Abstract:Background: One of the common causes of respiratory distress in neonate is persistent pulmonary hypertension of newborn (PPHN) and has been estimated to occur in 2 per 1000 live born term infants. Objective: To evaluate the effect of injectable Magnesium Sulphate (MgSO4) in the treatment of Persistent Pulmonary Hypertension of Newborn. Methodology: It was a prospective, nonrandomized, clinical study conducted from August 2015 to July 2017 among 25 neonates having moderate to severe PPHN in the Pediatric Cardiac Intensive Care Unit (CICU) of Dhaka Shishu (Children) Hospital. Injectable Magnesium Sulphate was used along with other supportive management. Outcome measures include drop of pulmonary vascular resistance and increase oxygenation. Side effects of Magnesium Sulphate were observed and outcome was recorded. Data were analyzed by using SPSS version 17. Results: There was significant improvement of oxygenation and decrease in pulmonary vascular resistance at 72 hours after use of MgSO4 (p=000). Complications were present in 28% cases which include hypotension in 16% patients, urinary retention in 8% and altered GI function in 8% cases. Mortality was 16% among study population. Use of magnesium sulphate in severe persistent pulmonary hypertension of the newborn

Abstract:Persistent pulmonary hypertension in the newborn (PPHN) is a neonatal emergency due to the failure to achieve normal cardiopulmonary adaptation following delivery. A clinical trial to assess the efficacy of magnesium sulphate (MgSO4) in the treatment of PPHN was conducted in the tertiary level neonatal intensive care unit of RIPAS Hospital, Brunei Darussalam over a period of 2 years. Twelve newborn babies admitted consecutively to the neonatal intensive care unit with severe hypoxemia due to persistent pulmonary hypertension were included in this trial. A loading dose of 200 mg/kg MgSO4 was given over a period of 20 min, followed by a continuous infusion at the rate of 20-150 mg/kg/h to obtain a serum magnesium level between 3.5 and 5.5 mmol/l. No other vasodilators were used and babies were not hyperventilated. Dopamine was commenced at 5-10 microg/kg/min before the loading dose of MgSO4 was given. Mean blood pressure was maintained with short periods of dopamine alone or in combination with dobutamine. Oxygen index (OI) and alveolar-arterial oxygen gradient (A-aDO2) showed significant improvement within 24 h of treatment. In this study, MgSO4 was found to be a safe and effective pulmonary vasodilator. MgSO4 could be used as a first-line vasodilator in developing countries because of its low cost and high efficacy. Parenteral MgSO4 is easy to administer and monitor during treatment. When MgSO4 is used, systemic hypotension can be adequately controlled with ionotropes.
Intravenous magnesium sulfate in pulmonary hypertension of the newborn: a systematic review

Abstract:Objective: Pulmonary hypertension in the newborn (PPHN) is a significant clinical condition characterized by elevated pulmonary artery pressures, leading to serious health consequences. Magnesium sulfate, known for its vasodilatory properties, has been studied for its potential benefits in managing PPHN. This systematic review evaluates the efficacy and safety of magnesium sulfate in neonates with PPHN. Materials and methods: A systematic literature search was conducted on PubMed and Scopus up to March 10, 2024. Studies were included based on predefined Population, Intervention, Comparison, Outcome, Study (PICOS) criteria focusing on pediatric patients with PPHN treated with magnesium sulfate, compared against placebo or other pharmacological interventions. Outcomes of interest included resolution of PPHN, improved oxygenation, and decreased oxygenation index. Results: From a total of 1,233 articles screened, four studies met the inclusion criteria, including three randomized controlled trials and one multicentric retrospective study. The comparisons included nebulized magnesium sulfate, oral sildenafil, and inhaled nitric oxide. The outcomes varied, with none reported consistently across more than two studies, making a meta-analysis unfeasible. Results indicated a potential benefit of magnesium sulfate in improving pulmonary pressures and oxygenation, but the evidence was insufficient to establish definitive conclusions due to the heterogeneity and a limited number of studies. Conclusions: The limited data suggest that, while magnesium sulfate may have a role in the management of PPHN, it should not replace established therapies. Further research is needed to better define its efficacy and safety profile.
Efficacy of Prostaglandin E Combined with Magnesium Sulfate Treatment of Neonatal Pulmonary Hypertension

Abstract:Objective To investigate the efficacy and safety of prostaglandin E with magnesium sulfate on persistent pu1monary hypertension of newborn(PPHN).Methods 26 children with PPHN were treated with prostaglandin E with magnesium sulfate.Results 24 children were cured,while 2 died.6h after treatment,PaO2,SaO2 gradually increased,while PAMP decreased significantly(P0.05).The course of treatment in 2 patients the low calcium convulsions,be symptomatic treatment after symptoms disappear.Conclusion Prostaglandin E and magnesium sulfate combination treatment of neonatal persistent pulmonary hypertension is effective,safe,worthy of recommendation.

Abstract:Purpose: To investigate the efficacy of the combined use of sildenafil and magnesium sulfate in the treatment of persistent pulmonary hypertension of the newborn (PPHN), and its influence on hemodynamics.Methods: A total of 174 children with persistent pulmonary hypertension who were treated in Ganzhou People’s Hospital, Ganzhou, China were selected and randomly assigned to joint group (JG) and control group (CG), with 87 patients in each group. The CG group received magnesium sulfate, while the JG group received sildenafil plus magnesium sulphate. The respiratory parameters of the children were analyzed using blood gas analyzer, while their hemodynamic indices were evaluated using color Doppler echocardiography. The levels of cytokines and inflammatory factors were determined by enzyme-linked immunosorbent assay (ELISA).Results: Time taken for symptom disappearance, oxygen therapy, and hospitalization period were shorter in JG than in CG (p < 0.05). Post-treatment, the respiratory parameters (PaO2, PaCO2, and SaO2) in both groups s improved, with lower levels of PaO2 and PaCO2, and a higher level of SaO2 in JG (p < 0.05). Following treatment, the levels of systemic vascular resistance (SVR), posterior pulmonary vascular resistance (PVR) and pulmonary artery pressure (PA) in JG were significantly reduced, relative to CG (p < 0.05). Similarly, the expression of endothelin -1 (ET-1), brain natriureticpeptide (BNP), and angiotensin 1 (ANG-1) improved, with lower levels of ET-1 and BNP, and a higher level of ANG-1 in JG (p < 0.05). There was post-treatment reduction as well in IL-6 and TNF-α, with lower levels in JG (p < 0.05). Patients in JG showed higher total treatment effectiveness and a lowerincidence of adverse reactions than those in CG (p < 0.05).Conclusion: The combined use of sildenafil and magnesium sulfate enhances the management of PPHN, ameliorates respiratory parameters, hemodynamics, and levels of cytokines and inflammatory factors. These findings provide evidence-based medical references for a new treatment strategy for PPHN.

Abstract:Persistent pulmonary hypertension of neonatal (PPHN) refers to the continuous increase of pulmonary vascular resistance after birth, which causes the abnormal development from fetal circulation to ″adult″ type circulatory and the subsequent right to left shunt through atrial septum or arterial duct. Clinical symptoms include severe hypoxemia and even respiratory failure. PPHN can be seen in the congenital dysplasia of the lung, as well as in the substantial lung diseases. The incidence of the disease is around 0.2%, and mortality about 10%. In addition to the treatment of the underlying disease, timely and effective treatments like respiratory support and vasoactive drugs can significantly reduce the incidence and mortality of the disease. This article summarizes the current treatment progress in persistent pulmonary hypertension of neonatal, and provides useful information for clinicians. Abstract:Persistent pulmonary hypertension of the newborn (PPHN) is a condition caused by failure of pulmonary vascular adaptation at birth, resulting in severe hypoxia. Several therapeutic modalities are being tried in developing countries where established therapies (inhaled nitric oxide and extracorporeal membrane oxygenation) are widely unavailable. This study aimed to assess the efficacy of milrinone versus sildenafil as available alternative therapeutics in treating PPHN. Forty neonates (>34 weeks) admitted to neonatal intensive care units with evidence of PPHN were randomly allocated to receive either oral sildenafil (0.5–2 mg/kg/6 hours) or intravenous milrinone (0.25–0.75 mic/kg/min). Primary outcomes included improvements in systolic pulmonary artery pressure and oxygen saturation index (OSI) at 24 and 48 hours after treatment. Secondary outcomes included the duration of hospitalization and mechanical ventilation. The ClinicalTrials identifier is NCT04391478. Both groups showed significant improvement in the post-treatment hemodynamic variables compared with pretreatment levels ( P < 0.05 for all parameters). Systolic pulmonary artery pressure and OSI values significantly improved in both study groups compared with baseline ( P < 0.001). The 24-hour and 48-hour post-treatment OSI values were much lower in the milrinone group than those in the sildenafil group ( P < 0.05). The length of hospital stay was significantly shorter in the milrinone group than that in the sildenafil group ( P < 0.05). There were no significant differences in the duration of mechanical ventilation, incidence of intracranial hemorrhage and pulmonary hemorrhage, or mortality between the 2 groups ( P > 0.05). In conclusion, milrinone and sildenafil are effective and well-tolerated in neonates with PPHN, particularly when inhaled nitric oxide and extracorporeal membrane oxygenation are not available. Milrinone is superior to sildenafil in improving oxygenation without lowering blood pressure parameters.

Abstract:Background: Persistent pulmonary hypertension of the newborn (PPHN) is a serious disease among newborns. Pulmonary vasodilators such as Sildenafil, Bosentan and Milrinone are important treatment modalities to treat persistent pulmonary hypertension of the newborn, especially in resource limited centers where inhaled nitric oxide is not available. Objective: To assess and compare the effectiveness of Bosentan and Sildenafil in persistent pulmonary hypertension of the newborn. Method: This randomized controlled trial was done in the NICU, ICMH, Matuail, Dhaka during July 2017 to July 2019. Following echocardiographic diagnosis, a total of 76 patients were randomly assigned into group A and group B (by lottery). Bosentan was given to group A (1mg/kg/body wt) 12 hourly for 7 days and Sildenafil was given to group B (2mg/kg/body wt.) 8 hourly for 7 days via oro-gastric tube/orally. Then heart rate, respiratory rate, cyanotic changes, SPO2 was recorded every day for 7 days and finally on the 8th day echocardiographic findings were obtained. Effects of two groups were compared. Results: The age of presentation of persistent pulmonary hypertension of newborn was < 24 hours and male newborns were predominant in both groups. Magnesium Sulfate in Treatment of Hypoxic Pulmonary Hypertension in Experimental Animal

Abstract:Objective To evaluate the effects of magnesium sulfate (MgSO 4) in the treatment of persistent pulmonary hypertension of newborn (PPHN). Methods Hypoxia-induced pulmonary hypertension models of seven piglets were constructed. A catheter was inserted through the external jugular vein and advanced to the right atrium, right ventricle and into the pulmonary artery to measure pulmonary arterial pressure (PAP) and cardiac output (CO). Another catheter was inserted into the right femoral artery to measure systemic arterial pressure (SAP). Heart rate (HR), arterial oxygen saturation (SaO 2), arterial blood gases, and serum magnesium (Mg ) concentrations were estimated. Results After hypoxia was induced for 10 minutes, there were significant increases in systolic PAP (SPAP), diastolic PAP (DPAP) and mean PAP (MPAP) (42.1%±19.4%, 46.8%±23.9% and 39.7%±22.1% respectively; P 0.01). Post MgSO 4 infusion, significant decreases occurred in SPAP, DPAP and MPAP (25.2%± 5.4%, 25.5%±6.3% and 22.5%±5.8% respectively; P 0.01), while systolic SAP remained unchanged ( P 0.05), but serum Mg  rose significantly from (0.79±0.12) mmol/L to (2.78±1.11) mmol/L following the infusion of MgSO 4. Conclusion Magnesium sulfate decreases PAP significantly and improves PO 2 with less effect on the SAP, HR and CO.

Abstract:Persistent Pulmonary Hypertension of the Newborn (PPHN) is defined as a failure of the normal circulatory transition after birth. PPHN manifests as a progressive and potentially fatal hypoxemic respiratory failure. The condition is associated with a persistent increase in pulmonary vascular resistance and reduced blood flow in the pulmonary vessels without an accompanying congenital heart defect. The abnormalities in pulmonary circulation lead to right-to-left shunting through the foramen ovale and usually also through the ductus arteriosus, with resultant hypoxemia, which manifests clinically as cyanosis. Management options in PPHN include optimal lung inflation, oxygenation, prevention of acidosis, stabilization of blood pressure, sedation and pulmonary vasodilator therapy. While inhaled nitric oxide (iNO) is the most commonly used treatment option, alternative methods can be used whenever is unavailable or contraindicated. We report three cases of severe PPHN, in which due to the lack of adequate response to intravenous MgSO4, the patients were switched to milrinone.
Oral Sildenafil in Infants With Persistent Pulmonary Hypertension of the Newborn: A Pilot Randomized Blinded Study

Abstract:BACKGROUND. Persistent pulmonary hypertension (PPHN) occurs in as many as 6.8 of 1000 live births. Mortality is ∼10% to 20% with high-frequency ventilation, surfactant, inhaled nitric oxide, and extracorporeal membrane oxygenation but is much higher when these therapies are not available. Sildenafil is a phosphodiesterase inhibitor type 5 that selectively reduces pulmonary vascular resistance. OBJECTIVE. Our goal was to evaluate the feasibility of using oral sildenafil and its effect on oxygenation in PPHN. DESIGN. This study was a proof-of-concept, randomized, masked study in infants >35.5 weeks' gestation and <3 days old with severe PPHN and oxygenation index (OI) >25 admitted to the NICU (Hospital Niño Jesús, Barranquilla, Colombia). The sildenafil solution was prepared from a 50-mg tablet. The first dose (1 mg/kg) or placebo was given by orogastric tube <30 minutes after randomization and every 6 hours. Preductal saturation and blood pressure were monitored continuously. OI was calculated every 6 hours. The main outcome variable was the effect of oral sildenafil on oxygenation. Sildenafil or placebo was discontinued when OI was <20 or if there was no significant change in OI after 36 hours. RESULTS. Six infants with an OI of >25 received placebo, and 7 received oral sildenafil at a median age of 25 hours. All infants were severely ill, on fraction of inspired oxygen 1.0, and with similar ventilatory parameters. Intragastric sildenafil and placebo were well tolerated. In the treatment group, OI improved in all infants within 6 to 30 hours, all showed a steady improvement in pulse oxygen saturation over time, and none had noticeable effect on blood pressure; 6 of 7 survived. In the placebo group, 1 of 6 infants survived. CONCLUSIONS. Oral sildenafil was administered easily and tolerated as well as placebo and improved OI in infants with severe PPHN, which suggests that oral sildenafil may be effective in the treatment of PPHN and underscores the need for a large, controlled trial.

The efficacy of nitric oxide inhalation and magnesium sulfate infusion in the treatment of neonatal pulmonary hypertension

Abstract:Objective To evaluate the effects of inhaled nitric oxide (NO) and magnesium sulfate (MgSO4) infusion on neonatal pulmonary hypertension. Methods A total of 34 neonates with acute hypoxic pulmonary hypertension were treated with inhaled nitric oxide (iNO group, n= 25) or MgSO4 infusion (MgSO4 group, n = 11), respectively. All the neonates underwent serial echocardiography evaluation before and during therapy to estimate systolic pulmonary artery pressure (SPAP) with tricuspid regurgitation jet (TR) or PDA shunting prior to initiation of iNO or MgSO4 infusion, 30-120 min and 18-24 h after initiation of treatment, and at the end of treatment. Ratio of pulmonary to systemic pressure (SPAP/SBP) was also calculated to evaluate the effectiveness of treatment. Results In iNO group, SPAP decreased from (59±9) mm Hg to (43±9) mm Hg, (40±14) mm Hg and (25±8) mm Hg, and SPAP/SBP decreased from 0.93±0.17 to 0.69±0.13, 0.65± 0.24 and 0.36±0.15, 30-120 min, 18-24 h after initiation of treatment, and at the end of treatment, respectively (P0.01). However, no significant change in SBP was found. In MgSO4 group, SPAP decreased from (62±9) mm Hg to (54±6) mm Hg, (44±8) mm Hg and (35±21) mm Hg, and ratio of SPAP to SBP decreased from 0.93±0.12 to 0.82±0.29、0.65±0.28 and 0.54±0.35, 30-120 min, 18-24 h after initiation of treatment, and at the end of treatment, respectively ( P 0.05). SBP decreased significantly in 30-120 min after MgSO4 infusion and recovered to normal 18-24 h after infusion. Conclusions Both low-dose of inhaled nitric oxide and MgSO4 infusion are good in lowering pulmonary vascular resistance in the newborn with pulmonary hypertension, and inhaled nitric oxide is a highly selective therapy as pulmonary vasodilator and more rapidly effective.
Nebulized Salbutamol with or without Magnesium Sulphate in the Management of Acute Asthma in Children in India: A Randomized Controlled Trial

Abstract:Background: There is inconclusive evidence on the role of nebulized magnesium sulphate (MgSO4) in the management of acute asthma in paediatric population. Objectives: Whether the use of nebulized salbutamol with or without MgSO4 in the management of acute asthma results in clinically significant improvement in lung function in Indian children? The primary outcome measure was to assess improvements in peak expiratory flow rate (PEFR), heart rate, respiratory rate and SpO2. Methods: This was a single centre; prospective double-blind randomized control trial conducted in paediatric intensive care unit of a tertiary care centre. Ninety children of 6-14 years with acute exacerbations of bronchial asthma were enrolled to receive either inhaled magnesium sulphate (95 mg) with salbutamol (5 mg) or inhaled salbutamol (5 mg) alone. All patients got three nebulizations done during the first hour at 20 min intervals, two nebulizations during the second hour at 30 min intervals, hourly for the next 2 h and then at 24 and 48 h. Results: Eighty-five patients were finally analysed as per protocol analysis. The trial showed that PEFR increased gradually in both groups over the study duration, but it was statistically not significant. Heart rate decreased significantly in both groups over the study duration. Respiratory rate decreased significantly between the groups at 24 and 48 h only. SpO2 improved too in both groups but was not significant statistically. Conclusion: The addition of nebulized MgSO4 to salbutamol does not seem to result in improvement in lung function in the management of acute asthma in Indian children.
Role of Intravenous Magnesium Sulphate in Term Neonates with Hypoxic Ischemic Encephalopathy (HIE) in a Low-income Country: A Randomised Clinical Trial

Abstract:Objective: To determine the effects of magnesium sulphate in term neonates with hypoxic ischemic encephalopathy (HIE) in reducing mortality and morbidity. Study design: Randomised clinical trial. Place and duration of study: Department of Neonatology, Services Hospital, Lahore, Pakistan from April to December 2019. Methodology: Term babies (inborn or outborn), fulfilling the operational definition of hypoxic ischemic encephalopathy, reaching within 6 hours of delivery in Nursery Department of Pediatric Medicine Unit-II, Services Hospital, Lahore, were included. Sarnat score was used for staging the severity of HIE. Cases were administered magnesium sulphate (MgSO4) as intravenous infusion. Rest of the management was similar for cases and controls. Mortality was defined as death due to birth asphyxia; whereas, morbidity was assessed by comparing the following at discharge: the grade of hypoxic ischemic encephalopathy, presence and frequency of seizures, duration of seizures, ability to suck feed and neurological problems such as abnormalities of muscle tone and neonatal reflexes. Babies with prematurity, dysmorphism comorbidities or arriving after 6 hours of birth, were excluded. Chi-square test was used for comparison; and p value <0.05 was considered significant. Results: Gender, mode of delivery, mode of resuscitation at birth, major risk factors (prolonged labour, premature rupture of membranes, presence of meconium-stained amniotic fluid) were comparable in both groups. The duration of seizures, ability to suck feed and presence of neurological problems at discharge were significantly better in magnesium sulphate group as compared to control group. Conclusion: Magnesium sulphate is better in establishing earlier suck feed and reducing the duration of seizures and neurological problems in babies with birth asphyxia. Abstract:Objectives. To assess the dose-related effects of inhaled nitric oxide (I-NO) as a specific adjunct to early conventional therapy for term infants with persistent pulmonary hypertension (PPHN), with regard to neonatal outcome, oxygenation, and safety. Methods. Randomized, placebo-controlled, double-masked, dose-response, clinical trial at 25 tertiary centers from April 1994 to June 1996. The primary endpoint was the PPHN Major Sequelae Index ([MSI], including the incidence of death, extracorporeal membrane oxygenation (ECMO), neurologic injury, or bronchopulmonary dysplasia [BPD]). Patients required a fraction of inspired oxygen [Fio2] of 1.0, a mean airway pressure ≥10 cm H2O on a conventional ventilator, and echocardiographic evidence of PPHN. Exogenous surfactant, concomitant high-frequency ventilation, and lung hypoplasia were exclusion factors. Control (0 ppm) or nitric oxide (NO) (5, 20, or 80 ppm) treatments were administered until success or failure criteria were met. Due to slowing recruitment, the trial was stopped at N = 155 (320 planned). Results. The baseline oxygenation index (OI) was 24 ± 9 at 25 ± 17 hours old (mean ± SD). Efficacy results were similar among NO doses. By 30 minutes (no ventilator changes) the Pao2 for only the NO groups increased significantly from 64 ± 39 to 109 ± 78 Torr (pooled) and systemic arterial pressure remained unchanged. The baseline adjusted time-weighted OI was also significantly reduced in the NO groups (-5 ± 8) for the first 24 hours of treatment. The MSI rate was 59% for the control and 50% for the NO doses (P = .36). The ECMO rate was 34% for control and 22% for the NO doses (P = .12). Elevated methemoglobin (>7%) and nitrogen dioxide (NO2) (>3 ppm) were observed only in the 80 ppm NO group, otherwise no adverse events could be attributed to I-NO, including BPD. Conclusion. For term infants with PPHN, early I-NO as the sole adjunct to conventional management produced an acute and sustained improvement in oxygenation for 24 hours without short-term side effects (5 and 20 ppm doses), and the suggestion that ECMO use may be reduced.

Abstract:BACKGROUND: Persistent pulmonary hypertension of the newborn causes systemic arterial hypoxemia because of increased pulmonary vascular resistance and right-to-left shunting of deoxygenated blood. Inhaled nitric oxide decreases pulmonary vascular resistance in newborns. We studied whether inhaled nitric oxide decreases severe hypoxemia in infants with persistent pulmonary hypertension.METHODS: In a prospective, multicenter study, 58 full-term infants with severe hypoxemia and persistent pulmonary hypertension were randomly assigned to breathe either a control gas (nitrogen) or nitric oxide (80 parts per million), mixed with oxygen from a ventilator. If oxygenation increased after 20 minutes and systemic blood pressure did not decrease, the treatment was considered successful and was continued at lower concentrations. Otherwise, it was discontinued and alternative therapies, including extracorporeal membrane oxygenation, were used.RESULTS: Inhaled nitric oxide successfully doubled systemic oxygenation in 16 of 30 infants (53 percent), whereas conventional therapy without inhaled nitric oxide increased oxygenation in only 2 of 28 infants (7 percent). Long-term therapy with inhaled nitric oxide sustained systemic oxygenation in 75 percent of the infants who had initial improvement. Extracorporeal membrane oxygenation was required in 71 percent of the control group and 40 percent of the nitric oxide group (P=0.02). The number of deaths was similar in the two groups. Inhaled nitric oxide did not cause systemic hypotension or increase methemoglobin levels.CONCLUSIONS: Inhaled nitric oxide improves systemic oxygenation in infants with persistent pulmonary hypertension and may reduce the need for more invasive treatments.

The Neuroprotective Efficacy of Postnatal Magnesium Sulfate in Term or Near-Term Infants With Moderate-to-Severe Birth Asphyxia

Abstract:Background In Pakistan, the neonatal mortality rate is 41 per 1,000 live births and birth asphyxia is one of the leading causes of neonatal mortality and morbidity. The goal of this study was to determine whether postnatal magnesium sulfate therapy can improve short- and long-term neurological outcomes in term or near-term neonates with moderate-to-severe birth asphyxia. Methodology This prospective double-blind randomized controlled trial was conducted in the Neonatology Department of the Children's Hospital & The Institute of Child Health, Lahore. A total of 62 neonates (31 in each group) were randomized to receive either three doses of magnesium sulfate infusion at 250 mg/kg per dose, 24 hours apart (treatment group), or three doses of injection 10% distilled water infusion at 3 mL/kg, 24 hours apart (placebo group). Both groups received similar supportive care. The neurodevelopmental assessment was done at six months of age using the ShaMaq Developmental Inventory. Results Demographic data such as gestational age, mean weight, age at presentation, gender, hypoxic-ischemic encephalopathy grade, mode of delivery, and the presence of seizures at presentation were comparable between both groups. In the magnesium sulfate group, statistically significant results were seen in terms of early seizure control (p = 0.001), early initiation of feed (p = 0.002), and shorter duration of hospital stay (p = 0.003). Moreover, the magnesium sulfate group had lower mortality compared to the control group, though it was not statistically significant (p = 0.390). There was no significant difference in terms of cranial ultrasound findings between the two groups (p = 0.783) at the time of discharge. Regarding the neurodevelopmental delay, there was no significant difference between the magnesium sulfate and control groups (p = 0.535). Conclusions Postnatal magnesium sulfate treatment improves short-term neurologic outcomes at discharge in term or near-term neonates with moderate-to-severe perinatal asphyxia. However, no difference was noted in the neurodevelopmental outcome at six months.

Abstract:Persistent pulmonary hypertension (PPHN) of the newborn is a lung parenchymal disorder that causes a wide range of hemodynamic changes in the newborn's systemic circulation. Arising from a multifactorial web of etiology, PPHN is one of the most common reasons for neonatal intensive care unit hospitalization and is associated with increased morbidity and mortality. Historically, multiple treatment modalities have been explored, ranging from oxygen and surfactant therapy to newer upcoming medications like magnesium sulfate and adenosine. This review article has discussed the pathogenesis of PPHN and its relationship with the clinical implications of PPHN, such as heart failure and so on. This article has also explored the diagnostic guidelines and analyzed the existing and the upcoming modalities for treating PPHN.

Abstract:Objective To investigate the clinical effect of high-frequency oscillatory ventilation combined with sildenafil and magnesium sulfate in treatment of newborn with persistent pulmonary hypertension.Methods 122 newborn with persistent pulmonary hypertension from Jan.2012 to Jan.2015 were selected in this study, and they were equally and randomly divided into the observation group and the control group.All of them received sildenafil and magnesium sulfate, the control group was given conventional mechanical ventilation treatment, and the observation group was given the high frequency oscillatory ventilation therapy.The value of PaO2, PaCO2, OI, pH, SaO2, SPAP and SBP were detected.The treatment time, hospital stay and cost were recorded and compared.Results The remarkable response rate was 70.5% and 52.5%, and the clinical efficacy rate was 93.4% and 85.3% respectively in the observation group and the control group.The values of arterial PaO2, PaCO2, OI, pH, SaO2, SPAP and SBP were (44.39±5.12) mmHg, (62.93±8.95) mmHg, (18.12±2.31), (7.39±0.09), (96.28±3.26)%, (26.71±6.07) mmHg and (73.51±9.26) mmHg in the observation group after treatment, which improved more significantly than those in the control group.The treatment time and hospital stay in the observation group were shorter than those in the control group, and the cost was obviously cheaper in the observation group.Conclusion High-frequency oscillatory ventilation combined with sildenafil and magnesium sulfate has good efficacy in treatment of newborn with persistent pulmonary hypertension, which can effectively improve their blood gas and pressure indexes.

Abstract:Persistent pulmonary hypertension in newborn (PPHN) is a serious and possibly fatal syndrome characterized by sustained foetal elevation of pulmonary vascular resistance at birth. PPHN may manifest secondary to other conditions as meconium aspiration syndrome, infection and congenital diaphragmatic hernia. This MiniReview provides the reader with an overview of current and future treatment options for patients with PPHN without congenital diaphragmatic hernia. The study is based on systematic searches in the databases PubMed and Cochrane Library and registered studies on Clinicaltrials.gov investigating PPHN. Inhaled nitric oxide (iNO) is well documented for treatment of PPHN, but 30% fail to respond to iNO. Other current treatment options could be sildenafil, milrinone, prostaglandin analogues and bosentan. There are several ongoing trials with sildenafil, but evidence is lacking for the other treatments and/or for the combination with iNO. Currently, there is no evidence for effect in PPHN of other treatments, for example tadalafil, macitentan, ambrisentan, riociguat and selexipag used for pulmonary arterial hypertension in adults. Experimental studies in animal models for PPHN suggest effect of a series of approaches including recombinant human superoxide dismutase, L-citrulline, Rho-kinase inhibitors and peroxisome proliferator-activated receptor-γ agonists. We conclude that iNO is the most investigated and the only approved pulmonary vasodilator for infants with PPHN. In the iNO non-responders, sildenafil currently seems to be the best alternative either alone or in combination with iNO. Systematic and larger clinical studies are required for testing the other potential treatments of PPHN.
