Abstract:Menstrual synchrony has been typically studied among women who live together: dormitory roommates or family members sharing a bedroom or living in the same house. The current study examined menstrual synchrony in 51 pairs of women working together under conditions optimally conducive to synchrony. They had been together for at least 1 year, shared a relatively small office, worked there all day full time and contact with other people during the day was minimal. Prospective records of three menstrual dates showed a significant degree of synchrony for each of the 3 months. Menstrual onsets of close friends tended to occur on the average within 3.5-4.3 days of each other while onsets of co-workers who were not close friends were significantly more broadly ranged (7.7-9.0 days of each other). This is the first unequivocal demonstration of menstrual synchrony outside of the household.

Abstract:Objective To study whether the menstrual cycle has a circaseptan (7 days) rhythm and whether it is associated with the lunar cycle (also defined as the synodic month, it is the cycle of the phases of the Moon as seen from Earth, averaging 29.5 days in length). Design Cross-sectional study. Subjects A total of 35,940 European and North American women aged 18–40 years. Exposure Data were collected in real-life conditions. Intervention No intervention was performed. Main Outcome Measure The onset of menstruation was assessed in prospectively measured menstrual cycles (311,064 cycles) over 3 full years (2019–2021). Associations were calculated between the onset of menstruation and the day of the week, and between the onset of menstruation and the lunar phase. Results In this large data set, a circaseptan (7-day) rhythmicity of menstruation was observed, with a peak (acrophase) of menstrual onset on Thursdays and Fridays. This circaseptan rhythm was observed in every age group, in every phase of the lunar cycle, and in all seasons. This feature was most pronounced for cycle durations between 27 and 29 days. In winter, the circaseptan rhythm was found in cycles of 27–29 days, but not in other cycle lengths. A circalunar rhythm was also statistically significant, but not as clearly defined as the circaseptan rhythm. The peak (acrophase) of the circalunar rhythm of menstrual onset varied according to the season. In addition, there was a small but statistically significant interaction between the circaseptan rhythm and the lunar cycle. Conclusion Although relatively small in amplitude, the weekly rhythm of menstruation was statistically significant. Menstruation occurs more often on Thursdays and Fridays than on other days of the week. This is particularly true for women whose cycles last between 27 and 29 days. Circalunar rhythmicity was also statistically significant. However, it is less pronounced than the weekly rhythm.

Abstract:The menstrual cycle is an essential life rhythm governed by interacting levels of progesterone, estradiol, follicular stimulating, and luteinizing hormones. To study metabolic changes, biofluids were collected at four timepoints in the menstrual cycle from 34 healthy, premenopausal women. Serum hormones, urinary luteinizing hormone and self-reported menstrual cycle timing were used for a 5-phase cycle classification. Plasma and urine were analyzed using LC-MS and GC-MS for metabolomics and lipidomics; serum for clinical chemistries; and plasma for B vitamins using HPLC-FLD. Of 397 metabolites and micronutrients tested, 208 were significantly (p < 0.05) changed and 71 reached the FDR 0.20 threshold showing rhythmicity in neurotransmitter precursors, glutathione metabolism, the urea cycle, 4-pyridoxic acid, and 25-OH vitamin D. In total, 39 amino acids and derivatives and 18 lipid species decreased (FDR < 0.20) in the luteal phase, possibly indicative of an anabolic state during the progesterone peak and recovery during menstruation and the follicular phase. The reduced metabolite levels observed may represent a time of vulnerability to hormone related health issues such as PMS and PMDD, in the setting of a healthy, rhythmic state. These results provide a foundation for further research on cyclic differences in nutrient-related metabolites and may form the basis of novel nutrition strategies for women.

Abstract:The ovarian cycle has a well-established circa-monthly rhythm, but the mechanisms involved in its regularity are unknown. Is the rhythmicity driven by an endogenous clock-like timer or by other internal or external processes? Here, using two large epidemiological datasets (26,912 cycles from 2303 European women and 4786 cycles from 721 North American women), analyzed with time series and circular statistics, we find evidence that the rhythmic characteristics of the menstrual cycle are more likely to be explained by an endogenous clock-like driving mechanism than by any other internal or external process. We also show that the menstrual cycle is weakly but significantly influenced by the 29.5-day lunar cycle and that the phase alignment between the two cycles differs between the European and the North American populations. Given the need to find efficient treatments of subfertility in women, our results should be confirmed in larger populations, and chronobiological approaches to optimize the ovulatory cycle should be evaluated.

Importance of Daily Sex Hormone Measurements Within the Menstrual Cycle for Fertility Estimates in Cyclical Shifts Studies

Abstract:Recent discussions have highlighted the importance of fertility measurements for the study of peri-ovulatory shifts in women’s mating psychology and mating-related behaviors. Participants in such studies typically attend at least two test sessions, one of which is, at least in theory, scheduled to occur during the high-fertility, peri-ovulatory phase of the menstrual cycle. A crucial part of this debate is whether luteinizing hormone (LH) tests alone are sufficient to accurately assign test sessions to the peri-ovulatory phase. This article adds to this ongoing debate by presenting analyses of a detailed database of daily estradiol levels and LH tests for 102 menstrual cycles. Based on more than 4,000 hormonal measurements, it is clear that individual differences in length of the cycle, length of the luteal phase and, perhaps most importantly, the discrepancy between the timing of the LH surge and the drop in estradiol that follows it are pronounced. Less than 40% of analyzed cycles followed the textbook pattern commonly assumed to occur in fertility-based research, in which the LH surge is assumed to occur not more than 48 hr before the estradiol drop. These results suggest that LH tests alone are not sufficient to assign test sessions to the peri-ovulatory phase and that analyses of sex hormones are essential to identify whether the participant was tested during the peri-ovulatory phase.

Abstract:Dimensions of human mood, behaviour and vital signs cycle over multiple timescales. However, it remains unclear which dimensions are most cyclical, and how daily, weekly, seasonal and menstrual cycles compare in magnitude. The menstrual cycle remains particularly understudied because, not being synchronized across the population, it will be averaged out unless menstrual cycles can be aligned before analysis. Here, we analyse 241 million observations from 3.3 million women across 109 countries, tracking 15 dimensions of mood, behaviour and vital signs using a women's health mobile app. Out of the daily, weekly, seasonal and menstrual cycles, the menstrual cycle had the greatest magnitude for most of the measured dimensions of mood, behaviour and vital signs. Mood, vital signs and sexual behaviour vary most substantially over the course of the menstrual cycle, while sleep and exercise behaviour remain more constant. Menstrual cycle effects are directionally consistent across countries.

Abstract:This study examined menstrual synchrony among roommates residing in 20 university housing units. Menstrual synchrony was examined for roommates who were close friends and roommates who were not close friends, and by housing unit. The impact of personality, social-interaction and menstrual-related factors was also studied. To avoid limitations found in previous studies, (i) data were collected prospectively and (ii) over the entire academic year; (iii) high participation rate was obtained (86%); and (iv) women who used oral contraceptives were excluded. Menstrual synchrony was found among roommates who were close friends. Synchrony was not found for roommates who were not close friends, nor by housing units. The phenomenon of menstrual synchrony may more likely occur among close friends and women with intensive social contact than under the conditions common to university dormitories.

Abstract:Summary Menstruating individuals experience an increased risk for sleep and affective disorders, attributed in part to monthly oscillations in sex hormones. Emotional functioning and sleep continuity worsens during the perimenstrual phase of the menstrual cycle. This study examined the interactive effects of sleep, menstrual phase, and emotion in healthy women. Participants (N = 51, 43% Caucasian) aged 18–35 (m = 24 years) completed actigraphy and daily sleep/emotion diaries over two menstrual cycles (m days = 51.29). Abstract:Background: Menstrual synchrony is also known as the Mc Clintock effect when a person who attained menarche come in physical contact with another person who menstruates, the pheromones influence each other so that eventually, the monthly cycles lineup. To assess the synchrony (frequency of coincidence) of menstrual cycles among hostel girls.Methods: A cross-sectional study was conducted among 121 hostel girls in Tamil Nadu during February 2019. A semi-structured questionnaire was used to collect the data participant’s age, synchronization of the menstrual cycle, the experience of synchrony common among roommates /friends other than roommates, regularity of periods, number of days of bleeding, intake of pills to delay/regularize periods, coinciding symptoms during menses. The data collected was entered in Microsoft excel and analysed using SPSS version 16 software. The descriptive analysis was analysed using frequencies, mean, standard deviation and proportions.Results: Among 121 hostel girls, the mean (SD) age was 20.20 (1.31) years. The 86 (71.1%) girls get their menses during the same time. The 58.7% experience synchrony more common among roommates whereas 41.3% experience it with friends other than roommates.Conclusions: High proportion of the participants reported menstrual synchrony among them. Future research is needed to prove these hypotheses under the influence of pheromones. This paves the way to study the role of pheromones in other physiological mechanisms.

Abstract:Women's health concerns are generally underrepresented in basic and translational research, but reproductive health in particular has been hampered by a lack of understanding of basic uterine and menstrual physiology. Menstrual health is an integral part of overall health because between menarche and menopause, most women menstruate. Yet for tens of millions of women around the world, menstruation regularly and often catastrophically disrupts their physical, mental, and social well-being. Enhancing our understanding of the underlying phenomena involved in menstruation, abnormal uterine bleeding, and other menstruation-related disorders will move us closer to the goal of personalized care. Furthermore, a deeper mechanistic understanding of menstruation-a fast, scarless healing process in healthy individuals-will likely yield insights into a myriad of other diseases involving regulation of vascular function locally and systemically. We also recognize that many women now delay pregnancy and that there is an increasing desire for fertility and uterine preservation. In September 2018, the Gynecologic Health and Disease Branch of the Eunice Kennedy Shriver National Institute of Child Health and Human Development convened a 2-day meeting, "Menstruation: Science and Society" with an aim to "identify gaps and opportunities in menstruation science and to raise awareness of the need for more research in this field." Experts in fields ranging from the evolutionary role of menstruation to basic endometrial biology (including omic analysis of the endometrium, stem cells and tissue engineering of the endometrium, endometrial microbiome, and abnormal uterine bleeding and fibroids) and translational medicine (imaging and sampling modalities, patient-focused analysis of menstrual disorders including abnormal uterine bleeding, smart technologies or applications and mobile health platforms) to societal challenges in health literacy and dissemination frameworks across different economic and cultural landscapes shared current state-of-the-art and future vision, incorporating the patient voice at the launch of the meeting. Here, we provide an enhanced meeting report with extensive up-to-date (as of submission) context, capturing the spectrum from how the basic processes of menstruation commence in response to progesterone withdrawal, through the role of tissue-resident and circulating stem and progenitor cells in monthly regeneration-and current gaps in knowledge on how dysregulation leads to abnormal uterine bleeding and other menstruation-related disorders such as adenomyosis, endometriosis, and fibroids-to the clinical challenges in diagnostics, treatment, and patient and societal education. We conclude with an overview of how the global agenda concerning menstruation, and specifically menstrual health and hygiene, are gaining momentum, ranging from increasing investment in addressing menstruation-related barriers facing girls in schools in low- to middle-income countries to the more recent "menstrual equity" and "period poverty" movements spreading across high-income countries.

Abstract:Cyclic fluctuations in hypothalamic-pituitary-gonadal axis (HPG-axis) hormones exert powerful behavioral, structural, and functional effects through actions on the mammalian central nervous system. Yet, very little is known about how these fluctuations alter the structural nodes and information highways of the human brain. In a study of 30 naturally cycling women, we employed multidimensional diffusion and T1-weighted imaging during three estimated menstrual cycle phases (menses, ovulation, and mid-luteal) to investigate whether HPG-axis hormone concentrations co-fluctuate with alterations in white matter (WM) microstructure, cortical thickness (CT), and brain volume. Across the whole brain, 17β-estradiol and luteinizing hormone (LH) concentrations were directly proportional to diffusion anisotropy (μFA; 17β-estradiol: β1 = 0.145, highest density interval (HDI) = [0.211, 0.4]; LH: β1 = 0.111, HDI = [0.157, 0.364]), while follicle-stimulating hormone (FSH) was directly proportional to CT (β1 = 0 .162, HDI = [0.115, 0.678]). Within several individual regions, FSH and progesterone demonstrated opposing relationships with mean diffusivity (Diso) and CT. These regions mainly reside within the temporal and occipital lobes, with functional implications for the limbic and visual systems. Finally, progesterone was associated with increased tissue (β1 = 0.66, HDI = [0.607, 15.845]) and decreased cerebrospinal fluid (CSF; β1 = -0.749, HDI = [-11.604, -0.903]) volumes, with total brain volume remaining unchanged. These results are the first to report simultaneous brain-wide changes in human WM microstructure and CT coinciding with menstrual cycle-driven hormone rhythms. Effects were observed in both classically known HPG-axis receptor-dense regions (medial temporal lobe, prefrontal cortex) and in other regions located across frontal, occipital, temporal, and parietal lobes. Our results suggest that HPG-axis hormone fluctuations may have significant structural impacts across the entire brain.

Abstract:Despite decades of research on the physiological and psychological effects of the menstrual cycle, studies have not sufficiently adopted consistent methods for operationalizing the menstrual cycle. This has resulted in substantial confusion in the literature and limited possibilities to conduct systematic reviews and meta-analyses. In order to facilitate more rapid accumulation of knowledge on cycle effects, the present paper offers a set of integrative guidelines and standardized tools for studying the menstrual cycle as an independent variable. We begin with (1) an overview of the menstrual cycle and (2) premenstrual disorders, followed by (3) recommendations and tools regarding data collection in cycle studies. These recommendations address selecting the appropriate study design, sampling strategy, managing demand characteristics, identifying a sample of naturally-cycling individuals, measuring menstrual bleeding dates, ovarian hormones, and ovulation. We proceed with suggestions for (4) data preparation and coding of cycle day and phases, (5) data visualization, statistical modeling, and interpretation of menstrual cycle associations. We also provide (6) recommendations for using menses start day and ovulation testing to schedule visits in laboratory studies and end with a (7) comprehensive summary and conclusion. Regardless of whether the influence of the menstrual cycle is of central interest in a study or should be controlled to accurately assess the effects of another variable, the use of these recommendations and tools will help make study results more meaningful and replicable.

Abstract:The aim of the study was to investigate whether menstrual cycle phases influence the physical performance during soccer match-play.Fifteen elite female soccer players, with physiologically normal menstrual cycles, competed in matches over a four-month period. Physical performance was assessed via GPS and expressed as meters per minute, and separated into four individualised thresholds (low, high, very high and sprinting). Seventy-six complete individual match observations, 36 from the follicular and 40 from the luteal phase were recorded. The differences in physical match performance parameters between the cycle phases were evaluated using a mixed linear model.The results of the current study indicate that very high-intensity running distance was significantly greater during the luteal phase compared to the follicular phase (5.90 ± 2.16 mmin−1 vs. 6.64 ± 2.72 mmin−1; p = 0.02). However, this finding was accompanied by large variations across matches (CV = 39.5%)..Overall, the results suggest that the menstrual cycle phase does not influence match physical performance of female soccer players to a significant degree. Therefore, at present interventions or other methods of coping with menstrual cycle phase do not seem necessary on a group/team level to maximise competitive physical performance.

Menstrual Cycle Changes in Daily Sexual Motivation and Behavior Among Sexually Diverse Cisgender Women

Abstract:We used a one-month daily diary assessment to measure menstrual cycle-related changes in same-gender and other-gender sexual motivation and behavior in 148 cisgender women (32% lesbian-identified, 35% bisexually identified, and 33% heterosexual-identified). Women with exclusive same-gender orientations reported increased motivation for same-gender sexual contact during the higher-fertility phase of the cycle, but women with exclusive other-gender orientations did not show a parallel increase in other-gender sexual motivation during the higher-fertility phase. Bisexually attracted women showed no phase-related changes in same-gender or other-gender sexual motivation, regardless of whether they generally preferred one gender versus the other. Rates of partnered sexual contact did not increase during the higher-fertility phase. During the 14 midcycle days during which we assayed salivary estrogen and testosterone, we found no significant associations between daily hormones and sexual motivation. However, daily estrogen levels were positively related to sexual behavior among women currently partnered with women, and negatively related to sexual behavior among women currently partnered with men. Our results suggest that traditional evolutionary models of menstrual cycle-related changes in sexual motivation do not adequately reflect the full range of cycle-related changes observed among sexually diverse women.

This study used the Oura ring to track changes in sleep and related physiological features, and also tracked self-reported daily functioning and symptoms across the regular, healthy menstrual cycle. Methods: Twenty-six healthy women (age, mean (SD): 24.4 (1.1 years)) with regular, ovulatory cycles (length, mean (SD): 28.57 (3.8 days)) were monitored across a complete menstrual cycle. Four menstrual cycle phases, reflecting different hormone milieus, were selected for analysis: menses, ovulation, mid-luteal, and late-luteal. Objective measures of sleep, sleep distal skin temperature, heart rate (HR) and vagal-mediated heart rate variability (HRV, rMSSD), derived from the Oura ring, and subjective daily diary measures (eg sleep quality, readiness) were compared across phases. Results: Wearable-based measures of sleep continuity and sleep stages did not vary across the menstrual cycle. Women reported no menstrual cycle-related changes in perceived sleep quality or readiness and only marginally poorer mood in the midluteal phase. However, they reported moderately more physical symptoms during menses (p < 0.001). Distal skin temperature and HR, measured during sleep, showed a biphasic pattern across the menstrual cycle, with increased HR (p < 0.03) and body temperature (p < 0.001) in the mid- and late-luteal phases relative to menses and ovulation. Correspondingly, rMSSD HRV tended to be lower in the luteal phase. Further, distal skin temperature was lower during ovulation relative to menses (p = 0.05). Conclusion: The menstrual cycle was not accompanied by significant fluctuations in objective and perceived measures of sleep or in mood, in healthy women with regular, ovulatory menstrual cycles. However, other physiological changes in skin temperature and HR were evident and may be longitudinally tracked with the Oura ring in women over multiple cycles in a natural setting. Keywords: sleep, wearables, menstrual cycle, ovulation, body temperature, women's health This study aimed to use a wearable device (Oura ring), which has been previously tested against gold standard methods for sleep study, to examine changes in sleep, nocturnal heart rate and skin temperature, across four phases of the ovulatory menstrual cycle in 26 regularly menstruating, healthy women. We show that measures of sleep continuity and stages derived from the Oura ring stay relatively stable across menstrual cycle phases in women, consistent with their self-reported sleep assessments. On the other hand, distal skin temperature and heart rate, measured during sleep, increased in the luteal phase, consistent with ovulatory cycles. Skin temperature also showed a decrease in the ovulatory phase, likely reflecting the temperature-lowering effect of estrogen. Thus, wearable ring technology shows promise as a sensitive and informative tool for longitudinally tracking physiological changes across the menstrual cycle in women. Abstract:Abstract This study aimed to investigate the fluctuations of muscle performance in the Smith machine half-squat exercise during three different phases of the menstrual cycle. Thirteen resistance-trained and eumenorrheic women volunteered to participate in the study (58.6 ± 7.8 kg, 31.1 ± 5.5 years). In a pre-experimental test, the half-squat one-repetition maximum (1RM) was measured. Body mass, tympanic temperature and urine concentration of the luteinizing hormone were estimated daily for ~30 days to determine the early follicular phase (EFP), the late follicular phase (LFP), and the mid-luteal phase (MLP) of the menstrual cycle. On the second day of each phase, performance of the Smith machine half-squats was assessed using 20, 40, 60 and 80% of one repetition maximum (1RM). In each load, force, velocity, and power output were measured during the concentric phase of the exercise by means of a rotatory encoder. The data were analyzed using one-way repeated measures ANOVA coupled with magnitude-based inferences. Overall, force, velocity and power output were very similar in all menstrual cycle phases with unclear differences in most of the pairwise comparisons and effect sizes >0.2. The results of this investigation suggest that eumenorrheic females have similar muscle strength and power performance in the Smith machine half-squat exercise during the EFP, LFP, and MLP phases of the menstrual cycle.

Abstract:This study aimed to explore how natural menstrual cycle phases and dosage of oral hormonal contraceptives (OC) influence the diurnal rhythm of distal skin temperature (DST) under real-life conditions. Participants were 41 healthy females (23.9 ± 2.48 y), comprising 27 females taking monophasic hormonal oral contraceptives (OC users) and 14 females with menstrual cycles (non-OC users). Wrist DST was continuously recorded and averaged over two consecutive 24-hour days during (pseudo)follicular and (pseudo)luteal menstrual phases. Diurnal rhythm characteristics, i.e. acrophase and amplitude, describing timing and strength of the DST rhythm, respectively, were calculated using cosinor analysis. Results show that non-OC users experienced earlier diurnal DST maximum (acrophase, p = 0.019) and larger amplitude ( p = 0.016) during the luteal phase than during the follicular phase. This was observed in most (71.4%) but not all individuals. The OC users showed no differences in acrophase or amplitude between pseudoluteal and pseudofollicular phases. OC users taking a higher dosage of progestin displayed a larger amplitude for DST rhythm during the pseudoluteal phase ( p = 0.009), while estrogen dosage had no effect. In conclusion, monophasic OC cause changes in diurnal DST rhythm, similar to those observed in the luteal phase of females with menstrual cycles, suggesting that synthetic progestins act in a similar manner on skin thermoregulation as progesterone does.

Abstract:The menstrual cycle affects women's emotional states, with estrogen and progesterone having predominant roles. However, it remains unclear whether the phases of the menstrual cycle also affect women's motivational behaviors. In this study, the main aim was to investigate how the menstrual cycle influences approach–avoidance behavior under conditions of conscious versus unconscious processing of emotions. Briefly, after recruitment by advertisement and screening with a menstrual cycle survey questionnaire, 27 naturally cycling, healthy women participated in an improved "manikin task" and were presented both positive and negative emotional stimuli during early follicular, late follicular, and mid-luteal phases. Estrogen and progesterone levels were measured. Women in the late follicular phase exhibited the shortest response times for approaching positive stimuli, while women in the mid-luteal phase exhibited the shortest response times for avoiding negative stimuli. Estrogen and progesterone levels significantly correlated with the speed of the approach–avoidance responses observed for the women, indicating the important role that sex hormones have in mediating emotionally motivated behavior. Overall, these findings suggest that the menstrual cycle has strong and specific influences on women's approach–avoidance behaviors that are in part mediated by estrogen and progesterone. By identifying characteristics of these behaviors in the late follicular and mid-luteal phases, greater insight can be provided to women regarding the physiological influences of the menstrual cycle on their personal growth and security.

Abstract:This article presents a mathematical model for hormonal regulation of the menstrual cycle which predicts the occurrence of follicle waves in normally cycling women. Several follicles of ovulatory size that develop sequentially during one menstrual cycle are referred to as follicle waves. The model consists of 13 nonlinear, delay differential equations with 51 parameters. Model simulations exhibit a unique stable periodic cycle and this menstrual cycle accurately approximates blood levels of ovarian and pituitary hormones found in the biological literature. Numerical experiments illustrate that the number of follicle waves corresponds to the number of rises in pituitary follicle stimulating hormone. Modifications of the model equations result in simulations which predict the possibility of two ovulations at different times during the same menstrual cycle and, hence, the occurrence of dizygotic twins via a phenomenon referred to as superfecundation. Sensitive parameters are identified and bifurcations in model behaviour with respect to parameter changes are discussed. Studying follicle waves may be helpful for improving female fertility and for understanding some aspects of female reproductive ageing.
