selected to determine the morphology of the alveolar bone at the maxillary anterior teeth according to the degree of bone loss, tooth type, sex and age. The differences in the dimensions between periodontitis patients and healthy individuals were compared, and the distribution of alveolar bone defects was analyzed. A classification system was established regarding the sagittal positions and angulations of the teeth. The buccal residual bone was thicker and the lingual bone was thinner in the periodontitis patients than in the periodontally healthy individuals, and there were differences between the different tooth types, sexes and age subgroups. The buccal undercut was close to the alveolar ridge, while fenestration was reduced and the apical bone height was higher in periodontitis patients than in periodontally healthy individuals. The apical bone height increased with the aggravation of bone loss and age. The proportions of different sagittal positions changed with the aggravation of bone loss. Moreover, the teeth moved more buccally regarding the positions of the maxillary anterior teeth. The morphology of the alveolar bone at the maxillary anterior teeth differed between periodontitis patients and healthy individuals, and the differences were related to the degree of bone loss, tooth type, sex and age.

An In Vivo and Cone Beam Computed Tomography Investigation of the Accuracy in Measuring Alveolar Bone Height and Detecting Dehiscence and Fenestration Defects

Abstract:Purpose: To investigate cone beam computed tomography (CBCT) accuracy in measuring facial bone height and detecting dehiscence and fenestration defects around teeth. Materials and methods: Patients who were treatment planned for periodontal flap or dental implant surgeries were enrolled (n = 25). CBCT imaging (Carestream CS 9300) was obtained at 0.09-mm voxels (n = 10 patients, 23 teeth) and at 0.18-mm voxels (n = 15 patients, 33 teeth). Facial bone height measurements, from cusp tip to crest of bone height along the long axis of the tooth, and presence or absence of dehiscence or fenestration defects were recorded from CBCT images in triplicates independently by two examiners. The corresponding clinical measurements were made at the time of surgery. Comparisons of CBCT and clinical measurements were made using paired t tests for teeth: anterior and posterior, maxillary and mandibular, with or without restorations, or root canal therapy. Level of agreement between investigators was assessed by concordance correlation coefficients (CCC), Pearson's correlation coefficient (PCC), and Cohen's Kappa. Results: Comparing mean CBCT and clinical measurements, statistically significant differences were noted for 0.09-mm and 0.18-mm voxel sizes, for anterior and posterior teeth, for maxillary and mandibular teeth, for teeth with or without restorations, and for teeth without root canal therapy (P < .05). Clinical and CBCT measurements were similar for teeth with crowns and with root canal therapy (P > .05). CBCT measurements underestimated mean facial bone height from 0.33 ± 0.78 to 0.88 ± 1.14 mm (mean ± SD) and absolute facial bone height values from 0.56 ± 0.35 to 1.08 ± 0.92 mm. Intraexaminer and interexaminer reliability for measuring facial bone height ranged from poor to substantial (PCC = 0.78 to 0.97 and CCC = 0.63 to 0.96, respectively). Interexaminer reliability for detection of dehiscence and fenestration defects ranged from poor to moderate (Cohen's Kappa = -0.09 to 0.66). Conclusion: CBCT imaging underestimated facial bone height and overestimated the presence of dehiscence and fenestration defects.
Using Cone-Beam Computed Tomography to Assess Changes in Alveolar Bone Width around Dental Implants at Native and Reconstructed Bone Sites: A Retrospective Cohort Study

Abstract:The aim of this study was to use a cone-beam computed tomography (CBCT) to assess changes in alveolar bone width around dental implants at native and reconstructed bone sites before and after implant surgery. A total of 99 implant sites from 54 patients with at least two CBCT scans before and after implant surgery during 2010–2019 were assessed in this study. Demographic data, dental treatments and CBCT scans were collected. Horizontal alveolar bone widths around implants at three levels (subcrestal width 1 mm (CW1), subcrestal width 4 mm (CW4), and subcrestal width 7 mm (CW7)) were measured. A p-value of < 0.05 indicated statistically significant differences. The initial bone widths (mean ± standard deviation (SD)) at CW1, CW4, and CW7 were 6.98 ± 2.24, 9.97 ± 2.64, and 11.33 ± 3.00 mm, respectively, and the postsurgery widths were 6.83 ± 2.02, 9.58 ± 2.55, and 11.19 ± 2.90 mm, respectively. The change in bone width was 0.15 ± 1.74 mm at CW1, 0.39 ± 1.12 mm at CW4 (p = 0.0008), and 0.14 ± 1.05 mm at CW7. A statistically significant change in bone width was observed at only the CW4 level. Compared with those at the native bone sites, the changes in bone width around implants at reconstructed sites did not differ significantly. A significant alveolar bone width resorption was found only at the middle third on CBCT scans. No significant changes in bone width around implants were detected between native and reconstructed bone sites.

Abstract:Objective: This retrospective study aimed to measure the labial, palatal, mesial, and distal bone thickness around maxillary central and lateral incisors and canines and height from crest to apex, using cone beam computed tomography (CBCT) images and compare the results based on gender. The second objective of the study was to measure root angulation on CBCT images and its relation with the labial cortical thicknesses. Material and Methods: After the Institutional Review Board (IRB) approval, a total of 140 CBCT volumes were included in this study according to the set criteria. On each scan, right-side maxillary central, lateral incisors, and canine were selected for the measurements. All the measurements were done at three levels at the alveolar crest (L1), mid-root (L2), and apical region (L3) for each tooth. Results: The Student's t-test was performed to compare the result of buccal, palatal, mesial, and distal bone thickness, angulation, and height of all subjects. Buccal alveolar bone thickness was minimum at the mid-root region, and the palatal bone thickness was minimum at the crestal region. The mesial bone thickness was minimum at the mid-root level, and distal bone thickness was minimum at the crest level. The available bone height was maximum at the lateral incisor and equal for the central incisor and canine. The canine was the most angulated tooth. Conclusion: Cone beam computed tomography is a reliable imaging modality to evaluate pre-surgical immediate implant sites and measure alveolar bone thickness. The canine was the most angulated tooth with more buccal alveolar bone thickness.
Analysis of Facial Bone Wall Dimensions and Sagittal Root Position in the Maxillary Esthetic Zone: a Retrospective Study Using Cone Beam Computed Tomography.

Abstract:PURPOSE:To determine the thickness of the facial bone wall and the sagittal angulation between the long axis of the teeth and the long axis of the associated alveolar bone in the maxillary esthetic zone using cone beam computed tomography (CBCT).MATERIALS AND METHODS:A retrospective radiographic study of CBCT images was conducted in 300 patients. The distance between the cementoenamel junction (CEJ) and the facial bone crest, the thickness of the facial bone wall (4 mm apical to the CEJ and midroot), and the sagittal angle between the long axis of teeth and the long axis of the respective alveolar bone were measured. Descriptive statistics and frequency analyses were performed.RESULTS:The sample included 133 men and 167 women (mean age, 36.9 years; range, 18 to 60 years). The distance between the CEJ and the facial bone crest varied between 0.1 and 4 mm and showed an overall tendency to increase with age. Almost 80% of anterior teeth and 40% of premolars exhibited a thin facial bone wall (< 1 mm), and nearly 30% of sites had a bone wall thinner than 0.5 mm. The sagittal angle at approximately 80% of central incisor and second premolar sites was < 20 degrees, but the angle at more than 40% of canine sites was ≥ 30 degrees. There was a significant decrease in facial bone wall thickness from premolars to anterior teeth.CONCLUSION:The facial bone wall in most maxillary anterior teeth was very thin. There was a substantial sagittal angulation between the long axes of teeth and those of their respective alveolar bone in most esthetic zone positions. CBCT analyses of the facial bone wall and the sagittal angle are recommended to ensure the most appropriate dental implant treatment approach.
Accuracy of cone-beam computed tomography at different resolutions assessed on the bony covering of the mandibular anterior teeth

Abstract:Introduction: The aim of this study was to determine the accuracy of cone-beam computed tomography (CBCT) with different voxel resolutions. Measurements were made of the bony covering of the mandibular anterior teeth because this region is crucial in orthodontic treatment planning. Methods: CBCT data at 2 resolutions (0.125-mm and 0.4-mm voxels) were collected from 8 intact cadaver heads. The vertical position of the mucogingival junction was clinically assessed. After removal of the gingiva, vertical and horizontal bony measurements were taken, and the buccal alveolar bone margin was determined. Anatomic bony measures were compared with the CBCT measures, and the correlation of the mucogingival junction measures to the buccal alveolar bone margin measures was evaluated. Results: Bony measures obtained with CBCT were accurate and differed only slightly from the physical findings. The mean differences, ranging from -0.13 to +0.13 mm, were statistically not significant, but the limits of agreement showed discrepancies in the measurements as large as 2.10 mm, depending on measurement and resolution. Buccal alveolar bone margin measurements correlated with the mucogingival junction measurements (P <0.001). On average, the mucogingival junction was 1.67 mm more apical than the buccal alveolar bone margin (CI 95%, 1.35-1.98 mm). Conclusions: CBCT renders anatomic measures reliably and is an appropriate tool for linear measurements. Presence of soft tissue as well as different voxel size affect the precision of the data. A customized resolution protocol must be chosen according to the accuracy needed. However, even the 0.125-mm voxel protocol does not depict the thin buccal alveolar bone covering reliably, and there is a risk of overestimating fenestrations and dehiscences. The mucogingival junction appears to follow the buccal alveolar bone margin in a parallel line.

Abstract:The components and dimensions of the periodontal and peri‐implant phenotype have a high relevance in contemporary dental research and should be taken into consideration in the decision‐making process in the management of a variety of clinical scenarios to optimize the outcomes of therapy. Various assessment methods for quantifying and classifying the phenotypical dimensions have emerged and developed in recent decades. Nevertheless, the use of cone‐beam computed tomography (CBCT) scans remains the most commonly used approach worldwide. However, the accuracy to adequately imaging and measuring the dimensions of the hard and soft tissue components around teeth may represent a significant challenge in different clinical scenarios due to factors such as the age of the patient and motion during the scan, presence of metallic artifacts causing streaks and gray‐value distortion, overlapping of soft tissue structures, machine performance, file processing, and small voxel size among others. These factors pose a particular challenge when tiny structures are under investigation, for example, the buccal/lingual bony or soft tissue layer of lower/upper incisors. Therefore, this review addresses the underlying technical information of the use of CBCT scans, and suggests some recommendations on the utilization of this method of assessment to optimally use it despite its' system‐inherent limitations.

Abstract:In the diagnosis and treatment of oral diseases, imaging examination is a more important auxiliary method. Among them, the application of digital X-ray examination is the most common. However, because the image information provided by this technology is a two-dimensional model, it is difficult to obtain a comprehensive image, so there is a missed diagnosis. With the development of imaging technology, cone-beam CT (CBCT) is widely used in the diagnosis and treatment of oral diseases. This is a non-invasive imaging examination. The diagnosis effect is achieved by cone-beam computed tomography or volume tomography. It has less radiation and low cost. Clear imaging and high spatial resolution. This study compared the diagnostic effect of cone-beam CT and traditional X-ray examination in diseases related to oral medicine to observe the diagnostic superiority of cone-beam CT. In this study, we can know that 30 patients with periapical periodontal disease have been treated by this technique, and the detection accuracy rate is 100.00%. The damage of the apical root of the patient’s tooth is completely displayed in the image. The measurement results of the bone defect around the tooth are accurate and space and the location image is presented in front of the doctor, so the detection accuracy of this technology is high. The study found that cone beam CT is effective in the diagnosis and treatment of diseases in oral medicine, and can clearly show the three-dimensional structure of oral tissue, which is convenient for physicians to observe the shape of the disease and improve the accuracy of disease diagnosis.
Prevalence and Severity of Circumferential Alveolar Bone Loss Using CBCT Images: A Retrospective Study of 20,620 Surfaces of 5155 Teeth

Abstract:Periodontal disease is a site-specific disease affecting the supporting tissues of the teeth. It is useful for the clinician to have information about the prevalence and severity of alveolar bone loss (ABL) according to the site, location, and position of the teeth for a better treatment plan and expected prognosis. This study aimed to assess the prevalence and severity of ABL at mesial, distal, buccal and lingual sites of teeth in different locations, positions and sides of the dentition. The ABL of 20,620 sites of 5155 teeth in 212 patients was assessed using cone-beam computed tomography from the cemento-enamel junction to the crest of the alveolar bone. The prevalence of ABL was higher in the interproximal sites as well as anterior and mandibular teeth compared to their counterparts. Buccal sites and anterior teeth revealed higher ABL levels than the other tooth sites and posterior teeth, respectively. Furthermore, associations in the severity of ABL were observed between distal and mesial sites, buccal and lingual sites, maxillary and mandibular teeth, anterior and posterior teeth, and right and left sides. This study showed that the prevalence and severity of ABL differ from one tooth site to another and according to the tooth's location in the dentition. Higher prevalences were found in the interproximal sites, anterior teeth and mandibular teeth; higher ABL was found in buccal and distal sites, with the strongest associations between distal and mesial sites, buccal and lingual sites, and right and left sides.

Ex vivo assessment of the buccal and oral bone by CBCT

Abstract:Purpose: Identifying buccal and oral bone as an important supporting periodontal structure for teeth provides important information for treatment planning in periodontics and orthodontics. This study aims to add evidence to the knowledge of preciseness of cone beam computed tomography (CBCT) measurements of the vertical dimension of buccal and oral bone. The hypothesis is that CBCT is an accurate and reliable method to measure vertical vestibular and oral bone loss. Methods: The amount of vertical buccal and oral bone loss (bl) of 260 sites of 10 human cadavers was investigated clinically and radiographically by CBCT. Radiographic measurements were rated by two blinded raters. Measurements and the corresponding differences between clinical and radiological findings are described by medians and quartiles (Q1-Q3). For statistical analysis, Lin's concordance correlation coefficient (CCC) and Bland-Altman plots were calculated. Results: The CCC between the raters was 0.994 (95% confidence interval 0.992-0.995). The median bone loss (bl) distance from the cementoenamel junction (CEJ) to the bony defect (BD) was 3.5 mm (range 3-5 mm). The median bl measured in the CBCT was 3.8 mm (range 3.1-4.8 mm). The median difference of the 2 measurements for all sites included in the study (N = 260) was -0.2 mm (-0.7 to 0.3 mm). Conclusions: CBCT seems to be an accurate and highly reliable method to detect and describe vertical buccal and oral bone loss. It could improve planning and prediction for successful combined periodontal and orthodontic therapies.
Comparative Assessment of Reliability and Accuracy of Cone-Beam Computed Tomography (CBCT) Over Direct Surgical Measurement for Periodontal Bone Loss: A Prospective, Cross-Sectional Study

Abstract:Introduction: Assessing bone condition holds significant value in the diagnosis, treatment planning, and prognosing the periodontal disease; its importance is undeniable. The main aim of the present study was to evaluate the accuracy of alveolar bone measurements due to periodontal disease using cone-beam computed tomography (CBCT), by comparing with surgical measurements, considered as the gold standard. Materials and methods: A prospective cross-sectional study included a sample of 40 individuals diagnosed with chronic periodontitis who required periodontal surgery. A total of 202 sites were assessed for vertical and horizontal bone loss in the anterior (76 sites) and posterior (126 sites) teeth. Bone loss was measured using CBCT and a UNC 15 periodontal probe during the surgical intervention, and then compared. The statistical analysis involved employing a Student's t-test to compare measurements. Unpaired t-tests and correlation analyses were conducted using Pearson's correlation coefficient test. To establish statistical significance, a threshold of p<0.05 was considered appropriate. Results: The statistical analysis carried out on the mean values of CBCT and direct surgical measurements for vertical bone loss demonstrated a significant difference (p<0.01). However, the values obtained for horizontal bone loss did not display statistical significance. A strong correlation of 0.94-0.99 existed between surgical and CBCT measurements. A statistically significant distinction was observed between the two methods in measuring bone loss at the distal and palatal sites of the anterior teeth. Conclusion: Both CBCT and direct surgical measurement exhibit comparable accuracy potential in assessing alveolar bone loss. CBCT provides an accessibility advantage by enhancing visual access to challenging sites during surgical interventions, including palatal and distal areas of the teeth.

Abstract:Introduction: The maxillary anterior region is becoming a major concern due to its aesthetic relevance. The buccal bone thickness is important for implant placement, orthodontic treatment and restorative treatment. Objective: To assess the thickness of alveolar bone in the maxillary central incisor using cone beam computed tomography (CBCT). Methods: A cross-sectional observational study was conducted at Department of Dental Surgery, Bir Hospital where CBCT of 53 samples from July 2019 till December 2019, the archived CBCT images was assessed retrospectively. The thickness of the labial bone in a direction perpendicular to the outer surface of the tooth root was measured at a distance of 2 mm from the cementoenamel junction (CEJ). The measurement was taken thrice and the mean measurement was considered. Results: The labial alveolar bone thickness in maxillary central incisor was found to be 0.55±0.27 mm at a distance of 2 mm from the CEJ. Only 2 (3.8%) of the samples had an alveolar thickness of >1 mm. No statistically significant difference was found with respect to gender and age. Conclusion: The average thickness of the labial alveolar bone in maxillary central incisor using cone beam computed tomography was found to be thin.
Relationship Between Different Skeletal Facial Types and Anterior Alveolar Bone Thickness with Cone-Beam Computed Tomography in an Asian Population

Abstract:Background: To investigate the relationship between different skeletal facial types and anterior alveolar bone thickness with cone-beam computed tomography (CBCT) in an Asian population. Methods: A total of 130 patients with 1,560 healthy anterior teeth were enrolled. On three-dimensional reconstructed images, Frankfurt-mandibular plane angle (FMA) value and angle formed by subspinale, nasion, and supramental (ANB) value were measured, and subjects were categorized into different groups based on their vertical skeletal patterns as well as sagittal jaw relationships. For each tooth, the thickness of alveolar bone was measured at three locations: 1, 3, and 5 mm apical to alveolar bone crest. Descriptive statistics were used. Kruskal-Wallis test, one-way ANOVA, and independent-samples t-test were used for further analysis. Results: Men's maxillary anterior teeth's lingual alveolar bone thickness was significantly greater than women's (P<0.05). Strong correlations were found between vertical skeletal patterns and lingual alveolar bone thickness of maxillary/mandibular anterior teeth (R-2=0.302, P<0.01 in the maxilla; R-2=0.311, P<0.01 in the mandible). However, no significant difference was shown in the alveolar bone thickness among people with different sagittal bone profiles. Conclusions: The lingual alveolar bone of the maxillary anterior teeth is thicker in males than in females. With the increase of FMA, the anterior alveolar bone gradually became thinner.
Cone Beam Computerized Tomography Measurement of Alveolar Ridge at Posterior Mandible for Implant Graft Estimation

Abstract:Damaging the inferior alveolar nerve (IAN) is the most serious complication when harvesting an autogenous graft from posterior mandible. The objective of this study was to use cone beam computerized tomography (CBCT) to measure dimensions of the alveolar ridge in the posterior mandible for estimation of a safe graft size, and then analyze how it is related to the gender, age, and dentition status of subjects. CBCT scans were screened to include 59 subjects without interfering pathologies. Alveolar height was measured from the alveolar crest to superior border of IAN and also to the inferior border of the mandible. Alveolar width (from buccal to lingual cortical plates) and buccal bone thickness (from buccal cortical plate to mandibular molar mesial root buccal surface) were measured at the coronal, middle, and apical thirds divided from the alveolar crest to the IAN. It was found that males and dentate sites had larger alveolar dimensions than did females and edentulous sites, respectively. Bone volume did not correlate significantly with age. Buccal bone thicknesses increased from coronal to apical and from the first to the third molar generally. A larger bone graft could be harvested from male than female patients, with a mean harvestable graft dimension (height × width in mm) for male was 15.5 × 3.2, and for female was 14.1 × 2.9. In conclusion, males and dentate arches demonstrate larger alveolar volumes than do females and edentulous regions, respectively. Larger alveolar grafts can be harvested from males compared to the females. Age does not seem to affect alveolar dimension/graft volume.
Cone beam computed tomography assessment of maxillary anterior teeth cervix dimensions in healthy adults for optimal anatomic healing abutments

Abstract:Objectives The abutments produced with circular symmetry failed to accurately replicate the natural teeth's cervical shapes. The purpose of this study was to measure cervical cross‐sections of maxillary anterior teeth using cone beam computed tomography (CBCT) images to design anatomic healing abutments. Materials and Methods CBCT data of 61 patients were analyzed using Ez3D Plus software. Measurements were taken at the cemento‐enamel junction (CEJ) and 1 mm coronal to CEJ for maxillary central incisors, lateral incisors, and canines. Various parameters, including area, perimeter, and eight line segments in the distal (a), disto‐palatal (b), palatal (c), mesio‐palatal (d), mesial (e), mesio‐labial (f), labial (g), and disto‐labial (h) directions, were used to describe dental neck contours. The ratios (f/b and h/d) were analyzed, and differences based on sex and dental arch morphology were explored. Results Significant differences were found in area and perimeter between males and females, but not in f/b and h/d ratios. Differences in the f/b ratio were observed among dental arch morphologies for maxillary central incisors, lateral incisors, and canines. Conclusions CBCT measurements of cervical cross‐sections provide more accurate data for designing anatomic healing abutments. The fabrication of anatomical healing abutments needs to consider the influence of gender on cervical size and to explore the potential effect of arch shape on cervical morphology. Clinical Significance The novel method provides detailed measurements for the description of dental cervical contours for patients with bilateral homonymous teeth missing. The measurements of this study could be utilized to design more accurate anatomic healing abutments to create desired morphology of peri‐implant soft tissue.

Multidimensional tooth movement boundaries in the extended aesthetic zone: a cone-beam computed tomography study

Abstract:Abstract Purpose To establish tooth movement boundaries in four directions by a novel morphometric analysis of alveolar bone using conebeam computed tomography (CBCT) images. The aim was to assist clinicians in minimising the occurrence of bone fenestration and dehiscence, thereby facilitating safe and efficient orthodontic treatment. Methods CBCT images of 123 male volunteers comprising 55 individuals with hypodivergent vertical skeletal patterns and 68 individuals with normodivergent vertical skeletal patterns and normal occlusions but without a history of previous orthodontic treatment, were analysed. The mesiodistal and labiolingual alveolar bone height (ABH) as well as the labiolingual cancellous bone thickness (CBT) were measured in relation to the maxillary and mandibular anterior teeth and premolars. These parameters were subsequently compared between the different vertical skeletal patterns. Results Teeth within the extended aesthetic zone exhibited a limited range of safe movement on the labial aspect across all three root aspects, with 97.6% of the upper lateral incisors and 98.4% of the lower lateral incisors positioned against the labial cortical plate (<0.5 mm). The lingual aspects had significantly more space for tooth movement compared with the labial aspects ( P < 0.05). The ABH was significantly greater in the hypodivergent group than the normodivergent group involving particular teeth and sites in the maxilla ( P < 0.05), while no significant difference was noted in the mandible. The hypodivergent group also exhibited thicker cancellous bone on most labial and some lingual aspects. Conclusions The findings provided the average tooth movement range in the horizontal and vertical directions within the extended aesthetic zone in male individuals presenting with a normal occlusion. It was concluded that a hypodivergent growth pattern may have a relatively large range of safe tooth movement in the sagittal and vertical directions.

Abstract:Background: This study quantitatively analyzed the anatomic structure of the alveolar bone in the maxillary molar region at three potential locations for Temporary Anchorage Device (TAD) placement. Additionally, the study compared the variability in this region across different age groups, sagittal skeletal patterns, vertical facial types, and sexes. Methods: In this retrospective cone-beam computed tomography study, the buccal alveolar bone was analyzed in the posterior molar area of 200 patients, the measurement items include buccal alveolar bone height, alveolar bone thickness, interradicular distance, and maxillary retromolar space. Results: Buccal alveolar height was greatest in the U56 region. The interradicular space was largest in the U56 region and increased from the alveolar crest to the sinus floor. Buccal alveolar bone thickness was highest in the U67 region and generally increased from the alveolar crest to the sinus floor. The maxillary retromolar space gradually increased from the alveolar crest to the root apex. Conclusions: TADs are safest when placed in the buccal area between the maxillary second premolar and the first molar, particularly at the 9 mm plane. The U67 region is the optimal safe zone for TAD placement for maxillary dentition distalization. TADs placement in adolescents can be challenging. Maxillary third molar extraction can be considered for maxillary dentition distalization.
Changes in alveolar process dimensions following extraction of molars with advanced periodontal disease: A clinical pilot study

Abstract:Objective: To use cone beam computed tomography (CBCT) to assess the alterations of socket dimensions after a 6-month healing period following molar extraction. Materials and methods: Seventeen molars were extracted due to advanced periodontitis. CBCT scans were taken immediately after extraction and once more 6 months later. Superimposition of CBCT images was used to measure the following: horizontal changes at extraction sites at three corono-apical levels (-1, -3, and -5 mm) below the bone crest, three mesio-distal levels (mesial, central, and distal), vertical changes at nine regions of the alveolar crest, and alveolar bone volume. Results: The width of the central crests at -1 mm decreased by 0.59 and 0.72 mm in the buccal and 0.27 and 0.02 mm in the lingual, in the maxilla and mandible, respectively. No statistically significant decreases in the ridge height were observed except in the disto-palatal region with a 1.11 mm decrease in the maxilla (p < 0.05). Heights of the buccal and lingual bone plates decreased significantly and ranged from 0.56 to 1.38 mm in the mandible after 6 months of healing (p < 0.05). Overall, ridge height changes were not significantly different between the maxilla and mandible (p > 0.05) and no significant volumetric bone loss occurred in either maxillary or mandibular sockets after 6 months of healing. Conclusions: Socket dimensions of molars with advanced periodontal disease showed a significant increase at the middle-central portion, although there were no significant changes of horizontal width 6 months following the procedures.
Facial alveolar bone thickness and modifying factors of anterior maxillary teeth: a systematic review and meta-analysis of cone-beam computed tomography studies

Abstract:Background: Understanding the anatomy of the facial alveolar bone (FAB), provides a prognostic tool for estimating the degree of dimensional ridge alterations after tooth extraction. This systematic review and meta-analysis aims to determine the FAB thickness and modifying factors of anterior maxillary teeth measured by CBCT scans. A secondary objective was to assess the facial distance from the cementoenamel junction (CEJ) to the bone crest. Methods: An electronic search was made of Medline, Embase, Web of Science, Cochrane Library and Google Scholar up to December 2019. Studies that analyze and quantitatively compare FAB thickness at maxillary teeth by CBCT scans were included. The methodological quality of the included studies was appraised using the ROBINS-I tool and the overall meta-evidence certainty using the GRADE approach. A single means random-effects meta-analysis was performed to obtain the weighted mean for 95% confidence interval. A meta-regression of covariates and subgroup analysis was conducted. The nullity Qh test and I2 index for heterogeneity was estimated. Results: 2560 potentially relevant articles were recorded from which 29 studies were selected for the qualitative analysis, including 17,321 teeth. Seventeen studies considered the facial bone crest, and 12 the CEJ as a reference point for their measurements. Mean FAB thickness was ≤ 1 mm in maxillary incisors and canines (0.75-1.05 mm) and 1-2 mm in premolars. Patients over 50 years of age, females and thin gingival phenotype was associated with thinner FAB at some apico-coronal locations of maxillary incisors and canines. The geographical setting was an effect modifier that could explain up to 87% of the heterogeneity in FAB thickness, being Asian populations that showed the lowest FAB thickness values. The CEJ-bone crest distance was 2-2.5 mm in all teeth analyzed. Population over 50 years of age exhibited greater CEJ-bone crest distances, and males also showed a trend for greater distance. Evidence certainty has shown moderate quality in most analysis subsets. Conclusions: Facial alveolar bone at anterior maxillary teeth is thin, heterogeneous in width along its apico-coronal dimensions, and increases in thickness in maxillary premolars. The CEJ-bone crest distance presented homogeneous and similar values in all teeth analyzed.
