Abstract:Covid-19 has rapidly changed physician-patient interaction, from hands-on to hands-off medicine. In this essay, 2 family physicians on different continents reflect on the meaning of touch in clinical practice and how virtual care is transforming this tacit aspect of patient care. Although technology enables physicians to stay in touch with patients verbally, we have lost the ability to physically touch. Traditionally, touch is central to medical practice, physical examination guides diagnosis and informs management. But the silent language of touch fulfills a deeper symbolic function, enabling physicians to acknowledge patient concerns in a tangible way. Touch expresses healing, extending beyond skin-to-skin contact to express humanity, caring, and connection. As we adapt to novel technologies, we wonder how, as family physicians, we will adapt our clinical acumen to extend our ability to connect with patients.
Impact of the COVID-19 pandemic on paediatric otolaryngology: a nationwide study

Abstract:Objective: The COVID-19 pandemic profoundly modified the work routine in healthcare; however, its impact on the field of paediatric otorhinolaryngology (ORL) has been rarely investigated. The aim of this study was to assess the impact of COVID-19 on paediatric ORL. Methods: A questionnaire was developed by the Young Otolaryngologists of the Italian Society of ORL-Head and Neck Surgery (GOS). The questionnaire consisted of 26 questions related to workplace and personal paediatric ORL activities. The link was advertised on the official social media platforms and sent by e-mail to 469 Italian otolaryngologists. Results: The questionnaire was completed by 118 responders. During the pandemic, the main reduction was observed for surgical activity (78.8%), followed by outpatient service (16.9%). The conditions that were mostly impacted by a delayed diagnosis were respiratory infections in 45.8% of cases and sensorineural hearing loss in 37.3% of cases. Conclusions: Paediatric ORL was highly impacted by the COVID-19 pandemic, with a significant reduction of surgical and outpatient activities and a delay in time-sensitive diagnosis. Therefore, the implementation of new strategies, such as telemedicine, is recommended.

Abstract:Humans are social creatures, and touch is the most primitive form of relationship. In the context of the traditional medical examination, touch, palpation, is essential to diagnose or suspect pathology. But it is not only an issue of semeiotics. The physician's ability to examine a body through his or her hands is at the heart of the doctor-patient relationship. Moreover, we have been witnessing for many years to the progressive downgrading of the objective examination in clinical practice. First, the obsolescence of interaction with the patient due to the lure of diagnostic technology, then the "social distancing" of Covid-19 and, finally, the expansion of telemedicine has contributed to the distancing of caregivers from patients. The same technology that tends to distance doctor and patient provides haptic wearable devices that can digitize touch and open up application scenarios that are still largely unexplored. These, however, cannot replace human interaction. What can we do to preserve the culture of physical contact in the doctor-patient relationship? As always, awareness is the first step. The technology that separates us from patients is not the real problem. It is only when it becomes an end instead of a means that we risk losing centuries of medical tradition. It is therefore necessary to strike a balance between using technology and ensuring that health care professionals maintain a central role in providing emotional support and understanding to patients. Such issues should be introduced early in the medical student's training. In the course of study, not only the mechanisms of disease should be taught, but also how to approach care with compassion, empathy and "human touch". In any case, even if digital culture eventually prevails, it is necessary to practice at least "touch" as interpersonal sensitivity, gentleness, and the ability to intercept the other's state of mind and respect it.
Experience of Pediatricians and Pediatric Surgeons With Virtual Care During the COVID-19 Pandemic: Descriptive Study

Abstract:Background: Prior to the COVID-19 pandemic, in-clinic visits were the standard of care for pediatric physicians and surgeons at our center. At the pandemic onset, web-based care was adopted at an unprecedented scale and pace. Objective: This descriptive study explores the web-based care experience of pediatric physicians and surgeons during the pandemic by determining factors that supported and challenged web-based care adoption. Methods: This study took place at the Children's Hospital at London Health Sciences Centre, a children's hospital in London, Ontario, Canada, which provides pediatric care for patients from the London metropolitan area and the rest of Southwestern Ontario. The Donabedian model was used to structure a web-based survey evaluating web-based care experience, which was distributed to 121 department-affiliated pediatric physicians (including generalists and subspecialists in surgery and medicine). Recruitment occurred via department listserv email. Qualitative data were collected through discrete and free-text survey responses. Results: Survey response rate was 52.1% (63/121). Before the pandemic, few physicians within the Department of Paediatrics used web-based care, and physicians saw <10% of patients digitally. During March-May 2020, the majority transitioned to web-based care, seeing >50% of patients digitally. Web-based care use in our sample fell from June to September 2020, with the majority seeing <50% of patients digitally. Telephone and Ontario Telemedicine Network were the platforms most used from March to September 2020. Web-based care was rated to be convenient for most providers and their patients, despite the presence of technical difficulties. Challenges included lack of physical exam, lower patient volumes, and poor patient digital care etiquette. Regardless of demographics, 96.4% (116/121) would continue web-based care, ideally for patients who live far away and for follow-ups or established diagnoses. Conclusions: Transition to web-based care during COVID-19 was associated with challenges but also positive experiences. Willingness among pediatricians and pediatric surgeons to continue web-based care was high. Web-based care experiences at our center could be improved with patient education and targeting select populations. Future research is needed to improve practice efficiency and to inform regulatory guidelines for web-based care.

Abstract:Purpose:Describe a comprehensive overview of a telehealth implementation process that highlights attitudes and satisfaction scores toward telehealth from patients, providers, and staff in an academic pediatric ophthalmology practice during the early months of the coronavirus disease 2019 (COVID-19) pandemic.Methods:The electronic medical record data for telehealth and in-person visits, as well as a patient experience survey in pediatric ophthalmology were retrospectively reviewed for March 1 to July 31, 2020 and March 1 to July 31, 2019. Patient experience survey results were retrospectively reviewed. All current providers and staff were invited to participate in an anonymous and voluntary survey focused on attitudes at the time of telehealth implementation.Results:During March 1 to July 31, 2020, there was significant increase in telehealth visits (n = 1,006) compared with the same period in 2019 (n = 22). Evaluation and management (E & M) codes (n = 527) were the most commonly used billing codes, and strabismus, nystagmus, and irregular eye movement (n = 496) were the most common telehealth primary diagnoses. The telehealth attitudes survey showed more positive responses from providers than staff. The patient experience survey showed more favorable scores for telehealth visits compared with clinic visits. However, only about 50% of the respondents were satisfied with the technology in terms of ease and quality of connection during their telehealth visits.Conclusions:Telehealth was a satisfactory alternative to clinic visits in our academic pediatric ophthalmology practice during the early phase of the COVID-19 pandemic. Providers and staff had largely positive attitudes toward telehealth; however, future efforts should include strategies to increase staff buy in. Patients had high satisfaction scores with telehealth visits despite connection challenges.

Abstract:Background: Suspending ordinary care activities during the COVID-19 pandemic made it necessary to find alternative routes to comply with care recommendations not only for acute health needs but also for patients requiring follow-up and multidisciplinary visits. We present the 'Contactless' model, a comprehensive operational tool including a plurality of services delivered remotely, structured according to a complexity gradient, aimed to cover diagnostic procedures and monitor disease progression in chronic pediatric patients. Methods: A multidisciplinary and multiprofessional project team was recruited, in collaboration with patients' associations, to map a panel of available Evidence-Based solutions and address individual needs in full respect of the concept of personalized medicine. The solutions include a number of services from videoconsultations to more structure videotraining sessions. Results: A modular framework made up of four three Macro-levels of complexity - Contactless Basic, Intermediate and Advanced - was displayed as an incremental set of services and operational planning establishing each phase, from factors influencing eligibility to the delivery of the most accurate and complex levels of care. Conclusion: The multimodal, multidisciplinary 'Contactless' model allowed the inclusion of all Units of our Pediatric Department and families with children with disability or complex chronic conditions. The strengths of this project rely on its replicability outside of pediatrics and in the limited resources needed to practically impact patients, caregivers and professionals involved in the process of care. Its implementation in the future may contribute to reduce the duration of hospital admissions, money and parental absence from work.

Abstract:HCI research has explored a variety of technologies to support human-to-human touch by simulating or mediating the feeling of touch over distance. Restrictions in activities involving touch, close contact, and physical proximity due to the COVID-19 pandemic have made it more pertinent to understand and investigate the goals, opportunities, and limitations of technologically mediated touch. We conducted an exploratory interview study with 18 participants across the United States in April and June of 2021 to understand how the COVID-19 pandemic has changed people’s attitudes towards engaging in touch, being co-located, and sharing physical objects. We observed four themes: new efforts to evaluate safety, new preferences for physically co-locating without touch, technology solutions amplifying separation, and a newfound appreciation for human touch and touch-adjacent activities. These results suggest that future touch technologies may need to shift focus from simulating human touch convincingly to allowing for interactions that incorporate these emergent sensitivities to safety and preferences for physical co-location.

Abstract:The COVID-19 pandemic has led to the introduction of unprecedented safety measures, one of them being physical distancing recommendations. Here, we assessed whether the pandemic has led to long-term effects on two important physical distancing aspects, namely interpersonal distance preferences and interpersonal touch behaviors. We analyzed nearly 14,000 individual cases from two large, cross-cultural surveys – the first conducted 2 years prior to the pandemic and the second during a relatively stable period of a decreased infection rate in May-June 2021. Preferred interpersonal distances increased by 54% globally during the COVID-19 pandemic. This increase was observable across all types of relationships, all countries, and was more pronounced in individuals with higher self-reported vulnerability to diseases. Unexpectedly, participants reported a higher incidence of interpersonal touch behaviors during than before the pandemic. We discuss our results in the context of prosocial and self-protection motivations that potentially promote different social behaviors.

Abstract:Introduction: Studies on the impacts of COVID-19 on patient safety are emerging. However, few studies have elicited the perspectives of front-line clinicians. Methods: We interviewed clinicians from 16 US hospitals who worked in the emergency department, intensive care unit or inpatient unit during the COVID-19 pandemic. We asked about their experiences with both clinician well-being and patient care throughout the pandemic. We used a rigorous thematic analysis to code the interview transcripts. This study was part of a larger randomised control trial of an intervention to improve healthcare worker well-being during the COVID-19 pandemic; the findings described here draw from clinicians who spontaneously raised issues related to patient safety. Results: 11 physicians and 16 nurses in our sample raised issues related to patient safety. We identified two primary themes: (1) compromised access to healthcare and (2) impaired care delivery. First, clinicians discussed how changes in access to healthcare early in the pandemic-including a shift to telehealth and deferred care-led to delays in accurate diagnosis and patients presenting later in their disease course. Second, clinicians discussed the effects of COVID-19 on care delivery related to staffing, equipment shortages and space constraints and how they deviated from the standard of care to manage these constraints. Clinicians noted how these issues led to patient safety events such as central line infections, patient falls and serious medication administration errors. Conclusions: Several well-intentioned interventions implemented in the early weeks of the pandemic created a unique context that affected patient safety throughout the pandemic. Future pandemic preparedness should consider planning that incorporates a patient safety lens to mitigate further harm from occurring during a public health crisis.

Abstract:Qualitative Health Research, Ahead of Print. During the COVID-19 pandemic, medical residents had the task of being the frontline of the response, being exposed to high risk of infection, increased clinical duty, and long and irregular working hours in highly restricted environments, increasing their levels of stress. We sought to expose the experiences of a group of geriatrics residents during this period of change in their professional and personal lives through the photovoice methodology. Thirteen participants were recruited and had 2 weeks to take photographs. The photographs were discussed in group meetings; the content of the conversations was transcribed and analyzed using interpretive description. Sixteen themes were identified. They were divided into personal life (11 themes) and life as a resident (5 themes). Adaptation was the main theme that came into discussion. The photographs and themes show how life changed for the participants, having a feeling of isolation, especially from their families, and highlighting their experiences as a team and community. While the pandemic, particularly at its beginning, was a period of uncertainty and a heavy load of work, it also provided learning and experience to this group of young physicians, which should not hide the fact that mental health concerns and burnout were a common situation. An online gallery was created which is publicly accessible.

Abstract:Bajwa and colleagues in this month's journal describe perceptions of telemedicine use from both parent and pediatrician perspectives during the COVID-19 epidemic (September 2021–January 2022) in Geneva, Switzerland. 1 Medicine, especially in a fee for service system, has often been a technology in search of a disease rather than a patient in need of technology. The need to use expensive technology results in increased costs, overdiagnosis, and lack of availability of the technology for appropriate use. 2,3 These authors offer a refreshing outlook on the medical technology of telemedicine, giving a balanced view (advantages and disadvantages) as well as a more granular discussion of the clinical scenarios where telemedicine is most appropriate. Although telemedicine has been in use for decades, its use grew exponentially out of necessity during the recent pandemic. 4,5 Caregivers and patients had to adapt quickly to decrease transmission of a highly virulent pathogen while continuing to provide and receive care. 5 The epidemic provided a natural experiment for these authors to study effectiveness of this modality in a more universal setting. As the world returns to normal, telemedicine will remain as a viable option for health care; there is a critical need to understands how this modality fits into the healthcare system when in person visits are not as restricted. These data will help fill that gap.

Family experiences and perceptions of intensive care unit care and communication during the COVID-19 pandemic

Abstract:Introduction: In 2020, during the first wave of the COVID-19 pandemic in Melbourne, visitor access to acute hospitals including intensive care units (ICUs) was initially barred, followed by a limit of one person per patient for one hour per day. This study explores the care and communication experienced by family members of ICU patients during this time. Methods: This qualitative descriptive study was conducted at an Australian quaternary hospital. Semistructured phone interviews were conducted using an aide-memoire designed to understand participants' experiences as family of a patient during this time. Interviews were recorded, transcribed, and thematically analysed. Findings: Twenty family members of patients in the ICU participated. Three major themes were identified: 'impact of restricting visiting procedures', 'family experiences of communication', and 'care and support'. Inflexible visiting restrictions had a momentous impact on families. Participants objected to having to nominate only two people to visit during the admission and the short visiting time limit. Some family members suffered extreme stress and anxiety during their absence from the bedside. Additional challenges were experienced by rural families, visitors with disabilities, and the young children of patients who were excluded. Communication with clinicians varied. Telehealth was valued by some but not universally embraced. The relationship between staff members and families and involvement in decision-making were unaffected. Conclusion: Families experienced significant psychological distress from being separated from their critically ill relatives. Patient care and involvement in decision-making appeared to be unchanged, but communication with staff felt to be lacking. Better alternatives to face-to-face communication must be sought to limit the impact of family separation on mental health. Families are a key link between the patient and clinicians and often play a major role in patient support and recovery after discharge. There is an urgent need to support them and facilitate meaningful engagement despite the obstacles.

Abstract:ABSTRACT Since the first reported case of Coronavirus disease (COVID‐19) in the United States in January 2020, substantial changes to medical education have taken place secondary to the demands, chaos, and uncertainty related to the pandemic. The involvement of medical students, residents and fellows in COVID‐19 care has and continues to vary by specialty and regions across the country. Our goal was to learn more about how COVID‐19 has impacted Physical Medicine and Rehabilitation (PM&R) training and practice across the country. To do so, real time, in‐depth experiences and opinions from persons within, or committed, to PM&R were pooled and this information was used to inform the creation of an anonymous survey that was distributed online to the PM&R community. 501 people completed the survey, 178 (35.5%) attending physicians, 183 (36.5%) residents and fellows, 29 (5.8%) interns, and 111 (22.2%) medical students. The survey results explore the impact of the global crisis on patient care and education in the field of PM&R. The adaptation from traditional patient care to telehealth was notable with 86.5% of physiatrists practicing via telemedicine during the pandemic. There was also a massive shift of education to virtual didactics with 92.5% of residents participating in virtual learning. The COVID‐19 pandemic has caused PM&R to revisit traditional routines in medical education and adapt to new practices to enhance educational experiences while still maintaining safe social distance between physicians, trainees and patients. Based on the survey results, the adaptations the PM&R community has made have provided a diversified, easily accessible platform for medical students, physiatrists‐in‐training and physiatrists across the country that has been full of innovation and collaboration between residencies and national organizations. We foresee continued, successful adoption of these new educational platforms and the use of telemedicine to enhance medical education and patient care post COVID‐19. Abstract:Objective: To assess the experience of virtual care among both patients and physicians across a range of clinical scenarios during the COVID-19 pandemic. Methods: A web-based survey was disseminated to patients and physicians through a variety of media and healthcare communications from May 2020 to July 2021. Demographic details and attitudes across a range of virtual care domains were collected. Quantitative responses were analyzed descriptively. Open-text responses were gathered to contrast when a virtual visit was superior or inferior to an in-person one, and a thematic content analysis was used. Results: There were 197 patients and 93 physician respondents, representing a range of demographic and practice characteristics. Patients noted several benefits of virtual care and felt it should continue to be available. Physicians felt they could do a lot of their care virtually. Common themes related to the superiority of virtual care were for "quick" visits, reviewing test results, chronic disease monitoring, and medication needs. Virtual care was less ideal when a physical exam was needed, and was not perceived as a good fit for an individual's cultural, language, or emotional needs. Certain conditions were identified as both ideal and non-ideal for the virtual format (e.g. mental healthcare). Discussion: Certain situations are more amenable to virtual care with personal preferences among both patients and physicians. Future priorities should ensure that virtual care is effective across the range of clinical situations in which it may be used and that both virtual and in-person options are equally available to those who want them.
Social touch deprivation during COVID-19: effects on psychological wellbeing and craving interpersonal touch

Abstract:Social touch has positive effects on social affiliation and stress alleviation. However, its ubiquitous presence in human life does not allow the study of social touch deprivation ‘in the wild’. Nevertheless, COVID-19-related restrictions such as social distancing allowed the systematic study of the degree to which social distancing affects tactile experiences and mental health. In this study, 1746 participants completed an online survey to examine intimate, friendly and professional touch experiences during COVID-19-related restrictions, their impact on mental health and the extent to which touch deprivation results in craving touch. We found that intimate touch deprivation during COVID-19-related restrictions is associated with higher anxiety and greater loneliness even though this type of touch is still the most experienced during the pandemic. Moreover, intimate touch is reported as the type of touch most craved during this period, thus being more prominent as the days practising social distancing increase. However, our results also show that the degree to which individuals crave touch during this period depends on individual differences in attachment style: the more anxiously attached, the more touch is craved; with the reverse pattern for avoidantly attached. These findings point to the important role of interpersonal and particularly intimate touch in times of distress and uncertainty.
