Abstract:Over the past year, numerous important advances in our understanding of multiple aspects of asthma, ranging from disease pathogenesis to epidemiology to therapeutics, have been reported. This review is a compilation of highlights from articles published largely in the Journal of Allergy and Clinical Immunology and supplemented by articles published elsewhere that have substantially advanced the fields of asthma, chronic obstructive pulmonary disease (COPD), and asthma-COPD overlap and biologic therapies for these disorders. The intention of this article is not to provide a comprehensive review but rather to focus on several areas that have developed quickly and/or received extensive attention from our readers.

Abstract:Abstract The importance of allergen immunotherapy (AIT) is multifaceted, encompassing both clinical and quality‐of‐life improvements and cost‐effectiveness in the long term. Key mechanisms of allergen tolerance induced by AIT include changes in memory type allergen‐specific T‐ and B‐cell responses towards a regulatory phenotype with decreased Type 2 responses, suppression of allergen‐specific IgE and increased IgG 1 and IgG 4 , decreased mast cell and eosinophil numbers in allergic tissues and increased activation thresholds. The potential of novel patient enrolment strategies for AIT is taking into account recent advances in biomarkers discoveries, molecular allergy diagnostics and mobile health applications contributing to a personalized approach enhancement that can increase AIT efficacy and compliance. Artificial intelligence can help manage and interpret complex and heterogeneous data, including big data from omics and non‐omics research, potentially predict disease subtypes, identify biomarkers and monitor patient responses to AIT. Novel AIT preparations, such as synthetic compounds, innovative carrier systems and adjuvants, are also of great promise. Advances in clinical trial models, including adaptive, complex and hybrid designs as well as real‐world evidence, allow more flexibility and cost reduction. The analyses of AIT cost‐effectiveness show a clear long‐term advantage compared to pharmacotherapy. Important research questions, such as defining clinical endpoints, biomarkers of patient selection and efficacy, mechanisms and the modulation of the placebo effect and alternatives to conventional field trials, including allergen exposure chamber studies are still to be elucidated. This review demonstrates that AIT is still in its growth phase and shows immense development prospects.

Abstract:In this Synopsis Book, our reviewers have selected articles with relevant data that will optimize the clinical care of children affected by allergic and immunologic disorders. These include publications from the past year that have impactful findings which can support current practice and/or advance our understanding, leading to new insights and approaches to allergic and immunologic disorders. A wide spectrum of allergy disorders that affect children are covered, with articles providing observations and new knowledge ranging from possible approaches to prevention of atopic disorders, risk factors contributing to the development of atopic dermatitis, and emerging treatment options for the treatment of food allergy, atopic dermatitis, and asthma.

Abstract:New Trends in Allergy V" is the fifth volume of a series focussing on new trends in experimental and clinical allergology. The interdisciplinary character is documented by articles from such different fields as basic immunology, chemistry, molecular biology, pharmacology, epidemiology, pneumology, dermatology, otolaryngology, pediatrics and psychosomatic medicine. Special emphasis is given to the role of both anthropogenic (pollutants) as well as natural environmental factors in the development of allergy. Among new pathophysiological concepts, dendritic cells, lymphocyte interactions, mast cells, eosinophils and neutrophils as well as psycho-neuro-immunological influences are featured. Practical aspects focus on asthma, rhinoconjunctivitis, atopic eczema, occupational and food allergy as well as anaphylaxis. The most recent therapeutic strategies comprise novel pharmacotherapy and allergen-specific as well as unspecific (e. g. anti-IgE) immunotherapy.

Abstract:Abstract Recent developments in the understanding of the pathophysiology of inflammatory skin diseases have resulted in the emergence of new treatments. However, new therapies are frequently accompanied by new side effects. In the November issue of the Journal, we explore recent advances on how Cutibacterium acnes phylotypes are involved in biofilm formation and its significance in therapy resistance. The Janus kinase (JAK) inhibitor‐induced acneiform eruptions also raise intriguing questions about the complex relationship between JAK inhibition and acne pathophysiology. We also provide real‐world data on recent therapies for atopic dermatitis, hyperkeratotic hand and foot dermatitis, and generalized pustular psoriasis.

Advances and highlights in allergen immunotherapy: On the way to sustained clinical and immunologic tolerance

Abstract:Allergen immunotherapy (AIT) is an effective treatment strategy for allergic diseases and has been used for more than 100 years. In recent years, however, the expectations on concepts, conduct, statistical evaluation, and reporting have developed significantly. Products have undergone dose-response and confirmative studies in adults and children to provide evidence for the optimal dosage, safety, and efficacy of AIT vaccines using subcutaneous and sublingual delivery pathways in large patient cohorts, ensuring solid conclusions to be drawn from them for the advantage of patients and societies alike. Those standards should be followed today, and products answering to them should be preferred over others lacking optimization and proof of efficacy and safety. Molecular and cellular mechanisms of AIT include early mast cell and basophil desensitization effects, regulation of T- and B-cell responses, regulation of IgE and IgG4 production, and inhibition of responses from eosinophils, mast cells, and basophils in the affected tissues. There were many developments to improve vaccination strategies, demonstration of new molecules involved in molecular mechanisms, and demonstration of new biomarkers for AIT during the last few years. The combination of probiotics, vitamins, and biological agents with AIT is highlighting current advances. Development of allergoids and recombinant and hypoallergenic vaccines to skew the immune response from IgE to IgG4 and regulation of dendritic cell, mast cell, basophil, innate lymphoid cell, T-cell, and B-cell responses to allergens are also discussed in detail.

Abstract:Research efforts in allergic rhinitis have always been intense. Over the past 3 years, numerous breakthroughs in basic science and clinical research have been made, augmenting our understanding of this condition that afflicts a significant proportion of the global population. New epidemiological findings, novel insights into the molecular and cellular mechanisms of allergy, enhancement of current developmental theories, new concepts of the goals and endpoints of management, and latest therapeutic modalities that includes the harnessing of information technology and big data are some areas where important advances were made. We attempt to bring you a summary of the key research advances made in the field of allergic rhinitis from 2013 to 2015.

Abstract:This study aimed to present a bibliometric and altmetric Analyses of the Iranian Journal of Allergy, Asthma, and Immunology (IJAAI). The citation performance and altmetric data were extracted from Scopus and Altmetric Explorer, respectively. Analyses were done using SPSS 26, Microsoft Excel, VOSviewer, and CiteSpace. The results of the bibliometric analysis revealed that IJAAI had experienced respectable growth. Among the total citations, 4746 citations belong to the first decade (2005-2014) and 3,035 citations belong to the second (2015-2022). The findings demonstrated the significance of IJAAI among Iranian researchers. Pourpak, Z (66; 6.57%) is the top-producing author in IJAAI. The examination of research institutions reveals that the Tehran University of Medical Sciences (TUMS) is ranked first. The most highly cited article in IJAAI over the past 18 years is a review article which has received 138 citations. IJAAI is ranked first at the citing source and journal level, with the most citations (249 citations) to IJAAI. Iran has collaborated with 13 other countries. Overall, the analysis of co-occurred keywords indicates that IJAAI authors have used the following three high-frequency and important keywords: Asthma (162), Inflammation (48), and Multiple sclerosis (40). Co-citation analysis results demonstrated that a total of 6,718 sources were cited in this journal. The results of the altmetric analysis show that IJAAI has a reasonably low presence across various social media platforms, including Twitter, Facebook, Wikipedia, Mendeley, news and blogs. This study aids researchers in exploring and identifying emerging trends in the fields of allergy, asthma, and immunology.

Abstract:With its advent in 2018, the publishers and editors of JAMA Network Open confronted the risky proposition as to whether a high-volume, open access journal could be successful while maintaining the same standards as the most highly respected medical journals. Now, 6 years on, the answer is clear. Submissions to the journal have grown steadily, reaching nearly 15 000 in 2023, 14% more than in 2022. The reach of the journal has become global. Two-thirds of submissions originate from 82 countries outside the United States, particularly China, Korea, Taiwan, Canada, Japan, the United Kingdom, Germany, and France. We published 2187 articles in 2023, with an acceptance rate of 12% for research studies and 15% overall. Even given this volume, the quality and importance of the articles we published remained high, as reflected by the journal Impact Factor of 13.8, making JAMA Network Open the leading large-volume open access general medical journal in the world. Moreover, there were more than 30 million article views and downloads and 73 000 mentions in the news media last year, and the journal has more than 43 000 social media followers. The remarkable success of JAMA Network Open clearly validates the notion that an open access journal can reliably deliver the highest quality science as full-content articles to the world without compromise and without cost to readers. We firmly believe that much of the success of JAMA Network Open is due to an unwavering commitment to the journal's mission, "to improve health, health care, and health equity worldwide through open access dissemination of high quality, innovative, general medical research and commentary by and for a broad range of clinicians, researchers, policy makers, and public health and health care leaders." 1 In keeping with this charge, JAMA Network Open covers a remarkably broad range of topics on health and health care of worldwide importance and interest. 2 -10 For example, as seen in the Table, popular studies published in 2023 focused on clinical topics such as alcohol intake, 2 sleep patterns, 3 and stroke. 8 We continue to receive nearly 3000 submissions annually that are related to SARS-CoV-2, and this topic continues to be of great interest to our readers, as evidenced by the studies with the highest Altmetric scores 5 -7 (Table). Our focus has evolved along with the pandemic. We are most interested in studies that contribute important information about new, emerging trends in SARS-CoV2 infection; rigorous studies on the etiology, epidemiology, and clinical manifestations of post–COVID-19 condition; and intervention trials of treatments for this post–COVID-19 condition that include appropriate control groups. We are also interested in evaluations of interventions that increase vaccination rates or address other avenues for preventing disease, especially among high-risk populations. We remain deeply committed to addressing racism in medicine, health care, and society both in the US and abroad, 11 as is the JAMA Network as a whole. This past year, under the leadership of our diversity, equity, and inclusion associate editor, Monica Vela, MD, we broadened our efforts beyond publishing high-caliber articles related to research and scholarship that address the needs of all communities and established an editorial fellowship with an initial cadre of 6 outstanding junior faculty. Our goal is to help to train the next generation of medical journalists and to expand diversity among editorial board members and researchers across the scientific community. This year, in a related vein, the JAMA Network gathered data on the self-reported diversity of our editors and board members. 12 At JAMA Network Open , among 36 responders of 41 editors and editorial board members, 53% are men and 47% are women. By self-reported race and ethnicity, 8% are Asian or Pacific Islander individuals, 19% are Black individuals, 5% are Hispanic individuals, 61% are White individuals, 8% are from other racial backgrounds (including American Indian or Alaska Native and Middle Eastern or North African), and 5% chose not to disclose. We have and will continue to strongly consider diversity as we add new members to our editorial team. JAMA Network Open places a high priority on studies that help us better understand how the social constructs of race and racism in their many manifestations affect health across the globe. Two of our most viewed and cited studies 4 ,10 address equity issues for physicians underrepresented in medicine (Table). Abstract:IgE-mediated food allergy (IgE-FA) occurs due to a breakdown in immune tolerance that leads to a detrimental type 2 helper T cell (T H 2) adaptive immune response. While the processes governing this loss of tolerance are incompletely understood, several host-related and environmental factors impacting the risk of IgE-FA development have been identified. Mounting evidence supports the role of an impaired epithelial barrier in the development of IgE-FA, with exposure of allergens through damaged skin and gut epithelium leading to the aberrant production of alarmins and activation of T H 2-type allergic inflammation. The treatment of IgE-FA has historically been avoidance with acute management of allergic reactions, but advances in allergen-specific immunotherapy and the development of biologics and other novel therapeutics are rapidly changing the landscape of food allergy treatment. Here, we discuss the pathogenesis and immunobiology of IgE-FA in addition to its diagnosis, prognosis, and treatment.
