Abstract:Spatial approaches to epidemiological research with big social media data provide tremendous opportunities to study the relationship between the socio-ecological context where these data are generated and health indicators of interest. Such research poses a number of ethical challenges, particularly in relation to issues such as privacy, informed consent, data security, and storage. While these issues have received considerable attention by researchers in relation to research for physical health purposes in the past 10 years, there have been few efforts to consider the ethical challenges of conducting mental health research, particularly with geo-referenced social media data. The aim of this article is to identify strengths and limitations of current recommendations to address the specific ethical issues of geo-referenced tweets for mental health research. We contribute to the ongoing debate on the ethical implications of big data research and also provide recommendations to researchers and stakeholders alike on how to tackle them, with a specific focus on the use of geo-referenced data for mental health research purposes. With increasing awareness of data privacy and confidentiality issues (even for non-spatial social media data) it becomes crucial to establish professional standards of conduct so that compliance with ethical standards of conducting research with health-related social media data can be prioritized and easily assessed.

Legal, ethical, and wider implications of suicide risk detection systems in social media platforms

Abstract:Suicide remains a problem of public health importance worldwide. Cognizant of the emerging links between social media use and suicide, social media platforms, such as Facebook, have developed automated algorithms to detect suicidal behavior. While seemingly a well-intentioned adjunct to public health, there are several ethical and legal concerns to this approach. For example, the role of consent to use individual data in this manner has only been given cursory attention. Social media users may not even be aware that their social media posts, movements, and Internet searches are being analyzed by non-health professionals, who have the decision-making ability to involve law enforcement upon suspicion of potential self-harm. Failure to obtain such consent presents privacy risks and can lead to exposure and wider potential harms. We argue that Facebook's practices in this area should be subject to well-established protocols. These should resemble those utilized in the field of human subjects research, which upholds standardized, agreed-upon, and well-recognized ethical practices based on generations of precedent. Prior to collecting sensitive data from social media users, an ethical review process should be carried out. The fiduciary framework seems to resonate with the emergent roles and obligations of social media platforms to accept more responsibility for the content being shared.

Abstract:Social media are a rich source of insight for data mining and user-centred research, but the question of consent arises when studying such data without the express knowledge of the creator. Case studies that mine social data from users of online services such as Facebook and Twitter are becoming increasingly common. This has led to calls for an open discussion into how researchers can best use these vast resources to make innovative findings while still respecting fundamental ethical principles. In this position paper we highlight some key considerations for this topic and argue that the conditions of informed consent are often not being met, and that using social media data that some deem free to access and analyse may result in undesirable consequences, particularly within the domain of health research and other sensitive topics. We posit that successful exploitation of online personal data, particularly for health and other sensitive research, requires new and usable methods of obtaining consent from the user.

Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aim:Platforms on social media are increasingly used for public health research. While social media provides an exceptional opportunity to explore communication about public health topics, this practice is not without ethical dilemmas. Our aim was to identify and unfold some of these dilemmas and to suggest possible solutions and ways forward for future research.Methods:Using our own research within a closed forum for people experiencing suicidal thoughts as a case, we explored certain dilemmas and possible answers relating to whether what is to be researched falls under a public or private social media domain; we investigated avenues for obtaining access to participants in an evolving online environment; how to secure informed consent from participants; and ways of ensuring anonymity.Results:We provide recommendations and reflections that we hope will offer inspiration for researchers embarking on similar social media public health research within and beyond suicide research.Conclusions:The ethical framework commonly referred to in health research, based on confidentiality, anonymity, informed consent and doing no harm must be adjusted to be relevant for a social media context where technologies and regulations are constantly being altered.

Abstract:The growing use of social media field experiments demands a rethink of current research ethics in computational social science and psychological research. Here, we provide an exploratory empirical account of key user concerns and outline a number of critical discussions that need to take place to protect participants and help researchers to make use of the novel opportunities of digital data collection and field studies. Our primary contention is that we need to elicit public perceptions to devise more up-to-date guidelines for review boards whilst also allowing and encouraging researchers to arrive at more ethical individual study design choices themselves. To ground our discussion in real-world examples of online experiments, we focus on recent social media studies in the field of misinformation, polarization, and hate speech research. We conclude by discussing how we can better strike a balance between meeting ethical guidelines and the concerns of social media users alongside maximizing scientific impact and credibility.

Abstract:This study investigates the complex relationship between social media usage and mental well-being across different age groups in the United States. Employing a systematic literature review and content analysis, the research aims to unravel the multi-dimensional impacts of online platforms on mental health. The methodology involves a comprehensive search of peer-reviewed journals, conference proceedings, and digital libraries, focusing on studies conducted from 2000 to the present. Inclusion and exclusion criteria are meticulously applied to ensure the relevance and quality of the selected literature. Key findings reveal that the impact of social media on mental health is nuanced and varies significantly across age groups. Positive aspects such as enhanced connectivity and community support coexist with negative impacts like exposure to cyberbullying, addiction, and exacerbation of mental health issues. The study highlights the importance of considering psychological, social, and cultural dimensions in understanding these impacts. The future landscape of social media and mental health presents both challenges and opportunities. The digital age necessitates adaptive strategies to mitigate the risks associated with social media use while leveraging its potential benefits. Recommendations are provided for industry leaders, healthcare professionals, and policymakers, emphasizing the need for safer social media platforms, integration of social media literacy in healthcare, and robust regulatory frameworks. Conclusively, this study contributes to the understanding of social media's role in mental health, suggesting the need for ongoing research, particularly in exploring causal relationships and developing age-specific and culturally sensitive interventions. The findings underscore the importance of a collaborative approach among various stakeholders to foster a healthier digital environment.

Abstract:Social media platforms are popular venues for sharing personal experiences, seeking information, and offering peer-to-peer support among individuals living with mental illness. With significant shortfalls in the availability, quality, and reach of evidence-based mental health services across the United States and globally, social media platforms may afford new opportunities to bridge this gap. However, caution is warranted, as numerous studies highlight risks of social media use for mental health. In this commentary, we consider the role of social media as a potentially viable intervention platform for offering support to persons with mental disorders, promoting engagement and retention in care, and enhancing existing mental health services. Specifically, we summarize current research on the use of social media among mental health service users, and early efforts using social media for the delivery of evidence-based programs. We also review the risks, potential harms, and necessary safety precautions with using social media for mental health. To conclude, we explore opportunities using data science and machine learning, for example by leveraging social media for detecting mental disorders and developing predictive models aimed at characterizing the aetiology and progression of mental disorders. These various efforts using social media, as summarized in this commentary, hold promise for improving the lives of individuals living with mental disorders.
Exploring the Evolution of Social Media in Mental Health Interventions: A Mapping Review

Abstract:Background: With the rise of social media, social media use for delivering mental health interventions has become increasingly popular. However, there is no comprehensive overview available on how this field developed over time. Objectives: The objective of this paper is to provide an overview over time of the use of social media for delivering mental health interventions. Specifically, we examine which mental health conditions and target groups have been targeted, and which social media channels or tools have been used since this topic first appeared in research. Methods: To provide an overview of the use of social media for mental health interventions, we conducted a search for studies in four databases (PubMed; ACM Digital Library; PsycInfo; and CINAHL) and two trial registries (Clinicaltrials.gov; and Cochranelibrary.com). A sample of representative keywords related to mental health and social media was used for that search. Automatic text analysis methods (e.g., BERTopic analysis, word clouds) were applied to identify topics, and to extract target groups and types of social media. Results: A total of 458 studies were included in this review (n=228 articles, and n=230 registries). Anxiety and depression were the most frequently mentioned conditions in titles of both articles and registries. BERTopic analysis identified depression and anxiety as the main topics, as well as several addictions (including gambling, alcohol, and smoking). Mental health and women's research were highlighted as the main targeted topics of these studies. The most frequently targeted groups were "adults" (39.5%) and "parents" (33.4%). Facebook, WhatsApp, messenger platforms in general, Instagram, and forums were the most frequently mentioned tools in these interventions. Conclusions: We learned that research interest in social media-based interventions in mental health is increasing, particularly in the last two years. A variety of tools have been studied, and trends towards forums and Facebook show that tools allowing for more content are preferred for mental health interventions. Future research should assess which social media tools are best suited in terms of clinical outcomes. Additionally, we conclude that natural language processing tools can help in studying trends in research on a particular topic.
Reporting of Ethical Considerations in Qualitative Research Utilizing Social Media Data on Public Health Care: Scoping Review

Abstract:Background: The internet community has become a significant source for researchers to conduct qualitative studies analyzing users' views, attitudes, and experiences about public health. However, few studies have assessed the ethical issues in qualitative research using social media data. Objective: This study aims to review the reportage of ethical considerations in qualitative research utilizing social media data on public health care. Methods: We performed a scoping review of studies mining text from internet communities and published in peer-reviewed journals from 2010 to May 31, 2023. These studies, limited to the English language, were retrieved to evaluate the rates of reporting ethical approval, informed consent, and privacy issues. We searched 5 databases, that is, PubMed, Web of Science, CINAHL, Cochrane, and Embase. Gray literature was supplemented from Google Scholar and OpenGrey websites. Studies using qualitative methods mining text from the internet community focusing on health care topics were deemed eligible. Data extraction was performed using a standardized data extraction spreadsheet. Findings were reported using PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews) guidelines. Results: After 4674 titles, abstracts, and full texts were screened, 108 studies on mining text from the internet community were included. Nearly half of the studies were published in the United States, with more studies from 2019 to 2022. Only 59.3% (64/108) of the studies sought ethical approval, 45.3% (49/108) mentioned informed consent, and only 12.9% (14/108) of the studies explicitly obtained informed consent. Approximately 86% (12/14) of the studies that reported informed consent obtained digital informed consent from participants/administrators, while 14% (2/14) did not describe the method used to obtain informed consent. Notably, 70.3% (76/108) of the studies contained users' written content or posts: 68% (52/76) contained verbatim quotes, while 32% (24/76) paraphrased the quotes to prevent traceability. However, 16% (4/24) of the studies that paraphrased the quotes did not report the paraphrasing methods. Moreover, 18.5% (20/108) of the studies used aggregated data analysis to protect users' privacy. Furthermore, the rates of reporting ethical approval were different between different countries ( P =.02) and between papers that contained users' written content (both direct and paraphrased quotes) and papers that did not contain users' written content ( P <.001). Conclusions: Our scoping review demonstrates that the reporting of ethical considerations is widely neglected in qualitative research studies using social media data; such studies should be more cautious in citing user quotes to maintain user privacy. Further, our review reveals the need for detailed information on the precautions of obtaining informed consent and paraphrasing to reduce the potential bias. A national consensus of ethical considerations such as ethical approval, informed consent, and privacy issues is needed for qualitative research of health care using social media data of internet communities.

Social media recruitment for mental health research: A systematic review

Abstract:Background: Social media holds exciting promise for advancing mental health research recruitment, however, the extent and efficacy to which these platforms are currently in use are underexplored. Objective: A systematic review was conducted to characterize the current use and efficacy of social media in recruiting participants for mental health research. Method: A literature review was performed using MEDLINE, EMBASE, and PsychINFO. Only non-duplicative manuscripts written in the English language and published between 1/1/2004-3/31/2019 were selected for further screening. Data extracted included study type and design, participant inclusion criteria, social media platform, advertising strategy, final recruited sample size, recruitment location, year, monetary incentives, comparison to other recruitment methods if performed, and final cost per participant. Results: A total of 176 unique studies that used social media for mental health research recruitment were reviewed. The majority of studies were cross-sectional (62.5%) in design and recruited adults. Facebook was overwhelmingly the recruitment platform of choice (92.6%), with the use of paid advertisements being the predominant strategy (60.8%). Of the reviewed studies, substance abuse (43.8%) and mood disorders (15.3%) were the primary subjects of investigation. In 68.3% of studies, social media recruitment performed as well as or better than traditional recruitment methods in the number and cost of final enrolled participants. The majority of studies used Facebook for recruitment at a median cost per final recruited study participant of $19.47. In 55.6% of the studies, social media recruitment was the more cost-effective recruitment method when compared to traditional methods (e.g., referrals, mailing). Conclusion: Social media appears to be an effective and economical recruitment tool for mental health research. The platform raises methodological and privacy concerns not covered in current research regulations that warrant additional consideration.
Ethical Considerations at the Intersection of Social Media and Dementia Prevention Research

Abstract:BackgroundEthical social media use underpins effective online engagement for dementia prevention research. Existing social media guidelines are broad and lack empirical justification reflecting the values and priorities of the dementia community and the challenges specific to prevention research.ObjectivesBy engaging professional and community experts, we sought to identify the ethical issues, motivators, and barriers pertaining to social media engagement for dementia prevention research.DesignSemi-structured, qualitative interviews conducted online.SettingWe recruited participants using a combination of accessible online databases, advertisements/posters through organizational newsletters and websites, social media, registries, and from our network of colleagues.ParticipantsProfessional experts working in dementia research (n=15; e.g., researchers, coordinators) and experts with lived experience (n=14). Experts were from Canada, the USA, the UK, and Chile.MeasurementsDiscussions were analyzed using thematic qualitative analysis methods.ResultsProfessional experts revealed a dearth of social media guidelines for prevention research, relying on informal sources to supplement ethics board approval. They sought methods of strategic communication for public dialogue (e.g., misinformation, criticism). Experts by experience appreciated the educational benefits of social media but raised risks such as diminished online privacy, dementia-related stigma, being targeted for predatory practices, and misinformation. Various digital inequities (e.g., age, socioeconomic status) dampen social media’s reach to diverse publics. Participants acknowledged that younger aging populations have more digital fluency and may benefit more from social media research engagement.ConclusionsResearch professionals and community members identified ethical and contextual factors surrounding the use of social media for dementia prevention, and a need for more guidance. The next project phase will use these data to inform the co-creation of ethical guidelines for brain health research.

Abstract:Background Social media with real-time content and a wide-reaching user network opens up more possibilities for palliative and end-of-life care (PEoLC) researchers who have begun to embrace it as a complementary research tool. This review aims to identify the uses of social media in PEoLC studies and to examine the ethical considerations and data collection approaches raised by this research approach. Methods Nine online databases were searched for PEoLC research using social media published before December 2022. Thematic analysis and narrative synthesis approach were used to categorise social media applications. Results 21 studies were included. 16 studies used social media to conduct secondary analysis and five studies used social media as a platform for information sharing. Ethical considerations relevant to social media studies varied while 15 studies discussed ethical considerations, only 6 studies obtained ethical approval and 5 studies confirmed participant consent. Among studies that used social media data, most of them manually collected social media data, and other studies relied on Twitter application programming interface or third-party analytical tools. A total of 1 520 329 posts, 325 videos and 33 articles related to PEoLC from 2008 to 2022 were collected and analysed. Conclusions Social media has emerged as a promising complementary research tool with demonstrated feasibility in various applications. However, we identified the absence of standardised ethical handling and data collection approaches which pose an ongoing challenge. We provided practical recommendations to bridge these pressing gaps for researchers wishing to use social media in future PEoLC-related studies.

Abstract:We live in a digital age where information can be found instantaneously via the Internet. Studies have shown that consumers search for much of their medical information on the Internet, particularly utilizing blogs and social media platforms. As the mental health field is riddled with misinformation and stigma, this offers a unique opportunity for psychiatrists and mental health professionals to reach a broad audience for mental health education and advocacy. In this review, we discuss the various methods and techniques for blogging and social media. We then review the current recommendations for ethics and professionalism as well as make recommendations to strengthen our guidance in this new and evolving field.

Abstract:The use of social media as a recruitment tool for research with humans is increasing, and likely to continue to grow. Despite this, to date there has been no specific regulatory guidance and there has been little in the bioethics literature to guide investigators and institutional review boards (IRBs) faced with navigating the ethical issues such use raises. We begin to fill this gap by first defending a nonexceptionalist methodology for assessing social media recruitment; second, examining respect for privacy and investigator transparency as key norms governing social media recruitment; and, finally, analyzing three relatively novel aspects of social media recruitment: (i) the ethical significance of compliance with website "terms of use"; (ii) the ethics of recruiting from the online networks of research participants; and (iii) the ethical implications of online communication from and between participants. Two checklists aimed at guiding investigators and IRBs through the ethical issues are included as appendices.

Abstract:Background: The Internet and social media offer promising ways to improve the reach, efficiency, and effectiveness of recruitment efforts at a reasonable cost, but raise unique ethical dilemmas. We describe how we used social media to recruit cancer patients and family caregivers for a research study, the ethical issues we encountered, and the strategies we developed to address them. Objective: Drawing on the principles of Privacy by Design (PbD), a globally recognized standard for privacy protection, we aimed to develop a PbD framework for online health research recruitment. Methods: We proposed a focus group study on the dietary behaviors of cancer patients and their families, and the role of Web-based dietary self-management tools. Using an established blog on our hospital website, we proposed publishing a recruitment post and sharing the link on our Twitter and Facebook pages. The Research Ethics Board (REB) raised concern about the privacy risks associated with our recruitment strategy; by clicking on a recruitment post, an individual could inadvertently disclose personal health information to third-party companies engaged in tracking online behavior. The REB asked us to revise our social media recruitment strategy with the following questions in mind: (1) How will you inform users about the potential for privacy breaches and their implications? and (2) How will you protect users from privacy breaches or inadvertently sharing potentially identifying information about themselves? Results: Ethical guidelines recommend a proportionate approach to ethics assessment, which advocates for risk mitigation strategies that are proportional to the magnitude and probability of risks. We revised our social media recruitment strategy to inform users about privacy risks and to protect their privacy, while at the same time meeting our recruitment objectives. We provide a critical reflection of the perceived privacy risks associated with our social media recruitment strategy and the appropriateness of the risk mitigation strategies that we employed by assessing their alignment with PbD and by discussing the following: (1) What are the potential risks and who is at risk? (2) Is cancer considered "sensitive" personal information? (3) What is the probability of online disclosure of a cancer diagnosis in everyday life? and (4) What are the public's expectations for privacy online and their views about online tracking, profiling, and targeting? We conclude with a PbD framework for online health research recruitment. Conclusions: Researchers, REBs, ethicists, students, and potential study participants are often unaware of the privacy risks of social media research recruitment and there is no official guidance. Our PbD framework for online health research recruitment is a resource for these wide audiences.

Abstract:The prevalence of social media platforms and their use across the globe makes them attractive options for studying large groups of people, particularly when some of these platforms provide access to large amounts of structured data. However, with the collection, storage, and use of this data comes ethical and legal responsibilities, which are particularly important when looking at social groups such as migrants, who are often stigmatised and criminalised. Various guidelines, frameworks and laws have been developed to ensure social media data is used in the most ethical way. However, they have quickly evolved within the past few years and are scattered across various fields and domains. To help researchers navigate these issues, this chapter provides an overview of the ethical considerations of studying migration via social media platforms. Building on relevant academic literature, as well as national and supranational frameworks and legislations, we review how the main ethical issues related to social media research have been discussed in the past twenty years and outline good practice examples to mitigate them. This overview is designed to provide researchers with theoretical and practical tools to consider and mitigate the ethical challenges related to social media research in migration-related contexts.
Social media: a review and tutorial of applications in medicine and health care

Abstract:Background: Social media are dynamic and interactive computer-mediated communication tools that have high penetration rates in the general population in high-income and middle-income countries. However, in medicine and health care, a large number of stakeholders (eg, clinicians, administrators, professional colleges, academic institutions, ministries of health, among others) are unaware of social media's relevance, potential applications in their day-to-day activities, as well as the inherent risks and how these may be attenuated and mitigated. Objective: We conducted a narrative review with the aim to present case studies that illustrate how, where, and why social media are being used in the medical and health care sectors. Methods: Using a critical-interpretivist framework, we used qualitative methods to synthesize the impact and illustrate, explain, and provide contextual knowledge of the applications and potential implementations of social media in medicine and health care. Both traditional (eg, peer-reviewed) and nontraditional (eg, policies, case studies, and social media content) sources were used, in addition to an environmental scan (using Google and Bing Web searches) of resources. Results: We reviewed, evaluated, and synthesized 76 articles, 44 websites, and 11 policies/reports. Results and case studies are presented according to 10 different categories of social media: (1) blogs (eg, WordPress), (2) microblogs (eg, Twitter), (3) social networking sites (eg, Facebook), (4) professional networking sites (eg, LinkedIn, Sermo), (5) thematic networking sites (eg, 23andMe), (6) wikis (eg, Wikipedia), (7) mashups (eg, HealthMap), (8) collaborative filtering sites (eg, Digg), (9) media sharing sites (eg, YouTube, Slideshare), and others (eg, SecondLife). Four recommendations are provided and explained for stakeholders wishing to engage with social media while attenuating risk: (1) maintain professionalism at all times, (2) be authentic, have fun, and do not be afraid, (3) ask for help, and (4) focus, grab attention, and engage. Conclusions: The role of social media in the medical and health care sectors is far reaching, and many questions in terms of governance, ethics, professionalism, privacy, confidentiality, and information quality remain unanswered. By following the guidelines presented, professionals have a starting point to engage with social media in a safe and ethical manner. Future research will be required to understand the synergies between social media and evidence-based practice, as well as develop institutional policies that benefit patients, clinicians, public health practitioners, and industry alike.
“Participant” Perceptions of Twitter Research Ethics

Abstract:Social computing systems such as Twitter present new research sites that have provided billions of data points to researchers. However, the availability of public social media data has also presented ethical challenges. As the research community works to create ethical norms, we should be considering users’ concerns as well. With this in mind, we report on an exploratory survey of Twitter users’ perceptions of the use of tweets in research. Within our survey sample, few users were previously aware that their public tweets could be used by researchers, and the majority felt that researchers should not be able to use tweets without consent. However, we find that these attitudes are highly contextual, depending on factors such as how the research is conducted or disseminated, who is conducting it, and what the study is about. The findings of this study point to potential best practices for researchers conducting observation and analysis of public data.

Methods in predictive techniques for mental health status on social media: a critical review

Abstract:Abstract Social media is now being used to model mental well-being, and for understanding health outcomes. Computer scientists are now using quantitative techniques to predict the presence of specific mental disorders and symptomatology, such as depression, suicidality, and anxiety. This research promises great benefits to monitoring efforts, diagnostics, and intervention design for these mental health statuses. Yet, there is no standardized process for evaluating the validity of this research and the methods adopted in the design of these studies. We conduct a systematic literature review of the state-of-the-art in predicting mental health status using social media data, focusing on characteristics of the study design, methods, and research design. We find 75 studies in this area published between 2013 and 2018. Our results outline the methods of data annotation for mental health status, data collection and quality management, pre-processing and feature selection, and model selection and verification. Despite growing interest in this field, we identify concerning trends around construct validity, and a lack of reflection in the methods used to operationalize and identify mental health status. We provide some recommendations to address these challenges, including a list of proposed reporting standards for publications and collaboration opportunities in this interdisciplinary space.

Abstract:The surge in internet use to express personal thoughts and beliefs makes it increasingly feasible for the social NLP research community to find and validate associations between social media posts and mental health status . Cross-sectional and longitudinal studies of social media data bring to fore the importance of real-time responsible AI models for mental health analysis. Aiming to classify the research directions for social computing and tracking advances in the development of machine learning (ML) and deep learning (DL) based models, we propose a comprehensive survey on quantifying mental health on social media . We compose a taxonomy for mental healthcare and highlight recent attempts in examining social well-being with personal writings on social media. We define all the possible research directions for mental healthcare and investigate a thread of handling online social media data for stress, depression and suicide detection for this work. The key features of this manuscript are (i) feature extraction and classification, (ii) recent advancements in AI models, (iii) publicly available dataset, (iv) new frontiers and future research directions. We compile this information to introduce young research and academic practitioners with the field of computational intelligence for mental health analysis on social media. In this manuscript, we carry out a quantitative synthesis and a qualitative review with the corpus of over 92 potential research articles.
