Abstract:BACKGROUND: The SSaSS (Salt Substitute and Stroke Study) has shown that use of a potassium-enriched salt lowers the risk of stroke, total cardiovascular events, and premature death. The effects on cause-specific cardiac outcomes are reported here. METHODS: SSaSS was an unblinded, cluster-randomised trial assessing the effects of potassium-enriched salt compared with regular salt among 20 995 Chinese adults with established stroke and older age and uncontrolled hypertension. Post hoc efficacy analyses were performed using an intention-to-treat method and a hierarchical Poisson regression model adjusting for clustering to obtain rate ratios and 95% CIs. We assessed acute coronary syndrome, heart failure, arrhythmia, and sudden death. RESULTS: Over a mean 4.74 years follow-up, there were 695 acute coronary syndrome events, 454 heart failure events, 230 arrhythmia events, and 1133 sudden deaths recorded. The rates of events were lower in potassium-enriched salt group for all outcomes but CIs were wide for most: acute coronary syndrome (6.32 versus 7.65 events per 1000 person-years; rate ratio, 0.80 [95% CI, 0.65–0.99]); heart failure (9.14 versus 11.32 events per 1000 person-years; rate ratio, 0.88 [95% CI, 0.60–1.28]); arrhythmia (4.43 versus 6.20 events per 1000 person-years; rate ratio, 0.59 [95% CI, 0.35–0.98]); and sudden death (11.01 versus 11.76 events per 1000 person-years; rate ratio, 0.94 [95% CI, 0.82–1.07]; all P >0.05 with adjustment for multiple comparisons). CONCLUSIONS: These results suggest that use of potassium-enriched salt is more likely to prevent than cause cardiac disease but the post hoc nature of these analyses precludes definitive conclusions. Abstract:AIM:To investigate the effect of potassium treatment on coronary arterial impairment induced by high salt intake.METHODS:Sprague-Dawley rats (4-week-old, n=10 in each group) received distilled water (NS), water containing 1.5%NaCl (HS), or 1.5% NaCl and 0.5% KCl (HS+HP) for 16 weeks.Systolic blood pressure (SBP) was determined by tail plethysmography every 2 weeks.After 16 weeks of treatment, vascular remodeling, superox-ide production, malondialdehyde (MDA) content, and endothelial nitric oxide synthase (eNOS) and gp91 expression in the coronary arteries were detected .RESULTS:After 16 weeks of salt loading , the rats in HS group was divided into salt sensitive subgroup and salt resistance subgroup according to the tail-cuff blood pressure .In this experiment , the salt-sensi-tive rats were selected as HS group .In HS group, salt loading significantly increased SBP , serum MDA and gp91 expres-sion, decreased serum NO and eNOS expression in the coronary arteries , and induced the coronary artery remodeling com-pared with NS group .In salt-loaded SD rats , 16-week potassium treatment abrogated the effects induced by salt loading . CONCLUSION:High salt may affect structural and functional changes in coronary arteries by activating oxidative stress . Potassium treatment antagonizes the effect of high salt intake .

Abstract:Objective: High blood pressure, one of the most important risk factors for cardiovascular mortality, leads to vascular dysfunction and subsequently to arteriosclerosis, stroke, kidney and heart failure. High salt intake and low potassium intake are associated with high blood pressure and an increased risk of cardiovascular disease and mortality. Partial replacement of sodium with potassium led to a lower risk of cardiovascular events and death in a long-term follow-up study. However, the exact mechanism remains unclear. Our aim was to investigate the acute effects of sodium or potassium intake on vascular health to unravel the underlying mechanisms leading to vascular adaptations. Design and method: Six male and nine female healthy individuals (mean age 31 years) were randomized to either 9g of sodium chloride (high salt group), 6g of sodium chloride (regular salt group) or 6 g of sodium chloride plus 3g of potassium chloride group (substitution group). Individuals received a salty soup with the corresponding sodium or potassium load. Blood analyses, blood pressure, body composition and macro- as well as microvascular measurements were performed before as well as four (t4) and 24 hours (t24) after soup intake. To investigate macrovascular health, carotid-to-femoral pulse wave velocity was analyzed. Microvascular health was measured by investigating retinal vessel diameters. Results: The high (139±2mmol/l vs. 143±2mmol/l p=0,003) and regular salt group (140±2mmol/l vs. 141±0,6 mmol/l p=0,029) showed both higher serum sodium concentrations at t4 compared to baseline. These higher values were normalized at t24. The serum sodium concentration of the substitution group showed no statistically significant difference between timepoints. The substitution group showed improved microvascular health, quantified by higher arteriolar-to-venular diameter ratio from baseline to t4 (0,86±0,09 vs. 0,89±0,08 p=0,005). These improvements were regressive at t24 (0,88±0,09). No statistically significant changes were found on pulse wave velocity. The high and regular salt group showed no statistically significant differences at any timepoint on micro- or macrovascular health parameters. Conclusions: Replacement of sodium by potassium seems to improve microvascular function directly after potassium intake. These results might explain the long-term beneficial effects of potassium on cardiovascular outcomes.

Abstract:The influence of dietary sodium and potassium on blood pressure (BP) has been extensively studied, however their impact on endothelial function, particularly any interactive effects, has received less attention. The purpose of this study was to determine if dietary potassium can offset the deleterious effect of high dietary sodium on endothelial function independent of BP. Thirty-three adults with salt-resistant BP (16 M and 17 F; 27 ± 1 year) completed seven days each of the following diets in a random order: a moderate potassium/low sodium diet (65 mmol potassium/50 mmol sodium; MK/LS), a moderate potassium/high sodium diet (65mmol potassium/300 mmol sodium; MK/HS) and a high potassium/high sodium (120 mmol potassium/300 mmol sodium; HK/HS). On day seven of each diet, 24-h ambulatory BP and a urine collection were performed. Brachial artery flow-mediated dilation (FMD) was measured in response to reactive hyperemia. Between diets, 24-h BP was unchanged confirming salt resistance (p > 0.05). Sodium excretion increased on both HS diets compared to MK/LS (p < 0.05) and potassium excretion was increased on the HK diet compared to MK/LS and MK/HS (p < 0.05) confirming diet compliance. FMD was lower in MK/HS (5.4 ± 0.5%) compared to MK/LS (6.7 ± 0.5%; p 0.05). These data suggest that dietary potassium provides vascular protection against the deleterious effects of high dietary sodium by restoring conduit artery function.

Abstract:Hypertension, Ahead of Print. BACKGROUND:The SSaSS (Salt Substitute and Stroke Study) recently reported definitive effects of a potassium-enriched salt on cardiovascular outcomes and death. Quantifying the amount of potassium-enriched salt used by trial participants is important for understanding the magnitude of the effect of potassium-enriched salt on risk reduction and how population-wide scale-up might be achieved.METHODS:Baseline and annual 24-hour urine samples were collected from subgroups of participants in SSaSS throughout the 5-year follow-up. The mean difference in 24-hour potassium excretion between the 2 groups was used to estimate the quantity of potassium-enriched salt consumed in the intervention group. The corresponding projected difference in sodium intake between groups was calculated and compared with the observed difference.RESULTS:The potassium-enriched salt group, compared to the regular salt group, had a mean increase in 24-hour urinary potassium excretion of 0.80 g/d (95% CI, 0.71–0.90), which equates to consumption of 8.8 g/d (95% CI, 7.8–9.9) of potassium-enriched salt. Based on 8.8 g/d potassium-enriched salt consumption, the projected difference in 24-hour urinary sodium excretion was −0.79 g/d. This compares to an observed difference of −0.35 g/d (95% CI, −0.55 to −0.15) and suggests that 72% of baseline regular salt intake was replaced by potassium-enriched salt.CONCLUSIONS:The smaller than anticipated between-group difference in sodium excretion likely results from the joint use of regular salt and potassium-enriched salt in the intervention group. Abstract:Two recent pivotal studies offer an opportunity to shift the thinking on population strategies to reduce dietary sodium intake to improve cardiovascular health. The SSaSS (Salt Substitute and Stroke Study), a cluster-randomized controlled study in high-risk persons in rural China was the first randomized clinical trial in a community setting of dietary sodium reduction with a potassium enriched salt substitute to report a reduction not only in blood pressure, but, in stroke, heart disease events, and total mortality. 1 The sodium intervention was a potassium-enriched salt substitute and was provided for food preservation and food preparation in homes in individuals with high risk for cardiovascular disease. See related article, pp 2188–2198 A second pivotal study is reported in this issue of Hypertension. 2 The researchers modeled the effects of a population-wide intervention in India replacing salt with a potassium enriched salt substitute as used in the China study. The modeling study evaluated the impact of this intervention on cardiovascular disease morbidity and mortality as well as adverse effects from hyperkalemia both in the general population and in those with undiagnosed chronic kidney disease. A similar modeling study was performed on the Chinese population as a follow-up of the SSaSS study. 1 Results from the India study suggest that nation wide salt substitution could prevent 8% to 14% of cardiovascular events annually (240 000–315 000 lives saved per year in the total population). While intervention strategies have focused on reducing sodium intake, the importance of increasing potassium needs consideration, both the potential for harm from hyperkalemia and whether increased potassium intake reduces cardiovascular disease risk. Concern about the possibility of hyperkalemia with the use of potassium-enriched salt substitutes has likely reduced enthusiasm for its use as a population strategy to lower blood pressure and reduce cardiovascular disease risk. The SSaSS study addressed this issue carefully, evaluating reports of risk and benefit of the intervention in the general population. Importantly, the India study evaluated both risk and benefit in persons with undiagnosed chronic kidney disease. The India modeling study demonstrates a strong benefit in those with and without chronic kidney disease. The small morbidity and mortality from hyperkalemia in the chronic kidney disease group is overwhelmed by the large benefit on cardiovascular health. In the SSaSS study, as well as most studies on the effect of potassium-enriched salt substitutes on blood pressure, the focus has been on the benefit of sodium reduction. Sorting out the potential contribution of increasing potassium intake in these studies has been a matter of interest for decades. 3 Increasing potassium intake without lowering sodium intake has a modest effect on lowering blood pressure. In the early 1990s, Langford 4 raised the question of whether there was cardiovascular protection from potassium independent of the effect on blood pressure. Since then, multiple studies have identified the benefit of higher potassium intake along with sodium reduction lowering blood pressure as well as the risk of cardiovascular disease, especially stroke. 3 Understanding the relative contribution of decreasing dietary sodium versus increasing dietary potassium could be important in designing interventions for future studies and in clinical implementation. Another key issue, study design, will be considered in deciding whether these 2 studies dramatically shift thinking on adopting potassium-enriched salt substitution as a population strategy for cardiovascular disease prevention. The study from India is a modeling study using the most rigorous methods for this study design. Modeling studies have gained widespread acceptance under certain circumstances including the inability to perform a trial in humans. 5 However, some scientists, clinicians, and policy makers are not willing to consider modeling studies as definitive evidence to support clinical decisions among individuals or policy making at the population level. The China study was conducted in a single country in a high-risk population, so generalizability is a consideration. Thus, some might hesitate to embrace these 2 studies as pivotal. The ideal follow-up to the China study would be to conduct randomized controlled trials in the general population (as opposed to a high risk group) of various countries. If possible, this approach would satisfy current thinking of regulators on necessary evidence. Effects of potassium supplementation and deficiency on the progression of salt-sensitive hypertension

Abstract:While the physiological effects of sodium on blood pressure are well established, many of the effects of potassium remain unclear. Prior work found that increased potassium intake decreases blood pressure and risk of cardiovascular disease, improves vasodilation, has reno-protective effects, and can stimulate natriuresis. In this study, we examined how different sodium/potassium ratios affect the development of salt-sensitive hypertension, specifically examining changes in the kidney and the renin angiotensin aldosterone system (RAAS). We hypothesized that blood pressure would directly correlate to the Na + /K + ratio and that there would be compensatory changes in RAAS and in expression of renal potassium channels, particularly inwardly rectifying potassium (K ir ) channels. At 9 weeks of age, male and female Dahl SS rats were placed on a high salt (4% NaCl) diet for 5 weeks with normal potassium (NK, 0.36% K + ), high potassium (HK, 1.41% K + ), or potassium depleted (DK, <0.0001% K + ) diet. After 5 weeks of dietary intervention, DK animals were hypokalemic, and HK animals had significantly increased plasma potassium. Potassium supplementation attenuated the development of salt-sensitive hypertension in male rats fed the HK diet compared to NK; the DK group did not develop hypertension. Potassium supplementation did not attenuate blood pressure in female rats, who also had a higher mortality rate on the DK diet. Heart rates were unchanged in both male and female HK groups but were significantly lower in the DK groups of both sexes. After 5 weeks of diet, the total body weights of DK rats were significantly lower. The 2 kidney/total body weight ratios were not different between the HK and NK groups, but the kidneys of the DK group were significantly larger, likely due to their smaller body weights. Medullary protein casts in DK rats of both sexes were decreased but no renoprotective effects were observed in the HK group. Western blot analysis of renal potassium channels demonstrated no changes in the expression of ROMK, significantly increased K ir 5.1 in DK males and females, and a significant increase in K ir 4.1 and the alpha-subunit of the Na + /K + -ATPase in DK males. We analyzed RAAS metabolites from plasma samples collected at the endpoint: Ang I (1-10), Ang IV (3-8), and Ang (1-5) were significantly decreased in DK males and females, Ang II (1-8) and Ang III (2-8) were significantly reduced in DK males, and aldosterone was significantly increased in HK males and females. These results provide a comprehensive assessment of how various potassium diets affect the development of salt-sensitive hypertension in both males and females, which will provide a solid foundation for future mechanistic studies. Abstract:Objective: Switching regular salt (sodium chloride) for salt enriched with potassium chloride (25% potassium chloride, 75% sodium chloride) has been shown to reduce blood pressure and the risk of cardiovascular diseases. We sought to define the potential for current production of sodium chloride and potassium chloride to support a global switch to the use of potassium-enriched salt. Design: We summarised data from geological surveys, government reports and trade organisations describing the global production and supply of sodium chloride and potash (the primary source of potassium chloride) and compared this to potential requirements for potassium-enriched salt. Setting: Global. Subjects: N/A. Results: Approximately 280 million tonnes of sodium chloride were produced in 2020 with China and the United States the main producers. Global production of potash from which potassium chloride is extracted was about 44 million tonnes with Canada, Belarus, Russia and China providing 77% of the world's supply. There were 48 countries in which potassium-enriched salt is currently marketed with 79 different brands identified. Allowing for loss of salt between manufacture and consumption, a full global switch from regular salt to potassium-enriched salt would require about 9.7 million tonnes of sodium chloride to be replaced with 9.7 million tonnes of potassium chloride annually. Conclusions: Significant up-scaling of the production of potassium chloride and the capacity of companies able to manufacture potassium-enriched salt, as well as a robust business case for the switch to potassium chloride, would be required.

Abstract:BACKGROUND Salt substitutes with reduced sodium levels and increased potassium levels have been shown to lower blood pressure, but their effects on cardiovascular and safety outcomes are uncertain. METHODS We conducted an open-label, cluster-randomized trial involving persons from 600 villages in rural China. The participants had a history of stroke or were 60 years of age or older and had high blood pressure. The villages were randomly assigned in a 1:1 ratio to the intervention group, in which the participants used a salt substitute (75% sodium chloride and 25% potassium chloride by mass), or to the control group, in which the participants continued to use regular salt (100% sodium chloride). The primary outcome was stroke, the secondary outcomes were major adverse cardiovascular events and death from any cause, and the safety outcome was clinical hyperkalemia. RESULTS A total of 20,995 persons were enrolled in the trial. The mean age of the participants was 65.4 years, and 49.5% were female, 72.6% had a history of stroke, and 88.4% a history of hypertension. The mean duration of follow-up was 4.74 years. The rate of stroke was lower with the salt substitute than with regular salt (29.14 events vs. 33.65 events per 1000 person-years; rate ratio, 0.86; 95% confidence interval [CI], 0.77 to 0.96; P=0.006), as were the rates of major cardiovascular events (49.09 events vs. 56.29 events per 1000 person-years; rate ratio, 0.87; 95% CI, 0.80 to 0.94; P<0.001) and death (39.28 events vs. 44.61 events per 1000 person-years; rate ratio, 0.88; 95% CI, 0.82 to 0.95; P<0.001). The rate of serious adverse events attributed to hyperkalemia was not significantly higher with the salt substitute than with regular salt (3.35 events vs. 3.30 events per 1000 person-years; rate ratio, 1.04; 95% CI, 0.80 to 1.37; P=0.76). CONCLUSIONS Among persons who had a history of stroke or were 60 years of age or older and had high blood pressure, the rates of stroke, major cardiovascular events, and death from any cause were lower with the salt substitute than with regular salt.

Abstract:Objective Dietary sodium and potassium consumption is associated with blood pressure levels. The objective of this study was to define a practical and low- cost method for the control of blood pressure by modification of these dietary cations in rural Chinese.Methods This study was a double- blind, randomized, controlled trial designed to establish the long- term effects of a reduced- sodium, high- potassium salt substitute ( 65% sodium chloride, 25% potassium chloride, 10% magnesium sulphate) compared to normal salt ( 100% sodium chloride) on blood pressure among high- risk individuals. Following a 4- week run- in period on salt substitute, participants were randomly assigned to replace their household salt with either the study salt substitute or normal salt for a 12- month period.Results The mean age of the 608 randomized participants was 60 years and 56% of them were female. Sixty- four percent had a history of vascular disease and 61% were taking one or more blood pressure- lowering drugs at entry. Mean baseline blood pressure was 159/ 93mmHg ( SD 26/ 14). The mean overall difference in systolic blood pressure between randomized groups was 3.7mmHg ( 95% confidence interval 1.6 - 5.9, P< 0.001). There was strong evidence that the magnitude of this reduction increased over time ( PU0.001) with the maximum net reduction of 5.4mmHg ( 2.3 - 8.5) achieved at 12 months. There were no detectable effects on diastolic blood pressure.Conclusion Salt substitution produced a substantial and sustained systolic blood pressure reduction in this population, and should be actively promoted as a low- cost alternate or adjunct to drug therapy for people consuming significant quantities of salt.

Abstract:BACKGROUND The relation between sodium intake and cardiovascular disease remains controversial, owing in part to inaccurate assessment of sodium intake. Assessing 24-hour urinary excretion over a period of multiple days is considered to be an accurate method. METHODS We included individual-participant data from six prospective cohorts of generally healthy adults; sodium and potassium excretion was assessed with the use of at least two 24-hour urine samples per participant. The primary outcome was a cardiovascular event (coronary revascularization or fatal or nonfatal myocardial infarction or stroke). We analyzed each cohort using consistent methods and combined the results using a random-effects meta-analysis. RESULTS Among 10,709 participants, who had a mean (±SD) age of 51.5±12.6 years and of whom 54.2% were women, 571 cardiovascular events were ascertained during a median study follow-up of 8.8 years (incidence rate, 5.9 per 1000 person-years). The median 24-hour urinary sodium excretion was 3270 mg (10th to 90th percentile, 2099 to 4899). Higher sodium excretion, lower potassium excretion, and a higher sodium-to-potassium ratio were all associated with a higher cardiovascular risk in analyses that were controlled for confounding factors (P≤0.005 for all comparisons). In analyses that compared quartile 4 of the urinary biomarker (highest) with quartile 1 (lowest), the hazard ratios were 1.60 (95% confidence interval [CI], 1.19 to 2.14) for sodium excretion, 0.69 (95% CI, 0.51 to 0.91) for potassium excretion, and 1.62 (95% CI, 1.25 to 2.10) for the sodium-to-potassium ratio. Each daily increment of 1000 mg in sodium excretion was associated with an 18% increase in cardiovascular risk (hazard ratio, 1.18; 95% CI, 1.08 to 1.29), and each daily increment of 1000 mg in potassium excretion was associated with an 18% decrease in risk (hazard ratio, 0.82; 95% CI, 0.72 to 0.94). CONCLUSIONS Higher sodium and lower potassium intakes, as measured in multiple 24-hour urine samples, were associated in a dose-response manner with a higher cardiovascular risk. These findings may support reducing sodium intake and increasing potassium intake from current levels. (Funded by the American Heart Association and the National Institutes of Health.).

Abstract:Abstract Potassium often has a negative connotation in Nephrology as patients with chronic kidney disease (CKD) are prone to develop hyperkalaemia. Approaches to the management of chronic hyperkalaemia include a low potassium diet or potassium binders. Yet, emerging data indicate that dietary potassium may be beneficial for patients with CKD. Epidemiological studies have shown that a higher urinary potassium excretion (as proxy for higher dietary potassium intake) is associated with lower blood pressure (BP) and lower cardiovascular risk, as well as better kidney outcomes. Considering that the composition of our current diet is characterized by a high sodium and low potassium content, increasing dietary potassium may be equally important as reducing sodium. Recent studies have revealed that dietary potassium modulates the activity of the thiazide-sensitive sodium-chloride cotransporter in the distal convoluted tubule (DCT). The DCT acts as a potassium sensor to control the delivery of sodium to the collecting duct, the potassium-secreting portion of the kidney. Physiologically, this allows immediate kaliuresis after a potassium load, and conservation of potassium during potassium deficiency. Clinically, it provides a novel explanation for the inverse relationship between dietary potassium and BP. Moreover, increasing dietary potassium intake can exert BP-independent effects on the kidney by relieving the deleterious effects of a low potassium diet (inflammation, oxidative stress and fibrosis). The aim of this comprehensive review is to link physiology with clinical medicine by proposing that the same mechanisms that allow us to excrete an acute potassium load also protect us from hypertension, cardiovascular disease and CKD.

Abstract:The inverse effects that dietary sodium and potassium have on blood pressure have been known for some time. High sodium consumption is detrimental, while potassium supplementation is known to be beneficial, and the ratio of sodium/potassium consumption is also very important to consider when determining how these electrolytes affect salt-induced hypertension; however, the mechanisms behind the beneficial effects of potassium are still not yet entirely established. Inwardly rectifying potassium (K ir ) channels have been shown to play a major role in potassium transport and homeostasis in the kidney and might contribute to these effects. Here we aimed to examine how different variations in the sodium/potassium ratio affect the development of salt-sensitive hypertension and how channels and transporters in the kidney are altered to accommodate these changes. We hypothesize that these diets will affect the development of salt-sensitive hypertension, and that K ir channels will play a major role in the renal adaptations to varying intakes of potassium. At 9 weeks of age, male Dahl SS rats were given a high salt (4% NaCl) diet for 5 weeks either supplemented with high potassium (HK, 1.41% K + ), normal potassium (NK, 0.36% K + ), or a potassium depleted diet (DK). Telemetry blood pressure recordings revealed that potassium supplementation attenuated the development of salt-sensitive hypertension in rats fed HK diet compared to NK; surprisingly, the DK group did not develop hypertension. Diets had a similar effect on heart rates, we observed the largest heart rate in the NK group, which was reduced with HK, and even further reduced in DK. Comparing terminal body weights, there was no difference between the HK and NK groups, but rats fed DK diet were significantly smaller. The 2 kidney/total body weight ratios were not different in the HK and NK groups, but the potassium deficient rat kidneys were significantly larger, likely due to their smaller body weights. There was no difference in heart/body weight ratios in the HK, NK, or DK groups; however, there is a trend in increased heart weight the lower the dietary potassium. Female animals were also utilized for experiments and followed similar trends as seen in the male rats. Western blot analyses further revealed the expression of K ir channels in Dahl SS rats fed a high salt diet supplemented with varying amounts of potassium. Altogether we provided a comprehensive analysis of various potassium diets on the development of salt-sensitive hypertension and K ir channels. Abstract:The Salt Substitute and Stroke Study (SSaSS) recently reported blood pressure-mediated benefits of a potassium-enriched salt substitute on cardiovascular outcomes and death. This study assessed the effects of salt substitutes on a breadth of outcomes to quantify the consistency of the findings and understand the likely generalisability of the SSaSS results.We searched PubMed, Embase and the Cochrane Library up to 31 August 2021. Parallel group, step-wedge or cluster randomised controlled trials reporting the effect of salt substitute on blood pressure or clinical outcomes were included. Meta-analyses and metaregressions were used to define the consistency of findings across trials, geographies and patient groups.There were 21 trials and 31 949 participants included, with 19 reporting effects on blood pressure and 5 reporting effects on clinical outcomes. Overall reduction of systolic blood pressure (SBP) was -4.61 mm Hg (95% CI -6.07 to -3.14) and of diastolic blood pressure (DBP) was -1.61 mm Hg (95% CI -2.42 to -0.79). Reductions in blood pressure appeared to be consistent across geographical regions and population subgroups defined by age, sex, history of hypertension, body mass index, baseline blood pressure, baseline 24-hour urinary sodium and baseline 24-hour urinary potassium (all p homogeneity >0.05). Metaregression showed that each 10% lower proportion of sodium choloride in the salt substitute was associated with a -1.53 mm Hg (95% CI -3.02 to -0.03, p=0.045) greater reduction in SBP and a -0.95 mm Hg (95% CI -1.78 to -0.12, p=0.025) greater reduction in DBP. There were clear protective effects of salt substitute on total mortality (risk ratio (RR) 0.89, 95% CI 0.85 to 0.94), cardiovascular mortality (RR 0.87, 95% CI 0. 81 to 0.94) and cardiovascular events (RR 0.89, 95% CI 0.85 to 0.94).The beneficial effects of salt substitutes on blood pressure across geographies and populations were consistent. Blood pressure-mediated protective effects on clinical outcomes are likely to be generalisable across population subgroups and to countries worldwide.CRD42020161077.

Abstract:Hypertension, Ahead of Print. Excess dietary sodium intake and insufficient dietary potassium intake are both well-established risk factors for hypertension. Despite some successful initiatives, efforts to control hypertension by improving dietary intake have largely failed because the changes required are mostly too hard to implement. Consistent recent data from randomized controlled trials show that potassium-enriched, sodium-reduced salt substitutes are an effective option for improving consumption levels and reducing blood pressure and the rates of cardiovascular events and deaths. Yet, salt substitutes are inconsistently recommended and rarely used. We sought to define the extent to which evidence about the likely benefits and harms of potassium-enriched salt substitutes has been incorporated into clinical management by systematically searching guidelines for the management of hypertension or chronic kidney disease. We found incomplete and inconsistent recommendations about the use of potassium-enriched salt substitutes in the 32 hypertension and 14 kidney guidelines that we reviewed. Discussion among the authors identified the possibility of updating clinical guidelines to provide consistent advice about the use of potassium-enriched salt for hypertension control. Draft wording was chosen to commence debate and progress consensus building: strong recommendation for patients with hypertension—potassium-enriched salt with a composition of 75% sodium chloride and 25% potassium chloride should be recommended to all patients with hypertension, unless they have advanced kidney disease, are using a potassium supplement, are using a potassium-sparing diuretic, or have another contraindication. We strongly encourage clinical guideline bodies to review their recommendations about the use of potassium-enriched salt substitutes at the earliest opportunity.

Abstract:Increased sodium intake aggravates cardiovascular diseases. Salt substitute containing potassium chlorideimproves cardiovascular outcome in hypertensive patients with high cardiovascular risk, however it isunknown whether it is attributed to potassium (K+) supplementation or sodium reduction. Here, weinvestigate the role of high K+ intake on the development of hypertensive cardiac damage in the presenceor absence of high salt diet. 8-10 weeks old apolipoproteinE-deficient mice (apoE-KO) were fed a normalK+ (0,55%) or high K+ (5%) diet throughout the experiment. Two weeks after diet start,mice were infusedwith angiotensin (Ang)II (500ng/kg/min) for 28 days. Cardiac function was assessed by MRI. High K+ dietincreased serum K+ levels compared to mice fed a normal K+ diet (5.3±1.2 vs. 3.9±0.4mmol/L, p<0.05).Interestingly, high K+ diet did not affect blood pressure or cardiac function. As expected, aldosteroneexcretion was increased in high K+ diet group compared to normal K+ diet (132±20 vs 8±18 ng/24h,p<0.01). To evaluate the consequence of K+ mediated aldosterone secretion for hypertensivecardiovascular damage, we additionally treated our high K+ group with high salt diet (1%NaCl) in thedrinking water. High K+ induced aldosterone production in the presence of high NaCl intake aggravatedhypertensive cardiac damage characterized by higher left ventricular mass, more cardiac fibrosis andinflammation compared to mice fed a high K+ or a high NaCl diet solely. Furthermore, the simultaneousintake of high K+/NaCl diet, induced higher mitochondrial ROS production in cardiac endothelial cellscompared to high K+ or high NaCl diet alone. As a consequence, cellular senescence markers such as p16and p21 were significantly higher in mice fed a high K+ / NaCl diet. Of note, co-treatment withspironolactone (50mg/kg/day), significantly attenuated cellular senescence and cardiovascular damage inmice fed a high K+/NaCl diet. High K+ diet compared to normal K+ diet does not have a beneficial effecton hypertensive cardiac damage in apoE-KO mice. Moreover, the present study indicates that potassium-induced aldosterone production aggravates the detrimental effect of high salt intake on cardiac health inhypertension by influencing cardiac senescence.

Abstract:Cardiovascular diseases (CVD) continue to be the leading cause of deaths and disability worldwide and the major contributor is hypertension. Despite all the improvements in detecting hypertension together with technological advances and affordable, efficacious and relatively free of adverse effects anti-hypertensive agents, we continue to struggle to prevent the onset of hypertension and to control blood pressure (BP) to acceptable targets. The poor control of hypertension is commonly due to non-adherence to medications. Another reason is the failure to adopt diet and lifestyle changes. Reduction of dietary salt intake is important for lowering BP but the role of potassium intake is also important. Globally the intake of sodium is double that of the recommended 2 gm per day (equivalent to 5 gm of sodium chloride/salt) and half that of the daily recommended intake of potassium of 3500 mg/day, giving a sodium-to-potassium ratio of >1, when ideally it should be <1. Many studies have shown that a higher potassium intake is associated with lower BPs, particularly when coupled concurrently with a lower sodium intake giving a lower sodium to potassium ratio. Most hypertension guidelines, while recommending reduction of salt intake to a set target, do not specifically recommend a target for potassium intake nor potassium supplementation. Here we review the role of potassium and salt substitution with potassium in the management of hypertension. Hence, the focus of dietary changes to lower BP and improve BP control should not be on reduction of salt intake alone but more importantly should include an increase in potassium intake.

Abstract:The Salt Substitute and Stroke Study (SSaSS) demonstrated significant reductions in systolic blood pressure (SBP), and the risk of stroke, major cardiovascular events and total mortality with the use of potassium-enriched salt. The contribution of sodium reduction versus potassium increase to these effects is unknown. We identified four different data sources describing the association between sodium reduction, potassium supplementation and change in SBP. We then fitted a series of models to estimate the SBP reductions expected for the differences in sodium and potassium intake in SSaSS, derived from 24-h urine collections. The proportions of the SBP reduction separately attributable to sodium reduction and potassium supplementation were calculated. The observed SBP reduction in SSaSS was −3.3 mmHg with a corresponding mean 15.2 mmol reduction in 24-h sodium excretion and a mean 20.6 mmol increase in 24-h potassium excretion. Assuming 90% of dietary sodium intake and 70% of dietary potassium intake were excreted through urine, the models projected falls in SBP of between −1.67 (95% confidence interval: −4.06 to +0.73) mmHg and −5.33 (95% confidence interval: −8.58 to −2.08) mmHg. The estimated proportional contribution of sodium reduction to the SBP fall ranged between 12 and 39% for the different models fitted. Sensitivity analyses assuming different proportional urinary excretion of dietary sodium and potassium intake showed similar results. In every model, the majority of the SBP lowering effect in SSaSS was estimated to be attributable to the increase in dietary potassium rather than the fall in dietary sodium.
