Abstract:A series of explosions attributed to leakage of propylene from an underground pipeline resulted in 32 fatalities and 321 injuries in Kaohsiung, Taiwan, in 2014. An analysis of this gas explosion indicated that such an accident could have been prevented, or the consequences might not have been as severe if certain elements of process safety management (PSM) had been applied. Proper execution of PSM, including management of change, mechanical integrity, process hazard analysis (PHA), process safety information (PSI), and training, can prevent the occurrence of such explosion accidents. The impact of this explosion could have been reduced had these PSM elements, particularly PHA, operating procedures, training, PSI, and emergency planning and response, been executed. © 2016 American Institute of Chemical Engineers Process Saf Prog 35: 228–232, 2016

Abstract:This study examines a tragic explosion at Linyuan Petrochemical Park in Kaohsiung, Taiwan, resulting in two fatalities, two severe injuries, and significant financial losses. Taiwan's Industrial Development Bureau (IDB) investigation attributed the incident to a ruptured Ethyl Propyl Diene Monomer (EPDM) rubber hose during catalyst changeover, releasing flammable liquids and triggering a violent vapor cloud explosion. Because of lacking process safety information (PSI), the plant manager failed to identify the risk of incompatibility between EPDM rubber and olefin and failed to replace the EPDM hose with a compatible material or select a carbon steel pipe timely. By not carefully considering the impact of facility siting in the process hazard analysis, TASCO also was unaware that the control room was too close to the process area. As a result, the control room was destroyed at the onset of the accident. Inadequate Management of Change (MOC) impeded the hazard identification for the catalyst changeover procedure, thereby failing to identify defects in this operating procedure. Finally, a false level alarm caused the operator to partially close the control valve downstream of the hose, indirectly leading to the pipe rupture, and this highlights a lack of Mechanical Integrity (MI) in this plant.

Abstract:This paper performs an in-depth investigation and analysis on a catastrophic hazardous chemical accident involving domino effects in China based on an emerging accident causation model—the 24Model. The triggers and roots of the incident from the individual and organizational levels have been identified and several useful lessons have been summarized to avoid similar mistakes. This accident began with a leak of vinyl chloride caused by the failure of the gas holder’s bell housing and the operators’ mishandling. Leaked vinyl chloride was ignited by a high-temperature device in the process of diffusion and the fire quickly spread to the illegally parked vehicles. Several organizations were involved in this accident, and the chemical company should bear the main responsibility for it, and shall establish and implement an effective safety management system in its organizational structure and staffing, facilities management, hazards identification, emergency disposal, etc., to improve safety performance in a systematic way. Enterprises in the chemical industry park shall enhance the communication to clarify major hazard installations in their domains, and conduct regular safety evaluation for the plant as the external environment changed. Government agencies shall plan the layout of the chemical industry park scientifically and ensure safety starts with the design stage. The case study provides a practical procedure for accident investigation and analysis, and thus, preventive measures can be made according to the various causations at different levels.

Abstract:A number of chemical accidents have occurred in China over the past two decades with significant impact on humans and the environment. It is expected that lessons will have been learned from these accidents that will help industries to reduce the risk that catastrophic chemical accidents occur in future. In fact, to some extent there is evidence that lessons have been learned, to the extent that the Chinese government has substantially strengthened legislation and regulatory standards. Nonetheless, there remains a concern that much more still needs to be done to reduce chemical accidents risks in China. Important progress in this area requires not only government support but a commitment across all hazardous industries to learn from past accidents that may in many cases require establishment or considerable improvement of their safety management systems. To assist small and medium-sized enterprises (SMEs), in this effort, results of an analysis of common causes of the chemical accidents reported in the Major Accident Information (MAI) website of Chinese State Administration of Work Safety (SAWS) are presented in this paper In particular, inadequate process hazard analysis (PHA), training and emergency response planning (ERP) were identified as the top three process safety management (PSM) elements that contribute to most of the SMEs accidents in China. Seven recommendations are proposed in order to improve the effectiveness of lesson learning for government agencies and SMEs.

Abstract:Many coal gasification related accidents happened in China recently. Investigations of the accidents show that the performance of safety barriers and the risk influence factors contributing to it plays an important role. Lack of attention on the performance of safety barriers and associated risk influence factors hinder the thorough understanding of these accidents and, therefore, needs to be addressed by providing a new analysis methodology. Herein, we developed an approach from critical events and safety barriers to control the major hazards during the coal gasification process. The method consists of process hazard identification based on critical events, barrier performance evaluation based on barrier diagrams, and quantification of risk influence factors based on Bayesian network. The application of the method indicates that it is suitable for controlling major hazards of the coal gasification process as well as other chemical processes. © 2017 American Institute of Chemical Engineers Process Saf Prog 36: 382–391, 2017

Abstract:Cumene hydroperoxide (CHP) has been used in producing phenol, dicumyl peroxide (DCPO) and as an initiator for synthesizing acrylonitrile-butadiene-styrene (ABS) resin by copolymerization in Taiwan. Four incidents of fire and explosion induced by thermal runaway reactions were occurred in a same plant producing CHP, DCPO and bis-(tert-butylperoxy isopropyl) benzene peroxide (BIBP). The fourth fire and explosion occurred in the CHP reactor that resulted in a catastrophic damage in reaction region and even spread throughout storage area. Descriptions on the occurrences of these incidents were assessed by the features of processes, reaction schemes and unexpected side reactions. Calorimetric data on thermokinetics and pressure were used for explaining the practical consequences or which the worst cases encountered in this kind of plant. Acceptable risk associated with emergency relief system design is vital for a plant producing organic peroxide. These basic data for designing an inherently safer plant can be conducted from adiabatic calorimetry. An encouraging deduction has been drawn here, these incidents may be avoided by the implementation of API RP 520, API RP 521, DIERS technology, OSHA 1910.119 and AIChE's CCPS recommended PSM elements.

Abstract:Process hazard analysis and control technology emphasize on using systematic approach to identify the hazardous,and take necessary measures to eliminate and reduce the hazards,or mitigate the accident consequences caused by the hazards.At present,the global energy and chemical enterprises all adopt characterized process hazard analysis and control technology for process safety management.In this study,an integrated process hazard analysis and control technology which included HAZOP,LOPA and Bow-Tie analysis was introduced.Taking a furnace as an example,the hazards existing in the equipments were fully analyzed and identified by HAZOP,the sufficiency of protection layers was determined by LOPA,and through Bow-Tie method,the efficient activities ensuring the designated control measures or emergency measures are still effective were identified.Therefore,the integrated process hazard analysis and control technology could change the process safety management of the enterprise from experience-based management system to systematic management system,so as to effectively avoid the occurrence of major process safety accident,and improve risk management level of enterprises.

Abstract:Thermohydraulic analysis of postulated accidents will identify systems responses to accident scenarios and aid to develop design of safety systems and strategies to prevent and mitigate accident propagation. This paper reports recent progress in analysis of some accident scenarios of a water-cooled fusion DEMO reactor. Four types of accidents are particularly considered: ex-vessel loss-of-coolant of the primary cooling system, in-vessel loss-of-coolant of the first wall cooling pipes, loss-of-coolant in blanket modules and loss-of-vacuum. The analysis has identified responses of the DEMO systems to these accidents, pressure loads to confinement barriers for radioactive materials and radiological consequences. Effectiveness of the safety systems and integrity of the confinement barriers against the accidents are discussed.

Abstract:Explosion is the most dangerous accident during direct combustion of low-concentration gas. Few investigations have focused on explosion risk assessment of low-concentration gas safe combustion system. This study aims to analyze explosion risk of above system. Due to insufficient accident data for calculating the failure probability of explosion, expert evaluation method and fuzzy algorithm were adopted to obtain the failure probability of basic events (BEs) and Fuzzy Analytic Hierarchy (FAHP) Process method used to improve the calculation of expert weights. Then qualitative and quantitative analysis was carried out using fault tree analysis. Results show that the failure probability of system explosion is 0.085%. Events with high-risk probability are mainly on equipment management and personnel behavior norms. Safe combustion system has excellent performance on tempering prevention, explosion resistance and explosion relief. The outcomes of this study are expected to help the safety professionals while formulating safety management measures for low-concentration gas safety combustion system.

Abstract:A single presentation covering one person's first‐hand experience with a significant process safety incident, delivered at the beginning of OSHA's PSM regulation still provides contemporary lessons, even though the actual incident occurred more than 50 years ago. Lessons include: (a) stopping operations unilaterally or getting expert help when readings are unexpected or unusual, (b) establishment of a testing regimen that covers a broad range of conditions, especially those that can be achieved under abnormal circumstances (e.g., fire), and (c) the impact of “restore to original condition” maintenance activities that otherwise might not be analyzed if treated as a replacement‐in‐kind only. © 2015 American Institute of Chemical Engineers Process Saf Prog 35: 103–106, 2016

Abstract:Fire and explosion accidents frequently occur during hydrogen production, transportation, storage, and utilization processes, leading to significant casualties and economic losses. In this work, the characteristics of confined hydrogen explosions, hydrogen cloud explosions, and hydrogen blending explosions are summarized. Moreover, strategies for mitigating hydrogen deflagration, including explosion inhibition, explosion venting, and flame quenching, are analyzed. The results show that the deflagration behaviors of hydrogen under extreme initial conditions, such as low temperature and high pressure, are not yet well-understood. Research studies have demonstrate the marked effects of mitigating hydrogen deflagration, but this process has certain disadvantages. Porous materials and alkyl halides show dual promotion and inhibition effects. The suppression efficiency of inert gas and water mist must be improved. Powder suppressants with low pyrolysis temperatures and high decomposition efficiencies can be designed and employed to suppress hydrogen explosions.

Abstract:About frequently happened fire accidents in Power stations, an actual compartment fire accident in one room on 3nd floor was studied numerically with software FDS 5.3.0/SMV 5.3.10 in this article. According to the results of simulation, we investigated the different radiant heat flux parameters, burning losses under different heat release rates and rising rates. The developing process of fuel combustion was analyzed. Also, we compared the simulation results with the actual smoke trace and burning rates in fire accidents. It shows the numerical modeling software FDS is a reliable tool to analyze the power plant fire accident caused by casually placed inflammable objects in single room. It provides an applied valuable good way for the fire accident research in power plant.

Abstract:Traditional hazard and operability analysis (HAZOP) is one of the most widely applied methods for process safety management in process enterprises. Due to its principles based on the conservative and qualitative judgment, it often leads to too conservative risk identification results for the fluorine chemical industry usually with high-risk processes to keep the continuity of production. Most of improved quantitative and semi-quantitative methods are based on the layer of protection analysis (LOPA) to resolve the over-conservative problem of traditional HAZOP with the database of LOPA. However, the improved model, taking LOPA as the main line and HAZOP only as the provider of scenarios and influencing factors, is limited to the fact that LOPA can only analyze complete and independent protection layers (IPLs). Therefore, in order to realize the quantitative or semi-quantitative analysis of disaster causes and consequences, a new semi-quantitative HAZOP method takes HAZOP as the main line to integrate LOPA, F&EI (fire and explosion index) for quantitatively calculating the reduction factors, probability on failure demand (PFD) of general protection layers (GPLs) and PFD of IPLs. With the case comparison of fluorine chemical industry, it is proved that this new method can effectively improve the problem that traditional HAZOP are too conservative in complex scenarios.

Abstract:With the rapid development of the petrochemical industry, sulfur-related devices are increasing. In this paper, a fire accident occurred in a petrochemical storage tank of a petrochemical enterprise. Through analysis, we found that the direct cause of the accident was the spontaneous combustion of ferrous sulfide (FeS), which caused the explosion of the oil and gas mixture between the floating disk and the tank top. In addition, we analyzed the corrosion of aluminum floating plates in oil tanks by Scanning Electron Microscope (SEM) and Energy Dispersive Spectrometer (EDS). The analyses showed that although AlMn1 Cu aluminum alloy has high corrosion resistance, the material composition contains impurity elements such as silicon and iron, which weakens the corrosion resistance of aluminum alloy, and the intergranular corrosion and corrosion effects such as pitting corrosion cause cracks to form in the floating disk. The oil and gas that volatilize in the oil tank diffuse through the pores between the floating plate and the top of the tank to form an explosive mixture of oil gas and air. Next, the corrosion products of the manhole cover and the edge of the floating plate were sampled and were analyzed the sulfur and iron contents by elemental analysis. The results showed that the sulfur content in the sample of the manhole of the No. 1 tank was 25.04 wt%, and the sulfur content in the sample at the edge of the floating plate was 7.49 wt%. This was owing to the manhole at the bottom of tank No. 1 is not in contact with air. The edge of the floating plate of tank No. 1 has a low sulfur content due to the long-term contact with air and the burning process during an accident. Using Fault Tree Analysis (FTA), we conclude that the necessary conditions for the accident are appropriate heat storage conditions. The key factors are bad desulfurization effect on the refining process, sulfur in the crude oil, nonresistance of the tank material to corrosion and presence of S2- in the tank. Finally, corresponding countermeasures are proposed.

Abstract:Powdered materials are widely used in industrial processes, chemical processing, and nanoscience. Because most flammable powders and chemicals are not pure substances, their flammability and self-heating characteristics cannot be accurately identified using safety data sheets. Therefore, site staff can easily underestimate the risks they pose. Flammable dust accidents are frequent and force industrial process managers to pay attention to the characteristics of flammable powders and create inherently safer designs.This study verified that although the flammable powders used by petrochemical plants have been tested, some powders have different minimum ignition energies (MIEs) before and after drying, whereas some of the powders are released of flammable gases. These hazard characteristics are usually neglected, leading to the neglect of preventive parameters for fires and explosions, such as dust particle size specified by NFPA-654, MIE, the minimum ignition temperature of the dust cloud, the minimum ignition temperature of the dust layer, and limiting oxygen concentration. Unless these parameters are fully integrated into process hazard analysis and process safety management, the risks cannot be fully identified, and the reliability of process hazard analysis cannot be improved to facilitate the development of appropriate countermeasures. Preventing the underestimation of process risk severity due to the fire and explosion parameters of unknown flammable dusts and overestimation of existing safety measures is crucial for effective accident prevention.

Abstract:At present, chemical accidents have occurred frequently with disastrous consequences. In order to effectively prevent chemical accidents and their secondary disasters, it is urgent to develop detailed risk assessment for chemical processes. However, traditional methods tend to ignore the dynamic correlation coupling of chemical processes and devices; this results in the assessment results having poor accuracy. Therefore, a dynamic assessment method for risk evolution in chemical processes based on MFM-HAZOP-FDBN is proposed in this study. First, multilevel flow model (MFM) is established according to the transfer paths of material flow and energy flow in a chemical process, and further combined with hazard and operability analysis (HAZOP) to form the functional HAZOP technology. Second, risk identification results from the MFM-HAZOP are applied as the input of a fuzzy dynamic Bayesian network (FDBN); the device states and process errors are used as dynamic and static nodes of FDBN to present the risk propagation and evolution mechanism in the chemical process, respectively. Finally, the dynamic risk evolutionary trend throughout the chemical process is assessed quantitatively by determining the occurrence likelihood and consequence severity based on the risk matrix. The BP America (Texas City) refinery explosion is taken as a case study. Results show that the proposed method can provide a detailed characterization and dynamic assessment of risk evolution in the chemical process and the assessment results are more reasonable and effective.

Abstract:Explosion risk exists in many processes for manufacturing and handling of combustible materials. It is a continuous hazard to industrial processes. The impacts of fire and explosion may be severe in terms of death and injury to people, damage to physical property, and effects on the environment. Safe design of process control is an important part of risk management. Process control is widely applied in the field of explosion prevention and protection. The application of process control in sequence control and interlock, process monitoring and control, risk signal monitoring and safety protection, explosion suppression and isolation were introduced. Two case investigations of accidents because of process control design deficiency in chemical industry were presented. Clear status indication, reliable interlock and redundancy design of process control design were emphasized.
Hazard Identification on the Process of High-Level Liquid Waste Concentration and Denitration in Spent Fuel Reprocessing by HAZOP

Abstract:To develop China’s spent nuclear fuel reprocessing, safety analysis of reprocessing facility is of great importance and high priority. Any methodology of safety analysis, no matter whether it is in a kind of deterministic analysis, probabilistic assessment or so-called Integrated Safety Analysis, is beginning with the identification and systematic analysis of hazards as the very first essential step. Recognizing that reprocessing facilities are, to a large extent, chemical processing plants, HAZard and OPerability analysis (HAZOP) was introduced firstly in the US. It is featured as one of the most suitable methods for performing detailed identification of a wide range of hazards. In this paper, our work on part of the concentration and denitration process (C/D process) of high-level liquid waste (HLLW) was revealed to exemplify the procedure of hazard identification and analysis for a typical reprocessing process, as the C/D process of HLLW has many symbolic features of spent fuel reprocessing, such as high radiation, various chemicals, complex chemical reactions and operation stages. The purpose of this paper was to test the applicability of HAZOP for a typical process in reprocessing. The HAZOP approach was starting with the identifications of process, reactions, equipment, and system borders. Base on the features of the given system (part), specific elements and guidewords were selected and combined to generate deviations for different operation stages. The possible causes and consequences of deviation as well as existing safeguards were taken into consideration. To make the workflow of HAZOP complete and underscore its significance, a 4-by-5 risk matrix was established to evaluate the risk levels of all consequences resulted from deviations, based on the severity of consequence and associated likelihood of occurrence. The final results were shown in a HAZOP analysis worksheet, in which twenty-two deviations were presented, revealing the potential hazards found in the C/D part. After a preliminary risk assessment using a risk matrix, eight of them were recognized as undesirable risks (only accepted when risk reduction is impracticable). The results verified that the HAZOP analysis was suitable for the processes or parts involved with high radiation and complex chemical reactions in reprocessing facilities.

Abstract:As modern chemical plants are becoming more complex and bigger in scale, the associated chance of things going wrong is also increasing rapidly. Due to the flammable, explosive, toxic and corrosive nature of chemical process, any single accident may trigger a major catastrophe that brings tremendous environmental, social and economical loss. In order to prevent any accident from happening, hazard and operability (HAZOP) analysis has been brought in to monitor chemical process and provide early warning for signs of accident. However, most existing HAZOP is carried out manually, and there are always obstacles in terms of cost overrun and incompleteness of the analysis. To address the difficulties in current HAZOP method, this paper proposes a signed digraph (SDG)-based HAZOP analysis method. It is used to identify the most likely operating mistakes that may cause certain process variable deviating from its normal value, which is the main source of safety concern. A case study on polyvinyl chloride (PVC) plant is presented to demonstrate the effectiveness of SDG-based HAZOP analysis method in providing complete analysis result.

Process Safety Progress

Abstract:By figuring out the cause and learning the lessons, accident investigation serves to avoid the like accidents effectively. Incident Management is one of the twelve key factors among China's process safety management system (PSMS), while accident investigation is central to Incident Management. This paper selected “Tianjin Port 8.12 fire and explosion accident” (“Tianjin 8.12 Accident”) as an investigation case. Three core components: cause analysis, affixing the responsibility and corrective suggestions, were unfolded. Problems were analyzed from process safety and accident development point of view. A process analysis approach, which is based on event sequence‐barrier failed, was employed for accident investigation. Affixing responsibility process based on failed analysis was used for accident investigating and affixing. In order to give corrective suggestions, safety information‐based corrective suggestions are proposed by starting from systematic and interconnected perspective and combining with finding failed behaviors and an in‐depth analysis of the causes behind the behaviors. Subsequently, an integral accident investigation approach, which consists of cause analysis based on event sequence‐barrier failed, affixing responsibility process based on failed analysis and systematic corrective measures based on safety information, can effectively render scientific theoretical guidance to investigation and analysis of an accident. © 2017 American Institute of Chemical Engineers Process Saf Prog 36: 362–367, 2017
