Abstract:AIM: To investigate the etiology and clinical characteristics of severe acute pancreatitis (SAP) in elderly patients (>= 60 years of age).METHODS: We reviewed retrospectively all the SAP cases treated in Xuanwu Hospital in Beijing between 2000 and 2007.RESULTS: In 169 patients with SAP, 94 were elderly and 16 died. Biliary and idiopathic etiologies were the first two causes that accounted for over 90% of SAP in the elderly. Biliary, hyperlipemic and alcoholic etiologies were the first three causes in the young. The proportion of comorbidity of cholelithiasis, biliary infection, hypertension and coronary heart disease in the aged was significantly higher than that in their young partners. The scores of APACHE II and Ranson were also significantly higher in the elderly except the CT score. Organ failures were more common in the elderly, but the local pancreatic complications were not different between the two groups. Mortality of the aged was correlated with the severity of SAP, multiple co-morbidity and incidence of multiple organ dysfunction syndrome (MODS). MODS was the main cause of death.CONCLUSION: The etiology of SAP in the elderly is quite different from that in the young. Biliary and unknown factors are main causes in the aged. The elderly are subject to major organ failures but there is no difference in the occurrence of local pancreatic complications between the elderly and the young. It is crucial to monitor and improve the functions of major organs so as to prevent MODS in the aged with SAP. (C) 2008 WJG. All rights reserved.
Elevated Serum Ferritin Levels Are Associated with Severity and Prognosis of Severe Acute Pancreatitis: a Preliminary Cohort Study

Abstract:Abstract Background Serum ferritin (SF), as an acute-phase response protein, is used to reflect the degree of oxidative stress and systemic inflammatory responses. This study was designed to assess the effect of elevated SF levels on the severity of acute pancreatitis (AP). Methods From January 2013 to December 2020, 200 consecutive patients with AP were retrospectively reviewed to analyze the relationships among the etiologies of pancreatitis, the severity of the disease and SF levels. The receiver operating characteristic (ROC) curve and logistic regression analysis were used to assess whether elevated SF levels could predict the onset of organ failure in AP. Results 92 (46%) had high SF levels (> 275 ng/ml). SF levels were not associated with the etiology of AP disease. Among patients with high SF levels, there was a significant increase in the proportion of patients with severe AP (23.1% vs. 76.9%) and a higher proportion of systemic inflammatory response scores (25.9% vs. 44.6%) in comparison to patients with normal SF levels. The area under the ROC curve for SF in predicting persistent organ failure was 0.812 [95% confidence interval 0.721–0.904]. Conclusions F concentrations were positively correlated with the severity of AP, and quantitative assessment of SF can predict disease severity and organ failure in patients with AP.

Abstract:Objective To analyze the risk factors of Pseudomonas aeruginosa (PAE) infection in severe acute pancreatitis (SAP) patients. Methods Totally 102 patients diagnosed with SAP from January 2006 to December 2016 in our hospital were retrospectively reviewed. And the patients were divided into infection group (17 cases) and control group (85 cases) according to whether or not they were complicated with PAE infection. Risk factors of PAE infection were analyzed through logistic regression analysis. Results The propor-tion of PAE infection in the SAP patients in our hospital was 16.67%. There was no significant difference in gender, etiology or complication between the two groups (P>0.05). However, compared with the control group, the infection group showed older age, higher CT severity index (CTSI) and SAP severity grading, higher inci-dences of ARDS and shock, higher APACHE Ⅱ score and longer ICU stay (P<0.05). Multivariate logistic regression analysis revealed that the risk factors of PAE infection included higher CTSI and APACHE Ⅱscore, higher SAP severity, ARDS occurrence in the acute phase, and prolonged ICU and hospital stay (P<0.05). In the aspect of clinical outcomes, the infection group showed higher incidences of shock and sepsis, longer ICU stay and total hospital stay than the control group (P<0.05); however, no significant difference was found in mortality between the two groups (P>0.05). Conclusions Risk factors of PAE infection include higher CTSI, higher SAP severity, occurrence of ARDS in the acute phase, prolonged ICU and hospital stay, and higher APACHE Ⅱ score at admission.
Prediction of the Severity of Acute Pancreatitis Using Machine Learning Models.

Abstract:BACKGROUND:Acute pancreatitis (AP) is the most common pancreatic disease. Predicting the severity of AP is critical for making preventive decisions. However, the performance of existing scoring systems in predicting AP severity was not satisfactory. The purpose of this study was to develop predictive models for the severity of AP using machine learning (ML) algorithms and explore the important predictors that affected the prediction results.METHODS:The data of 441 patients in the Department of Gastroenterology in our hospital were analyzed retrospectively. The demographic data, blood routine and blood biochemical indexes, and the CTSI score were collected to develop five different ML predictive models to predict the severity of AP. The performance of the models was evaluated by the area under the receiver operating characteristic curve (AUC). The important predictors were determined by ranking the feature importance of the predictive factors.RESULTS:Compared to other ML models, the extreme gradient boosting model (XGBoost) showed better performance in predicting severe AP, with an AUC of 0.906, an accuracy of 0.902, a sensitivity of 0.700, a specificity of 0.961, and a F1 score of 0.764. Further analysis showed that the CTSI score, ALB, LDH, and NEUT were the important predictors of the severity of AP.CONCLUSION:The results showed that the XGBoost algorithm can accurately predict the severity of AP, which can provide an assistance for the clinicians to identify severe AP at an early stage.
The Clinical Characteristics and Outcomes of Acute Pancreatitis Are Different in Elderly Patients: A Single-Center Study over a 6-Year Period

Abstract:Objectives: This study aims to analyze the clinical characteristics of elderly patients with acute pancreatitis (AP) and investigate the effects of age on the clinical outcomes of AP. Methods: Patients aged ≥ 18 years with AP admitted within 72 h from 1 September 2013 to 31 August 2019 were included. Patients were divided into elderly (≥60 years) and non-elderly (<60 years) groups. Clinical data and outcomes were compared. Results: A total of 756 elderly and 4896 non-elderly patients with AP were included. The elderly patients had different etiological distributions and more severe clinical markers and scores. Age was an independent risk factor for mortality [odds ratio (OR): 2.911, 95% CI: 1.801-4.706, p < 0.001], intensive care unit admission (OR: 1.739, 95% CI: 1.126-2.685, p = 0.013), persistent organ failure (OR: 1.623, 95% CI: 1.326-1.987, p < 0.001), multiple organ failure (OR: 1.757, 95% CI: 1.186-2.604, p = 0.005), and infection (OR: 2.451, 95% CI: 1.994-3.013, p < 0.001). Adjusted multiple logistic regression and trend analysis confirmed the risk of the age for the outcomes. The deaths of elderly patients showed a biphasic pattern with peaks in the first and fifth weeks, in contrast to the single peak in the first week in the non-elderly patients. Conclusions: Elderly patients with AP were associated with worse clinical outcomes. It is crucial to devote considerable attention to the optimization of therapeutic approaches to reduce late mortality in this group of patients.

Abstract:Objective To study the prognostic factors of severe acute pancreatitis.Methods We retrospectively analyzed clinical data of 76 patients with severe acute pancreatitis(Ranson score ≥3 points and APACHE Ⅱ score ≥8 points)who were treated in our hospital from January 1998 to March 2010.The patients who died in 60 days after admission was enrolled into death group(n=11),and the others were set as a survival group(n=65).The Ranson score,APACHE Ⅱ score,age,oxygenation index,serum creatine,glucose,calcium,base excess,and hematocrit were compared between the two groups.Results Death group showed significantly higher APACHE Ⅱ(16.57±3.10 vs 12.04±2.95,t=2.859,P=0.006)and Ranson scores(4.71±0.76 vs 3.87±0.73,t=2.859,P=0.006),serum creatine [(103.1±24.4)μmol/L vs(78.4±14.5)μmol/L,t=2.607,P=0.037] and glucose [(13.7±2.8)mmol/L vs(11.0±1.5)mmol/L,t=2.448,P=0.047] than the survival group,but significantly lower oxygenation index [(221±41)mm Hg vs(285±48)mm Hg,t=-3.353,P=0.002],serum calcium [(1.77±0.39)mmol/L vs(1.98±0.39)mmol/L,t=-2.187,P=0.033],base excess [(-1.33±2.93)mmol/L vs(0.70±2.23)mmol/L,t=-2.149,P=0.036],and hematocrit(0.35±0.04 vs 0.40±0.04,t=-2.957,P=0.013).Conclusions The Ranson score,APACHE Ⅱ score,oxygenation index,serum creatine,glucose,calcium,base excess,and hematocrit can be prognostic factors for patients with severe acute pancreatitis in early stage.
Risk Factors for Death in Patients with Severe Acute Pancreatitis in Guizhou Province, China

Abstract:Aim. To compare the clinical characteristics of survival and nonsurvival patients with severe acute pancreatitis (SAP) and explore the risk of mortality in SAP patients. Methods. This was a single-center retrospective study performed in a severe acute pancreatitis diagnosis and treatment center. According to the outcome, SAP patients were divided into survival group and nonsurvival group. One-way ANOVA or independent t -test was used to compare the clinical characteristics of two groups of patients. Multivariate retrospective analysis was used to identify risk factors for mortality in SAP patients. Results. A total of 486 SAP patients were included in the study, and the 90-day mortality for SAP patients was 13.58%. The common etiologies of SAP are biliary tract diseases (69.75%) and hyperlipidemia (17.28%). The most common complications caused by SAP were organ failure (55.14%), ARDS (50.62%), AKI (30.45%), sepsis (27.16%), and abdominal fluid collection (27.57%). There were differences in age, complications, and medical intervention between the nonsurvival group and the survival group. The main causes of death were infection (46.97%), abdominal bleeding (28.79%), and organ failure (9.09%). The binary logistic regression analysis showed that there were significant differences in age, AKI, sepsis, abdominal hemorrhage, organ failure, laparotomy, creatinine, and APTT between the nonsurvival group and the survival group. Conclusion. Age, AKI, sepsis, abdominal hemorrhage, and organ failure are risk factors for mortality in SAP patients. SAP patients with high creatinine and prolonged APTT upon admission require doctors to be vigilant. The main cause of death in SAP patients is pancreatitis-related organ failure and secondary infection.

Abstract:目的探索与重症急性胰腺炎死亡相关的危险因素. 方法回顾性分析自1997年 8月至2004年2月入院的全部重症急性胰腺炎(SAP)57例,其中男性32例,女性25例,中位年龄 51岁.根据结局的不同将SAP患者分为生存组和死亡组,分别对两组资料的一般特征、严重程度和手术相关因素进行对比分析. 结果 SAP共57例,死亡11例,病死率为19.3%,手术治疗 30例,死亡8例,病死率为26.7%.两组在年龄、Ranson评分、APACHEⅡ评分、腹水、多器官功能不全、ARDS、心功能衰竭、肾衰竭等发生率差异有统计学意义.多因素回归分析显示,年龄、腹水、多器官功能不全是和死亡相关的独立危险因素. 结论年龄、腹水、多器官功能不全是和死亡相关的独立危险因素。
Analysis of early risk factors associated with the outcome of severe acute pancreatitis

Abstract:目的探讨入院24 h内重症急性胰腺炎转归的危险因素.方法自2000年1月至2003年12月我院收治的重症急性胰腺炎60例,按预后分为死亡组21例和生存组39例,回顾性分析入院24h内临床和实验室检查资料,并行Logistic回归分析.结果死亡组年龄为(66.6±16.1)岁,入院时呼吸频率为(29±9)次/min,24 h尿量为(1062±815)ml,血肌酐为(190.6±120.1)μmo1/L,动脉血氧分压为(58.5±17.3)mm Hg,APACHEⅡ评分为19.3±7.5;而生存组分别为(52.4±13.1)岁,(20±2)次/min,(1648±575)ml,(94.4±56.0)μmol/L,(78.8±18.0)mm Hg和9.1±2.0,两组比较差异有统计学意义(P＜0.01),心率、血糖、血清白蛋白、血pH值、24 h内通便与否差异亦有统计学意义(P＜0.05),而平均动脉压、血尿淀粉酶、白细胞、红细胞压积、血小板、血脂、肝功能、电解质及男女比例无显著差异.经多因素Logistic回归分析发现年龄、动脉血氧分压、血清白蛋白与重症急性胰腺炎死亡相关.结论年龄、动脉血氧分压和血清白蛋白是重症急性胰腺炎死亡独立的危险因素。

Abstract:Purpose: There is a lack of adequate models specifically designed for elderly patients with severe acute pancreatitis (SAP) to predict the risk of death. This study aimed to develop a nomogram for predicting the overall survival of SAP in elderly patients. Methods: Elderly patients diagnosed with SAP between January 1, 2017 and December 31, 2022 were included in the study. Risk factors were identified through least absolute shrinkage and selection operator regression analysis. Subsequently, a novel nomogram model was developed using multivariable logistic regression analysis. The predictive performance of the nomogram was evaluated using metrics such as the receiver operating characteristic curve, calibration curve, and decision curve analysis (DCA). Results: A total of 326 patients were included in the analysis, with 260 in the survival group and 66 in the deceased group. Multivariate logistic regression indicated that age, respiratory rate, arterial pH, total bilirubin, and calcium were independent prognostic factors for the survival of SAP patients. The nomogram demonstrated a performance comparable to sequential organ failure assessment (P = 0.065). Additionally, the calibration curve showed satisfactory predictive accuracy, and the DCA highlighted the clinical application value of the nomogram. Conclusion: We have identified key demographic and laboratory parameters that are associated with the survival of elderly patients with SAP. These parameters have been utilized to create a precise and user-friendly nomogram, which could be an effective and valuable clinical tool for clinicians.
Analysis of risk factors for severe acute pancreatitis in the early period (<24 hours) after admission

Abstract:Background Severe acute pancreatitis (SAP) has high mortality.Early identification of high-risk factors that may progress to SAP and active intervention measures may improve the prognosis of SAP patients. Objective Clinical data within 24 hours after admission were retrospectively analyzed to provide a evidence for early screening of high-risk factors in patients with SAP. Methods A review of clinical data of acute pancreatitis patients from January 1, 2018, to December 31, 2022, was conducted. We compared the clinical data of SAP and non-SAP patients, and a multivariable logistic regression model was used to identify the independent predictors of SAP. The ROC curve of SAP was drawn for continuous numerical variables to calculate the optimal clinical cutoff value of each variable, and the predictive value of each variable was compared by the area under the ROC curve. Results Based on the multivariate logistic regression analysis of Age(OR,1.032;95% CI,1.018-1.046,P<0.001), BMI(OR,1.181;95% CI,1.083-1.288, P<0.001), Non-HTGAP(OR,2.098;95% CI,1.276-3.45,P=0.003),WBC(OR,1.072;95% CI,1.034-1.111,P<0.001),PCT(OR,1.060;95% CI,1.027-1.095,P<0.001), serum Ca(OR,0.121;95% CI,0.050-0.292,P<0.001),CTSI≥4(OR,12.942;95% CI,7.267-23.049, P<0.001) were identified as independent risk factors for SAP. The AUC and optimal CUT-OFF values of continuous numerical variables for predicting SAP were Age(0.6079,51.5), BMI(0.6,23.25), WBC(0.6701,14.565),PCT(0.7086, 0.5175),Ca(0.7787,1.965), respectively. Conclusion Age, BMI, non-HTGAP, WBC, PCT, serum Ca and CTSI≥4 have good predictive value for SAP.

Acute Cholecystitis in the Late Phase of Severe Acute Pancreatitis A Neglected Problem

Abstract:Objective: This study aimed to evaluate the incidence and risk factors for acute cholecystitis (AC) in the late phase of severe acute pancreatitis (SAP).Methods: A review of patients with SAP from January 2008 to December 2009 was performed. Clinical characteristics were compared between patients with AC in the late phase and those without. Risk factors for AC were analyzed using a logistic regression analysis. Receiver operating characteristic curve was used to estimate the predictive value of the risk factors.Results: A total of 269 cases were included. Twenty-seven episodes of AC in the late phase were identified. Patients with AC had higher computed tomography severity index and Acute Physiology and Chronic Health Evaluation (APACHE) II score, as well as higher rate of intra-abdominal hypertension, infective pancreatic necrosis (IPN) of the pancreas head, fistula, abdominal bleeding, mechanical ventilation, and prolonged enteral nutrition (EN) via jejunal tube. Independent risk factors for AC, based on the results of logistic regression analysis, included higher APACHE II score, prolonged EN via jejunal tube, and IPN of the pancreas head.Conclusions: Approximately 10% of patients with SAP will develop AC in the late phase. Risk factors include higher APACHE II score, prolonged EN via jejunal tube, and IPN of the pancreas head.
Analysis of Influencing Factors of Severity in Acute Pancreatitis Using Big Data Mining

Abstract:OBJECTIVES: To evaluate the epidemiological characteristics of acute pancreatitis (AP) and explore potential relationships between these factors and severity. METHODOLOGY: Data-sets of 5,659 patients with AP from health statistics and the Information Center of Jiangsu province, between 2014 and 2016, were analyzed. A self-organizing map (SOM) neural network was used for data clustering. RESULTS: Biliary acute pancreatitis (BAP) (86.7%) was the most frequent etiological factor. A total of 804 (14.2%) patients had severe acute pancreatitis (SAP). The mean age of patients was 53.7 +/- 17.3 (range 12 similar to 94y). Most of the AP patients were married (75.4%); 6% of mild/moderately severe AP (MAP/MASP) patients were unmarried, which was less than SAP patients (P=0.016). AP patients with blood type AB in the general population (8.8%) was significantly lower than that of AP cases (13.9%) (P=0.019) and SAP cases(18.7%) (P=0.007). The number of AP patients in southern Jiangsu was much higher than that in northern Jiangsu province, especially in Nanjing (1229, 21.7%). The proportion of acute alcoholic pancreatitis (AAP) in the north of Jiangsu (Xuzhou 18.4%) was much higher than that in southern Jiangsu (Suzhou 2.6%). The whole sample was divided into five classes by SOM neural network. If BAP patients were male, old, divorced, and blood type AB or B, they were more likely to develop SAP. Middle-age, unmarried or divorced male patients with blood type B/AB who suffered from HAP or AAP were also more likely to develop SAP. CONCLUSIONS: The number of unmarried patients with MAP/MASP was smaller than that of SAP. Blood types AB and B were more frequent in AP, especially in SAP. The differences between southern Jiangsu and northern Jiangsu, in number of AP patients and the proportion of AAP, were significant. In class I and class IV, the ratio of SAP was much higher than in other classes and the whole sample.

Abstract:OBJECTIVE To analyze the postoperative complications and mortality in patients with severe acute pancreatitis(SAP). METHODS From Jan 1990 to Dec 2002,142 cases of SAP have been treated surgically. RESULTS Fifty-five patients (38.7%) had complications including abdominal infection,lung infection,hydrothorax,pseudocyst,ARDS,digestive tract bleeding, MODS, gastrointestinal fistula,pancreatic encephalopathy, and fungal infection.Twenty-three patients(16.2%) died of ARDS, MODS,septic shock, digestive tract bleeding or pancreatic encephalopathy.The main relative factors of mortality were: infection,age over 60,operation time and operation method. CONCLUSIONS It is important for us to control infection and choose operation time and method correctly in patients with severe acute pancreatitis.

Abstract:OBJECTIVE:To identify potential risk factors associated with pancreatic infection in severe acute pancreatisis (SAP) patients, thus providing evidence for clinical prediction and treatment. METHODS:A total of 42 patients with SAP collected in our hospital from January 2013 to July 2014 were divided into two groups according to the presence or absence of pancreatic infection and retrospectively analyzed. Clinical characteristics and laboratory examine results of the two groups including age, sex, APACHE II score, serum amylase, serum calcium, blood glucose, ALT, AST, hyoxemia, serum albumin, serum creatinine and blood urea nitrogen were investigated for their relevance to pancreatic infection. RESULTS:The overall occurrence of secondary pancreatic infection of the 42 patients was 52.38%. A significantly positive correlation was revealed between the incidence rate of the secondary pancreatic infection and the factors including hyoxemia, blood creatinine and urea nitrogen in SAP patients (P < 0.05 or P < 0.01). Meanwhile, the level of serum albumin was negatively correlated with the rate of secondary infection in SAP patients (P < 0.01). The rest factors showed no significant correlation (P > 0.05). CONCLUSION:Hyoxemia, blood creatinine and urea nitrogen are potential factors leading to pancreatic infection in SAP patients, while an increase of serum albumin may reduce the incidence of infection.
Development and validation of three machine-learning models for predicting multiple organ failure in moderately severe and severe acute pancreatitis

Abstract:<h3 class="Heading">Background</h3><p class="Para">Multiple organ failure (MOF) is a serious complication of moderately severe (MASP) and severe acute pancreatitis (SAP). This study aimed to develop and assess three machine-learning models to predict MOF.</p><h3 class="Heading">Methods</h3><p class="Para">Patients with MSAP and SAP who were admitted from July 2014 to June 2017 were included. Firstly, parameters with significant differences between patients with MOF and without MOF were screened out by univariate analysis. Then, support vector machine (SVM), logistic regression analysis (LRA) and artificial neural networks (ANN) models were constructed based on these factors, and five-fold cross-validation was used to train each model.</p><h3 class="Heading">Results</h3><p class="Para">A total of 263 patients were enrolled. Univariate analysis screened out sixteen parameters referring to blood volume, inflammatory, coagulation and renal function to construct machine-learning models. The predictive efficiency of the optimal combinations of features by SVM, LRA, and ANN was almost equal (AUC = 0.840, 0.832, and 0.834, respectively), as well as the Acute Physiology and Chronic Health Evaluation II score (AUC = 0.814, <em class="EmphasisTypeItalic">P</em> > 0.05). The common important predictive factors were HCT, K-time, IL-6 and creatinine in three models.</p><h3 class="Heading">Conclusions</h3><p class="Para">Three machine-learning models can be efficient prognostic tools for predicting MOF in MSAP and SAP. ANN is recommended, which only needs four common parameters.</p>

Early infection in patients with severe acute pancreatitis

Abstract:Objective To investigate predisposing factors for early infection in patients with severe acute pancreatitis.Methods The clinical and laboratory data including age、gender、APACHE Ⅱscore on admission、hemodiastase、mechanical ventilation、blood calcium、mean arterial blood pressure、blood glucose、 alanine aminotransferase、aspartate aminotransferase、total bilirubin、necrosis of the pancreas、hypoxemia、 entero-functional disturbance、etiological factor、serum albumin、serum creatinine、urea nitrogen and haematocrit were analyzed by multiple linear regression in relation with the infection incidence in the 86 SAP patients hospitalized from Jan 2002 to Mar 2007.Results The fasting time、hiliary panereatitis、 hypoxemia、necrosis of pancreas、entero-functional disturbance、serum creatinine、urea nitrogen and haematocrit were positively correlated with the incidence of pancreatic infection(all P<0.05)and serum albumin was negatively correlated with the incidence of pancreatic infection(P<0.01). Conclusions Fasting time、biliary pancreatitis、hypoxemia、necrosis of pancreas、entero-functional disturbance、operation、serum creatinine、urea nitrogen and haematocrit may be the important factors in the pancreatic infection.
