Abstract:The digital revolution is disrupting the ways in which health research is conducted, and subsequently, changing healthcare. Direct-to-consumer wellness products and mobile apps, pervasive sensor technologies and access to social network data offer exciting opportunities for researchers to passively observe and/or track patients 'in the wild' and 24/7. The volume of granular personal health data gathered using these technologies is unprecedented, and is increasingly leveraged to inform personalized health promotion and disease treatment interventions. The use of artificial intelligence in the health sector is also increasing. Although rich with potential, the digital health ecosystem presents new ethical challenges for those making decisions about the selection, testing, implementation and evaluation of technologies for use in healthcare. As the 'Wild West' of digital health research unfolds, it is important to recognize who is involved, and identify how each party can and should take responsibility to advance the ethical practices of this work. While not a comprehensive review, we describe the landscape, identify gaps to be addressed, and offer recommendations as to how stakeholders can and should take responsibility to advance socially responsible digital health research.

Abstract:This article addresses problems stemming from the implementation and development of digital health in general and telehealth in particular. It focuses on ethical and legal issues associated with the progress of new information technologies and other technologies used in health care. The aim of the article was to define the role of ethical and legal norms in the implementation and development of telehealth. The analysis of the currently effective legislation, its application and lacunae in the regulation of new forms of social relations suggests the need to expedite development of legal and ethical guidelines for the implementation of new technologies in health care. Higher standards of data security for vulnerable groups of patients should be established in the legislation and ethical guidelines.

Abstract:Digitalization in medicine offers a significant opportunity to transform healthcare systems by providing novel digital tools and services to guide personalized prevention, prediction, diagnosis, treatment and disease management. This transformation raises a number of novel socio-ethical considerations for individuals and society as a whole, which need to be appropriately addressed to ensure that digital medical devices (DMDs) are widely adopted and benefit all patients as well as healthcare service providers. In this narrative review, based on a broad literature search in PubMed, Web of Science, Google Scholar, we outline five core socio-ethical considerations in digital medicine that intersect with the notions of equity and digital inclusion: (i) access, use and engagement with DMDs, (ii) inclusiveness in DMD clinical trials, (iii) algorithm fairness, (iv) surveillance and datafication, and (v) data privacy and trust. By integrating literature from multidisciplinary fields, including social, medical, and computer sciences, we shed light on challenges and opportunities related to the development and adoption of DMDs. We begin with an overview of the different types of DMDs, followed by in-depth discussions of five socio-ethical implications associated with their deployment. Concluding our review, we provide evidence-based multilevel recommendations aimed at fostering a more inclusive digital landscape to ensure that the development and integration of DMDs in healthcare mitigate rather than cause, maintain or exacerbate health inequities.

Technology offers problem-solving applications for implementation of healthcare delivery that remains too unsafe, unreliable, unsatisfying, and expensive. The author offers insights into the ethics and implications of the healthcare providers’ dependence on digital technology. She further suggests possibilities for the utilization of technology from a nursing theoretical perspective that honors the importance of human discovery and the expansion of disciplinary theoretical knowledge for future professional nurse practice.

Abstract:Background: The integration of artificial intelligence (AI)-based applications in the medical field has increased significantly, offering potential improvements in patient care and diagnostics. However, alongside these advancements, there is growing concern about ethical considerations, such as bias, informed consent, and trust in the development of these technologies. Objective: This study aims to assess the role of ethics in the development of AI-based applications in medicine. Furthermore, this study focuses on the potential consequences of neglecting ethical considerations in AI development, particularly their impact on patients and physicians. Methods: Qualitative content analysis was used to analyze the responses from expert interviews. Experts were selected based on their involvement in the research or practical development of AI-based applications in medicine for at least 5 years, leading to the inclusion of 7 experts in the study. Results: The analysis revealed 3 main categories and 7 subcategories reflecting a wide range of views on the role of ethics in AI development. This variance underscores the subjectivity and complexity of integrating ethics into the development of AI in medicine. Although some experts view ethics as fundamental, others prioritize performance and efficiency, with some perceiving ethics as potential obstacles to technological progress. This dichotomy of perspectives clearly emphasizes the subjectivity and complexity surrounding the role of ethics in AI development, reflecting the inherent multifaceted nature of this issue. Conclusions: Despite the methodological limitations impacting the generalizability of the results, this study underscores the critical importance of consistent and integrated ethical considerations in AI development for medical applications. It advocates further research into effective strategies for ethical AI development, emphasizing the need for transparent and responsible practices, consideration of diverse data sources, physician training, and the establishment of comprehensive ethical and legal frameworks.

Abstract:Introduction Doctors hold a prima facie duty to respect the autonomy of their patients. This manifests as the patient's 'right' not to know when patients wish to remain unaware of medical information regarding their health, and poses ethical challenges for good medical practice. This paper explores how the emergence of digital health technologies might impact upon the patient's 'right' not to know. Method The capabilities of digital health technologies are surveyed and ethical implications of their effects on the 'right' not to know are explored. Findings Digital health technologies are increasingly collecting, processing and presenting medical data as clinically useful information, which simultaneously presents large opportunities for improved health outcomes while compounding the existing ethical challenges generated by the patient's 'right' not to know. Conclusion These digital tools should be designed to include functionality that mitigates these ethical challenges, and allows the preservation of their user's autonomy with regard to the medical information they wish to learn and not learn about.

Abstract:Ethics has long been a concern in medicine, education and scholarship. In the digital age, new complexities have arisen, and many medical education researchers are unprepared for the pitfalls ahead, often negotiating these in the absence of guidelines, and unaware of the many tools that can be used to assist them. This Guide takes the medical education scholar through a journey in which issues of ethics are discussed in all stages of digital scholarship: research preparation, research subject monitoring and data gathering, securing one's data (and balancing security against accessibility), anonymising textual and non-textual data, third party identifiability in digital data, writing one's own work (including plagiarism and paper mills), copyright (including issues of Creative Commons and royalty-free), accessing inaccessible reference material, ethically citing electronic material, and manuscript submission (including issues of selecting journals, open access and data sharing). The Guide ends with a brief look to the future. This Guide aims to be a useful tool to alert the readers to some of the most important ethical issues that need to be considered, and some practical solutions to ethical problems faced, when engaging in medical education digital scholarship.

Abstract:Digital health encompasses a wide range of novel digital technologies related to health and medicine. Such technologies rely on recent advances in the collection and analysis of ever increasing amounts of data from both patients and healthy citizens. Along with new opportunities, however, come new ethical and policy challenges. These range from the need to adapt current evidence-based standards, to issues of privacy, oversight, accountability and public trust as well as national and international data governance and management. This review illustrates key issues and challenges facing the rapidly unfolding digital health paradigm and reflects on the impact of big data in medical research and clinical practice both internationally and in Switzerland. It concludes by emphasising five conditions that will be crucial to fulfil in order to foster innovation and fair benefit sharing in digital health.

Differences between traditional ethics in the medical domain and emerging ethical challenges with AI-driven healthcare are presented, particularly as they relate to transparency, bias, privacy, safety, responsibility, justice, and autonomy. Open challenges and recommendations are outlined to enable the integration of ethical principles into the design, validation, clinical trials, deployment, monitoring, repair, and retirement of AI-based smart healthcare products.

Abstract:Digital Health Technologies (DHTs) are currently the subject of much debate both in terms of their technological frontiers as well as their ethical, legal and societal implications (ELSI). Regulation of such technologies as medical devices currently lacks behind their level of adoption. Digital Twins are the next evolution step of such DHTs and provide an opportunity to anticipate and act on ELSI before adoption again leaps before the necessary review. This paper introduces the concept and use cases of digital twins in medicine, then frames the debate through the lens of related technologies, machine learning and personalized medicine, and maps ethical challenges stemming from those. Finally, we lay out how digital twins may change and challenge the future practice of medicine.

Abstract:Knowledge and capabilities, particularly of a new technology or in a new area of study, frequently develop faster than the guidelines and principles needed for practitioners to practice ethically in the new arena; this is particularly true in medicine. The blending of medicine and healthcare with e-commerce and the Internet raises many questions involving what sort of ethical conduct should be expected by practitioners and developers of the medical Internet. Some of the early pioneers in medical and healthcare Web sites pushed the ethical boundaries with questionable, even unethical, practices. Many involved with the medical Internet are now working to reestablish patient and consumer trust by establishing guidelines to determine how the fundamentals of the medical code of ethical conduct can best be adapted for the medical/healthcare Internet. Ultimately, all those involved in the creation, maintenance, and marketing of medical and healthcare Web sites should be required to adhere to a strict code of ethical conduct, one that has been fairly determined by an impartial international organization with reasonable power to regulate the code. This code could also serve as a desirable, recognizable label-of-distinction for ethical Web sites within the medical and healthcare Internet community. One challenge for those involved with the medical and healthcare Internet will be to determine what constitutes "Medical Internet Ethics" or "Healthcare Internet Ethics," since the definition of medical ethics can vary from country to country. Therefore, the emerging field of Medical/ Healthcare Internet Ethics will require careful thought and insights from an international collection of ethicists in many contributing areas. This paper is a review of the current status of the evolving field of Medical/Healthcare Internet Ethics, including proposed definitions and identification of many diverse areas that may ultimately contribute to this multidisciplinary field. The current role that medicine and health play in the growing area of Internet communication and commerce and many of the ethical challenges raised by the Internet for the medical community are explored and some possible ways to address these ethical challenges are postulated.

Abstract:Big data are certainly an essential component of digital science and technology and also of machine learning, robotics, and new means of communication. They are also part of the digital revolution that is shaking our society to its very roots. The information that the data initially contain, is considerably enriched by cross-referencing data. Highly diverse, these data can berelated to health or well-being. One of the characteristics of big data in health is the blurring of the distinctions underpinningimplementation of the ethical principles that promote the protection of individual rights in health. Precise knowledge of individuals and of their state of health creates a risk of profiling, which threatens the protection of private life and may lead to stigmatization of people or groups. Such stigmatization threatens private life, but also the principles of solidarity and equity which are the basis of our health system. Care and business are becoming increasingly hard to distinguish, as aresult of the transformation of care and of the healthcare market. In addition, ethical principles can be weakened by the exploitation of big data: medical confidentiality, the responsibility of the medical decision and the personal relationship between the doctor and patient. The need for protection of the in-dividual must be reaffirmed and its modalities redefined, to dispel the threat of a society under the surveillance and control of multiple providers acting for various purposes.

Abstract:The online space has become a digital public square, where individuals interact and share ideas on the most trivial to the most serious of matters, including discussions of controversial ethical issues in science, technology and medicine. In the last decade, new disciplines like computational social science and social data science have created methods to collect and analyse such data that have considerably expanded the scope of social science research. Empirical bioethics can benefit from the integration of such digital methods to investigate novel digital phenomena and trace how bioethical issues take shape online. Here, using concrete examples, we demonstrate how novel methods based on digital approaches in the social sciences can be used effectively in the domain of bioethics. We show that a digital turn in bioethics research aligns with the established aims of empirical bioethics, integrating with normative analysis and expanding the scope of the discipline, thus offering ways to reinforce the capacity of bioethics to tackle the increasing complexity of present-day ethical issues in science and technology. We propose to call this domain of research in bioethics digital bioethics .

Abstract:Telemedicine, defined as the practice of delivering healthcare services remotely using information and communications technologies, raises a plethora of ethical considerations. As telemedicine evolves, its ethical dimensions play an increasingly pivotal role in balancing the benefits of advanced technologies, ensuring responsible healthcare practices within telemedicine environments, and safeguarding patient rights. Healthcare providers, patients, policymakers, and technology developers involved in telemedicine encounter numerous ethical challenges that need to be addressed. Key ethical topics include prioritizing the protection of patient rights and privacy, which entails ensuring equitable access to remote healthcare services and maintaining the doctor-patient relationship in virtual settings. Additional areas of focus encompass data security concerns and the quality of healthcare delivery, underscoring the importance of upholding ethical standards in the digital realm. A critical examination of these ethical dimensions highlights the necessity of establishing binding ethical guidelines and legal regulations. These measures could assist stakeholders in formulating effective strategies and methodologies to navigate the complex telemedicine landscape, ensuring adherence to the highest ethical standards and promoting patient welfare. A balanced approach to telemedicine ethics should integrate the benefits of telemedicine with proactive measures to address emerging ethical challenges and should be grounded in a well-prepared and respected ethical framework.

Medical educators must evolve alongside AI’s advancements, becoming stewards of ethical consciousness in an era where algorithms and data-driven decision-making play pivotal roles in patient care. The traditional paradigm of medical education, rooted in foundational ethical principles, must adapt to incorporate the complex ethical considerations introduced by AI. This pedagogical approach fosters dynamic engagement, cultivating a profound ethical awareness among students.
