[Robot-assisted Thoracoscopic Surgery for Mediastinal Tumor in Our Department]

Abstract:In 2019, our facility introduced robot-assisted surgery for mediastinal tumors using da Vinci Si, which was upgrade to da Vinci X and Xi in 2021. Initially, the unilateral thoracic cavity approach was used for robot-assisted surgeries. The unilateral approach has the advantage of facilitating the anatomical recognition of either the left or right side, with the side used selected depending on the location of the lesion. Notably, challenges such as difficulty in recognizing objects were still encountered. To compensate for these shortcomings, our hospital introduced robotic subxiphoid-optical thymectomy, a subxiphoid approach, which has been used for all robot-assisted mediastinal tumor surgeries since its introduction under the guidance of Kyoto Prefectural University of Medicine. Additionally, extended thymectomy, which has been covered by insurance since 2020 and used as the standard surgical procedure for robot-assisted mediastinal tumor surgery at our department, can be performed using the same approach.

Abstract:The robotic surgical system was designed to overcome the drawbacks of conventional endoscopic surgery. Since national health insurance in Japan began covering robotic-assisted thoracoscopic surgery (RATS) for malignant lung and mediastinal tumors in 2018, the number of RATS procedures being performed domestically has increased rapidly. This review evaluates the advantages and disadvantages of RATS for patients with lung cancers, based on an electronic literature search of PubMed. The main advantages of RATS are its ability to achieve excellent lymph-node removal with low morbidity and mortality, and minimal postoperative pain. Conversely, its disadvantages include a long operation time and the need for specialized instruments. However, the learning curve for RATS is reported to be shorter than that for VATS: some studies recommend that a surgeon needs to perform 18–22 robotic operations to attain sufficient skill. RATS for lung cancer is more expensive than VATS and the cost of training is high. Although the main disadvantage of RATS is that it reduces operator's tactile senses, the endoscope, which is directly manipulated by the surgeon at the console, using various magnifications, and 3D HD images on the monitor, may compensate for this. Ultimately, RATS offers better maneuverability, accuracy, and stability over VATS.

Abstract:Minimally invasive techniques have been widely applied in general thoracic surgery. Compared with video-assisted thoracoscopic surgery (VATS), due to its theoretic superiority, robotic surgery is challenging the traditional position of VATS. With its unique advantages, including 3D vision and a high-freedom endowrist, it leads to easier lymph node dissection, more convenient blood vessel dissection, a shorter learning curve and competence for the completion of complex surgery. However, as a new surgical technology, the safety and efficacy of robotic-assisted thoracoscopic surgery (RATS) still need to be further verified. Thus, in this article, we review and summarize the application of RATS versus VATS in general thoracic surgery.

Abstract:In the field of urology, robotic surgery has gained rapid and wide acceptance as a standard surgical approach in the majority of major surgeries over the last decade. To date, the da Vinci surgical system has been the dominant platform in robotic surgery; however, several newly developed robotic systems have recently been introduced in routine clinical practice. Of these, hinotori, the first made-in-Japan robotic system, is characterized by various unique and attractive features different from the existing system, and the use of this system has gradually increased mainly in urologic cancer surgeries, including radical prostatectomy, partial nephrectomy, radical nephrectomy, and radical nephroureterectomy. This review initially describes detailed characteristics of hinotori, then summarizes the early experience with urologic cancer surgeries using hinotori at our institution, and finally discusses the future prospects of robotic surgery using hinotori, considering problems associated with the use of this robotic system.

Abstract:In this paper, we focus on robots used for laparoscopic surgery, which is one of the most active areas for research and development of surgical robots. We introduce research and development of laparoscope-holder robots, master-slave robots and hand-held robotic forceps. Then, we discuss future directions for surgical robots. For robot hardware, snake like flexible mechanisms for single-port access surgery (SPA) and NOTES (Natural Orifice Transluminal Endoscopic Surgery) and applications of soft robotics are actively used. On the software side, research such as automation of surgical procedures using machine learning is one of the hot topics.

Abstract:Robotic liver resection has been reported in case series since the early 2000s. The surgical robot is capable of precise operation using articulated forceps with seven degrees of freedom. The robot also eliminates tremors and provides a good surgical field with highly detailed 3D high‐definition images. The clinical results demonstrating their usefulness have been increasing year by year. Initially, a shorter hospital stay was observed in comparison with open hepatectomy. Recent reports have also shown lower conversion and complication rates in comparison with laparoscopic hepatectomy. The clamp and crush method with bipolar forceps, sealing devices, ultrasonic shears, and the combined procedure of Cavitron ultrasonic surgical aspirator and robotic forceps as hybrid procedures have been reported as effective methods of parenchymal transection in robotic surgery. Theoretically, the advantages of the robotic platform allow for more complex liver resection around hilar structures and major blood vessels, as well as for vascular reconstruction or biliary reconstruction. The application of robotic liver surgery for hilar cholangiocarcinoma, living donor hepatectomy, and living donor liver transplantation has been reported. Robotic liver surgery is becoming more popular for certain indications; however, it is important to further evaluate its long‐term surgical and oncological outcomes and costs in comparison with conventional laparoscopic and open liver surgery, based on accumulated experience.

Abstract:In the 1990s, the introduction of thoracoscopy revolutionized minimally invasive thoracic surgery. At present, almost all thoracic surgeries (except lung transplantation) can be performed using thoracoscopy (video-assisted thoracoscopic surgery, VATS). However, VATS has limitations, including inconvenience of straight instruments (chopstick operation), two-dimensional surgical field, image instability, and unergonomic postures and angles for surgeons. In recent years, the emergence of robot-assisted thoracoscopic surgery has provided a solution to the limitations of conventional VATS. The robotic system can provide three-dimensional images, allowing surgical procedures to achieve unprecedented precision. In addition, the robotic instruments have a range of motion close to that of human hands and can move more than 360° in a very small space (EndoWrist technology). At present, robot-assisted thoracoscopic surgery has been widely performed in hospitals with robotic systems in China.

Abstract:Since the first successful gastric cancer surgery performed by Billroth 140 years ago, gastric cancer surgery has undergone major changes.<a class="bibLink tab-link" href="#ags312466-bib-0001">1</a> In the last 30 years, in particular, the main operative modality has shifted from open surgery to minimally invasive surgery; and now, robotic surgery is on the rise. Robotic surgery, with its multi-joint function, overcomes the disadvantages of motion limitation in laparoscopic surgery, prevents camera shake, and enables surgical precision with magnified stereoscopic effect. However, at this time, the superiority of robotic surgery over laparoscopic surgery is not fully clear in terms of short-term or long-term surgical outcomes. Practitioners anxiously await the findings of future randomized controlled trials.

Abstract:We used a high-speed 3-dimensional (3D) image analysis system (SYNAPSE VINCENT, Fujifilm Corp, Tokyo, Japan) to determine the best positioning of robotic arms and instruments preoperatively. The da Vinci S (Intuitive Surgical Inc, Sunnyvale, CA) was easily set up accurately and rapidly for this operation. Preoperative simulation and intraoperative navigation using the SYNAPSE VINCENT for robot-assisted thoracic operations enabled efficient planning of the operation settings. The SYNAPSE VINCENT can detect the tumor location and depict surrounding tissues quickly, accurately, and safely. This system is also excellent for navigational and educational use.

Abstract:: Unicompartmental knee arthroplasty (UKA) which has many potential advantages compared with total knee arthroplasty (TKA) was widely used across the world in recent years. The introduction of the robot systems greatly makes up for the defects of the conventional UKA surgery such as higher complication rates and revision rates. MAKO system, a new image-guided robot system relies on a preoperative computed tomography (CT) scan to assist in preoperative mapping and planning, offers an opportunity to improve the outcome of UKA surgeries. In order to have a more comprehensive and in-depth understanding of MAKO-assisted UKA, the studies on MAKO-assisted UKA were summarized. MAKO-assisted UKA is better than conventional UKA surgery on implant accuracy, soft tissue balance, patient function scores and satisfaction, complications rates, and learning curve in short-term, however, the mid-term and long-term outcomes of MAKO-assisted UKA need to be further studied. Abstract:Although the da VinciTM Surgical System is the most predominantly used surgical robot worldwide, other surgical robots are being developed. The Japanese surgical robot hinotoriTM Surgical Robot System was launched and approved for clinical use in Japan in November 2022. We performed the first robotic gastrectomy for gastric cancer using hinotori in the world. Here, we report our initial experience and evaluation of the feasibility and safety of robotic gastrectomy for gastric cancer using hinotori.

Abstract:Objectives To evaluate the safety and efficacy of the hinotoriTM Surgical Robot System in preclinical and clinical studies. Methods Preclinical studies on 10 living female porcine and four fresh male cadavers were conducted. Different types of urologic surgical procedures were performed by experienced urological surgeons using the hinotori system: partial nephrectomy, vesicourethral anastomosis, and pelvic lymph node dissection in porcine, and partial nephrectomy, radical prostatectomy, and pelvic lymph node dissection in cadavers. In a multi‐institutional single‐arm clinical study, radical prostatectomy was performed in 30 patients with localized prostate cancer to evaluate safety outcomes. The primary endpoint was safety profiles, including device errors and perioperative complications, and the secondary endpoints were console time, blood loss, and positive surgical margin rate. Results The preclinical results were similar to those reported for the daVinci surgical system. The performance of the hinotori was equivalent to that of the daVinci surgical system, based on a global assessment by the surgeons. Safety was confirmed in a first‐in‐human clinical study of robot‐assisted radical prostatectomy using hinotori in 30 patients. The procedures were completed as planned in all cases, and three (10%) perioperative adverse events were observed. Conclusions The safety of the newly developed hinotori surgical system was shown in the present preclinical and clinical studies. Further studies are required to confirm its clinical efficacy.

Abstract:The current status of robot‐assisted radical cystectomy was reviewed 16 years after the initial robot‐assisted radical cystectomy for the treatment of invasive bladder cancer. Articles associated with robot‐assisted radical cystectomy and written in English were selected from the PubMed database from January 2003 to February 2019. The present review article focused on the distribution of robot‐assisted radical cystectomy, patient selection, preoperative management, surgical technique, lymph node dissection, urinary diversion, recurrence pattern, oncological outcomes, cost, learning curve, complications and educational programs. A total of 400 articles were divided according to the country of the first author's affiliation. The USA was the most dominant at 198 (50%), whereas the number of articles from the countries belonging to the Urological Association of Asia was 15 (3.8%) for China, 17 (4.3%) for South Korea, 10 (2.5%) for Japan, eight (2%) for Taiwan, eight (2%) for Turkey and one (0.2%) for Iran. The percentage of robot‐assisted radical cystectomy carried out is increasing, and intracorporeal urinary diversion and ileal neobladder are also frequently carried out. With a refined technique being performed in high‐volume centers, robot‐assisted radical cystectomy has contributed to the reduction in transfusion rate, length of stay and severe complications; however, it has not yet shown any cancer‐specific survival benefits. Robot‐assisted radical cystectomy is not fully spread throughout the Urological Association of Asia. Further investigation with respect to worldwide results is needed to prove the real benefit of robot‐assisted radical cystectomy regarding low morbidity, reduced total medical cost, and survival benefit. In the era of precision medicine, appropriate drug and surgery will be given based on each genetic profile.
