Abstract:Objective To investigate the outcomes of radiotherapy for postoperative residual pilocytic astrocytoma. Methods Clinical data of 23 patients with postoperative residual pilocytic astrocytoma were analyzed retrospectively. The 23 patients were treated with three dimensional conformal radiotherapy(3D-CRT). Five patients were treated with X-knife(6-10 Gy) after 3D-CRT. The efficacy of radiotherapy was valued by MRI, and cognitive function assessed in children using Wechsler Intelligence Scale tests(WISC-CR).Results The median follow-up time was 44 months, ranging from 11 to 120 months. The Kaplan-Meier estimate of median progression-free survival(PFS) was 44 months. At 1 year, the MRI showed complete response in 5 patients, partial response in 2 and stable disease in 16. IQ mean of 8 school-age children who resumed their studies was 104.6 ± 13.4. Three patients had tumor recurrence28, 84 and 100 months respectively after initial therapy. Conclusions It is sensitive for pilocytic astrocytoma to radiotherapy. Adjuvant radiotherapy for postoperative residual pilocytic astrocytoma significantly improved local tumor control, prolonged PFS, but long-term outcomes needs further observation.

Abstract:Objective To discuss the clinical feature,diagnosis,and management of pseudoprogression after radiochemotherapy of high-grade glioma patients.Methods The clinical data of 31 high-grade glioma patients who underwent postoperative radiochemotherapy from June 2008 to June 2009 were reviewed.Pseudoprogression cases were analyzed.The treatment response was assessed through magnetic resonance imaging(MRI) according to the established response evaluation criteria in solid tumors.Results All the 31 high grade gioma patients received postoperative fractioned radiotherapy with concomitant TMZ chemotherapy,followed by TMZ maintenance chemotherapy.Four cases of pseudoprogression occurred after radiotherapy(14%).Conclusion Doctors should be careful in making early diagnosis for the suspected early progression after TMZ concomitant radiochemotherapy.It would be helpful for management to combine the clinical features of pseudoprogression with functional imaging technology.

Abstract:Radiation therapy is an important modality used in the treatment of patients with brain metastatic disease and malignant gliomas. Post-treatment surveillance often involves serial magnetic resonance imaging. A challenge faced by clinicians is in the diagnosis and management of a suspicious gadolinium-enhancing lesion found on imaging. The suspicious lesion may represent post-treatment radiation effects (PTRE) such as pseudoprogression, radiation necrosis or tumor recurrence. Significant progress has been made in diagnostic imaging modalities to assist in differentiating these entities. Surgical and medical interventions have also been developed to treat PTRE. In this review, we discuss the pathophysiology, clinical presentation, diagnostic imaging modalities and provide an algorithm for the management of pseudoprogression, radiation necrosis and tumor recurrence.

Abstract:Abstract BACKGROUND Post-gadolinium MRI is considered the mainstay of radiological monitoring of treatment effects in patients with a glioblastoma. However, the radiotherapy(RT)-induced abnormality, pseudoprogression, is difficult to distinguish from real tumor growth. Perfusion MRI (pMRI), including Arterial Spin Labeling (ASL) and Dynamic Susceptibility Contrast (DSC), may help in this differentiation. Evaluation of RT effects is usually discussed during multidisciplinary team meetings (MDTM), after which a decision regarding (dis)continuation of anti-tumor treatment is made. It is currently unknown whether pMRI impacts such treatment decisions. This study evaluated how accurate pMRI and the MDTM are in distinguishing tumor progression from pseudoprogression in glioblastoma. MATERIAL AND METHODS Population and outcome: Glioblastoma patients who received postoperative RT were retrospectively included from 2015 to 2020. ASL, DSC, FLAIR and post-gadolinium 3D-T1 at 3 months post-RT were visually scored for probability of radiological tumor progression. These radiological scores, as well as the outcome of MDTM, were compared to the final outcome (i.e. tumor progression or pseudoprogression) set at 9 months after RT initiation. Statistical analysis: Diagnostic accuracy of pMRI, conventional MRI and MDTM was assessed by 2x2 tables, sensitivity and specificity (including AUC values). In case the MDTM was inconclusive, subsequent analysis was used to determine whether pMRI has additional value. A multivariable logistic regression model was constructed to assess which clinical parameters (age, surgery type, IDH mutation status, MGMT methylation status, KPS score, chemotherapy and pMRI evaluation) were associated with the outcome of the MDTM assessment. RESULTS Sixty-five patients with a glioblastoma met the inclusion criteria (40 male, mean age 60+/-13 years, median KPS of 90). For all METHODS , the sensitivity for detecting true disease progression was poor to moderate (32-62%). Although sensitivity was higher when adding pMRI to conventional imaging (56% versus 32%), specificity was lower (80% versus 93%). AUC values were comparable (range 0.63-0.74), but highest for MDTM evaluation (0.74). For the 26% of patients where MDTM was inconclusive, pMRI shows increased sensitivity compared to conventional MRI in detecting tumor progression (respectively 36% versus 0%). The multivariable regression analysis showed significant associations for KPS score (OR = 0.84 (95% CI: 0.77-0.91)) and radiological assessment based on pMRI with MDTM outcome (OR = 0.09 (95% CI: 0.02-0.52)). CONCLUSION This study shows that perfusion MRI has added value for detection of tumor progression within the MDTM assessment in clinical decision-making for glioblastomas, especially when the MDTM outcome is uncertain. For this decision, the role of perfusion MRI and the KPS score are most important.

Abstract:Background: Juvenile pilocytic astrocytoma (JPA) is the most common primary brain tumor of childhood and is rarely seen in adults. Neurofibromatosis type 1 (NF1), a common tumor predisposition syndrome, demonstrates a strong association with low-grade gliomas, most notably pilocytic astrocytoma, which are relatively indolent. Unlike its juvenile counterpart, reports of adult pilocytic astrocytoma (APA) vary widely in terms of disease progression from benign to much more malignant courses. Moreover, current studies discussing APA report different treatment approaches and outcomes (e.g., malignant transformation of JPA and APA with or without radiation), as little is known regarding the management of recurrent tumors and how adjuvant therapies may alter disease progression. Observations: The authors report the unique case of an adult male with NF1 and APA who underwent rapid malignant conversion after intensity-modulated radiation therapy. Abstract:To study clinicopathologic features, treatment and prognosis of pilocytic astrocytoma (PA).Histopathological, ultrastructural, immunohistochemical (EnVision method) and clinical features of 68 cases of PA were studied by microscopic investigation with correlation of clinical follow-up information when available.Thirty-five male patients and 33 female patients were studied. The patient's age ranged from 3 to 66 years (mean = 20.1 years). The mean time from symptom onset to surgery was 371 days (range, 3 days to 14 years). Cystic degeneration was noted in 41 cases (60.3%), and enhancement of the tumor was noted in 43 cases (87.8%). On postcontrast imaging examination there were 33 cases involving the cerebellum (48.5%). Total tumor excision was performed in 35 patients, subtotal tumor excision was performed in 31 patients, and the procedures of other 2 patients were not clear. Among 51 patients with follow-up information, 44 were alive, 7 had recurrent tumor, and 7 died. The post-operative survival ranged from 2 months to 124 months (mean survival = 48.1 months). Five years and ten years survival rates were 89%, respectively. Tumors with classic histopathology demonstrated biphasic pattern of growth, consisting of compact elongated bipolar astrocytes associated with rosenthal fibers, and less cellular areas of multipolar cells with granular bodies and microcyst. Some cases showed atypia of nuclei, and occasional mitoses. Involvement of subarachnoid space was seen in 17 cases. One case had anaplastic features. All cases showed diffuse positive staining for GFAP and low expression for Ki-67, except 1 anaplastic tumor with 10% Ki-67 indices. Tumors with subarachnoid space involvement showed positive reticular fiber staining and negative EMA staining.PA is a benign, WHO grade I tumor with favorable prognosis, and does not require radiotherapy after total resection. The tumor can be mistaken as higher-grade astrocytoma when involving the subarachnoid space, and with cytological atypia, leading to unnecessary radiotherapy after surgery. Recurrence rate is increased when only partial resection is achieved. The outcome for patients with brainstem tumor or anaplastic PA is poor.

Abstract:Objective: The characteristics, imaging features, long-term surgical outcomes, and recurrence rates of primary spinal pilocytic astrocytomas (PAs) have not been clarified owing to their rarity and limited reports. Thus, this study aimed to analyze the clinical presentation, radiological features, pathological findings, and long-term outcomes of spinal PAs. Methods: Eighteen patients with spinal PAs who were surgically treated between 2009 and 2020 at 58 institutions were included in this retrospective multicenter study. Patient data, including demographics, radiographic features, treatment modalities, and long-term outcomes, were evaluated. Results: Among the 18 consecutive patients identified, 11 were women and 7 were men; the mean age at presentation was 31 years (3-73 years). Most PAs were located eccentrically, were solid or heterogeneous in appearance (cystic and solid), and had unclear margins. Gross total resection (GTR), subtotal resection (STR), partial resection (PR), and biopsy were performed in 28%, 33%, 33%, and 5% of cases, respectively. During a follow-up period of 65 ± 49 months, 4 patients developed a recurrence; however, the recurrence-free survival did not differ significantly between the GTR and non-GTR (STR, PR, and biopsy) groups. Conclusion: Primary spinal PAs are rare and present as eccentric and intermixed cystic and solid intramedullary cervical tumors. The imaging features of spinal PAs are nonspecific, and a definitive diagnosis requires pathological support. Surgical resection with prevention of neurological deterioration can serve as the first-line treatment; however, the resection rate does not affect recurrence-free survival. Investigation of relevant molecular biomarkers is required to elucidate the regrowth risk and prognostic factors.

Abstract:Purpose: Diffusion tensor imaging (DTI) can measure molecular mobility at the cellular level, quantified by the apparent diffusion coefficient (ADC). DTI may also reveal axonal fiber directional information in the white matter, quantified by the fractional anisotropy (FA). Juvenile pilocytic astrocytoma (JPA) is a rare brain tumor that occurs in children and young adults. Proton therapy (PT) is increasingly used in the treatment of pediatric brain tumors including JPA. However, the response of both tumors and normal tissues to PT is currently under investigation. We report tumor and normal brain tissue responses for a pediatric case of JPA treated with PT assessed using DTI. Methods: A ten year old male with JPA of the left thalamus received passive scattered PT to a dose of 50.4 Gy (RBE) in 28 fractions. Post PT, the patient has been followed up in seven years. At each follow up, MRI imaging including DTI was performed to assess response. MR images were registered to the treatment planning CT and the GTV mapped onto each MRI. The GTV contour was then mirrored to the right side of brain through the patient’s middle line to represent normal brain tissue. ADC and FA were measured within the ROIs. Results: Proton therapy can completely spare contra lateral brain while the target volume received full prescribed dose. From a series of MRI ADC images before and after PT at different follow ups, the enhancement corresponding to GTV had nearly disappeared more than 2 years after PT. Both ADC and FA demonstrate that contralateral normal brain tissue were not affect by PT and the tumor volume reverted to normal ADC and FA values. Conclusion: DTI allowed quantitative evaluation of tumor and normal brain tissue responses to PT. Further study in a larger cohort is warranted.

Abstract:The Neuroradiology Journal, Ahead of Print. Following completion of adjuvant radiation and chemotherapy imaging surveillance forms a major role in the management of diffuse gliomas. The primary role of imaging is to detect recurrences earlier than clinical symptomatology. Magnetic resonance imaging (MRI) is considered the gold standard in follow-up protocols owing to better soft tissue delineation and multiparametric nature. True recurrence can often mimic treatment-related changes, it is of paramount importance to differentiate between the two entities as the clinical course is divergent. Addition of functional sequences like perfusion, spectroscopy and metabolic imaging can provide further details into the microenvironment. In equivocal cases, a follow-up short interval imaging might be obtained to settle the diagnostic dilemma. Here, we present a patient with diagnosis of recurrent oligodendroglioma treated with adjuvant chemoradiation, presenting with seizures five years post-completion of chemotherapy for recurrence. On MRI, subtle new onset gyral thickening of the left frontal region with mild increase in perfusion and patchy areas of raised choline. FET-PET (fluoro-ethyltyrosine) showed an increased tumour-to-white matter (T/Wm) ratio favouring tumour recurrence. Based on discussion in a multi-disciplinary joint clinic, short interval follow-up MRI was undertaken at two months showing decrease in gyral thickening and resolution of enhancing areas in left frontal lobe. Repeat imaging one year later demonstrated stable disease status without further new imaging findings. Given the changes resolving completely without any anti-tumoral intervention, we conclude this to be peri-ictal pseudoprogression, being the second such case described in India.

Abstract:Objective To analyze the MR imaging features of atypical piloeytic astrocytoma.Methods The MRI imaging,clinical and pathological data of 22 patients with pilocytic astrocytoma,3 of them are pilomyxoid astrocytoma,were reviewed retrospectively.All cases were proved by surgery and pathological examinations.Results Clinic:Among all the cases,1 case was 2.5 years old,7 cases were more than 20 years old,and 1 case was 36,38 and 46 years old respectively.Sites Of 22 cases,1 case had multiple lesions;lesions of 7 cases were at cerebral hemisphere,5 cases at sellar region;4 cases at spinal cord.MRI:7 cases with atypical pilocytic astrocytoma performed as solid masses.MRI images showed iso-or long Tl and long T2 signalL and multiple microcysts could be foundL After contrast enhancement,the solid part showed uniform enhancement,while the cystic part showed non-enhancement;3 cases showed diffuse leisions,obscure boundary,heterogeneous long Tl and long T2 signal and non-enhancement.3 of 22 are WHO Ⅱ pilocytic astrocytoma.MRI showed solid lesions.1case had obscure boundary.1 case had diffuse lesions.2 cases showed peritumoral edema.After constrast enhancement,the mass showed non-enhancement or inhomogeneous enhancement.Pathology:The endoscopic findings of 3 cases with pilocytic astrocytoma showed oligodendrocyte,and arranged as honeycomb;The endoscopic findings of 3 cases with pilomyxoid astrocytoma showed formyxoid background and high cell density,and the nucleus were inequality of size,and most cellular showed obvious atypia.Conclusion Atypical piloeytic astrocytoma could occur in adult,has multiple lesions,locates in sellar region or medulla oblongata,and MRI could show solid mass.WHO Ⅱ pilocytic astrocytoma could show malignant features in MRI images and clinics.
Tumor pseudoprogression and true progression following gamma knife radiosurgery for recurrent ependymoma

Abstract:BACKGROUND: Gamma knife radiosurgery (GKRS) has become an effective salvage therapeutic option for recurrent ependymomas. However, its effectiveness can be assessed only by neuroimaging before clinical deterioration occurs. We analyzed the evolution of post-GKRS magnetic resonance imaging (MRI) features and sought to establish the feasibility of timely appropriate clinical management of the recurrent tumors.METHODS: We retrospectively investigated 19 recurrent ependymomas of 11 patients treated with GKRS in our hospital from 1994 to 2013. All included tumors had sequential MRI at 3-6-month intervals, and tumor response was volumetrically calculated on consecutive MRI.RESULTS: Post-GKRS tumors might show an increased enhancement or loss of enhancement associated with tumor enlargement or straight shrinkage. Seven of 19 tumors (37%) had continuously regressed or remained stable up to the last follow-up. Twelve of 19 tumors (63%) showed enlargement of enhancing lesions through examination of the post-GKRS follow-up MRI within the first 18 months. Five of 12 tumors (42%) showed continuous enlargement, which was interpreted as true progression; seven of 12 (58%) exhibited transient increasing enhanced volume that resolved within 6 months, and which was interpreted as pseudoprogression. There was no significant association between the presence of pseudoprogression and the pathological grades or locations of the tumors, and the concomitant chemotherapy or previous radiotherapy. Statistically significant differences were found for mean apparent diffusion coefficient (ADC) values and ADC ratio (prior to and after GKRS) of enhancing lesions between pseudoprogression and true progression.CONCLUSION: The MRI patterns of post-GKRS recurrent ependymomas are heterogeneous. Transient increased tumor volume may represent pseudoprogression, whose final tumor control rate was not significantly different from those cases with straight tumor shrinkage. ADC values, ADC ratio, and sequential follow-up MRI scans are beneficial to differentiate between pseudoprogression and true progression, and help guide clinical management.

Abstract:Pilocytic astrocytomas (PAs) are benign tumors. However, clinically aggressive PAs despite benign histology have been reported, and histological and molecular risk factors for prognosis have not been elucidated. 38 PAs were studied for clinical, histological, and molecular factors, including tumor location, extent of resection, post-operative treatment, glioma-associated molecules (IDH1/2, ATRX, BRAF, FGFR1, PIK3CA, H3F3A, p53, VEGF, Nestin, PD-1/PD-L1), CDKN2A/B deletion, and chromosomal number aberrations, to see if there is any correlation with patient's progression-free survival (PFS). Brainstem/spinal location, extent of resection and post-operative treatment, and VEGF-A, Nestin and PD-L1 expression, copy number gain of chromosome 7q or 19, TP53 mutation were significantly associated with shorter PFS. None of the histological parameters was associated with PFS. Multivariate analyses demonstrated that high Nestin expression, gain of 7q or 19, and extent of removal were independently predictive for early tumor recurrence. The brainstem/spinal PAs appeared distinct from those in the other sites in terms of molecular characteristics. Clinically aggressive PAs despite benign histology exhibited high Nestin expression. Brainstem/spinal location, extent of resection and some molecular factors including Nestin expression and gains of 7q and 19, rather than histological parameters, may be associated with early tumor recurrence in PAs.

Abstract:PurposeResponse evaluation in patients with glioblastoma after chemoradiotherapy is challenging due to progressive, contrast-enhancing lesions on MRI that do not reflect true tumour progression. In this study, we prospectively evaluated the ability of the PET tracer 18F-fluorothymidine (FLT), a tracer reflecting proliferative activity, to discriminate between true progression and pseudoprogression in newly diagnosed glioblastoma patients treated with chemoradiotherapy.MethodsFLT PET and MRI scans were performed before and 4 weeks after chemoradiotherapy. MRI scans were also performed after three cycles of adjuvant temozolomide. Pseudoprogression was defined as progressive disease on MRI after chemoradiotherapy with stabilisation or reduction of contrast-enhanced lesions after three cycles of temozolomide, and was compared with the disease course during long-term follow-up. Changes in maximum standardized uptake value (SUVmax) and tumour-to-normal uptake ratios were calculated for FLT and are presented as the mean SUVmax for multiple lesions.ResultsBetween 2009 and 2012, 30 patients were included. Of 24 evaluable patients, 7 showed pseudoprogression and 7 had true progression as defined by MRI response. FLT PET parameters did not significantly differ between patients with true progression and pseudoprogression defined by MRI. The correlation between change in SUVmax and survival (p = 0.059) almost reached the standard level of statistical significance. Lower baseline FLT PET uptake was significantly correlated with improved survival (p = 0.022).ConclusionBaseline FLT uptake appears to be predictive of overall survival. Furthermore, changes in SUVmax over time showed a tendency to be associated with improved survival. However, further studies are necessary to investigate the ability of FLT PET imaging to discriminate between true progression and pseudoprogression in patients with glioblastoma.

Abstract:Background: Glioblastoma is the most frequent malignant primitive brain tumor in adults. The treatment includes surgery, radiotherapy, and chemotherapy. During follow-up, combined chemoradiotherapy can induce treatment-related changes mimicking tumor progression on medical imaging, such as pseudoprogression (PsP). Differentiating PsP from true progression (TP) remains a challenge for radiologists and oncologists, who need to promptly start a second-line treatment in the case of TP. Advanced magnetic resonance imaging (MRI) techniques such as diffusion-weighted imaging, perfusion MRI, and proton magnetic resonance spectroscopic imaging are more efficient than conventional MRI in differentiating PsP from TP. None of these techniques are fully effective, but current advances in computer science and the advent of artificial intelligence are opening up new possibilities in the imaging field with radiomics (i.e., extraction of a large number of quantitative MRI features describing tumor density, texture, and geometry). These features are used to build predictive models for diagnosis, prognosis, and therapeutic response. Method: Out of 7350 records for MR spectroscopy, GBM, glioma, recurrence, diffusion, perfusion, pseudoprogression, radiomics, and advanced imaging, we screened 574 papers. A total of 228 were eligible, and we analyzed 72 of them, in order to establish the role of each imaging modality and the usefulness and limitations of radiomics analysis.

Fluorescein-guided surgery for the resection of pilocytic astrocytomas: A multicentric retrospective study

Abstract:Objective: Pilocytic astrocytomas (PAs) are relatively benign tumors, usually enhancing on post-contrast MRI and often characterized by a mural nodule within a cystic component. Surgical resection represents the mainstay of treatment, and extent of resection (EOR) is associated with improved survival. In this study, we analyzed the effect of sodium fluorescein (SF) on the visualization and resection of these circumscribed astrocytic gliomas. Methods: Surgical databases at two neurosurgical departments (Fondazione IRCCS Istituto Neurologico Carlo Besta, Milan, Italy and Department of Neurosurgery, University Medical Center Regensburg, Regensburg, Germany) were retrospectively reviewed to identify the cohort of patients with pilocytic astrocytoma who had undergone fluorescein-guided tumor resection at any of the centers between March 2016 and February 2022. SF was intravenously injected (5 mg/kg) immediately after the induction of general anesthesia. Tumors were removed using a microsurgical technique with the YELLOW 560 filter (Carl Zeiss Meditec, Oberkochen, Germany). Results: Forty-four patients (25 males and 19 females; 26 pediatric patients, mean age of 9.77 years, range 2 to 17 years; and 18 adult patients, mean age of 34.39 years, range 18 to 58 years) underwent fluorescein-guided surgery. No side effects related to SF occurred. In all tumors, contrast enhancement on preoperative MRI was correlated with intense, heterogeneous yellow fluorescence with bright fluorescent cystic fluid. Fluorescein was considered helpful in distinguishing tumors from viable tissue in all cases except three patients due to faint fluorescein enhancement. Biopsy was intended in two operations, and partial resection was intended in three operations. Gross total resection was achieved in 24 cases out of 39 patients scheduled for tumor removal (61.54%), in five cases a minimal residual volume was highlighted by postoperative MRI despite the intraoperative subjective evaluation of complete tumor removal (12.82%); in the other 10 cases, the resection was subtotal with fluorescent residual spots to avoid neurological worsening (25.64%). Conclusions: The use of SF is a valuable method for safe fluorescence-guided tumor resection. Our data showed a positive effect of fluorescein-guided surgery on intraoperative visualization during resection of Pas, suggesting a possible role in improving the extent of resection of these lesions.

Abstract:Objective:In order to improve the knowlegment of pilocytic astrocytoma,MRI findings correlated with pathologic changes of this tumor were investigated. Methods:The MRI findings of pilocytic astrocytoma were reviewed retrospectively. All cases were proved by operation and pathology.Results:The MRI characteristics of pilocytic astrocytoma were as follows:① The tumors appeared slight hyperintense,isointense,hypointense or mixed on T1 weighted images and hyperintense or mixed on T2 weighted images; After contrast injection,the lesions were usually enhanced as follows:cystic tumor with a nonenhancing cyst wall (only the mural nodule was enhanced); cystic tumor with an enhanced cyst wall (the mural nodule and the cyst wall were enhanced); cystic tumor with an enhanced cyst wall (only the cyst wall was enhanced); solid tumor(regular contrast enhancement); mainly solid tumor (most with irregular contrast enhancement);② Tumors were often mutiple microcysts. With hemorrhage,edema was seen in tumors usually,and vice versa;③ MRI study revealed a large lesion with cystic components and solid or cystic components and associated with hemorrhage.Conclusion:The MRI appearances of the pilocytic astrocytoma are characteristic. It is easily differentiated from other lesions by analysis of the correlation between ages and clinical feature.

Abstract:Objective To analyze clinical phenomenon of pseudoprogression(psPD) in glioma patients.Methods Ninety-three patients with pathologically diagnosed gliomas receiving chemotherapy,at Cancer Center of Sun Yat-sen University from January 2006 to July 2008,were reviewed.All patients underwent magnetic resonance imaging(MRI) scan for assessment of responses to the therapy and the responses to the therapy were assessed according to the established response evaluation criteria in solid tumours(RECIST).Results Fifteen cases(16.1%) of 93 cases were considered as psPD.among them,,10 occurred within 3 months and 5 cases occurred beyond 3 months after radiotherapy.Nine cases had the O6-methylguanine-DNA methyltransferase(MGMT)-postive tumors and 6 cases had MGMT-negative tumors.Conclusions The psPD is not uncommon in glioma patients receiving comprehensive therapy,and thus any alteration in current treatment on patients with suspected psPD,especially those undergoing chemotherapy,should be carefully considered.

Abstract:PurposeTo assess amide proton transfer-weighted (APTW) imaging features in patients with malignant gliomas after chemoradiation and the diagnostic performance of APT imaging for distinguishing true progression from pseudoprogression.Materials and MethodsAfter approval by the Institutional Review Board, 32 patients with clinically suspected tumor progression in the first 3 months after chemoradiation were enrolled and scanned at 3T. Longitudinal routine magnetic resonance imaging (MRI) changes and medical records were assessed to confirm true progression versus pseudoprogression. True progression was defined as lesions progressing on serial imaging over 6 months, and pseudoprogression was defined as lesions stabilizing or regressing without intervention. The APTW(mean) and APTW(max) signals were obtained from three to five regions of interests for each patient and compared between the true progression and pseudoprogression groups. The diagnostic performance was assessed with receiver operating characteristic curve analysis.ResultsThe true progression was associated with APTW hyperintensity (APTW(mean)=2.75%0.42%), while pseudoprogression was associated with APTW isointensity to mild hyperintensity (APTW(mean)=1.56%+/- 0.42%). The APTW signal intensities were significantly higher in the true progression group (n=20) than in the pseudoprogression group (P < 0.001; n=12). The cutoff APTW(mean) and APTW(max) intensity values to distinguish between true progression and pseudoprogression were 2.42% (with a sensitivity of 85.0% and a specificity of 100%) and 2.54% (with a sensitivity of 95.0% and a specificity of 91.7%), respectively.ConclusionThe APTW-MRI signal is a valuable imaging biomarker for distinguishing pseudoprogression from true progression in glioma patients.
