Abstract:Dedifferentiation of insulin-secreting beta cells in the islets of Langerhans has been proposed to be a major mechanism of beta-cell dysfunction. Whether dedifferentiated beta cells can be targeted by pharmacological intervention for diabetes remission, and ways in which this could be accomplished, are unknown as yet. Here we report the use of streptozotocin-induced diabetes to study beta-cell dedifferentiation in mice. Single-cell RNA sequencing (scRNA-seq) of islets identified markers and pathways associated with beta-cell dedifferentiation and dysfunction. Single and combinatorial pharmacology further show that insulin treatment triggers insulin receptor pathway activation in beta cells and restores maturation and function for diabetes remission. Additional beta-cell selective delivery of oestrogen by Glucagon-like peptide-1 (GLP-1-oestrogen conjugate) decreases daily insulin requirements by 60%, triggers oestrogen-specific activation of the endoplasmic-reticulum-associated protein degradation system, and further increases beta-cell survival and regeneration. GLP-1-oestrogen also protects human beta cells against cytokineinduced dysfunction. This study not only describes mechanisms of beta-cell dedifferentiation and regeneration, but also reveals pharmacological entry points to target dedifferentiated beta cells for diabetes remission.

Abstract:BACKGROUND:Diabetes mellitus is characterized by loss or dysfunction of insulin-producing β-cells in the pancreas, resulting in failure of blood glucose regulation and devastating secondary complications. Thus, β-cells are currently the prime target for cell-replacement and regenerative therapy. Triggering endogenous repair is a promising strategy to restore β-cell mass and normoglycemia in diabetic patients. Potential strategies include targeting specific β-cell subpopulations to increase proliferation or maturation. Alternatively, transdifferentiation of pancreatic islet cells (e.g. α- or δ-cells), extra-islet cells (acinar and ductal cells), hepatocytes, or intestinal cells into insulin-producing cells might improve glycemic control. To this end, it is crucial to systematically characterize and unravel the transcriptional program of all pancreatic cell types at the molecular level in homeostasis and disease. Furthermore, it is necessary to better determine the underlying mechanisms of β-cell maturation, maintenance, and dysfunction in diabetes, to identify and molecularly profile endocrine subpopulations with regenerative potential, and to translate the findings from mice to man. Recent approaches in single-cell biology started to illuminate heterogeneity and plasticity in the pancreas that might be targeted for β-cell regeneration in diabetic patients.SCOPE OF REVIEW:This review discusses recent literature on single-cell analysis including single-cell RNA sequencing, single-cell mass cytometry, and flow cytometry of pancreatic cell types in the context of mechanisms of endogenous β-cell regeneration. We discuss new findings on the regulation of postnatal β-cell proliferation and maturation. We highlight how single-cell analysis recapitulates described principles of functional β-cell heterogeneity in animal models and adds new knowledge on the extent of β-cell heterogeneity in humans as well as its role in homeostasis and disease. Furthermore, we summarize the findings on cell subpopulations with regenerative potential that might enable the formation of new β-cells in diseased state. Finally, we review new data on the transcriptional program and function of rare pancreatic cell types and their implication in diabetes.MAJOR CONCLUSION:Novel, single-cell technologies offer high molecular resolution of cellular heterogeneity within the pancreas and provide information on processes and factors that govern β-cell homeostasis, proliferation, and maturation. Eventually, these technologies might lead to the characterization of cells with regenerative potential and unravel disease-associated changes in gene expression to identify cellular and molecular targets for therapy.

Abstract:β-Cell injury plays an important role in the development of type 1 and type 2 diabetes. Most of the β-cell bioassays depend on labeling or endpoint assessments that might not capture the true physiology or pathology of the injury process. In the current study, we dynamically detected a broad range of pathological and pharmacological responses to four toxicants (cytokine mixture, free fatty acid mixture, streptozotocin, and hydrogen peroxide) in living β-cells (INS-1E and MIN6) by a label-free, cell-based assay system named xCELLigence, codeveloped by ACEA Biosciences and Roche Diagnostics. Our results suggest that the impedance readout is highly sensitive and provides more information than some of the conventional endpoint cytotoxicity assays for β-cell injury such as the Cell Counting Kit-8 (CCK-8) and morphology measurements. Furthermore, this system was used to evaluate the anti-injury effects of glucagon-like peptide-1 (GLP-1) and its nonpeptidic mimetic Boc5 by monitoring responses to four toxicants in two β-cell lines. This study confirms that the protective property of Boc5 on β-cells is similar to that of GLP-1.

Abstract:The development of diabetes mellitus is the interplay between insulin secretion and insulin resistance, while insufficient compensatory beta cell function plays a major role during the natural progression of the disease. In the past decades, a lot of work had been carried out to investigate the initiation and regulation of beta cell dysfunction, yet the underlying mechanism is largely unknown. In this issue, 11 interesting papers are compiled to discuss from experimental and clinical aspects the mechanism of beta cell dysfunction in the development of diabetes. MicroRNAs (miRNAs) are small noncoding 18–25 nucleotides that bind to the complementary 3′UTR regions of target mRNAs and function in transcriptional and posttranscriptional regulation of gene expression. In recent years, miRNAs were reported to regulate several metabolic pathways including insulin secretion, cholesterol biosynthesis, carbohydrate, and lipid metabolism. In this issue, 3 papers touched upon the regulatory effect of some miRNAs on beta cell function from different points of view. Dr. X. Chang et al. reported that the mean level of miR-375 methylation was significantly lower in T2DM patients from Kazak population than Han population, which might partly explain from genetic background that even though Kazak population clusters more risk factors for T2DM, prevalence rate of T2DM is 6 times less than that of the Han population in the same region. Q. Zhang et al. showed that 8-week treatment of Tianmai Xiaoke Tablet, a chromium picolinate hypoglycemic agent, in diabetic rat significantly upregulated the expression of multiple miRNAs such as miR-375 and miR-30d, which might be part of its effect on improving glucose control. X. Lin et al. investigated the direct suppression of Bcl-2 by miR-34a, which might account for palmitate-induced apoptosis in MIN6 cell, the latter of which is believed to be the most important mechanism of beta cell dysfunction. Another 2 manuscripts also discussed beta cell apoptosis. Dr. L. Zhou et al. examined the 3 signaling pathways of MAPKs in INS-1 cells treated with glucolipotoxicity conditions and concluded that P38 might be involved in the regulation of beta cell apoptosis through phosphorylation of IRS-2. Z. Zhang et al. simulated intermittent high glucose situation in vitro and investigated its effect on beta cell apoptosis. In another paper, the attenuating effect of metformin against high glucose-induced suppression of cell proliferation and osteogenic-related gene expression in osteoblast was investigated by X. Shao et al. Three papers in this issue presented with unique clinical picture of diabetes in the Chinese population. W. Tang et al. investigate the relationship between serum uric acid and residual beta cell function in 1021 T2DM patients. They concluded that patients with higher serum uric acid had greater insulin secretion at the early stage, but their residual beta cell function decayed more rapidly. Y. Ma et al. compared newly diagnosed T2DM patients with or without hyperlipidemia and found that the former were younger and had worse beta cell function. H. Lu et al. reported that ketosis onset type 2 diabetic patients had better beta cell function and were more insulin resistant. Two manuscripts brought new exploration in the treatment of type 1 diabetes. Dr. W. Li et al. revealed that, apart from higher density of insulin-producing beta cells, small islets transplantation expressed less angiotensin and more angiotrophic VEGF-A, which might be beneficial for the facilitation of microcirculation and revascularization in small islets. H. Luo et al. presented with a novel protocol that reprogrammed primary hepatocytes into functional insulin-producing cells using multicistronic vectors carrying Pdx1, Ngn3, and MafA. These cells activated multiple beta cell gene expression, synthesized and stored considerable amounts of insulin, and released the hormone in a glucose-regulated manner in vitro. We hope to bring about extensive concern and energetic discussion about beta cell function from experimental as well as clinical aspects. We wish that our readers enjoy this special issue. Small molecule-mediated insulin hypersecretion induces transient unfolded protein response and loss of function in pancreatic islet beta cells

Abstract:ABSTRACT Aims/hypothesis Insulin production and its regulated exocytosis by pancreatic islet beta cells is the major physiological mechanism controlling blood glucose concentrations. Beta cells are susceptible to failure due to genetic and environmental influences, leading to type 1 and 2 diabetes. In this work we aimed to advance understanding of the regulation of beta-cell function by identifying small molecule modulators of secretion. Methods For high-throughput screening and routine medium-throughput beta cell function assays we used mouse MIN6 beta cells stably expressing a Gaussia luciferase-linked insulin secretion reporter (InsGLuc-MIN6). We validated hit small molecules using cadaveric human islets in static culture insulin secretion assays. We measured effects on calcium influx and cAMP in MIN6 cells using Fura-2 and a bioluminescence resonance energy transfer reporter assay, respectively. Metabolism was analyzed by targeted metabolomics and Seahorse oxygen consumption assays. We measured relative gene expression and protein amounts by quantitative RT-PCR and SDS-PAGE immunoblotting, respectively. Results Through our high-throughput screen we discovered 2-(3-benzyl-2-iminobenzimidazol-1-yl)-1-thiophen-2-ylethanol (SW016789) which acutely potentiated nutrient-induced calcium influx and insulin secretion. More than 2 hours of exposure to SW016789 transiently induced the unfolded protein response of the endoplasmic reticulum and shut down of insulin secretory function. Distinct from the effects of thapsigargin, SW016789 did not affect beta cell viability or death as determined by multiplexed cytotoxicity assays and a lack of induction of cleaved PARP. This may be due in part to a more rapid induction of Hspa5 , and a lesser induction of signalling through the eIF2α kinase PERK and lack of expression of oxidative stress genes like Txnip . We determined that SW016789 acted upstream of the voltage-dependent calcium channel (VDCC) and potentiated nutrient-stimulated, but not KCl-stimulated, calcium influx. The potentiating effects of SW016789 were not due to altered metabolic pathways, mitochondrial function, or actions on G protein-coupled receptors. In chemical co-treatment experiments we discovered synergy between SW016789 and small molecule activators of protein kinase C and VDCCs, suggesting potential involvement of these pathways in the mechanism of action. Finally, chronically elevated calcium influx was required for the inhibitory impact of SW016789, as co-treatment with dihydropyridine-class VDCC inhibitors protected MIN6 beta cells and human islets from loss of function. Conclusions/interpretation These data suggest the major mechanism of action of SW016789 in beta cells is to potentiate opening of VDCCs. This activity may partially depend on protein kinase C. Beta cells under pharmacological hypersecretory conditions have the capacity to suppress their function to mitigate ER stress and avoid apoptosis. Further study of the mechanisms underlying these processes will increase understanding of beta cell function in normal and pathophysiological states. Research in context What is already known about this subject? Pharmacologic and environmental chemicals can stimulate aberrant beta cell activity. Hypersecretion of insulin from pancreatic beta cells can cause ER stress and lead to beta cell dysfunction. Defects in beta cell ER stress handling contribute to diabetes pathogenesis. What is the key question? Can the chemical tool SW016789 help uncover novel insulin secretion regulatory pathways and ER proteostasis factors? What are the new findings? SW016789 caused hypersecretion of insulin via enhanced nutrient-stimulated Ca 2+ influx followed by transient ER stress and shutdown of beta cell function without apoptosis. Blockade of voltage-dependent Ca 2+ channels protected human islets and beta cells from hypersecretion-induced dysfunction. How might this impact on clinical practice in the foreseeable future? These results have the potential to uncover beta cell ER stress mitigation factors and add support to beta cell rest strategies to preserve function.
High-Throughput Methods for the Discovery of Small Molecule Modulators of Pancreatic Beta-Cell Function and Regeneration

Abstract:The progression of type II diabetes (T2D) is characterized by a complex and highly variable loss of beta-cell mass, resulting in impaired insulin secretion. Many T2D drug discovery efforts aimed at discovering molecules that can protect or restore beta-cell mass and function have been developed using limited beta-cell lines and primary rodent/human pancreatic islets. Various high-throughput screening methods have been used in the context of drug discovery, including luciferase-based reporter assays, glucose-stimulated insulin secretion, and high-content screening. In this context, a cornerstone of small molecule discovery has been the use of immortalized rodent beta-cell lines. Although insightful, this usage has led to a more comprehensive understanding of rodent beta-cell proliferation pathways rather than their human counterparts. Advantages gained in enhanced physiological relevance are offered by three-dimensional (3D) primary islets and pseudoislets in contrast to monolayer cultures, but these approaches have been limited to use in low-throughput experiments. Emerging methods, such as high-throughput 3D islet imaging coupled with machine learning, aim to increase the feasibility of integrating 3D microtissue structures into high-throughput screening. This review explores the current methods used in high-throughput screening for small molecule modulators of beta-cell mass and function, a potentially pivotal strategy for diabetes drug discovery.
Metabolic control of adaptive β-cell proliferation by the protein deacetylase SIRT2

Abstract:Selective and controlled expansion of endogenous β-cells has been pursued as a potential therapy for diabetes. Ideally, such therapies would preserve feedback control of β-cell proliferation to avoid excessive β-cell expansion and an increased risk of hypoglycemia. Here, we identified a regulator of β-cell proliferation whose inactivation results in controlled β-cell expansion: the protein deacetylase Sirtuin 2 (SIRT2). deletion in β-cells of mice increased β-cell proliferation during hyperglycemia with little effect in homeostatic conditions, indicating preservation of feedback control of β-cell mass. SIRT2 restrains proliferation of human islet β-cells cultured in glucose concentrations above the glycemic set point, demonstrating conserved SIRT2 function. Analysis of acetylated proteins in islets treated with a SIRT2 inhibitor revealed that SIRT2 deacetylates enzymes involved in oxidative phosphorylation, dampening the adaptive increase in oxygen consumption during hyperglycemia. At the transcriptomic level, inactivation has context-dependent effects on β-cells, with controlling how β-cells interpret hyperglycemia as a stress. Finally, we provide proof-of-principle that systemic administration of a GLP1-coupled -targeting antisense oligonucleotide achieves β-cell selective inactivation and stimulates β-cell proliferation under hyperglycemic conditions. Overall, these studies identify a therapeutic strategy for increasing β-cell mass in diabetes without circumventing feedback control of β-cell proliferation.

Abstract:Insulin signaling at the target tissue results in a large array of biological outcomes. These events are essential for normal growth and development and for normal homeostasis of glucose, fat, and protein metabolism. Elucidating the intracellular events after activation of the IR has been the primary focus of a large number of investigators for decades, and for excellent reasons. Understanding the signaling pathways involved in insulin action could lead to a better understanding of the pathophysiology of insulin resistance associated with obesity and type 2 diabetes, and identifying key molecules and processes could lead to newer and more effective therapeutic agents for treating these common disorders. This review summarizes our previous understanding of how insulin acts and outlines some recent developments in our understanding of insulin action and insulin resistance at the cellular level, beginning with a discussion on the discovery of evolutionarily conserved molecules of the insulin signaling pathways. This article will also provide a summary of a few in vitro and cellular models of insulin resistance and a description of some new paradigms in the cellular mechanisms of insulin action. This review will not attempt to be all-inclusive; for a more comprehensive understanding, readers are referred to more complete reviews on insulin action (1–5).

Abstract:Pancreatic beta cells are the only cell type in our body capable of producing and secreting insulin to instruct the insulin-sensitive cells and tissues of our bodies to absorb nutrients after a meal. Accurate control of insulin release is of critical importance; too little insulin leads to diabetes, while an excess of insulin can cause potentially fatal hypoglycaemia. Yet, the pancreas of most people will control insulin secretion safely and effectively over decades and in response to glucose excursions driven by tens of thousands of meals. Because we only become aware of the important contributions of the pancreas when it fails to maintain glucose homeostasis, it is easy to forget just how well insulin release from a healthy pancreas is matched to insulin need to ensure stable blood glucose levels. Beta cells achieve this feat by extensive crosstalk with the rest of the endocrine cell types in the islet, notably the glucagon-producing alpha cells and somatostatin-producing delta cells. Here I will review the important paracrine contributions that each of these cells makes to the stimulation and subsequent inhibition of insulin release in response to a transient nutrient stimulation, and make the case that a breakdown of this local crosstalk contributes to the pathophysiology of diabetes.

Abstract:An imaging method for detecting β-cell function in real-time in the rodent pancreas could provide new insights into the biological mechanisms involving loss of β-cell function during development of type 2 diabetes and for testing of new drugs designed to modulate insulin secretion. In this study, we used a zinc-responsive MRI contrast agent and an optimized 2D MRI method to show that glucose stimulated insulin and zinc secretion can be detected as functionally active “hot spots” in the tail of the rat pancreas. A comparison of functional images with histological markers show that insulin and zinc secretion does not occur uniformly among all pancreatic islets but rather that some islets respond rapidly to an increase in glucose while others remain silent. Zinc and insulin secretion was shown to be altered in streptozotocin and exenatide treated rats thereby verifying that this simple MRI technique is responsive to changes in β-cell function.

Abstract:Diabetes is an epidemic caused by a multitude of factors. Despite the studies attempting to unravel its mechanism, there is still more to discover about glucose-insulin dynamics. Abstract:Diabetes mellitus involves both insufficient insulin secretion and dysregulation of glucagon secretion1. In healthy people, a fall in plasma glucose stimulates glucagon release and thereby increases counter-regulatory hepatic glucose production. This response is absent in many patients with type-1 diabetes (T1D)2, which predisposes to severe hypoglycaemia that may be fatal and accounts for up to 10% of the mortality in patients with T1D3. In rats with chemically induced or autoimmune diabetes, counter-regulatory glucagon secretion can be restored by SSTR antagonists4-7 but both the underlying cellular mechanism and whether it can be extended to humans remain unestablished. Here, we show that glucagon secretion is not stimulated by low glucose in isolated human islets from donors with T1D, a defect recapitulated in non-obese diabetic mice with T1D. This occurs because of hypersecretion of somatostatin, leading to aberrant paracrine inhibition of glucagon secretion. Normally, KATP channel-dependent hyperpolarization of β-cells at low glucose extends into the δ-cells through gap junctions, culminating in suppression of action potential firing and inhibition of somatostatin secretion. This 'electric brake' is lost following autoimmune destruction of the β-cells, resulting in impaired counter-regulation. This scenario accounts for the clinical observation that residual β-cell function correlates with reduced hypoglycaemia risk8.

Abstract:Key PointsThe pancreatic β-cell is a key regulator of glucose homeostasis, sensing blood glucose levels after a meal and secreting insulin appropriately.Loss of β-cell number and function feature prominently in both type 1 and type 2 diabetes.Promoting β-cell regeneration, survival or function, especially with small molecules, may prove to be an effective strategy for treating diabetes.β-cell regeneration can be accomplished by stimulating the division of existing β-cells. It will be important, however, to induce proliferation in a cell-specific manner to avoid oncogenic transformation.Another mechanism for β-cell regeneration is through cellular reprogramming. Such transformations can occur from stem-cell-like populations (directed differentiation) or from other terminally differentiated cell types (transdifferentiation).Prevention or slowing of β-cell death in the first place may avoid the loss of β-cell mass that enables the transition from insulin resistance to overt diabetes.Promoting glucose-dependent induction of insulin secretion would restore hormone levels following β-cell death. Compounds exist to enhance insulin secretion, but many are not glucose-dependent.Technical challenges to identifying novel compounds include a lack of many good human cell lines and the development of sophisticated techniques to measure β-cell function and number for high-throughput screening.

Abstract:Insulin-producing β-cells in pancreatic islets are regulated by systemic cues and, locally, by adjacent islet hormone-producing 'non-β-cells' (namely α-cells, δ-cells and γ-cells). Yet whether the non-β-cells are required for accurate insulin secretion is unclear. Here, we studied mice in which adult islets are exclusively composed of β-cells and human pseudoislets containing only primary β-cells. Mice lacking non-β-cells had optimal blood glucose regulation, enhanced glucose tolerance, insulin sensitivity and restricted body weight gain under a high-fat diet. The insulin secretion dynamics in islets composed of only β-cells was comparable to that in intact islets. Similarly, human β-cell pseudoislets retained the glucose-regulated mitochondrial respiration, insulin secretion and exendin-4 responses of entire islets. The findings indicate that non-β-cells are dispensable for blood glucose homeostasis and β-cell function. These results support efforts aimed at developing diabetes treatments by generating β-like clusters devoid of non-β-cells, such as from pluripotent stem cells differentiated in vitro or by reprograming non-β-cells into insulin producers in situ.

Abstract:Somatostatin, produced by pancreatic δ-cells, has been known to inhibit insulin secretion since its discovery in the 1970s. However, the role of somatostatin in the regulation of the glycaemic set point (the homeostatic blood level of glucose between meals) remains unclear. A study in Nature Metabolism now explores the contribution of pancreatic δ-cell somatostatin secretion to the glycaemic set point and β-cell insulin production.

Abstract:Insulin-secreting beta cells play an essential role in maintaining physiological blood glucose levels, and their dysfunction leads to the development of diabetes. To elucidate the signalling events regulating insulin secretion, we applied a recently developed phosphoproteomics workflow. We quantified the time-resolved phosphoproteome of murine pancreatic cells following their exposure to glucose and in combination with small molecule compounds that promote insulin secretion. The quantitative phosphoproteome of 30,000 sites clustered into three main groups in concordance with the modulation of the three key kinases: PKA, PKC and CK2A. A high-resolution time course revealed key novel regulatory sites, revealing the importance of methyltransferase DNMT3A phosphorylation in the glucose response. Remarkably a significant proportion of these novel regulatory sites is significantly downregulated in diabetic islets. Control of insulin secretion is embedded in an unexpectedly broad and complex range of cellular functions, which are perturbed by drugs in multiple ways.

Abstract:The main hallmark in the development of both type 1 and type 2 diabetes is a decline in functional β-cell mass. This decline is predominantly attributed to β-cell death, although recent findings suggest that the loss of β-cell identity may also contribute to β-cell dysfunction. This phenomenon is characterized by a reduced expression of key markers associated with β-cell identity. This review delves into the insights gained from single-cell omics research specifically focused on β-cell identity. It highlights how single-cell omics based studies have uncovered an unexpected level of heterogeneity among β-cells and have facilitated the identification of distinct β-cell subpopulations through the discovery of cell surface markers, transcriptional regulators, the upregulation of stress-related genes, and alterations in chromatin activity. Furthermore, specific subsets of β-cells have been identified in diabetes, such as displaying an immature, dedifferentiated gene signature, expressing significantly lower insulin mRNA levels, and expressing increased β-cell precursor markers. Additionally, single-cell omics has increased insight into the detrimental effects of diabetes-associated conditions, including endoplasmic reticulum stress, oxidative stress, and inflammation, on β-cell identity. Lastly, this review outlines the factors that may influence the identification of β-cell subpopulations when designing and performing a single-cell omics experiment.

Abstract:Objective: This report examines current perspectives regarding likely mechanisms of beta-cell failure in type 2 diabetes and their clinical implications for protecting or sparing beta-cells early in the disease progression. In addition, it considers translation strategies to incorporate relevant scientific findings into educational initiatives targeting clinical practice behavior. Participants: On January 10, 2009, a working group of basic researchers, clinical endocrinologists, and primary care physicians met to consider whether current knowledge regarding pancreatic beta-cell defects justifies retargeting and retiming treatment for clinical practice. Based on this meeting, a writing group comprised of four meeting participants subsequently prepared this consensus statement. The conference was convened by The Endocrine Society and funded by an unrestricted educational grant from Novo Nordisk. Evidence: Participants reviewed and discussed published literature, plus their own unpublished data. Consensus process: The summary and recommendations were supported unanimously by the writing group as representing the consensus opinions of the working group. Conclusions: Workshop participants strongly advocated developing new systems to address common barriers to glycemic control and recommended several initial steps toward this goal. These recommendations included further studies to establish the clinical value of pharmacological therapies, continuing basic research to elucidate the nature and mechanisms of beta-cell failure in type 2 diabetes mellitus, and exploring new educational approaches to promote pathophysiology-based clinical practices. The Endocrine Society has launched a new website to continue the discussion between endocrinologists and primary care physicians on beta-cell pathophysiology in type 2 diabetes and its clinical implications.
