Abstract:Purpose. In this study, we aimed to develop and validate a noninvasive method based on radiomics to evaluate the expression of Ki67 and prognosis of patients with non-small-cell lung cancer (NSCLC). Patients and Methods. A total of 120 patients with NSCLC were enrolled in this retrospective study. All patients were randomly assigned to a training dataset (n = 85) and test dataset (n = 35). According to the preprocessed F-FDG PET/CT image of each patient, a total of 384 radiomics features were extracted from the segmentation of regions of interest (ROIs). The Spearman correlation test and least absolute shrinkage and selection operator (LASSO), after normalization on the features matrix, were applied to reduce the dimensionality of the features. Furthermore, multivariable logistic regression analysis was used to propose a model for predicting Ki67. The survival curve was used to explore the prognostic significance of radiomics features. Results. A total of 62 Ki67 positive patients and 58 Ki67 negative patients formed the training set and test training dataset and test dataset. Radiomics signatures showed good performance in predicting the expression of Ki67 with AUCs of 0.86 (training dataset) and 0.85 (test dataset). Validation and calibration showed that the radiomics had a strong predictive power in patients with NSCLC survival, which was significantly close to the effect of Ki67 expression on the survival of patients with NSCLC. Conclusion. Radiomics signatures based on preoperative F-FDG PET/CT could distinguish the expression of Ki67, which also had a strong predictive performance for the survival outcome.

What problem does this paper attempt to address?
This paper aims to develop and validate a non - invasive radiomics - based method for evaluating Ki67 expression and prognosis in patients with non - small - cell lung cancer (NSCLC). Specifically, the researchers hope to predict the Ki67 expression in NSCLC patients by analyzing the radiomics features in 18F - FDG PET/CT images and explore the predictive value of these features for the survival prognosis of patients. ### Research Background - **Non - small - cell lung cancer (NSCLC)**: It is the most common type of lung cancer, accounting for 85% of all lung cancer patients. Due to the lack of specific clinical symptoms in the early stage, many patients have already had lymph node metastasis or distant metastasis at the time of diagnosis, resulting in poor treatment effects and prognosis. - **Ki67**: It is a nuclear protein expressed in the active phase of the cell cycle, and its expression level is often used as a marker of cell proliferation. High Ki67 expression is negatively correlated with disease - free survival, recurrence - free survival and overall survival in NSCLC patients. - **Radiomics**: It is a new medical imaging research method. By extracting and analyzing the quantitative features in medical images, it can predict the biological behavior of tumors and the prognosis of patients. ### Research Objectives - **Develop a non - invasive method**: Use the radiomics features of 18F - FDG PET/CT images to establish a model for predicting Ki67 expression and survival prognosis in NSCLC patients. - **Verify the effectiveness of the model**: Verify the predictive performance of the model through the data of the training set and the test set. ### Methods - **Data collection**: Retrospectively collected data from 120 NSCLC patients and randomly divided them into a training set (85 cases) and a test set (35 cases). - **Image processing**: Use the PyRadiomics tool to extract 384 radiomics features, including first - order and high - order features, from the pre - processed PET/CT images. - **Feature selection**: Screen out important radiomics features through Pearson correlation coefficient (PCC) and least absolute shrinkage and selection operator (LASSO) methods. - **Model establishment**: Use multivariate logistic regression analysis to establish a model for predicting Ki67 expression, and evaluate the predictive performance of the model through the receiver operating characteristic curve (ROC) and calibration curve. ### Results - **Feature selection**: Finally, 6 important radiomics features were screened out. - **Model performance**: In the training set and the test set, the AUC of the model was 0.86 and 0.85 respectively, showing good predictive performance. - **Survival analysis**: Positive Ki67 expression was associated with better survival prognosis, and the R - score also showed similar prognostic stratification ability. ### Conclusion The radiomics features based on 18F - FDG PET/CT images can effectively distinguish the Ki67 expression in NSCLC patients and have strong survival prediction ability. This method not only reduces the economic burden and physical harm of patients, but also provides a new means for the precise management of NSCLC patients.
