Abstract:A 48-year-old male presented to the emergency room with acute abdominal pain and distension that he had been experiencing for 2 days without passage of stool or flatus. He had a 2-year history of recurrent intestinal obstruction, but the condition was exacerbated this time. Physical examination revealed distension, epigastric tenderness, rebound tenderness, hyperactive bowel sounds and succussion splash. Plain abdominal X-ray showed differential air/fluid levels and a ‘string of pearls’ sign, indicating obstruction in the small intestine (Fig. 1A). CT scan showed an additional thickening and roughness of the bowel wall at the cecum and colon with partial calcification, accompanied by lumen stenosis and occlusion (Fig. 1B). The stool guaiac test was negative. The patient underwent laparoscopic exploration two days later due to the development of generalized peritonitis. The whole colon was found to be purple under laparoscope, and fine, red blood vessels were visible on its surface (Fig. 2A and video). We then turned to laparotomy and found extensive stenosis of the colon lumen and a dilated small intestine (Fig. 2B). The decision was made to resect the colon. Postoperative histologic examination of the specimen revealed diffuse angiodysplasia of the whole colon (Fig. 2C). The patient was recovering well at the follow-up visit 6 months after the surgery.
