Although we found several previous studies about the distribution, current trends, and resistance patterns of bacterial keratitis, few data are reported from East China, which is the most densely populated region in China, and has particular geographical and socioeconomic importance.2Sun X. We carried out a retrospective review of all patients with suspected bacterial keratitis who were examined at the Shanghai Eye, Ear, Nose and Throat Hospital between January 1, 2005, and December 31, 2010. The medical records of all participants, whose bacterial keratitis were confirmed by culture, histology, or clinical investigation, were reviewed. Bacterial isolates were identified and tested for antibiotic susceptibility with Kirby-Bauer disk diffusion and E test. Over the 6-year period, 436 bacterial keratitis isolates were identified from 1042 patients. The total number of Gram-positive and -negative isolates was 218 and 202, respectively. Sixteen patients had unidentified bacterial infection. No shifting trend in Gram-positive or -negative was identified. The most common isolate overall was Streptococcus species, and the most common Gram-negative bacteria isolated were Pseudomonas species. Methicillin-resistant Staphylococcus aureus (MRSA) was present in 8.3% of the S aureus isolates, whereas methicillin-resistant coagulase-negative Staphylococcus (MRCNS) was present in 53.1% of the coagulase-negative Staphylococcus isolates. Compared with the first 3 years (2005–2007), there was a trend toward increasing laboratory resistance to fluoroquinolones, which are prescribed as initial empirical monotherapy in many community-based ophthalmic cares in China. None of the Gram-positive isolates were found to be resistant to vancomycin. For the Gram-negative isolates, 90% and 81% were susceptible to gatifloxacin and levofloxacin, respectively. Based on the study by Lichtinger et al and our findings, we recommend initial empiric intensive antimicrobial therapy using either a fluoroquinolone with a fortified cefazolin or vancomycin. In conclusion, we have demonstrated that, within our Shanghai population, there was no significant change in the percentage of gram-positive or -negative microorganisms over time.
