Studying the effect of temperature on meropenem quantity in peritoneal dialysis solutions

Abstract:Background: Peritonitis is the common complication of peritoneal dialysis in chronic kidney disease (CKD) patients. This problem can change peritoneal membrane function and result in death. The standard treatment for peritonitis from the international society for peritoneal dialysis (ISPD) is antibiotics, meropenem is recommended for treating peritonitis that cause from the gram-negative bacteria especially Pseudomonas species. Meropenem is injected directly into PD solutions then take it to use for regularly peritoneal dialysis. However, the stability data of these admixture is not enough.Objective: To determine the stability of meropenem in PD solutions by using high performance liquid chromatography (HPLC).Method: Validation the analytical method by preparing and using the reference standard meropenem for accuracy, precision, specificity, range and linearity study.A chromatographic separation was performed on C18 column using isocratic system with mobile phase composed of acetonitrile:20 mM phosphate buffer (10:90 v/v) at a flow rate of 1 ml/min and detector wavelength of 300 nm. The 0.5 g meropenem was injected into PD solutions (1L) and stored them in various temperatures (4, 25, 30 and 37°) then analyzed by HPLC. Result: Linearity estimated by correlation coefficient was greater than 0.999 with five different concentrations of meropenem (0.1, 0.2, 0.5, 0.6, 0.8 mg/ml). The retention time of meropenem was about 5-6 minute. The quantity of meropenem retained more than 90% of their initial concentration for 72 hours when stored in refrigerator (4°). At room temperature (25 and 30°), meropenem is reported to be stable in PD solutions at least 9 hours. Nevertheless, meropenem was stable in PD fluids less than 6 hours at body temperature (37°). Conclusion: The increasing temperature effect to the stability of meropenem in PD solutions. Meropenem could injected into the PD solutions then take it to the regularly peritoneal dialysis for one cycle (4-6 hours). However, admixture meropenem in PD solutions must be used with caution due to its lack of stability.
