Abstract:Objective To evaluate the efficacy and safety of endoscopic submucosal dissection for the treatment of colorectal large laterally spreading tumor. Methods ESD was applied to treat 150 cases of colorectal LST with diameter larger than 4 cm. The morphological features of LST, distribution, the clinicopathological data and the en-bloc resection rate, complete resection rate, complications were retrospectively evaluated. Results There were 87 patients with LST-granular lesions and 63 patients with LST-nongranular lesions. Colorectal LST mainly distributed in the rectum for 109 cases (72.7%), sigmoid colon for 13 cases (8.7%), descending colon for 5 cases (3.3%), transverse colon for 8 cases (5.3%), ascending colon for 13 cases (8.7%), cecum for 2 cases (1.3%). There were 23 patients with low-grade neoplasia, 104 patients with high-grade intraepithelial neoplasia, 7 with intramucosal carcinoma and 16 with submucosal carcinoma. The en-bloc resection rate and complete resection rate were 92.7% (139/150) and 89.3%(134/150). Adverse events were intra-operative bleeding in 12 patients (8.0%), postoperative bleeding in 2 patients (1.3%), perforation in 3 patients (2.0%), postoperative stenosis in 3 patients (2.0%). Conclusion Colorectal large LST-NG has higher potential for malignancy. ESD is a safe and effective method to provide en-bloc and complete resection of colorectal large LST.
Clinical Outcomes of Endoscopic Submucosal Dissection for Laterally Spreading Tumors Involving the Dentate Line

Abstract:ObjectiveEndoscopic submucosal dissection (ESD) for laterally spreading tumors (LST) involving the dentate line (LST‐DL) is challenging because of the specific anatomical features of the anorectum. This study aimed to evaluate the efficacy and safety of ESD for LST‐DL.MethodsConsecutive patients with LST‐DL who had undergone ESD at our hospital between January 2010 and December 2015 were retrospectively enrolled in this study. Rates of en bloc resection, R0 resection, and complications, pathological characteristics, and tumor recurrence were analyzed and compared with those of LST in the rectum not involving the dentate line (LST‐NDL).ResultsAltogether 49 patients with LST‐DL (median age 63 years; 39 women; median lesion size 57 mm; median follow‐up period of 24 months) and 96 patients with LST‐NDL (median age 67 years; 31 women; median lesion size 47 mm; median follow‐up period of 31 months) were enrolled. En bloc resection (93.9% [46/49] vs 94.8% [91/96]) and en bloc R0 resection rates (83.7% [41/49] vs 88.5% [85/96]), respectively, for LST‐DL and LST‐NDL, with no significant differences. However, ESD for LST‐DL had a longer procedure time (77 min vs 54 min, P = 0.02), a greater postprocedural perianal pain rate (28.6% vs 0%, P < 0.001), and more anal strictures (4.1% vs 0%, P = 0.04). The complication rates of perforation, bleeding and fever, recurrence rate, and pathological characteristics did not differ between the two groups.ConclusionsESD is a safe and effective therapeutic modality for LST‐DL. However, this procedure should be performed by experienced endoscopists and the difficulty needs to be fully considered.

Abstract:Objective Endoscopic submucosal tunnel resection (ESTD) is defined as the endoscopic operation finished in the tunnel established between the mucosa and muscle layer in the digestive tract. The purpose of this paper is to evaluate the application of ESTD in the upper gastrointestinal tumor resection. Methods Eligible clinical reports about ESTD for the treatment of upper gastrointestinal tumors were identified from the database (2007-2014). The operation techniques, clinical application and complications were systematically analyzed. Results A total of 16 studies involving 196 cases (33 with malignant lesions, 163 benign) were included. The longest diameters of most tumors were less than 3 cm. The complete resection rate and whole block excision rate were close to 100%. Perforation was one of the common complications. The incidences of intraoperative perforation fluctuated between 0 and 100%, depending on the tumor size, tumor location, etc. However, there was no postoperative continuous gastrointestinal perforation. Serious complications such as peripheral viscera injury, bleeding, severe mediastinum infection were not reported in the studies. Conclusions ESTD is an effective, safe and minimally invasive surgical technique in the resection of upper gastrointestinal submucosal benign lesions. It may yield similar outcome with surgical treatment when the longest diameter is less than 3 cm. Abstract:Objective This study aims to investigate the clinicopathological features of rectal laterally spreading tumor( LST) and assess the effect and safety of endoscopic submucosal dissection( ESD) for treating rectal LSTs.Methods Data of 120 cases of rectal LSTs,which were treated by ESD in endoscopy center in Zhongshan Hospital,Fudan University from April 2008 to January 2012,were collected.All LSTs were divided into different types according to surface morphology under endoscopic view.Location,size and morphological characteristics of each subtype of LST were studied.Clinicopathological features of LST, resection rate of ESD,procedure time,complications and recurrence rate were analyzed.Results Among 120 LSTs in the rectum,84 cases were LST-G(84/120,70%),36 cases were LST-NG(36/120,30%).Average size of the lesion was 35 mm.Villioustublar adenoma accounted for 38.3%, high grade intraepithelial neoplasia 28.3%and submucosal carcinoma 7.5%.The average procedure time was 51.8 ±28.16(17-110) min.En bloc resection rate was 98.3%, complete resection rate was 95.0% and en bloc R0 curative resection rate was 90.8%.Bleeding after ESD developed in 5 cases(4.2%),perforation developed in 4 cases(3.3%).The average time of the follow-up was 32.6 months,recurrence rate was 0.8%.Conclusions High malignant potential existed in LSTs, which were lager than 2 cm.ESD was an effective and safe treatment for rectal LSTs for its high en bloc resection rate and low recurrence rate.
Efficacy Observation of Endoscopic Submucosal Dissection in Rectal Laterally Spreading Tumors

Abstract:Objective:To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) in the treatment of rectal laterally spreading tumors (LSTs) .Methods:The clinical data of 20 patients with LSTs ,who were admitted in Jiangyin Tradi‐tional Chinese Medicine Hospital from September 2012 to September 2014 ,were retrospectively analyzed .According to the su‐perficial morphology of rectal mucosa ,the subtypes of LSTs were determined by endoscopy .Tumor size and morphological characteristics ,pathological features and operation time of ESD were counted .Furthermore ,postoperative complications and recurrence were followed up and statistically analyzed .Results:There were 8 (40% ) cases of LST granular type and 12 (60% ) cases of LST nongranular type .There were 18 cases of adenomas associated with low grade neoplasia and 2 (10% ) cases of high grade intraepithelial neoplasia .There were 7 cases of tubulovillous adenoma and 13 cases of tubular adenoma .The mean diameter of lesions was (33 .9 ± 11 .5) mm .The enbloc resection rate was 100% and the average time of ESD operations was (40 ± 15) min .The postoperative bleeding rate was 10% (2/20) and no perforation occurred .The mean follow‐up period was 12 .6 months ,during which no recurrence was found .Conclusions:With high enbloc resection rate and low complication rate ,ESD is a safe and efficacious treatment for rectal LSTs ,especially for the lesions larger than 2 cm which have higher potential of malignancy .

Abstract:Objective To investigate the clinicopatholgical features of colorectal laterally spreading tumor (LST) with a diameter ＞ 2 cm,and assess the efficacy of endoscopic submucosal dissection (ESD) for the treatment of colorectal LST.Methods The clinical data of 151 patients with colorectal LST who were admitted to the Zhongshan Hospital from December 2007 to April 2011 were retrospectively analyzed.The distribution,size and morphological features of LST were recorded,and the clinicopathological data and resection rate,operation time,complications and recurrent rate of ESD were analyzed.The coherent rate of the LST subtype analysis was analyzed by the κ correlation coefficient test.Results Colorectal LST mainly distributed in the rectum (58 cases),and then followed by ascending colon (47 cases),cecum (15 cases),transverse colon (14 cases),sigmoid colon (13 cases) and descending colon (4 cases).The diameter of the tumors was (3.4 ± 1.2) cm.There were 105 patients with LST-granular lesions,including 75 patients with nodular mixed type and 30 with homogenous type;46 with LST-nongranular lesions,including 31 patients with elevated type and 15 with pseudodepressed type.The κ value of the 2 types coherence was 0.87,and the κ value of 4 subtypes coherence was 0.80.There were 96 patients with low-grade neoplasia,44 with high-grade intraepithelial neoplasia and 11 with submucosal neoplasia.Fifty-eight patients were with tubulovillous adenoma,33 with tubular adenoma,3 with villous adenoma,and 2 with serrated lesions.The en-bloc resection rate,complete resection rate and complete curative resection rate were 98.01％ (148/151),94.70％ (143/151) and 90.73％ (137/151),respectively.The operation time was (52 ± 31) minutes.The postoperative bleeding rate and perforation rate were 3.97％ (6/151) and 3.31％ (5/151),respectively.The mean time of follow-up was 28.6 months,and the recurrent rate was 0.66％ (1/151).Conclusions Colorectal LST with a diameter ＞ 2 cm has higher potential for malignancy.ESD is safe and feasible for the treatment of colorectal LST with advantages of high resection rate and low recurrent rate.

Abstract:Objective To analyze the short-term outcomes of patients undergoing endoscopic submucosal dissection (ESD) for superficial colorectal neoplasms, and to identify predictors of malignant pathology of these lesions. Methods ESD was performed in 111 patients with 152 high-risk colorectal tumors [ ≥ 10 mm in diameter or laterally spreading tumor (LST) morphology]at the Peking University Cancer Hospital. We retrospectively reviewed the patient records, therapeutic outcomes, and procedure-related adverse events. We used univariate and multivariate analyses to identify the risk factors for malignancy. Results There were 78 adenocarcinomas and 74 adenomas. The complete resection rates were 95.4％ and 91.0％ for all and malignant cases, respectively. Adverse events were confirmedor highly suspected in seven (4.6％) cases. As to the biopsy pathology,thefindings were consistent with the postoperative pathology in 99 (65.1％) lesions. The majority of malignancies (67.9％) were diagnosed as adenomas before resection. Older patients were more susceptible to developing malignant lesions (mean age, 64.4 vs 59.8 years in the carcinoma and adenoma group, respectively, P < 0.001), and the tumor size was significantly larger in the malignant group (2.13 cm vs 1.45 cm, P=0.009). The proportions of malignant tumors were 67.5％, 49.0％, and 42.4％ for rectal, left colon, and right colon lesions, respectively (P=0.045). Moreover, a significant difference was found according to morphology, with 63.2％ of LSTs diagnosed as malignancies, compared to only 41.6％ of protruding tumors (P=0.008). Conclusion ESD is a generally safe and effective procedure for treating colorectal neoplasms when performed by experienced endoscopists. Biopsy is not always reliable for the diagnosis. Several factors such as old age, large size, rectal location, and LST morphology are related to malignant pathology. For tumors with these high-risk factors, treatment with ESD instead of endoscopic mucosal resection should be considered to achieve enbloc resection.

Though these tumors are relatively neglected and were previously considered to be benign, they do have malignant potential. Knowledge regarding the nature of SMTs presented therapic dilemmas in the past, but with the development of digestive endoscopic technology, more gastrointestinal SMTs can be diagnosed and treated. Clinical evaluation of pre-cut-endoscopic mucosal resection in treatment of colorectal lateral spreading tumor

Abstract:Objective To evaluate the effect of pre-cut-endoscopic mucosa resection of colorectal laterally spreading tumor. Methods 65 patients with LST were enrolled from January 2014 to February 2014. LST was detected by chromoendoscopy and NBI combined with magnifying endoscopy technique. The size, site, morphological features, were observed and the histopathological features of the specimen of LST was analyzed. All the 65 LSTs were resect by pre-cut-EMR. The clinical results including enbloc resection rate, all bloc resection rate, procedure time, complication and recurrence rates were retrospectively evaluated. Results All the 65 LSTs lesions ranged from 2.0 cm to 5.0 cm, with a mean diameter of (2.4 ± 1.7) cm. The site of 65 LSTs was in rectum 28 (43.1 %), 11 LSTs in sigmoid colon (16.9 %), 6 LSTs in descending colon (9.2 %), 2 LSTs in splenic flexure of colon (3.1 %), 9 LSTs in transverse colon (13.8 %), 4 LSTs in Hepatic flexure of colon (6.2 %), 2 LSTs in ascending colon(3.1 %), and 3 LSTs in cecum (4.6 %). Morphology of 23 LSTs were homogeneous granular type (35.4 %), 27 LSTs were mixed non-granular type (41.5 %), 13 LSTs were flat elevated type (20.0 %), and 2 LSTs were pseudo-depressed type (3.1 %). The histopathological diagnoses of LST included 12 tubular adenoma (18.5 %), 19 villous-tubular adenoma (29.2 %), 26 villous adenoma (40.0 %), 7 advanced intraepithelial tumor (10.7 %), 1 intramucosal carcinoma (1.5 %). Enbloc resection was achieved in 65 patients (100.0 %) with a mean operation time of (18.0 ± 11.7) min. 5 cases were bleeding during the operation (7.7 %), 1 case was bleeding 7 days after operation (1.5 %), no perforation was happened. 65 patients were followed up for 3 ~ 12 months, and no local recurrence was found. Conclusion Pre-cut-endoscopic mucosal resection an effective and safe therapy for colorectal LST larger than 2.0 cm.
Endoscopic Mucosal Resection Versus Endoscopic Submucosal Dissection for Colorectal Laterally Spreading Tumors: a Meta-Analysis.

Abstract:OBJECTIVETo assess the efficacy and safety of endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) in the treatment of colorectal laterally spreading tumors (LSTs).METHODSA systematic literature search was performed in Pubmed, Embase, the Cochrane Library, CNKI and WANFANG databases. The related literatures were selected according to certain inclusion and exclusion criteria. The Cochrane Collaboration's Revman 5.3 software was used for data analysis.RESULTSA total of 12 studies were included in the analysis. The total number of lesions was 3062 (EMR: 1906; ESD: 1156). The en bloc resection rate of ESD was 95% (1098/1156), which was significantly higher than that of EMR (42.8%, 815/1906) [OR=0.07, 95% CI (0.02, 0.07), P<0.00001]. The complete resection rate of ESD was 93.2% (109/117), which was significantly higher than that of EMR as well (71.9%, 92/128) [OR=0.12，95% CI (0.05, 0.29), P<0.00001]. The bleeding rate showed no significant difference between EMR and ESD (4.2% vs. 3.5%) [OR=1.04, 95% CI (0.68, 1.60), P=0.85]. The perforation rates of EMR and ESD were 1.8% and 2.4%, respectively, which displayed significant difference [OR=0.56, 95% CI (0.32,0.97), P=0.04]. Nevertheless, the recurrence rate of EMR was significantly higher than that of ESD (15.9% vs. 0.5%) [OR=23.06, 95% CI (11.11, 47.85), P<0.00001].CONCLUSIONSEndoscopic resection of LSTs is safe and effective. As compared with EMR, ESD has higher en-bloc and complete resection rates but a lower recurrence rate. Therefore, ESD is highly recommended for treatment of LSTs.

Abstract:The increasing popularity of endoscopic submucosal dissection (ESD) as a treatment for early gastric cancer has highlighted the importance of quality assessment in achieving curative resections. This article emphasizes the significance of evaluating ESD quality, not only for curative cases but also for non-curative ones. Postoperative assessment relies on the endoscopic curability (eCura) classification, but management strategies for eCuraC-1 tumour with a positive horizontal margin are unclear. Current research primarily focuses on comparing additional surgical procedures in high-risk patients, while studies specifically targeting eCuraC-1 patients are limited. Exploring management strategies and follow-up outcomes for such cases could provide valuable insights. Furthermore, the application of molecular imaging using near-infrared fluorescent tracers holds promise for precise tumour diagnosis and navigation, potentially impacting the management of early-stage gastric cancer patients. Advancing research in these areas is essential for improving the overall efficacy of endoscopic techniques and refining treatment indications.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background and Aims</h3><p>Endoscopic submucosal dissection (ESD) is a promising technique for removing superficial gastrointestinal tumors, but ESD is technically difficult. The aims of this study were to establish a clinical score model for grading technically difficult colorectal ESD (CR-ESD).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Data on patients, lesions, and outcomes of CR-ESD at 2 centers were analyzed. The objective parameter of successful ESD within 60 minutes was set as an end point to evaluate the difficulty. Independent predictors of difficulty in the derivation cohort were identified by multiple logistic regression analysis and used to develop a clinical score. We validated the score model in the validation cohort.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The clinical score comprised the following: tumor size of 30 to 50 mm (1 point) or ≥50 mm (2 points); ≥2/3 circumference of the lesion (2 points); location in the cecum (1 point); flexure (2 points) or dentate line (1 point); and laterally spreading tumor non-granular lesions (1 point). Areas under receiver-operator characteristic curves for the score model were comparable (derivation [0.70] vs internal validation [0.69] vs external validation [0.69]). The probability of successful ESD within 60 minutes in easy (score = 0), intermediate (score = 1), difficult (score = 2-3), and very difficult (score ≥4) categories were 75.0%, 51.3%, 35.6%, and 3.4% in the derivation cohort, 73.3%, 47.9%, 31.8%, and 16.7% in the internal validation cohort, and 79.5%, 66.7%, 43.3%, and 20.0% in the external validation cohort, respectively.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>This clinical score model accurately predicts the probability of successful ESD within 60 minutes and can be applied to grade the technical difficulty before the procedure.</p>

Abstract:Endoscopic resection of colonic submucosal tumors (SMTs) is challenging and carries a high risk of perforation. In this prospective pilot study we aimed to evaluate the feasibility, safety, and efficacy of endoscopic full-thickness resection of colonic SMTs. The study enrolled 19 consecutive patients with colonic SMTs 3 cm in size or smaller. In 18 of the 19 patients full-thickness resection was carried out endoscopically with the entire tumor capsule intact. In 16 of these18 patients, full-thickness resection was carried out and the colonic wall defect closed all endoscopically; in the other 2 patients, laparoscopic closure was needed. Two cases of local peritonitis were managed with conservative treatment. No deaths occurred within 30 days, and no recurrence was detected after a median of 18 months' follow-up. Endoscopic full-thickness resection is a novel method enabling resection of colonic SMTs. The colonic wall mucosal defect can be closed endoscopically in the majority of cases. It appears to be a safe and effective endoscopic technique for managing these tumors, which traditionally are managed by colonic resection.

Abstract:PURPOSE: The objective of this study was to determine the efficacy and safety of endoscopic submucosal dissection for locally recurrent colorectal cancer after previous endoscopic mucosal resection. METHODS: A total of 16 patients with locally recurrent colorectal lesions were enrolled. A needle knife, an insulated-tip knife and a hook knife were used to resect the lesion along the submucosa. The rate of the curative resection, procedure time, and incidence of complications were evaluated. RESULTS: Of 16 lesions, 15 were completely resected with endoscopic submucosal dissection, yielding an en bloc resection rate of 93.8 percent. Histologic examination confirmed that lateral and basal margins were cancer-free in 14 patients (87.5 percent). The average procedure time was 87.2 +/- 60.7 minutes. None of the patients had immediate or delayed bleeding during or after endoscopic submucosal dissection. Perforation in one patient (6.3 percent) was the only complication and was managed conservatively. The mean follow-up period was 15.5 +/- 6.8 months; none of the patients experienced lesion residue or recurrence. CONCLUSIONS: Endoscopic submucosal dissection appears to be effective for locally recurrent colorectal cancer after previous endoscopic mucosal resection, making it possible to resect whole lesions and provide precise histologic information.

Abstract:Endoscopic submucosal tunnel dissection (ESTD) has been used for dissection of esophageal and gastric lesions. However, outcomes of ESTD for large lesions in the lesser gastric curvature had not been acknowledged because previous reports had the limitations of being single-center studies. We aimed to clarify the outcomes of ESTD for large lesser gastric curvature superficial neoplasms and provide our experience to accelerate its application.
Predictive factors for technically difficult endoscopic submucosal dissection in large colorectal tumors

Abstract:Background/aims: Endoscopic submucosal dissection (ESD) for colorectal tumors is dangerous, particularly those that are large. However, the technical difficulty in resecting large tumors in the colonrectum has seldom been investigated. Materials and methods: Between October 2012 and January 2015, 36 consecutive large colorectal tumors were resected by ESD at the endoscopic center of PLA Army General Hospital. Five factors were investigated in predicting the technical difficulty in resecting such tumors. Results: En bloc resection, complete (R0) resection, and curative resection rates were 83.33% (30/36), 80.56% (29/36), and 77.78% (28/36), respectively. Tumor location in a flexure was risk a factor for difficult ESD in the colonrectum as measured by perforation (4.55, 0.09-6.25), non-en bloc resection (4.94, 0.10, 9.45), and dissection speed (β±SE: 1.75±0.05). When tumor size increased, the perforation rate also increased (9.93, 0.96-10.32). Conclusion: ESD was more technically demanding in flexures for resecting large colorectal tumors, and for large tumor effective technique to close perforation is essential. Our study will guide endoscopists in using ESD to remove large colorectal tumors.
