Abstract:INTRODUCTION The Cox proportional hazards model is a multivariate method used in survival analysis. Undoubtedly, many factors influence survival rates. Some of them are well documented; some are suspected but difficult to measure, such as 'clinical management' while others are unsuspected and unmeasured, and cannot be controlled except by randomization in prospective studies.' However, in analysing epidemiological data, the investigator often wishes to adjust for the effect of some variables (confounders; which are associated with the study variable or exposure and the outcome) so that the effect of other variables can be defined more clearly.' That is, multivariate analysis takes account of correlations between variables when estimating the effect of the study variable with survival period. Univariate analysis cannot allow for these correlations and, as a result, may fail to identify some factors that affect survival while falsely identifying others. For example, in a univariate analysis, a long duration of pretransplant dialysis may have an apparent beneficial effect on kidney graft survival. However, this may be entirely due to the correlation between the duration of pretransplant dialysis and the number of pretransplant transfusions. A multivariate analysis including both variables simultaneously would help distinguish the benefits of each factor.' There are two main reasons for modelling survival data. One objective is to determine which combination of potential explanatory variables affect the form of the hazard function. Another reason for modelling the hazard function is to obtain an estimate of the hazard function itself for an individual. We discuss here the concept, assumptions, analysis and interpretation of multivariate survival analysis using the Cox proportional hazards model.

Abstract:Cox proportional hazards model is a semi-parametric model that leaves its baseline hazard function unspecified. The rationale to use Cox proportional hazards model is that (I) the underlying form of hazard function is stringent and unrealistic, and (II) researchers are only interested in estimation of how the hazard changes with covariate (relative hazard). Cox regression model can be easily fit with coxph() function in survival package. Stratified Cox model may be used for covariate that violates the proportional hazards assumption. The relative importance of covariates in population can be examined with the rankhazard package in R. Hazard ratio curves for continuous covariates can be visualized using smoothHR package. This curve helps to better understand the effects that each continuous covariate has on the outcome. Population attributable fraction is a classic quantity in epidemiology to evaluate the impact of risk factor on the occurrence of event in the population. In survival analysis, the adjusted/unadjusted attributable fraction can be plotted against survival time to obtain attributable fraction function.

Abstract:In medical research, analyzing the time it takes for a phenomenon to occur is sometimes crucial. However, various factors can contribute to the length of survival or observation periods, and removing specific data can lead to bias results. In this paper, we discuss the Kaplan-Meier analysis and Cox proportional hazards regression model, which are the most frequently used methods in survival analysis. For the first step, we shall discuss the temporal concepts needed in survival analysis, such as cohort studies and then the basic statistical functions dealt with in survival analysis. After solidifying the concepts, methods of understanding and practical application of the Kaplan-Meier survival analysis is noted. After that, we will discuss the analysis methods for the Cox proportional hazards regression model, which includes multiple covariates. With the interpretation method of Cox proportional hazards regression result, we then discuss methods for checking the assumptions of the Cox proportional hazards regression, such as log minus log plots. Finally, we briefly explain the concept of time-dependent regression analysis. It is our aim that through this paper, readers can obtain an understanding on survival analysis and learn how to perform it.

Survival analysis under non-proportional hazards: investigating non-inferiority or equivalence in time-to-event data

Abstract:The classical approach to analyze time-to-event data, e.g. in clinical trials, is to fit Kaplan-Meier curves yielding the treatment effect as the hazard ratio between treatment groups. Afterwards commonly a log-rank test is performed in order to investigate whether there is a difference in survival, or, depending on additional covariates, a Cox proportional hazard model is used. However, in numerous trials these approaches fail due to the presence of non-proportional hazards, resulting in difficulties of interpreting the hazard ratio and a loss of power. When considering equivalence or non-inferiority trials, the commonly performed log-rank based tests are similarly affected by a violation of this assumption. Here we propose a parametric framework to assess equivalence or non-inferiority for survival data. We derive pointwise confidence bands for both, the hazard ratio and the difference of the survival curves. Further we propose a test procedure addressing non-inferiority and equivalence by directly comparing the survival functions at certain time points or over an entire range of time. We demonstrate the validity of the methods by a clinical trial example and by numerous simulation results.

Abstract:Data that have a multilevel structure occur frequently across a range of disciplines, including epidemiology, health services research, public health, education and sociology. We describe three families of regression models for the analysis of multilevel survival data. First, Cox proportional hazards models with mixed effects incorporate cluster-specific random effects that modify the baseline hazard function. Second, piecewise exponential survival models partition the duration of follow-up into mutually exclusive intervals and fit a model that assumes that the hazard function is constant within each interval. This is equivalent to a Poisson regression model that incorporates the duration of exposure within each interval. By incorporating cluster-specific random effects, generalised linear mixed models can be used to analyse these data. Third, after partitioning the duration of follow-up into mutually exclusive intervals, one can use discrete time survival models that use a complementary log-log generalised linear model to model the occurrence of the outcome of interest within each interval. Random effects can be incorporated to account for within-cluster homogeneity in outcomes. We illustrate the application of these methods using data consisting of patients hospitalised with a heart attack. We illustrate the application of these methods using three statistical programming languages (R, SAS and Stata).

Abstract:The classical Cox model is the most popular procedure for studying right-censored data in survival analysis. However, it is based on the fundamental assumption of proportional hazards (PH). Modified Cox models, stratified and extended, have been widely employed as solutions when the PH assumption is violated. Nevertheless, prior comparisons of the modified Cox models did not employ comprehensive Monte-Carlo simulations to carry out a comparative analysis between the two models. In this paper, we conducted extensive Monte-Carlo simulation to compare the performance of the stratified and extended Cox models under varying censoring rates, sample sizes, and survival distributions. Our results suggest that the models' performance at varying censoring rates and sample sizes is robust to the distribution of survival times. Thus, their performance under Weibull survival times was comparable to that of exponential survival times. Furthermore, we found that the extended Cox model outperformed other models under every combination of censoring, sample size and survival distribution.

Abstract:Clinical research usually involves time-to-event survival analysis, in which the presence of a competing event is prevalent. It is acceptable to use the conventional Cox proportional hazard regression to model cause-specific hazard. However, this cause-specific hazard cannot directly translate to the cumulative incidence function, and the latter is usually clinically relevant. The subdistribution hazard regression directly quantifies the impact of covariates on the cumulative incidence. When estimating the subdistribution hazard, subjects experiencing competing event continue to contribute to the risk set, and censoring weights are assigned to them after the competing event time. The weights are the conditional probability that a subject remains uncensored, and can be modelled to depend on the covariates of a subject. The first option to perform regression on the subdistribution hazard was the crr() function in the cmprsk package. However, it is not straightforward to draw a nomogram, which is a user-friendly tool for risk prediction, with the crr() function. To overcome this problem, we show an alternative method to use a nomogram function based on result of subdistribution hazard modeling.
CaseCohortCoxSurvival: an R Package for Case-Cohort Inference for Relative Hazard and Pure Risk under the Cox Model

Abstract:The case-cohort design allows analysis of multiple endpoints and only requires covariates to be measured for cases and non-cases in a random subcohort from the cohort. Stratification of subcohort sampling and weight calibration increase efficiency of estimates of log-relative hazards and covariate-specific pure risk, but they may require specifically adapted variance estimators. Some recent articles in epidemiology and medical journals used an inappropriate "robust" variance estimator. In addition, stratification, weight calibration and analysis of pure risk seem underutilized in case-cohort studies, possibly because practitioners are put off by the varied technical methodologic literature and lack of convenient software. We recently proposed a unified approach to variance estimation for Cox model log-relative hazards and pure risks, and we implemented it in an R package, CaseCohortCoxSurvival, available on CRAN, that allows appropriate and convenient analysis of case-cohort data, with and without stratification, weight calibration, or missing at random phase-two data. Here we illustrate how easy it is to use CaseCohortCoxSurvival to analyze data from the Prostate, Lung, Colorectal and Ovarian Cancer Screening Trial to estimate pure covariate-specific risk of prostate cancer incidence with various case-cohort design and analysis options. These analyses also indicate situations where the simple "robust" variance is too large.

Abstract:One of the most common ways researchers compare survival outcomes across treatments when confounding is present is using Cox regression. This model is limited by its underlying assumption of proportional hazards; in some cases, substantial violations may occur. Here we present and compare approaches which attempt to address this issue, including Cox models with time-varying hazard ratios; parametric accelerated failure time models; Kaplan-Meier curves; and pseudo-observations. To adjust for differences between treatment groups, we use Inverse Probability of Treatment Weighting based on the propensity score. We examine clinically meaningful outcome measures that can be computed and directly compared across each method, namely, survival probability at time T, median survival, and restricted mean survival. We conduct simulation studies under a range of scenarios, and determine the biases, coverages, and standard errors of the Average Treatment Effects for each method. We then apply these approaches to two published observational studies of survival after cancer treatment. The first examines chemotherapy in sarcoma, where survival is very similar initially, but after two years the chemotherapy group shows a benefit. The other study is a comparison of surgical techniques for kidney cancer, where survival differences are attenuated over time.

Abstract:The Cox proportional hazards model is commonly used to examine the covariate-adjusted association between a predictor of interest and the risk of mortality for censored survival data. However, it assumes a parametric relationship between covariates and mortality risk though a linear predictor. Generalized additive models (GAMs) provide a flexible extension of the usual linear model and are capable of capturing nonlinear effects of predictors while retaining additivity between the predictor effects. In this paper, we provide a review of GAMs and incorporate bivariate additive modeling into the Cox model for censored survival data with applications to estimating geolocation effects on survival in spatial epidemiologic studies.

Abstract:The log-rank test and the Cox proportional hazards model are commonly used to compare time-to-event data in clinical trials, as they are most powerful under proportional hazards. But there is a loss of power if this assumption is violated, which is the case for some new oncology drugs like immunotherapies. We consider a two-stage test procedure, in which the weighting of the log-rank test statistic depends on a pre-test of the proportional hazards assumption. I.e., depending on the pre-test either the log-rank or an alternative test is used to compare the survival probabilities. We show that if naively implemented this can lead to a substantial inflation of the type-I error rate. To address this, we embed the two-stage test in a permutation test framework to keep the nominal level alpha. We compare the operating characteristics of the two-stage test with the log-rank test and other tests by clinical trial simulations.

Abstract:Survival analysis is commonly conducted in medical and public health research to assess the association of an exposure or intervention with a hard end outcome such as mortality. The Cox (proportional hazards) regression model is probably the most popular statistical tool used in this context. However, when the exposure includes compositional covariables (that is, variables representing a relative makeup such as a nutritional or physical activity behaviour composition), some basic assumptions of the Cox regression model and associated significance tests are violated. Compositional variables involve an intrinsic interplay between one another which precludes results and conclusions based on considering them in isolation as is ordinarily done. In this work, we introduce a formulation of the Cox regression model in terms of log-ratio coordinates which suitably deals with the constraints of compositional covariates, facilitates the use of common statistical inference methods, and allows for scientifically meaningful interpretations. We illustrate its practical application to a public health problem: the estimation of the mortality hazard associated with the composition of daily activity behaviour (physical activity, sitting time and sleep) using data from the U.S. National Health and Nutrition Examination Survey (NHANES).

Abstract:The Cox model is a regression technique for performing survival analyses in epidemiological and clinical research. This model estimates the hazard ratio (HR) of a given endpoint associated with a specific risk factor, which can be either a continuous variable like age and C-reactive protein level or a categorical variable like gender and diabetes mellitus. When the risk factor is a continuous variable, the Cox model provides the HR of the study endpoint associated with a predefined unit of increase in the independent variable (e.g., for every 1-year increase in age, 2 mg/L increase in C-reactive protein). A fundamental assumption underlying the application of the Cox model is proportional hazards; in other words, the effects of different variables on survival are constant over time and additive over a particular scale. The Cox regression model, when applied to etiological studies, also allows an adjustment for potential confounders; in an exposure-outcome pathway, a confounder is a variable which is associated with the exposure, is not an effect of the exposure, does not lie in the causal pathway between the exposure and the outcome, and represents a risk factor for the outcome.

Abstract:In many biomedical applications, outcome is measured as a "time-to-event" (e.g., disease progression or death). To assess the connection between features of a patient and this outcome, it is common to assume a proportional hazards model and fit a proportional hazards regression (or Cox regression). To fit this model, a log-concave objective function known as the "partial likelihood" is maximized. For moderate-sized data sets, an efficient Newton-Raphson algorithm that leverages the structure of the objective function can be employed. However, in large data sets this approach has two issues: (i) The computational tricks that leverage structure can also lead to computational instability; (ii) The objective function does not naturally decouple: Thus, if the data set does not fit in memory, the model can be computationally expensive to fit. This additionally means that the objective is not directly amenable to stochastic gradient-based optimization methods. To overcome these issues, we propose a simple, new framing of proportional hazards regression: This results in an objective function that is amenable to stochastic gradient descent. We show that this simple modification allows us to efficiently fit survival models with very large data sets. This also facilitates training complex, for example, neural-network-based, models with survival data.
Covariate-adjusted Group Sequential Comparisons of Survival Probabilities

Abstract:In confirmatory clinical trials, survival outcomes are frequently studied and interim analyses for efficacy and/or futility are often desirable. Methods such as the log rank test and Cox regression model are commonly used to compare treatments in this setting. They rely on a proportional hazards (PH) assumption and are subject to type I error rate inflation and loss of power when PH are violated. Such violations may be expected a priori, particularly when the mechanisms of treatments differ such as immunotherapy vs. chemotherapy for treating cancer. We develop group sequential tests for comparing survival curves with covariate adjustment that allow for interim analyses in the presence of non-PH and offer easily interpreted, clinically meaningful summary measures of the treatment effect. The joint distribution of repeatedly computed test statistics converges to the canonical joint distribution with a Markov structure. The asymptotic distribution of the test statistics allows marginal comparisons of survival probabilities at multiple fixed time points and facilitates both critical value specification to maintain type I error control and sample size/power determination. Simulations demonstrate that the achieved type I error rate and power of the proposed tests meet targeted levels and are robust to the PH assumption and covariate influence. The proposed tests are illustrated using a clinical trial dataset from the Blood and Marrow Transplant Clinical Trials Network 1101 trial.

Abstract:Proportional hazard (PH) models can be formulated with or without assuming a probability distribution for survival times. The former assumption leads to parametric models, whereas the latter leads to the semi-parametric Cox model which is by far the most popular in survival analysis. However, a parametric model may lead to more efficient estimates than the Cox model under certain conditions. Only a few parametric models are closed under the PH assumption, the most common of which is the Weibull that accommodates only monotone hazard functions. We propose a generalization of the log-logistic distribution that belongs to the PH family. It has properties similar to those of log-logistic, and approaches the Weibull in the limit. These features enable it to handle both monotone and nonmonotone hazard functions. Application to four data sets and a simulation study revealed that the model could potentially be very useful in adequately describing different types of time-to-event data.

Abstract:Time-varying covariance occurs when a covariate changes over time during the follow-up period. Such variable can be analyzed with the Cox regression model to estimate its effect on survival time. For this it is essential to organize the data in a counting process style. In situations when the proportional hazards assumption of the Cox regression model does not hold, we say that the effect of the covariate is time-varying. The proportional hazards assumption can be tested by examining the residuals of the model. The rejection of the null hypothesis induces the use of time varying coefficient to describe the data. The time varying coefficient can be described with a step function or a parametric time function. This article aims to illustrate how to carry out statistical analyses in the presence of time-varying covariates or coefficients with R.

Abstract:Survival analysis, or more generally, time-to-event analysis, refers to a set of methods for analyzing the length of time until the occurrence of a well-defined end point of interest. A unique feature of survival data is that typically not all patients experience the event (eg, death) by the end of the observation period, so the actual survival times for some patients are unknown. This phenomenon, referred to as censoring, must be accounted for in the analysis to allow for valid inferences. Moreover, survival times are usually skewed, limiting the usefulness of analysis methods that assume a normal data distribution. As part of the ongoing series in Anesthesia & Analgesia, this tutorial reviews statistical methods for the appropriate analysis of time-to-event data, including nonparametric and semiparametric methods—specifically the Kaplan-Meier estimator, log-rank test, and Cox proportional hazards model. These methods are by far the most commonly used techniques for such data in medical literature. Illustrative examples from studies published in Anesthesia & Analgesia demonstrate how these techniques are used in practice. Full parametric models and models to deal with special circumstances, such as recurrent events models, competing risks models, and frailty models, are briefly discussed.
