Abstract:Pretreatment of super-low-dose lipopolysaccharide (SL-LPS) induces a more hyperresponsive state on the production of proinflammatory mediators to a subsequent secondary challenge with high-dose LPS in innate immune cells. Low-dose glucocorticoids (GCs) are also known to induce inflammation and immunosuppression in the immune cells. However, there is limited knowledge on whether preconditioning of low-dose GCs enhances inflammatory responses and dysregulates T lymphocyte responses to secondary LPS in SL-LPS-primed immune cells. In the present study, RAW 264.7 and EL4 cells were pretreated with SL-LPS (50 pg/ml) or low-dose corticosterone (CORT50: 50 ng/ml and CORT100: 100 ng/ml) in fresh complete medium once a day for 2–3 days, consecutively, and then cultured in fresh complete medium for 6 or 24 h in the presence or absence of LPS (1–10 μg/ml) or concanavalin A (Con A). The results demonstrated that the repeated pretreatment of CORT50 strongly enhanced production of IL-6, IL-10, TNF-α, and nitric oxide (NO) by RAW 264.7 cells in EP (SL-LPS-primed cells: endotoxin priming) in the absence of LPS compared to those in control (vehicle-pretreated cells), whereas CORT100 reduced production of TNF-α and IL-10. Further, the repeated pretreatment of CORT50 markedly enhanced LPS-induced production of IL-6, IL-10, TNF-α, PGE2, and NO by RAW 264.7 cells in EP compared to those in control, whereas CORT100 attenuated LPS-induced production of IL-6, IL-10, and NO. Moreover, the repeated pretreatments of CORT50 and CORT100 greatly attenuated the Con A-stimulated production of IFN-γ and IFN-γ/IL-10 and LPS-stimulated production of IL-10, IFN-γ, and IFN-γ/IL-10 by SL-LPS-primed EL4 cells (EP). These findings suggest that double preconditionings of low grade hypercortisolemia and metabolic endotoxemia may act as important risk factors for metabolic disorder and severe morbidity and mortality in septic shock via upregulated production of inflammatory mediators and immunosuppression of IFN-γ-mediated responses.
What problem does this paper attempt to address?
This paper aims to explore the effect of low - dose corticosterone pretreatment on the production of inflammatory mediators by ultra - low - dose lipopolysaccharide (SL - LPS) - preactivated immune cells under subsequent high - dose LPS stimulation. Specifically, the study focuses on the following points: 1. **Whether low - dose corticosterone pretreatment enhances the inflammatory response of ultra - low - dose LPS - preactivated immune cells**: - The study found that pretreatment with low - dose corticosterone (50 ng/ml and 100 ng/ml) for several consecutive days significantly enhanced the ability of RAW 264.7 cells preactivated by ultra - low - dose LPS to produce IL - 6, IL - 10 and TNF - α in the absence of LPS. - In particular, corticosterone pretreatment at 50 ng/ml significantly enhanced the production of these inflammatory mediators in cells preactivated by ultra - low - dose LPS, while corticosterone at 100 ng/ml weakened these effects in some cases. 2. **Whether low - dose corticosterone pretreatment enhances the production of inflammatory mediators induced by high - dose LPS**: - The study further found that low - dose corticosterone pretreatment significantly enhanced the production of IL - 6, IL - 10 and TNF - α induced by high - dose LPS. - Corticosterone pretreatment at 50 ng/ml significantly enhanced the production of inflammatory mediators induced by high - dose LPS in cells preactivated by ultra - low - dose LPS, while corticosterone at 100 ng/ml weakened these effects in some cases. 3. **Whether low - dose corticosterone pretreatment affects the function of T cells**: - The study also explored the effect of low - dose corticosterone pretreatment on EL4 cells (T - cell line) preactivated by ultra - low - dose LPS. - The results showed that low - dose corticosterone pretreatment significantly reduced the production of IFN - γ and IFN - γ/IL - 10 induced by Con A and LPS, indicating that low - dose corticosterone may affect the immune response by inhibiting T - cell function. ### Main conclusions - **Low - dose corticosterone pretreatment**: Low - dose corticosterone pretreatment can significantly enhance the production of inflammatory mediators by ultra - low - dose LPS - preactivated immune cells in the absence of LPS or under high - dose LPS conditions, especially the effect of corticosterone at 50 ng/ml is more obvious. - **T - cell function**: Low - dose corticosterone pretreatment also significantly weakens the response of T cells to Con A and LPS, indicating that it may affect the immune response by inhibiting T - cell function. - **Potential mechanism**: Low - dose corticosterone pretreatment may increase the risk of metabolic disorders and severe diseases by enhancing the production of inflammatory mediators and inhibiting T - cell function, especially in the case of low - grade endotoxemia and chronic stress. These findings suggest that the dual pretreatment of low - grade cortisolemia and metabolic endotoxemia may be an important risk factor for chronic inflammatory diseases, potential multiple organ failure and mortality related to septic shock.
