Abstract:Deep bite is one of the most common malocclusion which affects children and even adults leading to a number of oral health problems. Patients with deep bite encounter periodontal and temporomandibular problems. Deep overbite refers to the increase of overlap of maxillary incisors over mandibular incisors beyond normal coverage of 30-40%. There are many factors which lead to the development of deep bite like inherent and acquired factors. Inherent factors like tooth morphology, skeletal patterns, rotation of condyle affects the bite. Acquired factors like muscle habits, loss of posterior tooth support or lateral tongue thrust may affect the bite. Deep bite can be corrected by number of different methods which include intrusion of anterior teeth, extrusion of posterior teeth or a combination of both. In non-growing patients surgical treatment is a suitable option to achieve stable results.This article describes various appliances and their biomechanics involved for correcting deepbite malocclusion

Biomechanical and clinical considerations in deep bite treatment using temporary skeletal anchorage devices

Abstract:Deep bite is usually treated by intrusion of the anterior teeth, extrusion of the posterior teeth, or a combination thereof. Various traditional methods have been used to correct deep bites, but they can cause negative side effects. Recently, temporary skeletal anchorage devices (TSADs) have been used to intrude the anterior teeth simply and efficiently without side effects. The amount of incisal exposure at rest should be considered as the first factor in determining which tooth segment should be intruded to correct a deep bite. The center of resistance of the anterior teeth should be considered to achieve the proper biomechanics when intruding them. The location of TSADs should be determined to ensure sufficient interradicular bone to achieve the desired tooth movement. Therefore, clinicians should understand the biomechanical and clinical considerations to correct a deep bite using TSADs.

Abstract:While deep overbite typically accompanies all classes of malocclusion, it is singled out in this article as the prominent component of malocclusion. We review the evidence-based treatment of deep overbite, mostly of lower tier on the evidence hierarchy. Accordingly, challenges to treatment emerge with the lack of firm guidelines for treatment. The main concerns relate to long-term stability and facial esthetics. Treatment options in children focus on possibilities of growth modification and avoidance of more severe development of the condition. Camouflage, surgical options, and compromised outcome are considered in the non-growing patient. We illustrate the various treatment strategies, including maxillary incisor esthetic differential extrusion, along with the side effects that may occur and ways to avoid them. The need for structured controlled trials and associated formulation of guidelines is demonstrated.

Abstract:To achieve a more appealing smile, an orthodontic technique known as Invisalign might be utilised. Invisalign procedures may be beneficial if you have an overbite, crossbite, or deepbite. Transparent plastic aligner trays are used to gradually straighten the teeth during the operation. Patients may maintain their oral health and wellbeing by simply removing the trays, without having to worry about brackets and wires that may make it difficult. The primary goal of this study is to evaluate previous studies and review all available evidence on how Invisalign can treat deep bites. The comprehensive search will use Medline, PubMed Central, Embase, and other databases. The literature review is completed in English with the goal of reviewing the material that describes how Invisalign can treat deep bites. In recent years, the importance of appearance in personal and professional lives has spurred significant interest in orthodontic treatment among adults. The transparency of the Invisalign appliance boosts its visual appeal for mature patients who are hesitant using conventional labial fixed braces.

An evidence-based approach to treatment of open bite and deep bite: case reports

Abstract:Aim: Evidence-based decision making is gaining increased emphasis in medicine and dentistry. Since orthodontics is both an art and a science, not all decisions can be based solely on scientific findings. However, to the extent that orthodontics is a science and is based on the principles of scientific method, the clinician can practice evidence-based decision making. This article summarizes the results of 6 case-controlled studies on treatment changes in deep bite or open bite. Material and methods: All studies used the Bolton Brush Growth Study as a source for untreated controls. Strategies for correction of deep bite included cervical pull headgear, bionator therapy, and Tweed edgewise mechanotherapy. Open-bite strategies included 4 premolar extraction, 4 first molar extraction, and active vertical corrector therapy. The changes in 6 variables involved in overlap of the incisor teeth (changes in the maxilla and mandible, as well as tipping and bodily movements of the maxillary and mandibular incisors) were summed at the occlusal plane. Results: Extraction of permanent teeth influences vertical facial growth, growth of the mandible is a major factor in the correction of deep bite, and tipping of the incisor teeth is an important contributor to open-bite correction. Two clinical cases that demonstrate the application of this analysis are presented. The first case involves an open bite treated with extraction of 4 premolars, and the second is a nonextraction deep-bite case treated with a Herbst appliance, followed by fixed appliances. Conclusion: Case-controlled studies can help practitioners decide among various treatment strategies for vertical overbite problems.
Brodie Bite: A Clinical Challenge

Abstract:Treatment of the posterior crossbite (Brodie bite) case is always challenging for orthodontics. The case requires meticulous treatment planning and is often difficult and time-consuming to treat Brodie bite. This kind of malocclusion develops partially because of lingual tipping of the lower segments, and partially because of a lower jaw too small, relative to the maxilla. A young male 12 years of age came to the dental department with chief complaints of unable to chew food and with lower jaw teeth contained within the upper jaw. Clinical examination revealed class II div I malocclusion, increased overjet and lingually locked upper left lateral incisor with just one occlusal contact at the left first molar region (mandibular teeth contained within the maxillary dentition). Though there are various treatment options available such as extractions, expansion, dental arch compensation, or orthognathic surgery for treating Brodie bite, the best treatment option should be chosen, which requires proper diagnosis. Bite Opening Effect with Fixed Bite Plate

Abstract:Objective To evaluate the bite opening effect of the fixed bite plate in the treatment of deep bite patients.Methods 12 deep bite patients with low or average mandibular plane angle were collected (6 male,6 female,12-30 years old,mean 17 years old) in experiment group.They received straight wire appliances with fixed bite plate.Another 12 matched patients were collected as control group to receive straight wire appliances only.Study models were taken before treatment and after the deep bite corrected.The overbite change was measured and the time needed for bite opening was calculated.Results Model analysis showed that overbite decreased significantly.The mean overbite reduction in the patients with fixed bite plate was 3.37 mm.The average time for bite opening was shorter significantly in the experimental group (average 3.75 months) than in the control group.Conclusions Fixed bite plate on the upper anterior teeth was effective on bite opening.It can be used throughout the treatment without interrupting the orthodontic procedures.
Relapse of orthodontically corrected deepbites in accordance with growth pattern

Abstract:Introduction: A common orthodontic problem is a deep overbite malocclusion. Because of its high relapse tendency, it is also one of the most challenging problems to treat. To minimize relapse, the morphologic characteristics of patients need to be considered. The aim of this study was to compare deepbite relapse in 3 groups of patients categorized by vertical growth type. Methods: The total sample included 60 patients treated at the University of Washington in Seattle, all with initial overbites greater than 50%. Data were collected from casts and cephalometric radiographs at 3 time points: pretreatment, posttreatment, and 10 years postretention. A mixed-effects model (analysis of variance) and post-hoc t tests were used for the statistical evaluations. Results: The high-angle subjects showed the least deepbite relapse (0.1 ± 1.1 mm), whereas the low-angle (1.2 ± 0.9 mm) and the normal-angle (1.4 ± 1.3 mm) subjects had statistically significant relapses P <0.001. This overbite relapse might be partially due to changes in the mandibular and interincisal angles, which were also observed in these 2 groups. Conclusions: High-angle subjects tend to relapse less in overbite than do low-angle and normal-angle subjects in the long term.
A Pilot Clinical Study on Deep Bite Correction with Clear Aligner

Abstract:Objective To evaluate the effectiveness on correction of anterior deep bite in adult patients with clear aligners.Methods Twelve adult patients with anterior deep bite were enrolled in this study.Conventional lateral cephalometric and panorami radiographs were taken at pretreatment and at the end of orthodontic treatment.All of the patients were treated with clear aligners.The distance between lower central incisor and mandibular plane,the inclination of lower central incisor to the mandibular plane,the distance between lower first molar and mandibular plane,and the mandibular plane angle were measured at the start of treatment and at the end of treatment by cephalometrics.Intragroup comparisons were made using a paired t-test.Results The anterior overbite was decreased in all patients after orthodontic treatment.The duration of treatment was (15.8±5.5) months.The mean amount of intrusion on lower incisors were [(2.1±1.9) mm,P＜ 0.05] and these teeth were also uprighted by [(4.9±1.8)°,P＜0.05].The position of the lower first molars and the mandibular plane angle were not changed significantly at the end of treatment,respectively mean amount [(0.1±0.6) mm,P＞0.05] and [(0.5±0.4)°,P＞0.05].Conclusions Deep bite correction was achieved effectively by using clear aligner.The intrusion of lower incisors was the main mechanics.

Abstract:Introduction: Orthodontics a unique branch in dentistry is inter-related with biology, mathematics and engineering sciences. Biomechanics is fundamental to the practice of orthodontics. Stresses generated at any point during orthodontic treatment are critical in remodeling of bone and type of tooth movement. Aims and Objectives: The aim of the following study is to determine the initial stresses produced in the tooth, periodontal ligament (PDL) and bone when force is applied on the tooth for intrusion and tipping on labial and lingual sides in the presence of varying alveolar bone heights. Materials and Methods: A geometric model generated using AutoCAD (Autodesk, Inc.). Six three-dimensional finite element models of a maxillary central incisor were designed with PDL and varying alveolar bone heights after applying the boundary conditions of the model, force was applied on the tooth.Results: Results were obtained with color coded three-dimensional fringe patterns. Conclusion: Alveolar bone loss causes an increase in the maximum initial stresses relative to the bone heights.
Full-mouth rehabilitation of a patient with severe deep bite: a clinical report

Abstract:Increasing the vertical dimension is essential to resolving problems associated with deep bite. Restorative dentistry, orthodontia, and oral surgery are the three disciplines that can help to gain the vertical dimension necessary in these patients. This clinical report presents the results of increasing vertical dimension with a full-mouth restorative treatment procedure for a 40-year-old male patient who exhibited severe deep bite. After clinical evaluation, extraoral examination showed a reduction of the lower facial height and protuberant lips, wrinkles, drooping, and overclosed commissures. In addition, intraoral examination showed a severe anterior deep-bite articulation, and upper incisors were in contact with the lower incisor labial tissue. A removable partial denture was made at increased occlusal vertical dimension (OVD) to use in the first stage of rehabilitation. Diagnostic wax-up was performed at the increased vertical dimension. Then, provisional crowns were fabricated according to this increased vertical dimension. Interim prostheses were used for 3 months as a guide for preparing the definitive restorations. The adaptation of the patient to the increased OVD was evaluated. During this period, he was asymptomatic. Following the evaluation period, definitive restorations were completed, and routine clinical assessments were made after 1 week, 1 month, 3 months, and 6 months, then after 1 and 2 years with visual and radiographic examinations.

Influence of Invisalign precision bite ramp utilization on deep bite correction and root length in adults

Abstract:Objectives: To assess the influence of Invisalign precision bite ramp use on skeletal deep overbite correction and root length and volume of maxillary anterior teeth. Materials and methods: This was a retrospective study of 60 adults with skeletal deep overbite. Patients were divided into three groups: Invisalign (Align Technology, San Jose, Calif) with precision bite ramps (Invisalign with Bite Ramps [IBR] = 12), Invisalign with no bite ramps (INBR = 22), and full-fixed appliances (FFA = 26). Cone beam computed tomography records at T1 (pretreatment) and T2 (posttreatment) were used to measure eight skeletal, nine dental, and three soft-tissue cephalometric variables. Maxillary anterior teeth root length (mm), root volume (mm3), and percent root volume loss between T1 and T2 (%) were also recorded. Results: Significant changes from T1 to T2 among the three groups were seen in ANB(o), lower face height (%), ODI (overbite depth indicator) (o), and U1-SN (o). Reduction in root length was significantly less (P < .001) in the INBR and IBR groups compared to the FFA group. Reduction in root volume and percent volume loss were significantly higher in the INBR group compared to the IBR group (P < .001), but the difference between the two Invisalign groups and the FFA group was not significant. Conclusions: Skeletal deep overbite correction using Invisalign with or without bite ramps is comparable to FFA. Reduction in root length was significantly less with Invisalign compared to FFA. Bite ramps influenced root volume and volume loss but not root length.

Abstract:Purpose: The primary objective of this study was to investigate the biomechanical effects and stresses on bone, periodontal ligament (PDL), cementum and displacement along X-, Y- and Z-axis during true intrusion of incisors using mini-implants with Finite Element Analysis; the secondary objective of the study was to find out the best method for anterior intrusion in clinical practice to treat anterior deep bite malocclusions. Materials and methods: A 3D finite element method was used to simulate true anterior intrusion with sliding mechanics using mini-implants. Two groups were modelled with mini-implants placed distal to lateral incisors for Model 1, and below the anterior nasal spine (ANS) for Model 2, to achieve intrusion. von Mises stress, principal stress on PDL and alveolar bone, displacements in all 3 planes were determined. Results: Amongst the modalities compared in the present study, the stresses on bone and PDL were showing best behavior for mini-implants placed distal to lateral incisors (Model 1). The teeth showed controlled tooth movement in Model 1. Conclusion: Maximum stress was found in the cortical bone and in the PDL. Nature of the stress changed from compressive to tensile from cervical area to root apex, concentrating mainly at the apical area. Amongst the modalities compared, the best controlled tooth movements for anterior intrusion to treat anterior deep bite malocclusions, was for mini-implants placed distal to lateral incisors (Model 1).

Abstract:Orthodontic treatment is an appliance-intensive endeavor, where an array of mechanical devices is used to bring about tooth movement. By virtue of remaining in close proximity to the enamel, gingiva and periodontal ligament intra-orally over a prolonged period of time, orthodontic appliances have a significant impact on the paradental tissues, oral environment and oral microbiome. Orthodontic appliances by acting as anchors for biofilm and plaque formation, accumulate bacteria and other micro-organisms in amounts unfavorable for maintenance of healthy equilibrium. The resulting loss of balance in the oral microbiome causes dysbiosis which manifests clinically as increased enamel demineralization, dental caries and periodontal disease. Mechanical removal of the accumulated plaque by maintaining rigorous oral hygiene has been proven to be the single most important factor to mitigate the harmful effects of dysbiosis. This review investigates how each of the various components of the orthodontic appliances, different types of appliances and unique surface properties of biomaterials have contributory effects at the interface of orthodontic biomaterials and oral biology. The information thus obtained will be critical in instituting the best diagnostic and therapeutic measures at the clinical level. It will also be instrumental in devising improvements and providing new directions for future research in general and precision orthodontics in particular.

Abstract:Dentistry has made significant progress since the era when teeth were commonly extracted for dental pain, extensive dental caries, and endodontic disease. In this manuscript, we introduce the cupping depth of the alveolar bone and propose a new way to assess malalignment of teeth in orthodontics. Advancements in restorative dentistry and endodontic procedures have enabled the restoration of dental hard tissue using innovative techniques and advanced materials. However, despite these strides in restorative dentistry, a considerable gap persists in orthodontic diagnosis and practice. Fortunately, the universal application of Edward R. Angle's arbitrary treatment goal, irrespective of individual facial and developmental morphology, is finally waning due to advancements in orthodontic technologies. FASTBRACES® Technologies introduce technical advancements and a proper knowledge base for evaluating and treating the overlooked alveolar bone architecture of malaligned teeth. The axial ("horizontal") restoration of alveolar bone is promoted, which is analogous to the sagittal ("vertical") restoration of alveolar bone in periodontal treatment planning and therapy. This approach is biologically based and consistent with a patient's natural dentition and individual genetic morphologic appearance, rather than relying on subjective or arbitrary ideals. Deep Tooth LLM: Neural Trajectory Optimization for Tooth Alignment

Abstract:A orthodontic treatment simulation is normally made up of several cycles of treatment, that regularly covers for more than 12 months. Thus, the fundamental determinant in orthodontic treatment is to simulate medically reasonable teeth position in long-term progress. However, existing orthodontic treatment simulation system heavily rely on duplication of efforts from dentists or only estimate final tooth arrangement without orthodontically intermediate procedure. Toward clinically reasonable simulate 3D orthodontic treatment progress, we present DeepOrtho, a deep learning based novel system to simulate medically 3D tooth position for orthodontic treatment planning. Our system takes 3D tooth meshes from patients with malocclusion as input, and sequentially simulates the orthodontically proper 3D rotation and translation for each tooth within the long-term treatment. Notably, we formulate the 3D orthodontic treatment simulation as a reverse process of iteratively denoising teeth arrangements, where DeepOrtho gradually reduces medically uncertain sequences from all the teeth adjustable positions until reaching the desired positions. To the best of our knowledge, we are the first medical simulation system to explore progress 3D orthodontic treatment. Extensive experiments demonstrate that the proposed DeepOrtho outperforms existing solutions in terms of performance and clinical feasibility.
Three-Dimensional Evaluation of Treatment Effects and Post-Treatment Stability of Maxillary Molar Intrusion Using Temporary Anchorage Devices in Open Bite Malocclusion

Abstract:Background: We investigated treatment outcomes and post-treatment stability in 10 patients with an anterior open bite and nonsurgical orthodontics. Methods: The patients underwent maxillary molar intrusion using temporary anchorage devices (TADs) to deepen the overbite due to mandibular autorotation. Lateral cephalograms and dental cast models were obtained before treatment (T0), immediately after it (T1), and >1 year after it (T2). Skeletal and dental cephalometric changes and three-dimensional movements of the maxillary dentitions were evaluated. Results: At T0, cephalometric analysis indicated that patients had skeletal class I with tendencies for a class II jaw relationship and a skeletal open bite. During active treatment (T0 to T1), the maxillary first molar intruded by 1.6 mm, the mandibular first molar extruded by 0.3 mm, the Frankfort-mandibular plane angle decreased by 1.1°, and the overbite increased by 4.1 mm. Statistically significant changes were observed in the amount of vertical movement of the maxillary first molar, Frankfort-mandibular plane angle, and overbite. Three-dimensional (3D) dental cast analysis revealed that the maxillary first and second molars intruded, whereas the anterior teeth extruded, with the second premolar as an infection point. In addition, the maxillary molar was tipped distally by 2.9° and rotated distally by 0.91°. Statistically significant changes were observed in the amount of vertical movement of the central incisor, lateral incisor, canine and first molar, and molar angulation. From T1 to T2, no significant changes in cephalometric measurements or the 3D position of the maxillary dentition were observed. The maxillary and mandibular dentitions did not significantly change during post-treatment follow-up. Conclusions: Maxillary molar intrusion using mini-screws is an effective treatment for open bite correction, with the achieved occlusion demonstrating 3D stability at least 1 year after treatment.

Abstract:Aims: This study was conducted to systematically evaluate the biomechanical impact of varying degrees of root and bone resorption resulting from periodontitis and orthodontic tooth movement (OTM) on the mandibular anterior teeth. The objective was to determine whether these distinct resorption patterns exert a specific influence on tooth displacement and strain patterns. Methods: A finite element (FE) model of an idealized anterior mandible from the first premolar in the third to the fourth quadrant was developed without bone or root resorption and a constant periodontal ligament (PDL) thickness of 0.2 mm. Variations included three root resorption levels (0%, 20%, 50%) and three bone resorption types (circular 50%, circular 80%, vestibular 80%). Models ranged from 200,000 to 440,000 elements and 55,000 to 130,000 nodes. Orthodontic forces, namely root torque (5 Nmm), intrusion (0.2 N), and distalization (0.5 N) were applied for subsequent crown displacement and PDL strain analysis. Results: A total of 180 simulations were performed. Simulations showed that displacement was similar across different bone resorption conditions, irrespective of modeled root resorptions. Circumferential bone resorption increased tooth displacement, regardless of root resorption status. Vestibular bone resorption exhibited less increase in tooth displacement. However, when accompanied by root resorption, the combination exacerbated tooth displacement. Strains in the PDL clearly increased with a circumferential bone resorption of 80%. Conclusions: This study highlights the critical role of bone resorption in tooth displacement during OTM, particularly the challenges associated with circumferential resorption. Clinicians must consider both bone and root resorption for personalized medicine treatment of patients with severe periodontitis, in favor of low-force application strategies to optimize outcomes and minimize complications linked to excessive tooth displacement.
