Cognitive workload, complications and visual outcomes of phacoemulsification cataract surgery: Three-dimensional versus conventional microscope

Abstract:Purpose To compare the surgical workload, complications, and visual outcomes using the three-dimensional visualization system with the conventional microscope in phacoemulsification cataract surgery. Design Prospective, non-randomized, open-label interventional study. Methods All patients underwent phacoemulsification cataract surgery using the three-dimensional visualization system or conventional microscope. Results Of the 203 eyes, 80 underwent surgery with the three-dimensional system while 123 underwent with the conventional microscope. No difference was noted in the total surgical duration, complication rates, and visual outcomes between the two groups. However, capsulorhexis was significantly faster using the conventional microscope while posterior chamber intraocular lens insertion was quicker using the three-dimensional system. In terms of cognitive workload comparison, no difference was seen in the surgeons’ heart rate, oxygen saturation levels, and surgery task load index total workload score and workload score for all six dimensions of the questionnaire, between the three-dimensional system and conventional microscope groups. As compared to baseline, the heart rate increased significantly during all surgical steps and at the end in both groups. When compared to baseline, the oxygen saturation levels were significantly raised during capsulorhexis, irrigation, and aspiration and posterior chamber intraocular lens insertion and at the end of the surgery in the three-dimensional group and during incision and at the end of the surgery in the conventional microscope group. Conclusions The duration of surgery, complications, and visual acuity outcomes remain unaffected while performing phacoemulsification cataract surgeries with the three-dimensional viewing system when compared to the conventional microscopes. Moreover, the surgeons’ cognitive workload too remains unaffected while utilizing this revolutionary three-dimensional surgical technology.

Abstract:<h3>Background</h3><p>Virtual reality simulators combined with head-mounted displays enable highly immersive virtual reality (VR) for surgical skills training, potentially bridging the gap between the simulation environment and real-life operating room conditions. However, the increased complexity of the learning situation in immersive VR could potentially induce high cognitive load thereby inhibiting performance and learning. This study aims to compare cognitive load and performance in immersive VR and conventional VR simulation training.</p><h3>Methods</h3><p>A randomized controlled trial of residents (<em>n</em> = 31) performing laparoscopic salpingectomies with an ectopic pregnancy in either immersive VR or conventional VR simulation. Cognitive load was estimated by secondary-task reaction time at baseline, and during nonstressor and stressor phases of the procedure. Simulator metrics were used to evaluate performance.</p><h3>Results</h3><p>Cognitive load was increased by 66% and 58% during immersive VR and conventional VR simulation, respectively (<em>p</em> </p><h3>Conclusion</h3><p>Immersive VR simulation training induces a higher cognitive load and results in a poorer performance than conventional VR simulation training in laparoscopy. High extraneous load and element interactivity in the immersive VR are suggested as mechanisms explaining this finding. However, immersive VR offers some potential advantages over conventional VR such as more real-life conditions but we only recommend introducing immersive VR in surgical skills training after initial training in conventional VR.</p>
Three-Dimensional Versus Two-Dimensional Laparoscopy in Laparoscopic Liver Resection: A Systematic Review and Meta-Analysis

Abstract:Background: Three-dimensional (3D) laparoscopy provides stereopsis and may reduce operating time (OT) and morbidity. However, there is a paucity of literature on its use in laparoscopic liver resection (LLR). This study aims to compare outcomes between 3D and two-dimensional (2D) LLR. Materials and Methods: PubMed, Embase, Scopus, and the Cochrane Library were systematically searched from inception to November 2022. The inclusion criterion was studies comparing intraoperative characteristics and/or postoperative outcomes between 3D and 2D LLR. Studies on the use of 3D image reconstruction techniques for preoperative planning were excluded. Primary outcomes were OT, estimated blood loss (EBL), and overall morbidity. Secondary outcomes were other postoperative complications, need for reoperation, and in-hospital mortality. Results: Four studies with 361 patients (3D: n = 159, 2D: n = 202) were included. There were 65.3% males (overall: n = 236/361). Age, sex, body-mass index, incidence of diabetes mellitus, hepatitis B and/or C carrier, receipt of neoadjuvant chemotherapy, tumor size, and incidence of multiple tumors were comparable between 3D and 2D LLR. No studies reported on Child-Pugh status. One study included only patients with hepatocellular carcinoma, two studies included patients with mixed histopathology, and one study did not report on histopathology. There was no significant difference in OT (mean difference [MD] -31.6 minutes, 95% confidence interval [CI]: -89.7 to 26.5), EBL (MD -454.1 mL, 95% CI: -978.8 to 70.6), need for reoperation (odds ratio [OR] 0.91, 95% CI: 0.18-4.61), and in-hospital mortality (OR 0.52, 95% CI: 0.06-5.50) between 3D and 2D LLR. Overall morbidity was lower in 3D LLR (OR 0.56, 95% CI: 0.32-0.98, P = .04). However, the learning curve (LC) was not described in the included studies and may confound outcomes. Conclusions: 3D LLR may reduce overall postoperative morbidity compared with 2D LLR, but results may be confounded by the lack of standardization of surgeons' experience and the LC of 3D LLR.

Abstract:With advances in technology in surgery, it is important to determine if they are gimmicks or if it has any clinical value. This paper explores the impact of 3D visualisation systems on difficult laparoscopic cholecystectomy. It was determined that the critical step in achieving critical view of safety was completed faster and more efficiently with 3D systems when compared to traditional 2D. Background 3D visualization systems in laparoscopic surgery have been proposed to improve manual task handling compared to 2D, however, few studies have compared the intra‐operative efficacy in laparoscopic cholecystectomy (LC). The aim of this study is to determine if there is a benefit in intra‐operative efficiency when using a 3D visualization system in difficult LC compared to traditional 2D visualization systems. Methods Retrospective analysis of 'difficult' LCs (Grades 3 or 4) was completed. The assessor was blinded as all cases were recorded and viewed in 2D only. Variables collected included time to complete steps, missed hook diathermy attempts, failed grasp attempts, missed clip attempts and preparation steps for intra‐operative cholangiogram (IOC). Multiple linear regression was undertaken for time variables, Poisson regression or negative binomial regression was completed for continuous variables. Results Fifty‐two operative videos of 'difficult' LC were reviewed. 3D systems were associated with reduced operative times, although this was not statistically significant (CI: −2.93–14.93, P‐value = 0.183). Dissection of the anterior fold to achieve the critical view of safety was significantly faster by 3.55 min (CI: 1.215–9.206, P‐value = 0.002), and with considerably fewer errors when using 3D systems. Fewer IOC preparation errors were observed with a 3D system compared with a 2D system. Conclusions 3D systems appear to enhance operator efficiency, allowing faster completion of critical steps with fewer errors. This pilot study underscores the utility of video annotation for intra‐operative assessment and suggests that, in larger multi‐centre studies, 3D systems may demonstrate superior intra‐operative efficiency over 2D systems during a 'difficult' LC.

Abstract:Background/Aim: Three-dimensional (3D) laparoscopy is being steadily adopted instead of two-dimensional (2D) for various procedures. Our aim was to compare the outcomes between 2D and 3D laparoscopic procedures for colorectal cancer in order to ascertain the safety, efficacy and potential advantages of 3D imaging systems. Materials and Methods: A systematic database search was conducted in March 2019. Comparative studies reporting clinical outcomes between patients undergoing elective colorectal procedures using either 2D or 3D laparoscopic equipment were eligible. Results: Six studies were selected, including 614 patients in total. Minor reduction in operative time, similar blood loss and increased number of harvested lymph nodes was noted for the 3D group. There was no difference for conversion to open surgery, time to flatus, postoperative hospital stay or postoperative complications. Conclusion: 3D Laparoscopic surgery for colorectal cancer may result in reduction of operative time and higher lymph node yields, leading to improved survival.

Abstract:Introduction: Cognitive Load Theory (CLT) relates to the efficiency with which individuals manipulate the limited capacity of working memory load. Repeated training generally results in individual performance increase and cognitive load decrease, as measured by both behavioral and neuroimaging methods. One of the known biomarkers for cognitive load is frontal theta band, measured by an EEG. Simulation-based training is an effective tool for acquiring practical skills, specifically to train new surgeons in a controlled and hazard-free environment. Measuring the cognitive load of young surgeons undergoing such training can help to determine whether they are ready to take part in a real surgery. In this study, we measured the performance of medical students and interns in a surgery simulator, while their brain activity was monitored by a single-channel EEG. Methods: A total of 38 medical students and interns were divided into three groups and underwent three experiments examining their behavioral performances. The participants were performing a task while being monitored by the Simbionix LAP MENTOR™. Their brain activity was simultaneously measured using a single-channel EEG with novel signal processing (Aurora by Neurosteer®). Each experiment included three trials of a simulator task performed with laparoscopic hands. The time retention between the tasks was different in each experiment, in order to examine changes in performance and cognitive load biomarkers that occurred during the task or as a result of nighttime sleep consolidation. Results: The participants' behavioral performance improved with trial repetition in all three experiments. In Experiments 1 and 2, delta band and the novel VC9 biomarker (previously shown to correlate with cognitive load) exhibited a significant decrease in activity with trial repetition. Additionally, delta, VC9, and, to some extent, theta activity decreased with better individual performance. Discussion: In correspondence with previous research, EEG markers delta, VC9, and theta (partially) decreased with lower cognitive load and higher performance; the novel biomarker, VC9, showed higher sensitivity to lower cognitive load levels. Together, these measurements may be used for the neuroimaging assessment of cognitive load while performing simulator laparoscopic tasks. This can potentially be expanded to evaluate the efficacy of different medical simulations to provide more efficient training to medical staff and measure cognitive and mental loads in real laparoscopic surgeries.
Laparoscopy training of novices with complex curved instruments using 2D- and 3D-visualization

Abstract:Abstract Purpose Beside many advantages, disadvantages such as reduced degrees of freedom and poorer depth perception are still apparent in laparoscopic surgery. 3D visualization and the development of complex instruments are intended to counteract the disadvantages. We want to find out whether the use of complex instruments and 3D visualization has an influence on the performance of novices. Methods 48 medical students with no experience in laparoscopic surgery or simulator-based laparoscopy training were included. They were randomized in four groups according to a stratification assessment. During a structured training period they completed the FLS-Tasks “PEG Transfer”, “Pattern Cut” and “Intracorporeal Suture” and a transfer task based on these three. Two groups used conventional laparoscopic instruments with 3D or 2D visualization, two groups used complex curved instruments. The groups were compared in terms of their performance. Results In 2D laparoscopy there was a better performance with straight instruments vs. curved instruments in PEG Transfer and Intracorporeal Suture. In the transfer task, fewer errors were made with straight instruments. In 2D vs. 3D laparoscopy when using complex curved instruments there was an advantage in Intracorporeal Suture and PEG Transfer for 3D visualization. Regarding the transfer exercise, a better performance was observed and fewer errors were made in 3D group. Conclusion We could show that learning laparoscopic techniques with complex curved instruments is more difficult with standard 2D visualization and can be overcome using 3D optics. The use of curved instruments under 3D vision seems to be advantageous when working on more difficult tasks.

Abstract:Background and Aims Published studies repeatedly demonstrate an advantage of three-dimensional (3D) laparoscopic surgery over two-dimensional (2D) systems but with quite heterogeneous results. This raises the question whether clinics must replace 2D technologies to ensure effective training of future surgeons. Methods We recruited 45 students with no experience in laparoscopic surgery and comparable characteristics in terms of vision and frequency of video game usage. The students were randomly allocated to 3D ( n = 23) or 2D ( n = 22) groups and performed 10 runs of a laparoscopic “peg transfer” task in the Luebeck Toolbox. A repeated-measures ANOVA for operation times and a generalized linear mixed model for error rates were calculated. The main effects of laparoscopic condition and run, as well as the interaction term between the two, were examined. Results No statistically significant differences in operation times and error rates were observed between 2D and 3D groups ( p = 0.10 and p = 0.72, respectively). The learning curve showed a significant reduction in operation time and error rates (both p 's < 0.001). No significant interactions between group and run were detected (operation time: p = 0.342, error rates: p = 0.83). With respect to both endpoints studied, the learning curves reached their plateau at the 7th run. Conclusion The result of our study with laparoscopic novices revealed no significant difference between 2D and 3D technology with respect to performance time and the error rate in a simple standardized test. In the future, surgeons may thus still be trained in both techniques.

Assessing the Efficacy of Enhanced Versus Standard Three-Dimensional Training Models in Laparoscopic Skills Acquisition: A Randomized Controlled Trial of Novice Medical Students

Abstract:Objective: The aim of the study was to determine if laparoscopic skills acquisition will be superior with the use of 3D-enhanced training models with increased dimensionality and spatial complexity compared to standard 3D LapPass training models in novice medical students. We hypothesized that adopting low-fidelity, 3D-enhanced training models in the 2D visual modality may expedite adaptation to monocular cues related to depth perception, enhancing performance. Design: A prospective, single-center, 2-arm, randomized controlled trial with participants randomized equally into either 3D-enhanced or standard 3D group (allocation ratio 1:1, block sizes 2 and 4) performing 2 standardized laparoscopic tasks: (1) polo grasping and manipulation and (2) intracorporeal suturing. Performance was assessed using the Global Operative Assessment of Laparoscopic Skills (GOALS) tool, completion time, and number of errors, with evaluations at baseline, interim, and post-training for each task. A post-study survey measured participant-perceived confidence and performance (ID: NCT06184854). Setting: Barts Cancer Institute, Queen Mary University of London, UK. Participants: Sixty-five medical students, all novices in laparoscopic surgery, were enrolled, with 3 dropouts. Results: Thirty-one participants in each group completed the study with homogeneity regarding demographic data (p > 0.05). Both groups had significantly superior performance post-training compared to baseline in the 2 tasks (intragroup comparison, p < 0.017). In the intergroup comparison, the 3D-enhanced group had statistically significantly greater improvements in the GOALS score parameter and depth perception domain across all comparisons for both tasks (p < 0.05). Time improvement and error reduction also favored the 3D-enhanced group, however, without reaching statistical significance. In the post-study survey, the 3D-enhanced group reported significantly greater perceived post-training confidence and performance in the suturing task alone (p < 0.05). Conclusions: The use of 3D-enhanced training models resulted in superior laparoscopic skills acquisition for novices in 2 tasks compared to standard 3D LapPass models. Therefore, incorporating 3D-enhanced models into laparoscopic training curricula alongside standard 3D LapPass models can supplement learning. Further work should focus on long-term efficacy with a competency-based endpoint.
Comparison of three-dimensional versus two-dimensional laparoscopic surgery for rectal cancer: a meta-analysis

Abstract:Purpose: Three-dimensional (3D) vision technology has recently been validated for the improvement of surgical skills in a simulated setting. This study assessed the current evidence regarding the efficiency and potential advantages of 3D compared with two-dimensional (2D) laparoscopic rectal surgery for rectal cancer. Methods: We comprehensively searched PubMed, EMBASE and the Cochrane Library and performed a systematic review and cumulative meta-analysis of all randomized controlled trials (RCTs) and non-randomized controlled trials (nRCTs) assessing the two approaches. Results: Four trials including a total 331 cases were identified. The positive circumferential resection margins (CRMs) were significantly lower for the 3D group (P = 0.02). The operative time was significantly shorter in the 3D group than in the 2D group (P < 0.00001). There was less estimated blood loss (EBL) in the 3D group than in the 2D group (P = 0.02). Perioperative complication rates, conversion rate, harvested lymph nodes, first flatus, length of stay, pneumonia, wound infection, ileus, anastomotic fistula and urinary retention did not differ significantly between the two groups (P > 0.05). Conclusions: In summary, 3D laparoscopic rectal surgery appears to have advantages over 2D laparoscopic rectal surgery in terms of positive CRM and operation time; however, it is not better than 2D laparoscopic rectal surgery in terms of the conversion rate and postoperative complications.
Randomized controlled study:comparative study on clinical efficacy between 3D and 2D laparoscopic radical colorectaomies in the cases of colorectal cancer

Abstract:Objective To investigate the operative technique,postopertaive recovery,feasibility and safety of the 3D laparoscopy system in colorectal surgery.Methods The clinical and follow-up data of 74 patients with colorectal carcinoma who underwent laparoscopic radical resection in Shanghai Minimally Invasive Surgery Center between November 2013 and June 2015 were analyzed prospectively.According to random number table,74 patients were divided into the following groups:2D laparoscopy system group and 3D laparoscopy system group.In the 38 cases of 2D laparoscopy system group (20 males,and 18 females),the average age was 60.5 years old (range 46-83 years);In the 36 cases of 3D laparoscopy system group (16 males,and 20 females),the average age was 62 years old (range 3784 years).In the 2D laparoscopy system group,12 cases were given laparoscopic right hemicolectomies,4 cases given laparoscopic left hemicolectomies,1 case given laparoscopic transverse colectomy,2 cases given laparoscopic sigmoidectomies,17 cases given laparoscopic radical resections of rectal carcinoma (3 cases subject to ileostomies) and 2 cases given laparoscopic abdominoperineal radical resections of the rectum.In the 3D laparoscopy system group,15 cases were subjected to laparoscopic right hemicolectomies,3 cases to laparoscopic left hemicolectomies,1 case to laparoscopic transverse colectomy,3 cases to laparoscopic sigmoidectomies,10 cases to laparoscopic radical resections of rectal carcinoma (5 cases to ileostomies) and 4 cases to laparoscopic abdominoperineal radical resections of the rectum.Results There was no conversion to open surgery,intraopertaive complications or operation related deaths.In the 2D laparoscopy system group,the mean operation time was (153.3 ± 54.9) min,and the mean blood loss was (117.9 ± 102.5) mL.The mean flatus time was (2.3 ± 0.9) days,and the mean hospital stay was 9.6 ± 7.8 days.In the 3D laparoscopy system group,the mean operation time was (152.9 ± 48.3) min (P =0.973),and the mean blood loss was (100.3 ± 66.3) mL (P =0.386).The mean flatus time was (2.1 ± 0.8) days (P =0.228),and the mean hospital stay was (8.1 ± 3.2)days (P =0.289).In the 2D laparoscopy system group,anastomotic leakage occurred in 2 cases in the group of laparoscopic right hemicolectomy and laparoscopic radical resections of rectal carcinoma.In the 3D laparoscopy system group,wound infection occurred in one case in the group of laparoscopic abdominoperineal radical resections of the rectum.Conclusions We concluded that the 3D laparoscopy system was similar to the traditional 2D laparoscopy system in surgical procedures and clinical effects.More studies with larger samples are required to evaluate the impact of 3D laparoscopy system.

Abstract:Abstract Laparoscopic surgery can be exhausting and frustrating, and the cognitive load experienced by surgeons may have a major impact on patient safety as well as healthcare economics. As cognitive load decreases with increasing proficiency, its robust assessment through physiological data can help to develop more effective training and certification procedures in this area. We measured data from 31 novices during laparoscopic exercises to extract features based on cardiac and ocular variables. These were compared with traditional behavioural and subjective measures in a dual-task setting. We found significant correlations between the features and the traditional measures. The subjective task difficulty, reaction time, and completion time were well predicted by the physiology features. Reaction times to randomly timed auditory stimuli were correlated with the mean of the heart rate ( $$r = - 0.29$$ r = - 0.29 ) and heart rate variability ( $$r = 0.4$$ r = 0.4 ). Completion times were correlated with the physiologically predicted values with a correlation coefficient of 0.84. We found that the multi-modal set of physiology features was a better predictor than any individual feature and artificial neural networks performed better than linear regression. The physiological correlates studied in this paper, translated into technological products, could help develop standardised and more easily regulated frameworks for training and certification.

Testing Distinct Three-Dimensional Effects in Laparoscopy: A Prospective Randomized Trial Using the Lübecker Toolbox Curriculum

Abstract:<span><b><i>Background:</i></b> The use of stereoscopic laparoscopic systems in minimally invasive surgery (MIS) allows a three-dimensional (3D) view of the surgical field, which improves hand-eye coordination. Depending on the stereo base used in the construction of the endoscopes, 3D systems may differ regarding the 3D effect. Our aim was to investigate the influence of different stereo bases on the 3D effect. <b><i>Methods:</i></b> This was a prospective randomized study involving 42 MIS-inexperienced study participants. We evaluated two laparoscopic 3D systems with stereo bases of 2.5 mm (system A) and 3.8 mm (system B) for differences in learning MIS skills using the Lübeck Toolbox (LTB) video box trainer. We evaluated participants' performance regarding the times and repetitions required to reach each exercise's goal. After completing the final exercise ("suturing"), participants performed the exercise again using a two-dimensional (2D) representation. Additionally, we retrospectively compared our study results with a preliminary study from participants completing the LTB curriculum with a 2D system. <b><i>Results:</i></b> The median number of repetitions until reaching the goals for LTB exercises 1, 2, 3, and 6 for system A were: 18 (range 7–53), 24 (range 8–46), 24 (range 13–51), and 21 (range 10–46), respectively, and for system B were: 12 (range 2–30), 16 (range 6–43), 17 (range 4–47), and 15 (range 6–29), respectively (<i>p</i> = not significant). Changing from a 3D to a 2D representation after completing the learning curve led to a longer average time required, from 95.22 to 119.3 s (<i>p</i> #x3c; 0.0001), for the last exercise (exercise 6; "suturing"). When comparing the results retrospectively with the learning curves acquired with the 2D system, there was a significant reduction in the number of repetitions required to reach the LTB exercise goals for exercises 1, 3, and 6 using the 3D system. <b><i>Conclusion:</i></b> Stereo bases of 2.5 and 3.8 mm provide acceptable bases for designing 3D systems. Additionally, our results indicated that MIS basic skills can be learned quicker using a 3D system versus a 2D system, and that when the 3D effect is eliminated, the corresponding compensatory mechanisms must be relearned.<br/>Visc Med</span>

Abstract:Abstract Introduction Surgeons strive to continually enhance performance. Currently, assessment relies on subjective, retrospective methods-hindering real-time improvement and error prevention. Learning from sports, surgeons can utilise surgical sabermetrics for data-driven analysis. Cognitive load(CogL) impacts performance, with overload impairing skills and judgement. Sabermetrics can be leveraged to measure CogL objectively(e.g.Electrodermal activity(EDA)) and subjectively. This study aimed to combine these measures and explore their relationship with surgeon factors and patient outcomes. Methods Fourteen surgeons conducting 58 benign surgery cases participated. Surgeons’ intraoperative EDA was captured using a digital sensor. Subjective data collected included (i)Surgery Task-Load-Index(SURG-TLX)), (ii)mood, (iii)sleep-disturbance and (iv)risk-aversion. EDA underwent decomposition into (a)tonic (background autonomic arousal) and (b) phasic(response to events) components. Patient outcomes included (i)30-day morbidity and (ii)case difficulty. Results Overall, mean phasic-EDA was 0.038μS and mean tonic-EDA was 0.483μS(ranges 0-1:1 indicating highest CogL). Mean SURG-TLX was 47.128(/120, where 120 is the highest workload). Negative mood related to changes in tonic-EDA(p<0.05,F=11.84). EDA climbed throughout each case (Tonic:p<0.05,F=2.035;Phasic:p<0.05,F=1.626). SURG-TLX were weakly correlated with sleep(p<0.05,ρ=-0.273) and moderately correlated with case duration(p<0.05, ρ=0.598), but not EDA(p>0.50). Discussion This study is the first in the UK to report combined CogL measures gathered during live surgery. Awareness of factors that affect CogL crucially allows for the management of factors to prevent overload and to inform targeted interventions for performance improvement. Using digital sensors to measure CogL provides opportunity for automated, real-time performance assessment, advanced feedback, and behavioural modifications.

Abstract:Simulation-based training is an effective tool for acquiring practical skills, specifically to train new surgeons in a controlled and hazard-free environment, it is however important to measure participants cognitive load to decide whether they are ready to go into a real surgery. In the present study we measured performance on a surgery simulator of medical students and interns, while their brain activity was monitored by a mobile EEG device. 38 medical studentswere underwent 3 experiments undergoing a task with Simbionix simulator, while their brain activity was measured using a single-channel EEG device (Aurora by Neurosteer). On each experiment, participants performed 3 repeats of a simulator task using laparoscopic hands. The retention between tasks was different on each experiment, to examine changes in performance and cognitive load biomarkers that occur during the task or as a results of night sleep consolidation. The participants behavioral performance improved with trial repetition in all 3 experiments. In Exps. 1 & 2, the theta band activity significantly decreased with better individual performance, as exhibited by some of the behavioral measurements of the simulator. The novel VC9 biomarker (previously shown to correlate with cognitive load), exhibited a significant decrease with better individual performance shown by all behavioral measurements. In correspondence with previous research, theta decreased with lower cognitive load and higher performance and the novel biomarker, VC9, showed higher sensitivity to load changes. Together, these measurements might be for neuroimaging assessment of cognitive load while performing simulator laparoscopic tasks. This could potentially be expanded to evaluate efficacy of different medical simulations to provide more efficient training to medical staff and to measure cognitive and mental load in real laparoscopic surgeries.

Abstract:With the increasing prevalence of 3D videos, investigating the differences of viewing experiences between 2D and 3D videos has become an important issue. In this study, we explored the cognitive load induced by 2D and 3D video stimuli under various cognitive tasks utilizing electroencephalogram (EEG) data. We also introduced the Cognitive Load Index (CLI), a metric which combines {\theta} and {\alpha} oscillations to evaluate the cognitive differences. Four video stimuli, each associated with typical cognitive tasks were adopted in our experiments. Subjects were exposed to both 2D and 3D video stimuli, and the corresponding EEG data were recorded. Then, we analyzed the power within the 0.5-45 Hz frequency of EEG data, and CLI was utilized to evaluate the brain activity of different subjects. According to our experiments and analysis, videos that involve simple observational tasks (P <0.05) consistently induced a higher cognitive load in subjects when they were viewing 3D videos. However, for videos that involve calculation tasks (P >0.05), the differences in cognitive load induced by 2D and 3D video were not obvious. Thus, we concluded that 3D videos could generally induce a higher cognitive load, but the extent of the differences also depended on the contents of the video stimuli and the viewing purpose.

Role of preoperative 3D rendering for minimally invasive parenchyma sparing liver resections

Abstract:Background: 3D rendering (3DR) represents a promising approach to plan surgical strategies. The study aimed to compare the results of minimally invasive liver resections (MILS) in patients with 3DR versus conventional 2D CT-scan. Methods: We performed 118 3DR for various indications; the patients underwent a preoperative tri-phasic CT-scan and rendered with Synapse3D® Software. Fifty-six patients undergoing MILS with pre-operative 3DR were compared to a similar cohort of 127 patients undergoing conventional pre-operative 2D CT-scan using the propensity score matching (PSM) analysis. Results: The 3DR mandated pre-operative surgical plan variations in 33.9% cases, contraindicated surgery in 12.7%, providing a new surgical indication in 5.9% previously excluded cases. PSM identified 39 patients in both groups with comparable results in terms of conversion rates, blood loss, blood transfusions, parenchymal R1-margins, grade ≥3 Clavien-Dindo complications, 90-days mortality, and hospital stay respectively in 3DR and conventional 2D. Operative time was significantly increased in the 3DR group (402 vs. 347 min, p = 0.020). Vascular R1 resections were 25.6% vs 7.7% (p = 0.068), while the conversion rate was 0% vs 10.2% (p = 0.058), respectively, for 3DR group vs conventional 2D. Conclusion: 3DR may help in surgical planning increasing resectability rate while reducing conversion rates, allowing the precise identification of anatomical landmarks in minimally invasive parenchyma-preserving liver resections.

Abstract:Introduction This study was designed to compare three-dimensional (3D) laparoscopy and conventional two-dimensional (2D) laparoscopy in surgical performance and clinical assessment during laparoscopic radical hysterectomy with pelvic lymphadenectomy (LRHND) for treating early-stage cervical cancer. Material and methods In this study, we included 67 consecutive patients underwent LRHND for treating early-stage cervical cancer by the experienced laparoscopic surgeons between August 2018 and December 2020. Among these patients, 32 patients underwent 3D laparoscopy (2D group) and 35 patients underwent 2D laparoscopy (2D group). Demographic data, clinical and surgical parameters were obtained from each patient. An end-of-operation questionnaire was administered regarding subjective perception of 3D laparoscopy system. Results Patient characteristics, including age, BMI, FIGO stage, and histology, were comparable between the two groups. Compared with 2D imaging system, 3D system significantly shortened the operation time, especially bilateral lymph node dissection time. Blood loss was lower in 3D group compared with 2D group. There were no significant differences regarding pelvic nodes retrieved, incidence of complications, hospital stay, the recovery time of bowel, abdominal drainage fluid, hospitalization costs and visual symptoms. In addition, 3D system significantly improved depth perception and precision, and reduced surgical strain and eye strain for surgeon. No statistical difference was observed in visual symptoms and adverse events between the two groups. The surgeon was more willing to accept 3D laparoscopy. Conclusion The 3D laparoscopy is safe, feasible and comfortable, with obvious advantage in depth perception, precision and surgical strain. It triggered no increase in the complications and adverse events.
