Abstract:Abstract The bacterium Mycobacterium tuberculosis ( Mtb ) causes tuberculosis (TB) and is responsible for more human mortality than any other single pathogen 1 . Although ~1.7 billion people are infected with Mtb 2 , most infections are asymptomatic. Progression to active disease occurs in ~10% of infected individuals and is predicted by an elevated type I interferon (IFN) response 3–8 . Type I IFNs are vital for antiviral immunity, but whether or how they mediate susceptibility to Mtb has been difficult to study, in part because the standard C57BL/6 (B6) mouse model does not recapitulate the IFN-driven disease that appears to occur in humans 3–5,8 . Here we examined B6. Sst1 S congenic mice that carry the C3H “sensitive” allele of the Sst1 locus that renders them highly susceptible to Mtb infections 9,10 . We found that B6.Sst1 S mice exhibit markedly increased type I IFN signaling, and that type I IFNs were required for the enhanced susceptibility of B6. Sst1 S mice to Mtb . Type I IFNs affect the expression of hundreds of genes, several of which have previously been implicated in susceptibility to bacterial infections 11,12 . Nevertheless, we found that heterozygous deficiency in just a single IFN target gene, IL-1 receptor antagonist (IL-1Ra), is sufficient to reverse IFN-driven susceptibility to Mtb . As even a partial reduction in IL-1Ra levels led to significant protection, we hypothesized that IL-1Ra may be a plausible target for host-directed anti-TB therapy. Indeed, antibody-mediated neutralization of IL-1Ra provided therapeutic benefit to Mtb-infected B6. Sst1 S mice. Our results illustrate how the diversity of inbred mouse strains can be exploited to better model human TB, and demonstrate that IL-1Ra is an important mediator of type I IFN-driven susceptibility to Mtb infections in vivo .

Abstract:Reasons for the spectrum of severity of active tuberculosis (TB) disease are incompletely understood. We sought to identify master regulators of host immune responses that determine disease severity in pulmonary TB. We performed molecular profiling of human in vivo immune responses to discover associations with the extent of radiographic TB disease in a patient cohort. We then undertook mechanistic studies to test causality for the observed associations using the zebrafish larval infection model. Transcriptional profiling of human immune recall responses to tuberculin skin test (TST) challenge, a surrogate for immune responses to TB in the lung, revealed that type I interferon activity in the TST transcriptome was inversely associated with radiological disease severity. Abrogation of type I interferon signalling, achieved by CRISPR-mediated mutagenesis of , led to increased susceptibility of zebrafish larvae to infection, as a result of reduced recruitment of myeloid cells required to restrict mycobacterial growth, to the site of disease. Our data support a host protective role for type I interferon responses in mycobacterial infection, with potential applications for risk-stratification of adverse outcomes and development of a host-directed therapy to mitigate against severe disease.

Abstract:Background: Inflammatory phenomena such as hyperinflammation or hemophagocytic lymphohistiocytosis are a frequent yet paradoxical accompaniment to virus susceptibility in patients with impairment of type I interferon (IFN-I) signaling caused by deficiency of signal transducer and activator of transcription 2 (STAT2) or IFN regulatory factor 9 (IRF9). Objective: We hypothesized that altered and/or prolonged IFN-I signaling contributes to inflammatory complications in these patients. Methods: We explored the signaling kinetics and residual transcriptional responses of IFN-stimulated primary cells from individuals with complete loss of one of STAT1, STAT2, or IRF9 as well as gene-edited induced pluripotent stem cell-derived macrophages. Results: Deficiency of any IFN-stimulated gene factor 3 component suppressed but did not abrogate IFN-I receptor signaling, which was abnormally prolonged, in keeping with insufficient induction of negative regulators such as ubiquitin-specific peptidase 18 (USP18). In cells lacking either STAT2 or IRF9, this late transcriptional response to IFN-α2b mimicked the effect of IFN-γ. Conclusion: Our data suggest a model wherein the failure of negative feedback of IFN-I signaling in STAT2 and IRF9 deficiency leads to immune dysregulation. Aberrant IFN-α receptor signaling in STAT2- and IRF9-deficient cells switches the transcriptional output to a prolonged, IFN-γ-like response and likely contributes to clinically overt inflammation in these individuals.
