[The orthodontic therapeutic principles for malocclusions concurring with temporomandibular joint disorders]

Abstract:The biological nature of temporomandibular joint (TMJ) featuring adaptive remodeling allows for TMJ structural changes in response to external stimuli, including alterations in occlusion and in mandibular posture. Correlative susceptibility exists between malocclusions and TMJ disorders (TMD), evidenced by high incidence of TMD coinciding with skeletal originated malocclusion with facial prognathsim (Angle Class I skeletal II high angle). Clinical manifestations include markedly retruded mandible together with increased overjet, or mandibular transverse deviation, due to condylar pathological resorption. It is critically imperative to execute differential diagnosis between the routine malocclusions and those infringed with TMD. The prime stipulations governing the orthodontic procedures include the patient's notification of the existential TMD conditions prior to any orthodontic interventions, close inspections of TMD developments during the treatment, and a specially designed retention protocol aftermath. For the cases where severe overjet and mandibular displacement occur as a result of TMD, some specific orthopedic modalities can be implemented to improve the occlusion by restoring the shifted mandible. TMD conditions are also expected to be relieved with mandibular repositioning, which encourages new bone formation in the degraded condyles. The newly developed clear version of orthopedic appliances, namely, S8-SGTB, S8-SGHB, S9 and S10 are of promising efficacy in treating this category of patients.

Abstract:The treatment of patients with severe temporomandibular joint (TMJ) disorders and associated skeletal discrepancies presents a complex challenge for oral and maxillofacial surgeons. It is widely recognized that TMDs can impact the stability and outcomes of surgical treatments for dentofacial deformities. Consequently, addressing TMDs prior to or in conjunction with orthodontic or surgical interventions may be necessary to achieve optimal and long-lasting results. Alloplastic temporomandibular joint replacement (TMJR) and orthognathic surgery have emerged as the standard approach due to their predictability, long-term stability and excellent outcomes when addressing end-stage TMJ disease in conjunction with DFDs as it provides a comprehensive solution to address both functional and aesthetic aspects of these patients' conditions. Understanding the appropriate utilization of TMJR in conjunction with orthognathic surgery can lead to improved treatment planning and successful outcomes for patients with complex TMJ disorders and associated dentofacial deformities. This review aims to discuss the indications, preoperative evaluation, staging, sequencing, and surgical considerations involved in utilizing alloplastic TMJ replacement in the presence of dentofacial deformities.

Abstract:Introduction: Temporomandibular joint (TMJ) ankylosis is a debilitating condition characterised by the fusion of bony or fibrous tissues in the TMJ, resulting in restricted mouth opening, functional challenges and facial deformities. This report highlights the complexity and multifaceted nature of TMJ ankylosis and emphasises the significance of early intervention and a multidisciplinary approach. Distraction osteogenesis has emerged as a successful method for managing TMJ ankylosis and associated facial asymmetry. Post-distraction orthodontic management is essential to address occlusal changes, including posterior open bite and crossbite. Diagnosis and treatment plan: A 17-year-old boy with unilateral right-sided TMJ ankylosis presented with restricted mouth opening, facial asymmetry and functional difficulties. Multidisciplinary treatment planning involved distraction osteogenesis, condylectomy, coronoidectomy and orthodontic correction to achieve proper occlusion and facial aesthetics. Results: The treatment significantly improved facial aesthetics, mouth opening, occlusion and the patient's confidence and self-esteem. Conclusion: This case report underscores the complexity of TMJ ankylosis and the need for a comprehensive approach, including surgical and orthodontic interventions. Collaboration among healthcare professionals and ongoing research is vital to refine treatment approaches and improve the quality of life for patients with TMJ ankylosis.
