Abstract:Purpose: Although osteoarthritis (OA) has been considered a disease of cartilage, recent studies reported that pathological subchondral bone remodeling precedes and accelerates cartilage degeneration in OA. Subchondral bone cyst has been identified as a cause for greater joint pain, cartilage loss and higher frequency of total joint replacement in knee OA patients. It has been proposed that subchondral bone cyst is linked with bone structure changes. However, the exact effect of subchondral bone cyst on bone structure alteration in OA is largely unknown. Therefore, this study aimed to investigate the association of subchondral bone cyst with subchondral bone remodeling in knee OA patients. Methods: A total number of 81 patients undergoing total knee arthroplasty for primary OA were recruited. The patients’ clinical and radiographic data were recorded. The tibial plateaus were collected during surgery and processed for micro-computed tomography (micro-CT) analysis. The presence of bone cyst was vilified in all three anatomical planes using software. And the samples were grouped as control (n=24) and bone cyst groups (n=57). Then the samples were further processed for histological analysis. Biomarkers of osteoclast (Trap), osteoblast (Osteocalcin) and osteoprogenitor (Osterix) were detected by immunohistochemistry. Results: Significant differences were not defined in age, body mass index (BMI), mechanical alignment or Knee Society Knee Score between the control and bone cyst groups. Subchondral bone cysts were observed in subchondral bone in the bone cyst group from multiple angle observations : The bone cyst group presented 34.9% higher fraction of bone volume (BV/TV) (43.7±7.4% vs. 32.4±7.2%, p=0.003) and 35.0% higher trabecular number (Tb.N) (2.7±0.45 vs. 2.0±0.4, p=0.017) in subchondral bone compared with the control group. Furthermore, 43.8% lower structure model index (SMI) (0.54±0.16 vs. 0.97±0.22, p=0.001) were detected in bone cyst group, indicating more trabeculae had shifted from rod-like to plate-like in the bone cyst group. Significant correlations were found between subchondral bone cyst volume and BV/TV (r= 0.407, p=0.010), Tb.N (r=0.342, p=0.029) and SMI (r= -0.541, p=0.001). Histology revealed a higher ratio of bone area/total area ratio (51.7±7.4% vs. 37.6±7.2%, p=0.002) in bone cyst group :. Besides, fibrocartilage tissue was found surrounding the bone cysts in bone cyst group. TRAP staining showed higher numbers of TRAP+ osteoclasts (61±7/mm2 vs. 36±4/mm2, p<0.05) in subchondral bone of the bone cyst group, indicating increased bone resorption. Higher numbers of Osteocalcin+ osteoblasts (58±5/mm2 vs. 35±4/mm2, p<0.05) and Osterix+ osteoprogenitors (65±6/mm2 vs. 37±4/mm2, p<0.05) were detected in areas surrounding bone cysts in the bone cyst group compared with the control group, indicating elevated level of osteoblastgenesis and bone formation. Conclusions: These experiments demonstrate that subchondral bone cysts coincides with impaired subchondral bone microstructure in patients with knee OA. The underlying mechanism is possibly due to turnovers of subchondral bone remodeling.

Abstract:Subchondral bone cysts represent an early postoperative sign associated with many articular cartilage repair procedures. They may be defined as an abnormal cavity within the subchondral bone in close proximity of a treated cartilage defect with a possible communication to the joint cavity in the absence of osteoarthritis. Two synergistic mechanisms of subchondral cyst formation, the theory of internal upregulation of local proinflammatory factors, and the external hydraulic theory, are proposed to explain their occurrence. This review describes subchondral bone cysts in the context of articular cartilage repair to improve investigations of these pathological changes. It summarizes their epidemiology in both preclinical and clinical settings with a focus on individual cartilage repair procedures, examines an algorithm for subchondral bone analysis, elaborates on the underlying mechanism of subchondral cyst formation, and condenses the clinical implications and perspectives on subchondral bone cyst formation in cartilage repair.

Abstract:The role of the subchondral bone in the pathophysiological processes of knee osteoarthritis(KOA)is receiving increasing attention.During the early stages of KOA,micro-fractures due to stress occurr with increased subchondral bone remodeling and decreased subchondral bone structural parameters,promoting the development of local microcirculatory disorders and atypical bone marrow lesions(BMLs).The micro-pores in cartilage provide pathways for blood vessel invasion into the deep layers of articular cartilage and subsequently bring in cartilage-degrading enzymes.With the pathogenesis of KOA,the loss of proteoglycans,which resist pressure to maintain the integrity of articular cartilage,results in stress overloading and then metabolic imbalances,leading to decreased bone resorption and increased bone formation.In the late stages of KOA,significant subchondral bone sclerosis further obstructs local blood microcirculation,which manifests as extended BMLs or increased signal intensity and even subchondral bone cysts.The acidic local environment impairs osteoblast function and erodes bone strength,contributing to subchondral bone collapse.Consequently,there is continued loss of the attached articular cartilage because of the positive feedback.Drug or surgical treatment aimed at cartilage protection should focus on functional modulation of osteoblasts and/or osteoclasts in the subchondral bone and the internal environment at different stages,instead of merely on the protection of articular cartilage.
Microstructural analysis of subchondral bone in knee osteoarthritis

Abstract:The results of this study show increased formation of bone in the subchondral areas in advanced stages of osteoarthritis of the knee. These changes seem to be influenced by mechanical factors. Introduction: Subchondral bone changes seem to contribute to the progression of knee osteoarthritis (OA). This study aimed to analyze subchondral bone microstructure in specimens of late-stage knee OA in respect to articular cartilage damage, meniscus integrity, and knee joint alignment. Methods: Thirty proximal tibiae of 30 patients (20 female and 10 male) with late-stage OA retrieved during total knee arthroplasty were scanned using a high-resolution micro-computed tomography. The scans were semi-automatically segmented into five volumes of interest. The volumes of interest were then further analyzed using commercially available software. The degree of articular cartilage damage was assessed semi-quantitatively by magnetic resonance imaging before surgery. Results: The mean bone fraction volume (bone volume/total volume (BV/TV)) in all weight-bearing locations was significantly higher compared to the non-weight-bearing reference point below the anterior cruciate ligament (p = 0.000). The mean BV/TV in the medial compartment was significantly higher compared to the lateral compartment (p = 0.007). As for the BV/TV in intact menisci, there was a significantly lower subchondral bone fraction volume compared to subluxated or luxated menisci in the medial (p = 0.020) and lateral compartment (p = 0.005). Varus alignment had a significantly higher subchondral BV/TV in the medial compartment, whereas valgus alignment had a significantly higher subchondral BV/TV in the lateral compartment (p = 0.011). Conclusions: The results show significant differences of subchondral bone microstructural parameters in respect to cartilage damage, meniscus' structural integrity, and knee joint alignment. Therefore, subchondral bone changes seem to be a secondary process in the late-stage OA of the knee caused by mechanical changes.

Abstract:Osteoarthritis (OA) is a joint degenerative disease characterized by articular cartilage degeneration and subchondral osteosclerosis. The pathological features were articular cartilage degeneration and destruction, subchondral osteosclerosis or cystic change, articular marginal hyperosteogeny, synovial lesions, articular capsule contracture, ligament relaxation or contracture, muscle atrophy and weakness, etc. In recent years, studies have found that subchondral osteosclerosis is a cause of OA, and subchondral osteosclerosis is superior to and accelerates the degeneration of cartilage, thus accelerating the development of OA. It is of great significance to inhibit or delay subchondral osteosclerosis in the treatment of OA. This article reviewed the research progress of OA treatment with subchondral bone sclerosis as the target, and provides help for follow-up basic research and clinical work.

Abstract:Purpose: Most patients with osteoarthritis have varus knee deformities with the change of Hip-knee-ankle (HKA) angle. The knee alignment influences load distribution at the tibial plateau. Meanwhile, change of subchondral bone microstruture is related to load bearing and osteoarthritis progression. However, the relationship between malalignment and the microstructural changes of subchondral bone has not yet been verified thoroughly. We aimed to investigate variation in medial tibial plateaus subchondral bone microstructure in knee osteoarthritis patients and their association with the severity of osteoarthritis with the change of varus angle. Methods: Tibial plateaus were collected from patients with end-stage knee osteoarthritis undergoing primary total knee arthroplasty (TKA). The varus angle (HKA angle) and Kellgren and Lawrence (K-L) grade was evaluated with full-leg standing posteroanterior radiographs. Tibial plateaus were divided into group A (varus angle≥10) and B (varus angle<10) according to whether the varus angle is greater than or equal to 10 degrees. Tibial plateaus were scanned by micro-computed tomography to analyze subchondral bone microstructure. The differences in microstructural parameters between the two groups were analyzed by the independent samples t-test. Pearson and spearman correlations were used to examine relationships among microstructural parameters, varus angle and K-L grade. Results: Group A had higher bone volume fraction (p<0.05), trabecular number (p<0.05), connectivity density (p<0.01) and lower trabecular separation (p<0.01) than group B. In addition, varus angle was significantly positively correlated with bone volume fraction (r=0.71, p<0.01) and trabecular number (r=0.63, p<0.01), respectively, and negatively correlated with trabecular separation (r=-0.59, p<0.05). Furthermore, the varus angle was significantly positively correlated with K-L grade (R=0.58, p<0.05). Conclusions: Increasing varus angle lead to severe knee osteoarthritis and sclerosis of medial tibial plateau subchondral bone, suggesting that abnormal malalignment may promote abnormal subchondral bone remodeling by altering joint load distribution, thereby affecting the progression of osteoarthritis.

Abstract:Osteoarthritis (OA) is a major cause of disability in the adult population. As a progressive degenerative joint disorder, OA is characterized by cartilage damage, changes in the subchondral bone, osteophyte formation, muscle weakness, and inflammation of the synovium tissue and tendon. Although OA has long been viewed as a primary disorder of articular cartilage, subchondral bone is attracting increasing attention. It is commonly reported to play a vital role in the pathogenesis of OA. Subchondral bone sclerosis, together with progressive cartilage degradation, is widely considered as a hallmark of OA. Despite the increase in bone volume fraction, subchondral bone is hypomineralized, due to abnormal bone remodeling. Some histopathological changes in the subchondral bone have also been detected, including microdamage, bone marrow edema-like lesions and bone cysts. This review summarizes basic features of the osteochondral junction, which comprises subchondral bone and articular cartilage. Importantly, we discuss risk factors influencing subchondral bone integrity. We also focus on the microarchitectural and histopathological changes of subchondral bone in OA, and provide an overview of their potential contribution to the progression of OA. A hypothetical model for the pathogenesis of OA is proposed.

Abstract:Osteoarthritis (OA) is the most common joint disease, and good therapeutic results are often difficult to obtain due to its complex pathogenesis and diverse causative factors. After decades of research and exploration of OA, it has been progressively found that subchondral bone is essential for its pathogenesis, and pathological changes in subchondral bone can be observed even before cartilage lesions develop. Osteoclasts, the main cells regulating bone resorption, play a crucial role in the pathogenesis of subchondral bone. Subchondral osteoclasts regulate the homeostasis of subchondral bone through the secretion of degradative enzymes, immunomodulation, and cell signaling pathways. In OA, osteoclasts are overactivated by autophagy, ncRNAs, and Rankl/Rank/OPG signaling pathways. Excessive bone resorption disrupts the balance of bone remodeling, leading to increased subchondral bone loss, decreased bone mineral density and consequent structural damage to articular cartilage and joint pain. With increased understanding of bone biology and targeted therapies, researchers have found that the activity and function of subchondral osteoclasts are affected by multiple pathways. In this review, we summarize the roles and mechanisms of subchondral osteoclasts in OA, enumerate the latest advances in subchondral osteoclast-targeted therapy for OA, and look forward to the future trends of subchondral osteoclast-targeted therapies in clinical applications to fill the gaps in the current knowledge of OA treatment and to develop new therapeutic strategies.

Multiple Subchondral Bone Cysts Cause Deterioration of Articular Cartilage in Medial OA of Knee: A 3D Simulation Study

Abstract:Aims: To investigate the impact of subchondral bone cysts (SBCs) in stress-induced osseous and articular variations in cystic and non-cystic knee models using finite element analysis.Materials and Methods: 3D knee joint models were reconstructed from computed tomography (CT) and magnetic resonance imaging (MRI). Duplicate 3D models were also created with a 3D sphere mimicking SBCs in medial tibia. Models were divided into three groups. In group A, a non-cystic knee model was used, whereas in groups B and C, SBCs of 4 and 12 mm size were simulated, respectively. Cyst groups were further divided into three sub-groups. Each of sub-group 1 was composed of a solitary SBC in the anterior half of tibia adjacent to joint line. In sub-group 2, a solitary cyst was modeled at a lower-joint location, and in sub-group 3, two SBCs were used. All models were vertically loaded with weights representing double- and single-leg stances.Results: During single-leg stance, increase in subchondral bone stress in sub-groups B-1 and B-3 were significant (p = 0.044, p = 0.026). However, in sub-group B-2, a slight increase was observed than non-cystic knee model (9.93 ± 1.94 vs. 9.35 ± 1.85; p = 0.254). All the sub-groups in group C showed significantly increased articular stress (p < 0.001). Conversely, a prominent increase in peri-cystic cancellous bone stress was produced by SBCs in groups B and C (p < 0.001). Mean cartilage shear stress in sub-groups B-1 and B-2 (0.66 ± 0.56, 0.58 ± 0.54) was non-significant (p = 0.374, p = 0.590) as compared to non-cystic model (0.47 ± 0.67). But paired cysts of the same size (B-3) produced a mean stress of 0.98 ± 0.49 in affected cartilage (p = 0.011). Models containing 12 mm SBCs experienced a significant increase in cartilage stress (p = 0.001, p = 0.006, p < 0.001) in sub-groups C-1, C-2, and C-3 (1.25 ± 0.69, 1.01 ± 0.54, and 1.26 ± 0.59), respectively.Conclusion: The presence of large-sized SBCs produced an increased focal stress effect in articular cartilage. Multiple cysts further deteriorate the condition by increased osseous stress effect and high tendency of peripheral cyst expansion in simulated cystic knee models than non-cystic knee models.

Three-dimensional analysis of subchondral cysts in hip osteoarthritis: An ex vivo HR-pQCT study

Abstract:INTRODUCTION: Subchondral cysts are deeply related to the pathogenesis of osteoarthritis (OA), but the factors contributing to cyst formation are not well known. A three-dimensional analysis of subchondral cysts at the micro-structural level was conducted using a high-resolution peripheral quantitative CT (HR-pQCT), and their relationships with cartilage attrition and subchondral bone microstructure were investigated.METHODS: Femoral heads extracted from ten female patients with hip OA were scanned using an HR-pQCT at a voxel size of 41μm. The volume fractions, numbers, and sizes of the cysts were measured in the subchondral bone region under the area of cartilage loss. Furthermore, the areas of cartilage loss, as well as the microstructure of the subchondral bones, were also measured, and their correlations with the cysts were analyzed.RESULTS: The volume fractions of cysts within subchondral bone regions varied from 2% to 33%, the numbers of cysts varied from 6 to 87, and the sizes varied from 1mm(3) to 657mm(3). There was a positive correlation between the number of cysts and bone volume (r>0.8, p<0.01).CONCLUSION: The degree of cyst formation showed a wide distribution in number and volume, and there was a close relationship between multiple cyst formation and bone sclerosis, which might be caused by reactive bone formation that occurred around each cyst.

Abstract:Subchondral bone has received increasing attention in both basic and clinical research on osteoarthritis (OA). Subchondral bone in OA presents abnormalities in structure, biochemical composition, biomechanics and cellular function. Overall, subchondral bone mainly shows bone resorption in early OA and bone formation in late OA. More and more evidence suggests that abnormalities in subchondral bone of OA promote joint pain generation and articular cartilage degeneration. Inhibition or amelioration of subchondral bone abnormalities can reduce joint pain and can delay cartilage degeneration; thus, subchondral bone-targeted treatment promises to be a new treatment approach for OA. The pathological changes and the role of subchondral bone in OA still require further investigation.

Abstract:Background: Subchondral cysts have always been taught to be one of the cardinal radiological features of knee osteoarthritis but are not well understood. We aimed to evaluate the radiological prevalence and epidemiology of subchondral cysts in patients with knee osteoarthritis to determine if they are truly a cardinal radiological feature. Methods: All patients of a single surgeon with symptoms of knee osteoarthritis were selected for this study. All patients had failed a trial of conservative therapy and were planned for total knee arthroplasty. Patients with symptoms of and documentary evidence of inflammatory arthritis, other neurological and orthopaedic problems causing functional deficits were excluded from this study. A total of 806 plain radiographs were analyzed with the aid of an atlas for the presence of narrowed joint space, osteophytes, subchondral sclerosis and subchondral cysts. The radiological prevalence of each feature was then calculated. Demographics and pre-operative measurements were compared between patients with and without radiological evidence of subchondral cysts. Results: Subchondral cysts were only present in 30.6% of the study population. Narrowed joint space was present in 99.5%, osteophytes in 98.1% and subchondral sclerosis in 88.3% of all radiographs. The differences in prevalence were statistically significant. There was a higher proportion of females in patients with radiological evidence of subchondral cysts. These patients also had a greater varus deformity preoperatively. Conclusion: With a radiological prevalence of 30.6%, subchondral cysts should not be considered a cardinal radiological feature of osteoarthritis. Subchondral cysts may be associated with the female gender and genu varum.

Abstract:Abnormal joint kinematics are commonly reported in the acute and chronic stages of recovery after anterior cruciate ligament (ACL) injury and have long been mechanistically implicated as a primary driver in the development of post‐traumatic osteoarthritis (PTOA). Though strongly theorized, it is unclear to what extent biomechanical adaptations after ACL injury culminate in the development of PTOA, as data that directly connects these factors does not exist. Using a pre‐clinical, non‐invasive ACL injury rodent model, our objective was to explore the direct effect of an isolated ACL injury on joint kinematics and the pathogenetic mechanisms involved in the development of PTOA. Thirty‐two, sixteen‐week‐old Long‐Evans rats were exposed to a non‐invasive ACL injury. Marker‐less deep learning software (DeepLabCut) was used to track animal movement for sagittal‐plane kinematic analyses and micro computed tomography was used to evaluate subchondral bone architecture at days 7, 14, 28 and 56 following injury. There was a significant decrease in peak knee flexion during walking (p<0.05), which had a moderate‐to‐strong negative correlation (r=‐.59 to r=‐.71, p<0.001) with subchondral bone plate porosity in all load bearing regions of the femur and tibia. Additional comprehensive analyses of knee flexion profiles revealed dramatic alterations throughout the step cycle. This occurred alongside considerable loss of epiphyseal trabecular bone and substantial changes in anatomical orientation. Knee flexion angle and subchondral bone microarchitecture are severely impacted after ACL injury. Reductions in peak knee flexion angle after ACL injury are directly associated with subchondral bone plate remodeling.This article is protected by copyright. All rights reserved.

Abstract:OBJECTIVE: To investigate changes in bone structure, turnover, and articular cartilage localized in subchondral bone cyst (SBC) regions associated with knee osteoarthritis (OA).METHODS: Tibial plateaus (n = 97) were collected from knee OA patients during total knee arthroplasty (TKA). SBCs were identified using micro-computed tomography, and the specimens were divided into non-cyst (n = 25) and bone cyst (n = 72) groups. Microstructure of subchondral bone was assessed using bone volume fraction (BV/TV), trabecular number (Tb.N), structure model index (SMI) and bone mineral density (BMD). In bone cyst group, the cyst subregion, which contained at least one cyst, and the peri-cyst subregion, which contained no cysts, were further selected for microstructure analysis. Articular cartilage damage was estimated using the Osteoarthritis Research Society International (OARSI) score. The numbers of TRAP(+) osteoclasts, Osterix(+) osteoprogenitors, Osteocalcin(+) osteoblasts and expression of SOX9 were evaluated by immunohistochemistry.RESULTS: Bone cyst group presented higher BV/TV, Tb.N and SMI at subchondral bone than non-cyst group. Furthermore, cyst subregion displayed increased BV/TV and Tb.N but lower BMD and SMI than peri-cyst subregion. Histology revealed a higher OARSI score in bone cyst group. SBC exhibited a weak relationship with BV/TV, etc. The numbers of TRAP(+) osteoclasts, Osterix(+) osteoprogenitors, Osteocalcin(+) osteoblasts and expression of SOX9, were higher in bone cyst group.CONCLUSION: SBCs within knee OA are characterized by focally increased bone turnover, altered bone structure and more severe articular cartilage damage. The increased bone turnover possibly contributes to altered bone structure localized in SBC areas, and thus aggravates articular cartilage degeneration.

Abstract:Subchondral bone alterations are emerging as considerable clinical problems associated with articular cartilage repair. Their analysis exposes a pattern of variable changes, including intra-lesional osteophytes, residual microfracture holes, peri-hole bone resorption, and subchondral bone cysts. A precise distinction between them is becoming increasingly important. Here, we present a tailored algorithm based on continuous data to analyse subchondral bone changes using micro-CT images, allowing for a clear definition of each entity. We evaluated this algorithm using data sets originating from two large animal models of osteochondral repair. Intra-lesional osteophytes were detected in 3 of 10 defects in the minipig and in 4 of 5 defects in the sheep model. Peri-hole bone resorption was found in 22 of 30 microfracture holes in the minipig and in 17 of 30 microfracture holes in the sheep model. Subchondral bone cysts appeared in 1 microfracture hole in the minipig and in 5 microfracture holes in the sheep model (n = 30 holes each). Calculation of inter-rater agreement (90% agreement) and Cohen's kappa (kappa = 0.874) revealed that the novel algorithm is highly reliable, reproducible, and valid. Comparison analysis with the best existing semi-quantitative evaluation method was also performed, supporting the enhanced precision of this algorithm.

Abstract:Objective To examine the natural history of subchondral bone cysts and to determine whether knee cartilage loss and risk of joint replacement are relevant to cysts and bone marrow lesions(BMLs).Methods 264 subjects with knee osteoarthritis(OA) were imaged by using magnetic resonance imaging at baseline and 2 years later.Tibial cartilage volume,subchondral bone cysts and BMLs were measured by using validated methods.We ascertained knee arthroplasty over a 4-year period.Results Bone cysts were present in 48.1% patients,97.1% of whom also had BMLs.Over a 2-year period,cysts progress was found in 31.4% of subjects,12.4% developed new cysts and 23.8% had cysts regress.Bone cysts at baseline were associated with lower tibial cartilage volume compared with those with BMLs only or those with neither.Annual medial cartilage volume loss was greatest in those with bone cysts.As the severity of bone abnormality in the medial compartment and the risk of knee replacement was most likely to happen in patients who had subchondral bone cysts.Conclusions Cartilage loss and risk of knee replacement are higher in patients who had cysts than if only BMLs are present.The result suggests that cysts should be a marker to identify those most likely to benefit from prevention of disease progression.As cysts can regress,it may also be a therapeutic targets in knee OA.

Abstract:Osteoarthritis (OA) is characterized by not only damaged articular cartilage , but also changes in subchondral bone .In addition to being one of main components keeping joint structure and function , subchondral bone plays an important role in maintaining healthy cartilage , in structure and function , through mechanical mutual support and biological information exchange .In OA, it may make cartilage bear abnormal force for subchondral bone remodeling and subsequent structural change .Moreover, the angiogenesis and structural change of subchondral bone enlarge possible pathway for abnormal exchange between bone and cartilage .Therefore, metabolism factors from subchondral bone can pro-mote cartilage degeneration .A further exploration on subchondral bone , including cellular and structural change , angio-genesis, biological exchange between bone and cartilage , subchondral bone remodeling and therapeutic approachs , will provide a great help for the research and therapy of OA .
Wolff’s law in action: a mechanism for early knee osteoarthritis

Abstract:There is growing interest in the role of bone in knee osteoarthritis. Bone is a dynamic organ, tightly regulated by a multitude of homeostatic controls, including genetic and environmental factors. One such key environmental regulator of periarticular bone is mechanical stimulation, which, according to Wolff’s law, is a key determinant of bone properties. Wolff’s law theorizes that repetitive loading of bone will cause adaptive responses enabling the bone to better cope with these loads. Despite being an adaptive response of bone, the remodeling process may inadvertently trigger maladaptive responses in other articular structures. Accumulating evidence at the knee suggests that expanding articular bone surface area is driven by mechanical stimulation and is a strong predictor of articular cartilage loss. Similarly, fractal analysis of bone architecture provides further clues that bone adaptation may have untoward consequences for joint health. This review hypothesizes that adaptations of periarticular bone in response to mechanical stimulation cause maladaptive responses in other articular structures that mediate the development of knee osteoarthritis. A potential disease paradigm to account for such a hypothesis is also proposed, and novel therapeutic targets that may have a bone-modifying effect, and therefore potentially a disease-modifying effect, are also explored.
