Abstract:BackgroundDigital forms of direct-to-consumer pharmaceutical marketing (eDTCA) have globalized in an era of free and open information exchange. Yet, the unregulated expansion of eDTCA has resulted in unaddressed global public health threats. Specifically, illicit online pharmacies are engaged in the sale of purportedly safe, legitimate product that may in fact be counterfeit or substandard. These cybercriminal actors exploit available eDTCA mediums over the Internet to market their suspect products globally. Despite these risks, a detailed assessment of the public health, patient safety, and cybersecurity threats and governance mechanisms to address them has not been conducted.DiscussionIllicit online pharmacies represent a significant global public health and patient safety risk. Existing governance mechanisms are insufficient and include lack of adequate adoption in national regulation, ineffective voluntary governance mechanisms, and uneven global law enforcement efforts that have allowed proliferation of these cybercriminals on the web. In order to effectively address this multistakeholder threat, inclusive global governance strategies that engage the information technology, law enforcement and public health sectors should be established.SummaryEffective global “eHealth Governance” focused on cybercrime is needed in order to effectively combat illicit online pharmacies. This includes building upon existing Internet governance structures and coordinating partnership between the UN Office of Drugs and Crime that leads the global fight against transnational organized crime and the Internet Governance Forum that is shaping the future of Internet governance. Through a UNODC-IGF governance mechanism, investigation, detection and coordination of activities against illicit online pharmacies and their misuse of eDTCA can commence.

Abstract:Patient confidentiality has remained a central issue in the current “big data” era of healthcare. Protections such as the Health Insurance Portability and Accountability Act of 1996 (HIPAA) exist to ensure that digital personal health information (PHI) are legally secure from threats and breaches that would threaten confidentiality. To be compliant with HIPAA regulations, steps must be taken by health care providers and digital health platforms, and these fall under the Privacy Rule, which outlines appropriate uses and disclosures of PHI, and the Security Rule, which lays out with granularity the steps that must be taken to adhere to the HIPAA regulations. Through deliberate design of secure digital health platforms, we can use technological advances in the collection, measurement, and delivery of health care to advance care and improve patient safety. Renewed efforts to optimize and standardize health care delivery has facilitated the implementation of electronic and digital health solutions that benefit medical and surgical training and efficiency while minimizing harm to patients. Cross-industry innovations such as the OR Black Box® will allow us to accomplish these lofty goals. Finally, we must strive to include patients in this digital health movement, as now more than ever we can create knowledge translation solutions that ensure that patients understand their health in a meaningful way.
Integration of personalized drug delivery systems into digital health

Abstract:Personalized drug delivery systems (PDDS), implying the patient-tailored dose, dosage form, frequency of administration and drug release kinetics, and digital health platforms for diagnosis and treatment monitoring, patient adherence, and traceability of drug products, are emerging scientific areas. Both fields are advancing at a fast pace. However, despite the strong complementary nature of these disciplines, there are only a few successful examples of merging these areas. Therefore, it is important and timely to combine PDDS with an increasing number of high-end digital health solutions to create an interactive feedback loop between the actual needs of each patient and the drug products. This review provides an overview of advanced design solutions for new products such as interactive personalized treatment that would interconnect the pharmaceutical and digital worlds. Furthermore, we discuss the recent advancements in the pharmaceutical supply chain (PSC) management and related limitations of the current mass production model. We summarize the current state of the art and envision future directions and potential development areas.

Abstract:The biomedical industry is developing at an unprecedented speed and the establishment and improvement of a digital infrastructure for biomedical research and development (R&D) is crucial for biotechnology innovation and Health China construction. This study analyzes the implication, classification, and systematic framework of the digital infrastructure for biomedical R&D in the new era. It summarizes the strategic layouts and development paths of the digital infrastructures for biomedical R&D in the United States, the European Union, and China, and examines the gap between China and European and American countries in terms of international influence and authority, development model design and efficiency, sustainability, and management organization construction. Moreover, it presents the challenges faced by the digital infrastructure of China regarding top-level design, management, and governance. Furthermore, we suggest that the following aspects should be strengthened: top-level framework design, coordination of biomedical and healthcare resources, clustered construction and governance of the platform, and capacity building and operationguarantee of the open platform.

Abstract:Digital health arrived to society and it generates data which is growing exponentially. Pharma and medical device industry recently embarked on this journey. Digital tools became everyday experience both in development as well as post-market settings. These non-traditional data sources may contain relevant safety, efficacy, effectiveness and other knowledge, which are valuable for understanding and further characterizing safety profile of a given medicinal and medical device product. It is however unclear what new responsibilities are associated with the use of such tools and data generated / collected by them. Current regulatory framework does not provide very clear guidance on it. Teams are struggling to interpret expectations originated from regulations, ethics and patients. We present practical approaches for data management and we suggest that a detailed assessment of projects is conducted to identify obligations for screening digitally-sourced data. Compliance with regulations is obligatory, but it is also incumbent on the sponsor to define a data management strategy covering the purpose of the activities, and the value of the data gathered. Decisions thereafter should be aligned with the mission, vision, and objectives defined by the sponsor.

Abstract:Tools and methods of precision medicine are developing rapidly, through both iterative discoveries enabled by innovations in biomedical research (e.g., genome editing, synthetic biology, bioengineered devices). These are strengthened by advancements in information technology and the increasing body of data—as assimilated, analyzed, and made accessible—and affectable—through current and emerging cyber—and systems- technologies. Taken together, these approaches afford ever greater volume and availability of individual and collective human data. Machine learning and/or artificial intelligence approaches are broadening this dual use risk; and in the aftermath of COVID-19, there is growing incentive and impetus to gather more biological data from individuals and their environments on a routine basis. By engaging these data—and the interventions that are based upon them, precision medicine offer promise of highly individualized treatments for disease and injury, optimization of structure and function, and concomitantly, the potential for (mis) using data to incur harm. This double-edged blade of benefit and risk obligates the need to safeguard human data from purloinment, through systems, guidelines and policies of a novel discipline, cyberbiosecurity, which, as coupled to ethical precepts, aims to protect human privacy, agency, and safety in ways that remain apace with scientific and technological advances in biomedicine. Herein, current capabilities and trajectories precision medicine are described as relevant to their dual use potential, and approaches to biodata security ( viz. - cyberbiosecurity) are proposed and discussed.

Abstract:The digitization of the medical data has been a sensitive topic. In modern times laws such as the HIPAA provide some guidelines for electronic transactions in medical data to prevent attacks and fraudulent usage of private information. In our paper, we explore an architecture that uses hybrid computing with decentralized key management and show how it is suitable in preventing a special form of re-identification attack that we name as the re-assembly attack. This architecture would be able to use current infrastructure from mobile phones to server certificates and cloud based decentralized storage models in an efficient way to provide a reliable model for communication of medical data. We encompass entities including patients, doctors, insurance agents, emergency contacts, researchers, medical test laboratories and technicians. This is a complete architecture that provides patients with a good level of privacy, secure communication and more direct control.

Abstract:Digitalization in medicine offers a significant opportunity to transform healthcare systems by providing novel digital tools and services to guide personalized prevention, prediction, diagnosis, treatment and disease management. This transformation raises a number of novel socio-ethical considerations for individuals and society as a whole, which need to be appropriately addressed to ensure that digital medical devices (DMDs) are widely adopted and benefit all patients as well as healthcare service providers. In this narrative review, based on a broad literature search in PubMed, Web of Science, Google Scholar, we outline five core socio-ethical considerations in digital medicine that intersect with the notions of equity and digital inclusion: (i) access, use and engagement with DMDs, (ii) inclusiveness in DMD clinical trials, (iii) algorithm fairness, (iv) surveillance and datafication, and (v) data privacy and trust. By integrating literature from multidisciplinary fields, including social, medical, and computer sciences, we shed light on challenges and opportunities related to the development and adoption of DMDs. We begin with an overview of the different types of DMDs, followed by in-depth discussions of five socio-ethical implications associated with their deployment. Concluding our review, we provide evidence-based multilevel recommendations aimed at fostering a more inclusive digital landscape to ensure that the development and integration of DMDs in healthcare mitigate rather than cause, maintain or exacerbate health inequities.
Keep an eye on your personal belongings! The security of personal medical devices and their ecosystems

Abstract:Today, personal medical devices (PMDs) play an increasingly important role in healthcare ecosystems as patient life support equipment. As a result of technological advances, PMDs now encompass many components and functionalities that open the door to a variety of cyber-attacks. In this paper we present a taxonomy of ten widely-used PMDs based on the five diseases they were designed to treat. We also provide a comprehensive survey that covers 17 possible attacks aimed at PMDs, as well as the attacks' building blocks. For each PMD type, we create an ecosystem and data and attack flow diagram, which comprehensively describes the roles and interactions of the players associated with the PMD and presents the most vulnerable vectors and components within the PMDs' ecosystems; such knowledge can increase security awareness among PMD users and their healthcare providers. We also present the basic, yet important, building blocks that constitute the steps by which each of the attacks presented is carried out. Doing so allowed us to establish the foundations for the future development of a novel risk analysis methodology for medical devices. For each attack we mapped the building blocks required to carry out the attack and found that 50% of the attacks rely upon the ability to remotely connect to the PMD, while 61% of them rely on the physical proximity of the attacker to the PMD. Finally, by surveying 21 existing security mechanisms and mapping their coverage for the attacks, we identify the gaps between PMDs' security mechanisms and the possible attacks. We show that current security mechanisms generally fail to provide protection from all of the attacks against PMDs and suggest the development of a comprehensive framework to secure PMDs and protect the patients that rely upon them.
Revolutionizing Medical Data Sharing Using Advanced Privacy Enhancing Technologies: Technical, Legal and Ethical Synthesis

Abstract:Multisite medical data sharing is critical in modern clinical practice and medical research. The challenge is to conduct data sharing that preserves individual privacy and data usability. The shortcomings of traditional privacy-enhancing technologies mean that institutions rely on bespoke data sharing contracts. These contracts increase the inefficiency of data sharing and may disincentivize important clinical treatment and medical research. This paper provides a synthesis between two novel advanced privacy enhancing technologies (PETs): Homomorphic Encryption and Secure Multiparty Computation (defined together as Multiparty Homomorphic Encryption or MHE). These PETs provide a mathematical guarantee of privacy, with MHE providing a performance advantage over separately using HE or SMC. We argue MHE fulfills legal requirements for medical data sharing under the General Data Protection Regulation (GDPR) which has set a global benchmark for data protection. Specifically, the data processed and shared using MHE can be considered anonymized data. We explain how MHE can reduce the reliance on customized contractual measures between institutions. The proposed approach can accelerate the pace of medical research whilst offering additional incentives for healthcare and research institutes to employ common data interoperability standards.

Abstract:Digital health encompasses a wide range of novel digital technologies related to health and medicine. Such technologies rely on recent advances in the collection and analysis of ever increasing amounts of data from both patients and healthy citizens. Along with new opportunities, however, come new ethical and policy challenges. These range from the need to adapt current evidence-based standards, to issues of privacy, oversight, accountability and public trust as well as national and international data governance and management. This review illustrates key issues and challenges facing the rapidly unfolding digital health paradigm and reflects on the impact of big data in medical research and clinical practice both internationally and in Switzerland. It concludes by emphasising five conditions that will be crucial to fulfil in order to foster innovation and fair benefit sharing in digital health.

Abstract:While the meteoric rise of medical AI promises transformative solutions to global health challenges, ensuring equitable access to these life-saving technologies remains a formidable obstacle. Policymakers wrestle with a seemingly impossible feat: fostering the continued innovation pipeline while bridging the stark divide in accessibility. Existing patent-sharing mechanisms, from compulsory licensing to voluntary pledges, have proven demonstrably insufficient, failing to strike the delicate balance between incentivizing innovation and guaranteeing widespread availability. This paper proposes a bold departure from these one-size-fits-all solutions. Drawing upon the principles of distributive justice, I present a two-pronged solution that tackles the issue from both ends of the spectrum. On the macro level, it calls for international collaboration and reciprocity among states, urging them to fulfill their shared responsibility in making patented medical technologies universally and affordably accessible. On the micro level, it compels patent holders to embrace ethical responsibility, challenging them to confront the inherent inequities of intellectual property protection. This novel framework represents a paradigm shift, holding immense potential to unlock a future where medical advancements reach every corner of the globe. Unlike its predecessors, it recognizes the intricate dance between innovation and access, offering a practical roadmap for policymakers and patent holders. By embracing cooperation and ethical responsibility, we can pave the way for a healthier world where the promise of medical AI translates into tangible improvements for people everywhere, not just a privileged few. This is not simply an academic pursuit but a call to action, a challenge to rewrite the narrative of medical technology distribution and forge a future where health equity reigns supreme.

Abstract:Cybersecurity has seen an increasing frequency and impact of cyberattacks and exposure of Protected Health Information (PHI). The uptake of an Electronic Medical Record (EMR), the exponential adoption of Internet of Things (IoT) devices, and the impact of the COVID-19 pandemic has increased the threat surface presented for cyberattack by the healthcare sector. Within healthcare generally and, more specifically, within anaesthesia and Intensive Care, there has been an explosion in wired and wireless devices used daily in the care of almost every patient—the Internet of Medical Things (IoMT); ventilators, anaesthetic machines, infusion pumps, pacing devices, organ support and a plethora of monitoring modalities. All of these devices, once connected to a hospital network, present another opportunity for a malevolent party to access the hospital systems, either to gain PHI for financial, political or other gain or to attack the systems directly to cause erroneous monitoring, altered settings of any device and even to access the EMR via this IoMT window. This exponential increase in the IoMT and the increasing wireless connectivity of anaesthesia and ICU devices as well as implantable devices presents a real and present danger to patient safety. There has, at the same time, been a chronic underfunding of cybersecurity in healthcare. This lack of cybersecurity investment has left the sector exposed, and with the monetisation of PHI, the introduction of technically unsecure IoT devices for monitoring and direct patient care, the healthcare sector is presenting itself for further devastating cyberattacks or breaches of PHI. Coupled with the immense strain that the COVID-19 pandemic has placed on healthcare and the changes in working patterns of many caregivers, this has further amplified the exposure of the sector to cyberattacks.

The Medical Science DMZ

Abstract:Objective: We describe use cases and an institutional reference architecture for maintaining high-capacity, data-intensive network flows (e.g., 10, 40, 100 Gbps+) in a scientific, medical context while still adhering to security and privacy laws and regulations. Materials and methods: High-end networking, packet filter firewalls, network intrusion detection systems. Results: We describe a "Medical Science DMZ" concept as an option for secure, high-volume transport of large, sensitive data sets between research institutions over national research networks. Discussion: The exponentially increasing amounts of "omics" data, the rapid increase of high-quality imaging, and other rapidly growing clinical data sets have resulted in the rise of biomedical research "big data." The storage, analysis, and network resources required to process these data and integrate them into patient diagnoses and treatments have grown to scales that strain the capabilities of academic health centers. Some data are not generated locally and cannot be sustained locally, and shared data repositories such as those provided by the National Library of Medicine, the National Cancer Institute, and international partners such as the European Bioinformatics Institute are rapidly growing. The ability to store and compute using these data must therefore be addressed by a combination of local, national, and industry resources that exchange large data sets. Maintaining data-intensive flows that comply with HIPAA and other regulations presents a new challenge for biomedical research. Recognizing this, we describe a strategy that marries performance and security by borrowing from and redefining the concept of a "Science DMZ"-a framework that is used in physical sciences and engineering research to manage high-capacity data flows. Conclusion: By implementing a Medical Science DMZ architecture, biomedical researchers can leverage the scale provided by high-performance computer and cloud storage facilities and national high-speed research networks while preserving privacy and meeting regulatory requirements.
Sharing for Caring? A Patients' and Clinicians' View on Handling Personal Medical Data in the Context of Digitization: An Exploratory Study

Abstract:Background: The healthcare sector is currently undergoing a significant transformation, driven by an increased utilization of data. In this evolving landscape, surveys are of pivotal importance to the comprehension of patient needs and preferences. Moreover, the digital affinity of patients and physicians within the healthcare system is reforming the manner in which healthcare services are accessed and delivered. The utilization and donation of data are influencing the future of medical research and treatment, while artificial intelligence (AI) is empowering patients and physicians with knowledge and improving healthcare delivery. Methods: In order to evaluate the opinions of patients and physicians regarding the management of personal health data and the functionality of upcoming data management devices in the context of healthcare digitization, we conducted an exploratory study and designed a survey. The survey focused on a number of key areas, including demographics, experience with digitization, data handling, the identification of needs for upcoming digitization, and AI in healthcare. Results: A total of 40 patients and 15 physicians participated in the survey. The results indicate that data security, timesaving/administrative support, and digital communication are aspects that patients associate with patient-friendly digitization. Based on the responses provided by physicians, it might be concluded that future digital platforms should prioritize usability, time efficacy, data security, and interoperability. Conclusions: In terms of expectations for future digital platforms, there is a notable overlap between the needs expressed by patients and those identified by physicians, particularly in relation to usability, time management, data security, and digital communication. This suggests that the requirements of different stakeholders can be combined in a future system, although individual issues may still require attention.

Abstract:We found the article on "The Digital Technology in Clinical Medicine: From Calculators to ChatGPT" interesting.1 According to Kulkarni et al., humanity has witnesses four important social system changes, starting with the primitive huntersgatherers and progressing to horticultural, agricultural, industrial, and the current fifth, which is based on digital information technology and has altered the way we present, recognize, and utilize different factors of production. In clinical medicine, digital technology has advanced significantly since the days of computations. According to Kulkarni et al., we have to benefit from these advancements as we all improve the lives of our patients while being cautious not to overturn the doctor-patient relationship. If technology, clinical expertise, and humanistic values are properly balanced, Kulkarni et al. concluded that the future is quite glorious.1 Regulatory organizations are pushing for improvements through clinical trials as a result of recognition of the expanding influence of digital technology in healthcare delivery. The "World Health Organizations Guidelines for Digital Interventions" and the "Food and Drug Administration's Digital Health Center of Excellence" are only two of the projects that are currently being highlighted in the study as efforts to analyze and implement digital health services.

Abstract:The pace of scientific progress over the past several decades within the biological, drug development, and the digital realm has been remarkable. The’omics revolution has enabled a better understanding of the biological basis of disease, unlocking the possibility of new products such as gene and cell therapies which offer novel patient centric solutions. Innovative approaches to clinical trial designs promise greater efficiency, and in recent years, scientific collaborations, and consortia have been developing novel approaches to leverage new sources of evidence such as real-world data, patient experience data, and biomarker data. Alongside this there have been great strides in digital innovation. Cloud computing has become mainstream and the internet of things and blockchain technology have become a reality. These examples of transformation stand in sharp contrast to the current inefficient approach for regulatory submission, review, and approval of medicinal products. This process has not fundamentally changed since the beginning of medicine regulation in the late 1960s. Fortunately, progressive initiatives are emerging that will enrich and streamline regulatory decision making and deliver patient centric therapies, if they are successful in transforming the current transactional construct and harnessing scientific and technological advances. Such a radical transformation will not be simple for both regulatory authorities and company sponsors, nor will progress be linear. We examine the shortcomings of the current system with its entrenched and variable business processes, offer examples of progress as catalysts for change, and make the case for a new cloud based model. To optimize navigation toward this reality we identify implications and regulatory design questions which must be addressed. We conclude that a new model is possible and is slowly emerging through cumulative change initiatives that question, challenge, and redesign best practices, roles, and responsibilities, and that this must be combined with adaptation of behaviors and acquisition of new skills.
