Abstract:BACKGROUND/AIM:The use of sternocleidomastoid muscle (SCM) flap for facial reanimation was established in the 1980s by the senior author of this paper. We aimed to analyze long-term outcome and complications of this procedure.PATIENTS AND METHODS:We conducted a retrospective chart review of all patients undergoing SCM reanimation for longstanding facial palsy between January 2009 and December 2015. Patients with follow-up longer than 12 months (range=12-96) were included in the study. Facial muscle function was evaluated before and at each follow-up after the surgery with the House-Brackmann (HB) scale-facial nerve grading system and Facegram analysis. Donor site morbidity and overall complication rates were documented and analyzed.RESULTS:Forty-two patients aged 18-66 years (mean age=37) with a mean duration of facial palsy of 5 years (range=2-48) met the inclusion criteria. The HB score 2 years after surgery improved significantly (p<0.05) in comparison to the pre-operative condition (3.6 vs. 4.7). Twelve months after surgery, oral commissure excursion improved by mean 8.95 mm. No flap necrosis occurred, nor compromise of neck and shoulder function despite an obvious contour defect in the SCM donor site. None of the patients presented head posture or movement issues.CONCLUSION:The SCM flap transfer is a reliable and effective procedure to achieve moderate improvement of the oral commissure excursion using a local method with moderate donor site morbidity. It can be regarded as a valuable option for dynamic facial reanimation in case of longstanding facial palsy.
The Effects of Modified Graded Recession, Anteriorization and Myectomy of Inferior Oblique Muscles on Superior Oblique Muscle Palsy

Abstract:Background: The aim was to investigate the effect of inferior oblique (IO) operation (IO myectomy or graded recession and anteriorization) for unilateral and bilateral superior oblique muscle palsy (SOP); Methods: A total of 167 eyes undergoing IO surgery by a single surgeon between 2008 and 2015 were retrospectively reviewed. The method for treating symmetric bilateral SOP was bilateral IO myectomy (n = 102) and the method for treating unilateral SOP or non-symmetric bilateral SOP was IO-graded recession and anteriorization (n = 65). Associated clinical results and other factors were analyzed; Results: Head tilt, vertical deviation, IO overaction, SO underaction degree and ocular torsion angle were all clearly changed, but there was no statistically significance between these two procedures. Mean preoperative torsional angle was 15.3 ± 6.4 degree, which decreased to 5.3 ± 2.7 degree after surgery. Preoperative torsional angle, IOOA and SOUA degree were all significantly affected in postoperative torsional angle (p = 0.025, 0.003 and 0.038). Horizontal rectus muscle and IO muscle operation did not interfere with each other’s results (p = 0.98); Conclusions: Symmetric bilateral SOP could be treated with bilateral IO myectomy and IO-graded recession and anteriorization should be reserved for unilateral SOP or non-symmetric bilateral SOP.

Abstract:Objective: This study aimed to investigate morphologic features of the sternocleidomastoid muscle (SCM) in children for pediatric head-neck surgeries. Methods: Magnetic resonance imaging images of 84 (46 males/38 females) pediatric patients aged ages 1 to 18 years (mean age: 9.38 ± 4.46 y) constituted the study universe. The size of SCM (including its height, width, and angulation) was measured, and the origin types of SCM were noted. Results: All patients had SCM bilaterally. The origin variation of SCM was observed in 22 sides (13.10%). The authors observed type 1 in 146 sides (86.90%), type 2a in 15 sides (8.90%), type 3 in 2 sides (1.20%), type 4 in 1 side (0.60%), and type 5 in 4 sides (2.40%). The absence and presence of SCM variation were not affected by sex ( P = 0.370) and side ( P = 0.360). All measured parameters were similar in right and left sides, and also in males and females, except from the angle between the junction of the sternal and clavicular heads of SCM. The angle was greater in males than females ( P = 0.004). All measured parameters increased with advancing age (between 1 and 18 y; P < 0.05). Conclusion: The authors’ SCM-related data may be useful for clinicians to estimate the size origin variations of SCM in children.

Surgical Management of Unilateral Superior Oblique Palsy: Thirty Years of Experience

Abstract:Introduction and purpose: We describe the clinical characteristics of 252 patients with unilateral superior oblique palsy who underwent strabismus surgery. We assess if a predetermined surgical strategy, based on preoperative alignment and motility measurements, was effective in treating these patients. On this basis, the patients were divided into three different treatment groups. Methods: Two-hundred fifty-two patients were identified retrospectively and classified into three groups according to the performed procedures: 1) inferior oblique weakening; 2) inferior rectus recession; 3) combined inferior oblique weakening and inferior rectus recession. Demographic and clinical data were recorded. Criteria for surgical success included good postoperative alignment (distance, primary position alignment ≤5Δ), and improvement of diplopia and of abnormal head posture. Subgroup analyses of surgical outcome were performed for small (<12Δ) versus large (>20Δ) preoperative hypertropia in the group that underwent inferior oblique weakening, and for inferior oblique disinsertion-myectomy versus inferior oblique recession. Results: Mean forced primary position (PP) hypertropia decreased from 14.3Δ (range 3-37Δ) to 4.5Δ (range 0-30Δ) in Group 1, from 13Δ (range 1-30Δ) to 2Δ (range -20-20Δ) in Group 2, and from 25.7Δ (range 6-40Δ) to 1.3Δ (range -12-18Δ) in Group 3. Group 1 had the lowest re-operation rate (7.6%), followed by Group 2 (16%) and Group 3 (25.9%). Final surgical success rates were similar in three groups. Inferior oblique weakening was more predictable for small primary position hypertropia, but still yielded 85% success rate in large deviations. Inferior oblique disinsertion-myectomy resulted in more favorable results than inferior oblique recession (P < 0.05). Conclusion: When a predetermined surgical strategy is applied to individual patients with unilateral superior oblique palsy, excellent functional improvement can be achieved in the majority of patients.

Abstract:Background: Recent reports postulate that the concomitant vertical deviation found in congenital superior oblique palsy is due to mechanical abnormalities rather than a congenitally paretic muscle, and is overcome in most patients by fusion. On the basis of the clinical characteristics alone a primary paresis is indeed unlikely. Although intraoperatively a different elasticity of the superior oblique tendon exists in congenital versus acquired cases of superior oblique palsy, preoperatively performed MR imaging shows that the clinical findings in congenital superior oblique muscle malfunction could nevertheless be of paretic origin. Materials and methods: Seventeen consecutive patients (males: n = 13; females: n = 4) were examined. The vertical deviation in adduction was concomitant in vertical versions, the excyclotropia was small and concomitant in all directions of gaze and was less than 10 degrees even after diagnostic occlusion. All patients showed a positive Bielschowsky head tilt phenomenon and large fusional ability. We performed preoperative MR imaging of both orbits in high resolution 3 mm sections in coronal and axial orientations with and without contrast enhancement. Results: In sixteen patients we found a significant reduction in muscle volume or even total aplasia of the superior oblique muscle of the affected side in comparison to the sound muscle on the other side. In contrast, two patients had a full blown clinical picture of a congenital superior oblique palsy but showed symmetrical muscle volumes on both sides in all coronal sections. Conclusions: Hypoplasia or aplasia of the superior oblique muscle on magnetic resonance imaging provides evidence for a primary paretic cause for the vertical squint found with congenital superior oblique dysfunction. It is not clear, however, whether this is caused by a primary hypoplasia or is of neurogenic origin. Our data together with the consistent difference in tendon morphology of the congenital and acquired forms of superior oblique palsy seem to exclude a purely neurogenic cause for the affection.

Abstract:Asymmetric superior oblique palsy (SOP) is a complicated, non-comitant type of strabismus. Patients usually present with small-angle hypertropia in the affected eyes, accompanied by inferior oblique muscle overaction (IOOA).<a class="ejp-citation-link js-ejp-citation-link">[1]</a> Compensatory head posture and asymmetric development of the face are typically observed in congenital SOP, while diplopia is more common in an acquired SOP.<a class="ejp-citation-link js-ejp-citation-link">[2]</a> Prism glasses can improve diplopia and vertical deviation in the primary position, but they are not so successful at correcting non-comitant paralytic strabismus. In addition, vertical strabismus and compensatory head posture will return after the prism glasses are removed, wearing prism glasses will lower the vision of children prominently. Surgery like an inferior oblique muscle (IO) weakening procedure is often required eventually in those patients. However, conventional procedures for IO weakening may cause overcorrection and/or secondary A pattern in patients who have mild to moderate IOOA together with small-angle asymmetric SOP, posing a risk to its safety.<a class="ejp-citation-link js-ejp-citation-link">[3]</a> In the study by Caldeira,<a class="ejp-citation-link js-ejp-citation-link">[4]</a> 21% of the patients with mild to moderate IOOA developed A pattern following the surgery. Therefore, it remains a clinical challenge to select the most appropriate type of surgery for the treatment of small-angle asymmetric SOP. Inferior oblique muscle belly transposition (IOMBT),<a class="ejp-citation-link js-ejp-citation-link">[5]</a> a new IO weakening procedure introduced to our practice lately, has shown positive results in treating small-angle asymmetric SOP as well as a lower risk of complications such as overcorrection and secondary A pattern. In this study, we evaluated the efficacy and safety of the IOMBT procedure for the treatment of small-angle asymmetric SOP.

Masquerading Superior Oblique Palsy

Abstract:Purpose: We evaluated patients with hypertropia compatible with a diagnosis of superior oblique (SO) palsy to ascertain whether the 3-step test (3ST) can distinguish SO atrophy characteristic of trochlear nerve pathology from masquerading conditions. Design: Prospective cross-sectional study. Methods: In an academic practice, we performed quasi-coronal plane, surface coil magnetic resonance imaging in 83 patients clinically diagnosed with SO palsy. We evaluated alignment, SO cross-sectional area, SO contractility, and rectus muscle pulley positions. Results: A total of 57 patients with mean age 39 years (SD = 21 years) had unilateral SO palsy manifested by SO atrophy (22 congenital and 35 acquired). There was normal SO size in 26 patients with an average age of 39 years (SD =16 years) considered masquerades (8 congenital and 18 acquired). Maximum palsied SO cross-section averaged 9.5 ± 3.8 mm2, less than 18.4 ± 3.9 mm2 contralaterally (P < 10-24). In masquerades, maximum hypertropic SO cross-section was 20.7 ± 3.1 mm2, which was not different from the hypotropic SO or the contralesional muscle in SO palsy. Head tilt testing in masquerades was indistinguishable from SO palsy. In SO palsy, central hypertropia averaged 13.2 ± 9.4Δ, increasing to 21.1 ± 14.0Δ in ipsilateral tilt, and decreasing to 4.3 ± 5.3Δ in contralateral tilt. In masquerades, central hypertropia averaged 13.1 ± 8.7Δ, and was 17.7 ± 11.1Δ in ipsilateral and decreasing to 4.9 ± 5.1Δ in contralateral tilt. Upright hypertropia was larger at 17.7 ± 9.9Δ in congenital than 12.0 ± 8.4Δ in acquired SO palsy (P = 0025) but was indistinguishable from congenital masquerades. Contractile change in SO cross-section was bilaterally similar in masquerades. Relevant coordinates of rectus pulleys were similar bilaterally in masquerades. Conclusions: The 3ST pattern characteristic of unilateral SO palsy may be mimicked in all respects by masquerades.
Magnetic resonance imaging of the functional anatomy of the inferior rectus muscle in superior oblique muscle palsy

Abstract:Purpose: Biomechanical modeling consistently indicates that superior oblique (SO) muscle weakness alone is insufficient to explain the large hypertropia often observed in SO muscle palsy. Magnetic resonance imaging (MRI) was used to investigate if any size or contractility changes in the inferior rectus (IR) muscle may contribute. Design: Prospective, case-control study. Participants: Seventeen patients with unilateral SO muscle palsy and 18 orthotropic subjects. Methods: Surface coils were used to obtain sets of contiguous, 2-mm-thick, high-resolution, coronal MRI views in different gazes. Cross-sectional areas of the IR and SO muscles were determined in supraduction and infraduction for evaluation of size and contractility. Diagnosis of SO muscle palsy was based on clinical presentations, subnormal contractility, and SO muscle size less than the normal 95% confidence limit. Main outcome measures: Cross-sectional areas of the IR and SO muscles. Results: Patients had 15.9+/-7.2 prism diopters (Delta; mean+/-standard deviation) of central gaze hypertropia and exhibited ipsilesional SO muscle atrophy and subnormal contractility. Mean ipsilesional, contralesional, and normal IR muscle cross-sections were 28.5+/-3.5 mm(2), 31.9+/-3.8 mm(2), and 31.8+/-5.8 mm(2), whereas mean contractility was 16.5+/-3.8 mm(2), 20.5+/-4.1 mm(2), and 16.6+/-4.8 mm(2), respectively. Ipsilesional IR muscle cross-section and contractility was significantly less than contralesional cross-section and contractility (P<0.01). Conclusions: In SO muscle palsy, the contralesional IR muscle is larger and more contractile than the ipsilesional IR muscle, reflecting likely neurally mediated changes that augment the relatively small hypertropia resulting from SO muscle weakness alone. Recession of the hyperfunctioning contralesional IR muscle recession in SO muscle palsy is a physiologic therapy.
Can Binocular Alignment Distinguish Hypertropia in Sagging Eye Syndrome from Superior Oblique Palsy?

Abstract:PURPOSE. Although the three-step test (3ST) is typically used to diagnose superior oblique palsy (SOP), sagging eye syndrome (SES) has clinical similarities. We sought to determine if alignment measurements can distinguish unilateral SOP from hypertropia in SES. METHODS. We studied hypertropic subjects who underwent surface-coil magnetic resonance imaging (MRI) demonstrating either SO cross-section reduction indicative of congenital or acquired palsy (SOP group) or lateral rectus muscle sag (SES group). Alignment was measured by Hess screen and prism-cover testing. Multiple supervised machine learning methods were employed to evaluate diagnostic accuracy. Rectus pulley coordinates were determined in SES cases fulfilling the 3ST. RESULTS. Twenty-three subjects had unilateral SOP manifested by SO atrophy. Eighteen others had normal SO size but MRI findings of SES. Maximum cross-section of the palsied SO was much smaller than contralaterally and in SES (P < 2 x 10(-5)). Inferior oblique cross-sections were similar in SOP and SES. In both SOP and SES, hypertropia increased in contralateral and decreased in ipsilateral gaze and was greater in ipsilateral than contralateral head tilt. In SES, nine subjects (50%) fulfilled the 3ST and had greater infraplacement of the lateral than medial rectus pulleys in the hypotropic orbit. Supervised machine learning of alignment data distinguished the diagnoses with areas under the receiver operating curves up to 0.93, representing excellent yet imperfect differential diagnosis. CONCLUSIONS. Because the 3ST is often positive in SES, clinical alignment patterns may confound SES with unilateral SOP, particularly acquired SOP. Machine learning substantially but imperfectly improves classification accuracy.
Effect of inferior oblique myectomy on ocular torsion according to the absence of the trochlear nerve in unilateral congenital superior oblique palsy

Abstract:Objectives: To evaluate the effect of inferior oblique (IO) myectomy on ocular torsion according to the absence of the trochlear nerve in unilateral congenital superior oblique palsy (UCSOP). Methods: We retrospectively reviewed the clinical data of patients who had been diagnosed with UCSOP and underwent ipsilateral IO myectomy (n = 43). Patients were classified into the present and absent groups according to the absence of the trochlear nerve and superior oblique hypoplasia on magnetic resonance imaging (MRI). For quantitative analysis of ocular torsion, disc-fovea angles (DFA) were collected in both eyes using fundus photographs taken within three months before surgery and one month after surgery. Results: DFA of the paretic eye did not differ according to the absence of the trochlear nerve (9.4±5.6° in the present group vs. 11.0±5.4° in the absent group, p = 0.508). However, the present group had a larger DFA in the non-paretic eye than the absent group (14.1±6.7° in the present group vs. 8.0±5.0° in the absent group, p = 0.003). The change of ocular torsion after IO myectomy in the paretic eye was -5.3±3.7° in the present group and -4.8±3.5° in the absent group, respectively (p = 0.801). In the non-paretic eye, the change in DFA was -1.5±3.0° in the present group, which was larger than that in the absent group (0.7±2.6°, p = 0.047). In the multivariate analysis, the change in DFA was correlated with only the preoperative DFA (standardized β = -0.617, p<0.001 in the paretic eye, and standardized β = -0.517, p<0.001 in the non-paretic eye). Conclusions: In the paretic eye, there was no significant difference in the change of ocular torsion between both groups, whereas in the non-paretic eye, the present group had a larger change in DFA after IO myectomy than the absent group. However, in the multivariable analysis, the change in ocular torsion was significantly correlated with preoperative excyclotorsion but not with the presence of the trochlear nerve itself.

Abstract:Purpose: To study size and contractility of the normal inferior oblique (IO) muscle using high-resolution magnetic resonance imaging (MRI) and to evaluate abnormalities of the superior oblique (SO) and IO muscles in chronic SO palsy. Design: Prospective, case control study. Participants: Thirteen patients with SO palsy and 17 orthotropic subjects. Methods: High-resolution, surface coil MRI was used to obtain sets of contiguous, 2-mm thick coronal and sagittal images repeated in multiple gaze directions. Digital image analysis was used to measure IO and SO muscle cross-sectional areas for evaluation of size and contractility. Diagnosis of SO palsy in one bilateral and 12 unilateral cases was based on subnormal contractility and SO size less than the normal 95% confidence limit. Ipsilesional and contralesional oblique muscles were compared with controls and correlated with clinical characteristics. Results: In all subjects, anterior movement and contractile thickening of the IO were observed in supraduction, with posterior movement and relaxational thinning in infraduction. The mean (+/- standard deviation) cross-sectional area of 15 normal control IO muscles was 13.4 +/- 3.9 mm(2), with mean contractile increase from infraduction to supraduction of 5.7 +/- 2.6 mm(2). Subjects with SO palsy had incomitant hypertropia with a wide range of overelevation and underelevation in adduction (i.e., upshoot, downshoot). SO atrophy correlated with underdepression in adduction (P < 0.0001). Contralesional SO cross-section was slightly greater than normal (P = 0.004). The IO cross-section ipsilesional and contralesional to SO palsy did not, however, differ significantly from normal and did not correlate with elevation in adduction (P > 0.2). Conclusions: Quantitative morphometry by MRI can demonstrate IO size and contractility. Even in cases of unequivocal SO palsy associated with ipsilesional SO atrophy and deficient contractility, the degree of elevation in adduction was not correlated with IO size. This finding suggests that the associated overelevation in adduction, commonly termed "inferior oblique overaction," actually arises from some other mechanism than IO hypertrophy or excess contractility. Revision of clinical terminology seems warranted.
The efficacy of superior rectus recession with simultaneous inferior oblique disinsertion on superior oblique palsy with superior rectus contracture

Abstract:Purpose: The information about superior rectus (SR) recession in cases with SR contracture coexisting with superior oblique palsy (SOP) is very limited in previous literature. The aim of this study is to evaluate the effect of SR recession, as a combined procedure with inferior oblique (IO) disinsertion, in long-standing SOP with secondary SR contracture. Methods: The medical records of the 145 patients operated for SOP were retrospectively reviewed and 15 patients who underwent SR recession met the inclusion criteria. The mean follow-up was 50.4 months. Results: The preoperative angle of deviation was within the range of 16-35 prism dioptres (PD) with a mean of 23.0 ± 5.03 PD. In all of the cases, SR recession was performed in combination with IO disinsertion. Adult patients (12) underwent adjustable SR recession. Postoperative overcorrection developed in 3 cases (20%), and in 12 patients, the postoperative vertical deviation was within the range of 0-5 PD with a mean of 1.41 ± 1.88 PD. The overcorrected three patients underwent botulinum toxin A (BTXA) injection into the ipsilateral inferior rectus muscle. The deviation is well controlled by BTXA injection in two patients, whereas the other one was considered as masked bilateral SOP and underwent IO disinsertion in the other eye. Conclusion: Our results suggested that SR recession in combination with IO disinsertion is an effective procedure to control large vertical deviations in SOP with SR contracture in the majority of cases. However, the risk of overcorrection should be considered despite adjustable SR weakening and BTXA injection seems efficient to rescue overcorrections in long term.

Methods: We performed a retrospective study of 76 patients who underwent surgery for SOP. We recorded data from the physical examination and the number and type of procedures performed. Favorable outcome was defined as resolution of or improvement in torticollis (≤5°) and diplopia in primary position (PP) and downgaze or as vertical deviation (VD) <5 prism diopters (pd) in PP and 10 pd in the oblique diagnostic position. Results: Mean age was 33.12 years. Congenital SOP was the most frequent type (65.8%). Mean preoperative VD was 15.89 ± 9.94 pd, decreasing to 3.07 ± 4.36 pd after surgery. Associated horizontal deviation was recorded in 51.32% of cases. The mean number of procedures was 1.37 ± 0.62 (range 1-4), with 69.7% of patients requiring only one procedure. The mean number of muscles operated on was 1.96 ± 1.01 (inferior oblique being the most frequent). A greater reduction in VD after surgery was observed in patients with congenital SOP (P = 0.04). Although none of the factors evaluated influenced surgical outcome, amblyopic patients had a greater risk of reoperation (P = 0.04). A favorable outcome was achieved in 75% of cases. Mean follow-up was 37.08 months. Conclusion: Congenital SOP was twice as frequent as acquired SOP and although surgery was successful in most cases, a greater reduction in VD was obtained in congenital cases. Amblyopia was identified as a risk factor for reoperation.
Evaluation of Mandibular Symmetry in Patients with Condylar Osteochondroma Who Underwent Intro-Oral Condylar Resection and Simultaneous Bimaxillary Orthognathic Surgery.

Abstract:Mandibular condylar osteochondroma (OC) can result in facial asymmetry, malocclusion, and temporomandibular joint dysfunction. The authors have previously demonstrated a novel method for conservative condylectomy and simultaneous orthognathic surgery for treatment of mandibular condylar OC. The purpose of this study was to evaluate the immediate improvement and long-term stability of mandibular symmetry in the treatment of condylar OC. Fifty-six patients with unilateral mandibular condylar OC combined with secondary facial asymmetry and malocclusion were enrolled in this retrospective study. The computerized tomography (CT) scans were acquired with the mandible in centric relation (CR) before surgery, 1 week and 12 to 18 months after surgery. The images were reconstructed and processed for the analysis with ProPlan CMF 2.1 software. After defining the skeletal landmarks and the reference planes, the chin deviation, chin rotation and mandibular asymmetry index were calculated. The operations and healing were uneventful and the patients showed no signs of recurrence or temporomandibular joint ankylosis during the follow-up. Facial symmetry was greatly improved right after the surgery in the chin deviation (from 9.2 to 1.7 mm, P < 0.01), chin rotation (from 11.2 to 2.3, P < 0.01) and the asymmetry index of three mandibular landmarks (Go, MF and Sg, all P < 0.01). It also showed a stable result after 12 to 18 months follow-up. We also established a novel measurement method, which showed that the combination of conservative condylectomy via the intraoral approach based on intraoperative navigation and simultaneous orthognathic surgery is effective for improving the facial symmetry when treat the mandibular condylar OC.

Abstract:Purpose: Dynamic asymmetry has not been as rigorously evaluated as static asymmetry for patients with skeletal deformity but could well be even more important. The aim of the present study was to evaluate the dynamic facial movement of Class III patients with facial asymmetry using a 3-dimensional (3D) motion capture system. Materials and Methods: The present cross-sectional study recruited patients with skeletal Class III malocclusion with and without facial asymmetry A 3D facial motion capture system was used to record the expression process of a maximal smile. Eleven orofacial landmarks were selected to analyze and calculate the cumulative distance and average speed of each landmark during smiling The predictor variable was mandibular symmetry The outcome variables consisted of the measurements of each soft tissue landmark and the absolute differences for the paired landmarks between 2 sides. Other variables consisted of descriptive data, including the age and gender of each patient. The data were analyzed using independent t tests and paired t tests. Bonferroni's adjustment was used to control for multiple comparisons. Results: A total of 63 patients were divided into 2 groups, an asymmetric group (n = 46) and a control group (n = 17), according to the degree of skeletal deviation. The difference in the cumulative distance of the bilateral cheilions was statistically significant between the 2 groups (P = .002). The difference for the asymmetric and control groups was 2.06 +/- 1.78 mm and 1.00 +/- 0.79 mm, respectively. In the asymmetric group, a comparison of the deviated side with the nondeviated side revealed statistically significant differences in the magnitude of motion for the cheilion (P < .01) and midlateral lower lip (P < .01). Conclusions: The patients with skeletal asymmetry also showed asymmetry in soft tissue functions while smiling The magnitude of movement in the nondeviated side was greater than that in the deviated side. Morphological Changes in the Sternocleidomastoid Muscle Following Functional Radical Neck Dissection.

Abstract:Objective:To investigate the morphological changes in the sternocleidomastoid muscle following functional radical neck dissection. Methods:We assessed the sternocleidomastoid muscle morphology in a cohort of 50 patients who underwent functional radical neck dissection. All patients had oral and maxillofacial cancers and received unilateral neck dissection as part of their primary treatment. Static measurements was performed at three different levels of the muscle,with the neck in the neutral position. Local muscle asymmetry patterns were calculated using the asymmetry index. Results:Significant sternocleidomastoid atrophy was noted on the side of functional radical neck dissection (P0.01). The atrophy was particularly marked in the caudal portions of the sternocleidomastoid muscle. Conclusion:Sternocleidomastoid atrophy was likely related to damage to the segmental blood supply in the caudal portions of the muscle as well as injury to the innervation.

Abstract:OBJECTIVE:To investigate the postoperative changes of the head tilt test in patients with unilateral congenial superior oblique palsy.METHODS:Thirty-eight cases with congenital unilateral superior oblique palsy underwent the ipsilateral inferior oblique or/and contralateral inferior rectus weakening procedure. Bielschowsky head tilt test was evaluated quantitatively before and after the operation by the prism cover test while the patient fixated at 33 cm and 6 m on the small light target. A positive Bielschowsky head tilt test was defined as one in which the vertical deviation when the head tilt to the side of the paresis was at least 5.0(Delta) greater than that on tilt to the uninvolved side.RESULTS:Before the operation, the hyperdeviation of the involved eye was 3.0(Delta) - 25.0(Delta) (average 11.0(Delta)) in primary position. The deviation on tilt to the side of the paresis was 23.0(Delta) (15.0(Delta) - 40.0(Delta)) and the deviation on tilt to the uninvolved side was 2.5(Delta) (0.0(Delta) - 10.0(Delta)). A positive Bielschowsky tilt test was found in all cases preoperatively. After the operation, the hyperdeviation of the involved eye was 0.0(Delta) - 5.0(Delta) (average 2.0(Delta)) in primary position. The result of Bielschowsky tilt test was still positive in 34 cases and negative in only 4 cases.CONCLUSION:In most cases with unilateral superior oblique paresis, the results of Bielschowsky tilt test do not change to negative postoperatively. The significance of using this test for evaluating surgical results remains to be determined.
Different Surgical Effect of Congenital Superior Oblique Muscle Paralysis

Abstract:Objective To evaluate the efficacy and indication of different surgical procedures for the congenital superior oblique muscle paralysis.Methods Sixty-five cases(seventy-five eyes) of congenital superior oblique muscle paralysis received different surgical procedures based on the degrees of the overaction of superior oblique muscle and the extension of vertical deviation.The degrees of 25 cases(29 eyes) were lower than 15△,inferior oblique muscle myotomy was chosen.When the degrees of deviation were between 15△and 20△,15 cases(15 eyes) using inferior oblique muscle myotomy and anterior transposition.When the degrees of deviation were 20△～25△,16 cases(22 eyes) using inferior oblique muscle myotomy and anterior transposition and augmentation.When the degrees of 9cases(9 eyes) were higher than 25△,inferior oblique muscle myotomy and inferior rectus muscle recession of the other eye were performed.Results Forty-two cases(forty-nine eyes) were cured(65.3%),21 cases(24 eyes) improved(32%)and 2 cases were unefficient(2.7%).Conclusion To weaken inferior oblique muscle,the surgica1 procedure of inferior oblique muscle myotomy and anterior transposition can solve more degrees of deviation than the procedure of inferior oblique muscle myotomy.And the surgical procedure of inferior oblique muscle myotomy and anterior transposition and augmentation can solve more degrees than the procedure of inferior oblique muscle myotomy and anterior transposition.Choosing suitable operation methods according to the degree of hyper deviation can get a good clinical effect to treat the congenital superior oblique paralysis.Postoperative complications can be avoided by master operating skill.

Abstract:Objective: Unilateral condylar hyperplasia (UCH) presents challenges with functional and esthetic concerns, including facial and chin asymmetry, prognathism, class III malocclusion, and temporomandibular joint symptoms. This study aims to precisely locate condylar and mandibular asymmetry in patients with UCH, aiding targeted surgical correction and addressing persistent skeletal changes. Methods: This retrospective study analyzed data from patients with UCH and controls, utilizing high-quality cone beam computed tomography scans with 1 mm or fewer slices. Morphometric points were placed using Planmeca software, measuring linear distances on both affected and unaffected sides. Results: In a cohort of 40 patients (29 UCH, 11 controls), significant differences in condylar head and neck lengths were observed between the affected and unaffected sides in patients with UCH ( P = 0.0019). However, ramus length showed no significant variation ( P = 0.65). When comparing differences in condylar head and neck length between controls and patients with UCH, a substantial contrast was evident ( P = 1.98e-07), while ramus length differences were not significant ( P = 1.00). The median condylar head and neck lengths were 20.3 mm for controls, 19.78 mm for the unaffected side in patients with UCH, and 23.48 mm for the affected side. Conclusion: Facial asymmetry in UCH primarily results from differences in condylar head and neck length, not ramus length. This emphasizes the importance of high condylectomy in addition to bilateral sagittal split osteotomy to achieve improved symmetry and reduced temporomandibular joint dysfunction. Understanding these regional skeletal changes is critical for effective UCH treatment.
