Treatment Decision-making in Chinese Inflammatory Bowel Disease Patients

Abstract:Background Inflammatory bowel disease (IBD) is rising in China, and the tendency for lifelong recurrence decreases patients' quality of life. However, no studies on treatment decision-making in Chinese patients with IBD exist. Thus, this study aimed to determine the actual and ideal decision-making, as well as factors affecting decision-making in Chinese IBD patients. Methods A multicenter online questionnaire was distributed among patients diagnosed with IBD. To assess factors that influence treatment decision-making, univariate and multivariate logistic regression analyses were performed. Results From March 20, 2018, to May 20, 2018, a total of 866 patients completed the questionnaires, including 222 patients with ulcerative colitis, 588 patients with Crohn's disease, and 56 patients with unclassified IBD. There was a significant difference between ideal and actual decision-making in Chinese IBD patients (P < .005). The factors affecting ideal decision-making included income, education, illness severity, religiosity, the importance of the treatment decision, the employment situation, and occupation area. The factors affecting actual decision-making included age, illness severity, religiosity, the employment situation, economic anxiety, concern about the side effects, and the importance of the treatment decision. Conclusions There is a significant difference between ideal and actual decision-making in IBD patients in China. That is, the economy, religiosity, illness severity, and concern about the side effects of treatment are the most important factors affecting treatment decisions in Chinese IBD patients.

Abstract:Inflammatory bowel disease (IBD) has become a common ailment in China in the past decade. At present, the level of diagnosis and treatment of inflammatory bowel disease in China still falls behind Western countries, while the incidence increases rapidly. Chinese IBD Working Group made a new consensus on diagnosis and treatment of inflammatory bowel disease in 2012. This article is to interpret some key issues in the consensus.

Abstract:Rapid progress in study on inflammatory bowel disease has been achieved in recent years both in China and abroad. It is necessary to revise the consensus statement issued in China on 2007. During the reforming of new consensus, emphasis was paid on collection of evidence-based materials and analyzing the research results obtained domestically. The main viewpoints in new consensus were approved by voting from experts participated. The newly revised consensus statement reflects new achievements, the contents were more deeply and comprehensively stated, and has better value of guidance for clinical practice. This article reported the main revisions based on 2007 consensus statement.

Abstract:Background Nowadays internet and social media community have provided a great amount of information for patients with inflammatory bowel disease (IBD). We aimed to assess the sources and needs of Chinese IBD patients through the internet and their impact on disease management. Methods Between November 24, 2017 and March 1, 2018. Totally 414 questionnaires were recruited online from seven different large urban hospitals in China. Internet usage, IBD knowledge and medication adherence were examined. One-way ANOVA, student t-tests and multivariate regression were used for statistical analysis. Results In all, 382 surveys were used for analysis, in which 291 (76.2%) had Crohn’s disease (CD), 74 (19.4%) ulcerative colitis (UC) and 17 (4.5%) undetermined colitis. The most common IBD-specific information sources were gastroenterologists (81.7%), Internet (58.6%), and WeChat or other social media (48.7%). Patients most frequently visited Baidu (65.2%), WeChat (58.9%) and Haodaifu network platform (33.2%) for personal information searching of IBD. Among ten WeChat public accounts of IBD, the China Crohn’s and Colitis Foundation (CCCF) (69.1%) and Changzaixin (20.2%) were most popular. A significant association was found between education (PP=0.003), frequent internet use in IBD-related information (PPP=0.003) and also had better medication adherence than undetermined patients (p=0.009). The majority of patients preferred obtaining information mainly through communication with (77.7%) or reference in the paper from IBD specialists (73%), only 18.6% patients preferred mainly from internet searching by themselves. They were interested in information about diet and daily life health care of IBD (81.2%), disease aetiology and course (80.1%), and medication advances and side effects (78.5%) from the Internet. Conclusions Internet and social media like WeChat have become the major information source for IBD education in China. This is beneficial to gain disease-related knowledge and improve future self-management for patients. Future initiatives should focus on providing more high-quality IBD information and increase interaction in patient education and disease organisation.

Abstract:The role of Chinese expert consensus in standardization and improvement of the clinical diagnosis and treatment of inflammatory bowel disease (IBD) is self-evident. As clinical and basic research in China becomes standardized under a better understanding of IBD, more data on Chinese patients have become available to develop a consensus and guideline. This guideline, collaboratively developed by the IBD Group, Chinese Society of Gastroenterology, incorporates the latest international consensus statements,[1-7] domestic research findings, and practical considerations, as an update based on the 2018 Chinese consensus on IBD management.[8] The IBD guideline consists of two parts: ulcerative colitis (UC) and Crohn's disease, and this manuscript presented the UC part. The evidence used in this guideline was collected and analyzed by the standard of guideline, and the contents were organized as problem statements, for clarity. The formulation of this guideline aimed to reflect the latest progress in IBD, providing comprehensive and valuable guidance for the clinical management of IBD.

Abstract:Since the Consensus on chronic gastritis in China was developed in September 2006 in Shanghai, much progress has been made in the diagnosis and treatment of chronic gastritis, including the Operative Link for Gastritis Assessment (OLGA), European Consensus On management of precancerous conditions of gastric cancer, and Maastricht-IV consensus. Therefore, the “2012 Chinese National Consensus Meeting on the Diagnosis and Treatment of Chronic Gastritis” was held in Shanghai in November 2012, sponsored by the Chinese Society of Gastroenterology, to revise the Consensus. Eighty-two gastroenterology experts from all over China discussed and passed the new consensus by an anonymous vote. The voting options were: 1. Totally agree; 2. Agree, but have some reservations; 3. Agree, but have greater reservations; 4. Disagree, but have reservations; 5. Totally disagree. If the item received “1”> 66.7% or “1+2” > 85% of the votes, the term was then considered as passed and included in the consensus. The terms with otherwise votes were considered failed to pass and not reported here. This consensus is aimed to serve as the position statement of Chinese Society Gastroenterology on the prevention, diagnosis, treatment and follow-up of chronic gastritis.
Patients’ Views of Shared Decision Making in Inflammatory Bowel Disease: a Survey in China

Abstract:Background Recently, decision-making process has become increasingly complex. But there is limited information on Chinese patients’ views of shared decision making (SDM) in inflammatory bowel disease (IBD). This questionnaire investigation aimed to understand Chinese patients’ perspectives and expectations of SDM in IBD and analyze the possible factors that influence their views. Methods An online survey was conducted from July 19th to 24th, 2020. A total of 1118 patients completed the survey. Results One-third of patients were dissatisfied with the current decision-making model, and the satisfaction of inpatients was lower than that of outpatients. 84% of patients preferred to participate in SDM, who were young and had a high education level, high income, commercial insurance, strong learning ability and knowledge of SDM. Most of those who did not want to participate (72%) were worried about the cost. The kind of medicine (948, 84.8%), surgical indications (505, 45.2%) and operation methods (482, 43.1%) were the topics that patients thought most require SDM. Side effects of medicine (837, 74.9%), costs of therapy (675, 60.4%), and surgical risks (563, 50.4%) were considered to be the most influential factors for SDM. 52.7% of all patients hoped experts in different disciplines would participate in SDM. The most desirable amount of time for discussion was 30 to 60 min (562/1118, 50.3%), that were associated with the cost of SDM. Conclusion We can meet the needs of patients by reducing costs and strengthening online patient education and exploring a model suitable for Chinese IBD patients.

Abstract:IBD Asian focus II was conducted in Shanghai,PR China,in March,2010.The meeting pointed out that in the past decade,the incidence and prevalence of IBD was reported widely in Asia and many variances were found compared with the West.So attention should be paid to explore the related critical factors affecting the features of Asian IBD compared with the West.A clinical guideline can provide standard guidance for clinical practice,but it is always long and complicated.Hence,the long reviewing time may lead to lag the steps for effective implementation to combat the disease.So it is significant to establish a brief and concise consensus statement.Consensus statement,which was established by Asian IBD work group at Macao in July,2009,was learned and discussed in this meeting.This article studied and summarized the consensus statement on IBD,including both Crohn's disease and ulcerative colitis,established by Asian IBD work group at Macao in July,2009.

Clinical Presentation of Inflammatory Bowel Disease： a Hospital Based Retrospective Study of 379 Patients in Eastern China

Abstract:Background Numerous studies from Europe and North America have provided a wealth of information regarding the epidemiological and clinical characteristics of inflammatory bowel disease (IBD) in Caucasians. Previous studies in mainland China have been limited by small patient numbers or by lack of detailed information about clinical subgroups of the disease. This study was carried out to assess the demographic and clinical characteristics of IBD in Chinese patients.Methods In the Sir Run Run Shaw Hospital between 1994 and 2003, 379 patients were diagnosed as IBD. Demographic and clinical data were collected and analysed.Results Of 379 patients, 317 had ulcerative colitis (UC) (83.6%, 168 male, 149 female, male-female ratio 1.13:1, age range at diagnosis 14 - 79 years, mean age 44 years) and 62 had Crohn's disease ( CD) (16.4%, 39 male and 23 female, male-female ratio 1.70:1, age range at diagnosis 13 - 70 years, mean age 33 years). In UC, 11.4% of patients had proctitis, 25.2% had proctosigniolditis, 18.6% were diseased to the splenic flexure and 44.8% had extensive colitis. Nine patients with UC (2.8%) had arthritis, three patients (0.9%) had iritis or conjunetivitis. Of the 62 CD patients, 16 (25.8%) had diseases restricted to the terminal ileum; 15 (24.2%) had colonic diseases; 20 (32.3%) had ileocolonic disease and 11 (17.7%) had disease involving the upper gastrointestinal tract.Conclusions This study shows similar caracteristics of IBD to that in the West but there are some differences with respect to severity and extraintestinal manifestations. The ethnic and geographic differences may give important clues to the aetiology of IBD.
P479 Trends in Medication Use and Treatment Patterns in Chinese Patients with Inflammatory Bowel Diseases: A Multicenter Cohort Study from 1999 to 2020

Abstract:Abstract Background The temporal changes in medication use and therapeutic paradigm in patients with Inflammatory bowel disease (IBD) have not been well elucidated in China over the past two decades. Therefore, we aimed to identify the temporal changes in medication use and treatment patterns for Chinese patients with IBD and to evaluate the factors associated with initial drug strategies. Methods A multicenter retrospective cohort study was conducted among Chinese patients newly diagnosed with Crohn’s disease (CD) and ulcerative colitis (UC) from January 1999 to December 2019. Medication utilization, treatment patterns, and associated factors with initial drug strategies were investigated. Results Prescriptions of 5-aminosalicylates (ASA) and corticosteroids (CS) gradually decreased, whereas prescriptions of immunosuppressants (IMS) and infliximab (IFX) increased in patients with CD. Except for stable prescriptions of ASA and CS, IMS and IFX increased since 2007 in patients with UC. Subgroup (N=957) analyses revealed that treatment patterns went through notable changes from ASA to IMS or IFX in patients with CD, while ASA and CS were still steadily prescribed in patients with UC. Logistic regression analyses revealed that perianal and gastrointestinal surgeries before diagnosis, disease behavior, and disease location were associated with different initial therapeutic strategies in patients with CD. However, medications before diagnosis, disease location, and diagnostic year might affect initial strategies in patients with UC. Conclusion Parallel to increasing uses of IMS and IFX in IBD over past two decades, a significant decrease in uses of ASA and CS were observed in CD, but not in UC. Treatment strategies analyses provided a unique insight in understanding long-term therapeutic patterns in the Chinese population.

Abstract:Inflammatory bowel disease (IBD), a traditionally “western disease”, has recently been reported to have risen in incidence in China. We summarize differences between Chinese and Western IBD based on relevant literatures published in both Chinese and English. Relevant references published in English were collected from an electronic database via PubMed and relevant references published in Chinese were also collected. Emerging clinical epidemiological data have shown a striking rise in the incidence rate of IBD in Mainland China over the last two decades. Although the current incidence rate of IBD in Mainland China is still lower than that in Western countries, the associated disease burden might be much heavier than previously thought due to the huge population. Clinical data have shown that disease characteristics (e.g., age of onset, male/female ratio, and location and disease severity) of Chinese IBD patients are different to those of Western IBD patients. Unlike the widely used bioimmunotherapeutic agents in Western countries, the management of IBD in China relies predominately on aminosalicylates and corticosteroids. The use of bioimmunotherapeutic agents is limited to a small number of IBD patients in China. Currently, many challenges remain, including the lack of properly trained IBD specialists, poor information availability to support patient self-management, and inappropriate treatment of some patients. Based on the current findings, the disease burden, characteristics, and treatment of Chinese IBD patients are slightly different to those of Western IBD patients. There are still great improvement requirements for the clinical management of IBD in China.

Abstract:The etiology of inflammatory bowel disease (IBD) is unclear, and its treatment is difficult. The correct diagnosis of IBD is crucial for treatment, but there is a lack of the diagnostic gold standard. A comprehensive diagnosis and analysis combined with clinical manifestations, endoscopy, imaging and histopathology is required. Therefore, the problem to be solved urgently is to develop a personalized treatment plan through a standardized multidisciplinary treatment (MDT) and the optimized combination of existing diagnosis and treatment methods, so as to improve the efficacy. Based on the current situation of IBD diagnosis and treatment, with reference to the successful experience of MDT diagnosis and treatment at home and abroad, and combined with China′s national conditions, this consensus is finally formulated by organizing multiple experts in relevant fields to conduct online voting. The significance, the institutional composition, the operating mode, the clinical practice and the MDT integrated management are discussed in detail. Through the formulation and publication of this consensus, we hope that it can promote the development of standardized IBD-MDT, improve the therapeutic efficacy of IBD in China, and strengthen the training of professionals.

Abstract:In August 2018, The Chinese Consensus on Endoscopic Diagnosis and Management of Gastrointestinal Submucosal Tumors, which was formulated by the Surgical Group of Chinese Society of Digestive Endoscopology, the Chinese Physicians Association Endoscopy Branch Digestive Endoscopy Professional Committee, the Gastrointestinal Surgery Group of Chinese Society of Surgery, was published in Chinese Journal of Gastrointestinal Surgery. This consensus is the first guideline in the area of gastrointestinal submucosal tumors (SMTs) in China, and it proposed the diagnosis and management procedure of SMTs from the endoscopists' viewpoint. This consensus presents case selection principles and technical principles of endoscopic treatment of SMTs. For and the indication and contraindication of endoscopic treatment of SMTs, it is suggested that clinician choose optimal procedure according to disease's characteristics and techniques of the clinician. In this review, the key contents of consensus are interpreted in detail. The application of endoscopic snare resection, endoscopic submucosal excavation, endoscopic full-thickness resection and submucosal tunneling endoscopic resection at different SMTs was summarized. At the same time, the controversies in endoscopic diagnosis and management of the SMTs, such as biopsy, indication of endoscopic treatment and new techniques of endoscopic therapy, were analyzed.

Current diagnosis and management of Crohn’s disease in China: results from a multicenter prospective disease registry

Abstract:Background This study aimed to understand the disease characteristics and treatment outcomes of Crohn’s disease (CD) in a real-world setting in China. Methods In this prospective, non-interventional, multicenter disease registry, adults (≥18 years) with existing and newly diagnosed CD were recruited from 14 medical centers across China from January 2015 to January 2017. The study consisted of the enrollment and follow-up periods, of 12 months each. Demographic, clinical characteristics, diagnostic duration and management of CD at enrollment were evaluated. Logistic regression analysis and stepwise multivariate logistic regression analysis used to assess the relationship between the risk factors and CD. Results Of 504 enrolled patients, 499 (99.0%) were eligible for analysis. The mean (SD) age at study enrollment was 32.3 (11.43) years and the majority (69.7%) of participants were male. In the past 15 years, a sustained decrease of the period of time in the diagnosis of CD was observed, at about 39.4 (24.11) months in 2010, which decreased to 3.1 (2.13) months in 2015. The most common presenting symptoms of CD included abdominal pain (78.0%), diarrhea (58.1%), weight loss (52.9%) and fever (30.1%). Oral ulcer (19.4%) and arthritis (9.8%) were the most common extra-intestinal manifestations. Non-stricturing non-penetrating (B1) (49.9%) behavior and ileocolonic involvement (L3) (56.2%) location were more frequent. Perianal disease was observed in 29.1% of the patients. Around 23.8% (119/499) patients had CD-related surgery other than perianal disease surgery. Older age at enrollment, longer disease course, complicated disease behavior and absence of perianal disease were all surgery risk factors ( p < 0.05). The most common medications was immunomodulators (e.g., azathioprine) (41.5%), anti-TNFα agents (32.9%) and aminosalicylates (20.6%). The mean (SD) Crohn’s Disease Active Index (CDAI) score was 159.1 (91.45) and almost half of the patients (49.1%, 81/165) were in remission. Conclusions This study demonstrated the CD-disease characteristics, risk factors of CD-related surgery and perianal disease, and treatment strategies in a real-world setting in China and may help in developing programs to diagnose and manage patients with CD.

Abstract:Inflammatory bowel disease mainly consisting of ulcerative colitis and Crohn's disease has been rising gradually during the last two decades in China. In this review article, we provide the latest epidemiological trends in incidence, prevalence, and mortality of IBD patients in China and summarize the risk factors and genetic susceptibility of Chinese IBD patients. We also compare these characteristics to those of IBD patients in Western countries.
