Abstract:Dear Editor, Gerhardt's syndrome is characteristic of the immobility of the vocal cords in a paramedian position, leading to symptoms of intermittent stridor and acute asphyxia.[1,2] It has been reported only in 12 studies since 1954, exhibiting extremely rare incidence. Due to atypical symptoms mimicking asthma, Gerhardt's syndrome is masqueraded as asthma for an average of 4.8 years or even 9 years,[3,4] leading to over-treatment of corticosteroids and bronchodilators. We are presenting one such case for readers. In July 2018, a 19-year-old young man presented with wheezing when asleep, anxious, or upon exposure to compound fertilizer. He experienced dyspnoea over 10 years when doing vigorous exercise. The wheezing had slightly worsened in the past 2 years prior to presentation. The physical examination was unremarkable except for wheezing during sleeping exhibited by sound recording. In local hospitals, chest computed tomography (CT), routine pulmonary function (PF), fractional exhaled nitric oxide (FeNO) testing, and a methacholine challenge test showed normal results. The patient was diagnosed with bronchial asthma based on clinical features, but he did not respond to regular treatment of bronchodilators and inhaled corticosteroids over the past 4 years. Bronchoscopy testing, three-dimensional airway reconstruction, and laryngoscopy showed normal tracheobronchial, pharynx, and larynx. Electrocardiogram (ECG), exercise ECG, transthoracic echocardiogram, and exercise stress test were undertaken to exclude heart disease, all exhibiting negative results. Polysomnography (PSG) was found to be non-contributory to apnoea or hypoventilation. PF test was repeated in July 2018. Post-bronchodilator spirometry showed no significant reversibility. Carbon monoxide diffusing capacity (DLCO) and arterial blood gas analysis were normal. Maximal inspiratory flow-volume curve (MIFV) was performed, revealing a truncated inspiratory flow volume loop with a forced flow at 50% of inspiration/a forced flow at 50% of expiration (FIF50:FEF50) ratio less than 1, indicating obvious inspiratory limitation [Figure 1], consistent with changes of extra-thoracic non-fixed airway obstruction. Laryngoscopy was repeated to exclude mild glottal dyskinesia. Vocal cord movement was observed carefully. Laryngoscopy showed bilateral abductor dysfunction with normal bilateral vocal cord structure [Figure 2]. Magnetic resonance (MRI) and CT imaging of the brain were performed. No denervation atrophy of the posterior cricoarytenoid muscle was found, and there were not lesions in brain stem or at the skull base.Figure 1: MEFV curve and MIFV curve on 18th July 2018Figure 2: Laryngoscope image on 18th July 2018. (1–3) Normal epiglottis and surrounding tissues; (4–6) The vocal cords with normal adduction and limited abductionAfter excluding laryngeal deformity, neoplastic causes, and neurological disease, the patient was finally diagnosed with idiopathic bilateral vocal cord abductor paralysis, known as Gerhardt's syndrome. The treatment of Gerhardt's syndrome depends on its cause. Surgical intervention is a better choice for severe patients. CO2 laser and diode laser are efficient, economical, simple, and cause less post-operative pain.[5] Temporary tracheostomy followed by CO2 laser-assisted Kashima's posterior cordectomy, which preserved the natural voice of the patient, adequate airway, and minimal scaring, was also performed by pulmonologists. Considering mild symptoms, the patient was asked for a routine follow-up visit. Over the past 5 years, his state has been stable except for slightly aggravated wheezing when asleep. The results of repeated PF tests and laryngoscopy in 2023 were essentially unchanged. Declaration of patient consent The authors certify that they have obtained all appropriate patient consent forms.
