Abstract:Journal of International Medical Research, Volume 51, Issue 5, May 2023. ObjectiveTo examine the effectiveness of decortication to treat chronic tuberculous empyema (TE) using uniport video-assisted thoracoscopic surgery (VATS) versus conventional triport VATS.MethodsData from consecutive patients with stage II or III TE who underwent decortication with either uniport VATS (uniportal group) between July and December 2017, or triport VATS between January and July 2018 (triportal group), were retrospectively analysed. VATS procedures were performed under general anaesthesia with double lumen endotracheal intubation and clinical outcomes were compared between the two groups.ResultsClinical data were comparable between the groups (20 patients in each) regarding demographic and baseline characteristics, operative and postoperative characteristics, surgical procedure-related complications, and postoperative adverse events. No surgical procedure-related complications occurred during the perioperative period in either group. Threshold values for mechanical pain at 8 h postoperatively were significantly higher in the triportal group versus the uniportal group. Furthermore, the incidence of nausea and vomiting was significantly lower in the uniportal versus triportal group. In the triportal group, one patient required readmission and further intervention due to recurrence.ConclusionsUniport VATS decortication for stages II and III TE may be a feasible and safe procedure in selected patients. Moreover, uniport VATS may be less painful than triport VATS.

Abstract:Stage III tuberculous empyema is a common disease of tuberculosis. Traditionally, it has been treated by thoracotomy or video-assisted thoracoscopic surgery with two to four incisions. But conventional surgery has large trauma, large bleeding volume and long recovery time. To our knowledge it is the first report of surgery for stage III tuberculous empyema with a mini single-port approach.

Abstract:Abstract OBJECTIVES The goal of this study was to evaluate the safety and efficacy of decortication for stage III drug-resistant tuberculous empyema (TE). METHODS We analysed all patients with stage III TE who underwent decortication between March 2015 and October 2019 at Wuhan Pulmonary Hospital. The patients were divided into 2 groups according to drug-susceptibility testing of bronchoscopy lavage fluid, pleural effusion and tissue specimens, including a drug-resistant group and a drug-sensitive group. We collected and compared the preoperative, perioperative and postoperative data from the 2 groups to evaluate the safety and efficacy of decortication for stage III drug-resistant TE. RESULTS In total, 135 cases met the inclusion criteria and were enrolled, including 30 cases in the drug-resistant group and 105 cases in the drug-sensitive group. No deaths were recorded for the entire study population. Compared to the drug-sensitive group, the drug-resistant group had longer operation times (259.8 ± 78.4 min vs 187.2 ± 56.0 min, P = 0.00), a larger volume of intraoperative blood loss [300 (200,400) ml vs 200 (130, 300) ml, P = 0.00] and a higher intraoperative transfusion rate (5/30, 16.7% vs 4/105, 3.8%, P = 0.04). The rate of complications was significantly higher in the drug-resistant group (23; 76.7%) than in the drug-sensitive group (53; 50.5%) (P = 0.01). Recurrence was not reported in any of the patients. Twenty-three (76.7%) patients in the drug-resistant group and 90 (85.7%) patients in the drug-sensitive group recovered to an “excellent” level, and 3 cases in each group recovered to a “poor” level; there was no significant difference between the 2 groups in surgical effects (P = 0.21). CONCLUSIONS Decortication is a safe, effective and feasible option for patients with stage III drug-resistant TE, although the operation is difficult and risky.
[Uniportal Video-assisted Thoracoscopic Surgery for Acute Empyema]

Abstract:A 70-year-old man was referred to our department for the treatment of acute empyema. Since the chest drainage was of little effect due to the fibrinous loculation, deloculation followed by curettage and decortication under uniportal video-assisted thoracoscopic surgery( VATS) with flexible endoscopy was performed. Complete re-expansion of the lung was successfully achieved and the chest tube was removed on the fourth postoperative day. However, on the seventh postoperative day, purulent discharge was noted from the wound resulting in relapse of empyema which was successfully treated with the cleaning and negative pressure suction by re-insertion of the chest tube.
Multiport Video-assisted Thoracoscopic Surgery in Stage III Empyema Thoracis—a Case Series Analysis of a Cohort of 128 Patients from a Tertiary Care Centre in Eastern India

Abstract:Clinico-patho-physiologically, there is a considerable overlap in stages II and III empyema thoracis (ET) (Table 1). Video-assisted thoracoscopic surgical decortication (VATSD) and open decortication (OD) both have been used in stage III ET when surgical intervention was required in patients not responding to chest tube/pigtail catheter drainage and/or antibiotic therapy. This study was an analysis of a cohort of 128 consecutive patients of stage III ET operated upon in a tertiary care centre from April 2015 to July 2021 by a single surgeon. Demographic and clinical details were analysed. VATSD was successful in 80/128 patients (62.5%) of stage III ET. Forty-two patients (32.8%) required elective OD. Six patients (6/86 = 6.98%) were converted from VATSD to OD. While head-to-head comparison was problematic in the two unmatched groups, we found that operating time was longer in VATSD than in the OD group contrary to recent published literature probably because of a higher (55%) prevalence of pulmonary tuberculosis in that group. Other discriminators (Table 2) included ICD duration less in VATSD group (median 6 days for OD, versus 5.5 days for VATSD, p 0.034), length of post-operative stay lower in VATSD group (median 7 days for OD and 6.5 days for VATSD, p 0.018). Post-operative pain on post-operation day (POD) 1 was also less in VATSD ( p < 0.0001) and POD 5 ( p < 0.0001) in VATSD patients. We conclude that VATSD may be an effective and safe first surgical option for stage III ET with less post-operative discomfort, reduced length of hospital stay and fewer post-operative complications in most patients. While our study reflects a real-world scenario, multicentric randomised control trials (RCTs) need to be undertaken across geographies for changes in the guidelines and recommendations for the treatment of stage III ET. In the consensus guidelines of the American Association of Thoracic Surgery (AATS), VATSD for stage III ET is a class IIA recommendation.

Abstract:Background: This retrospective intention-to-treat study aims to identify risk factors associated with intraoperative conversion from Video-Assisted Thoracoscopic Surgery (VATS) Decortication to open thoracotomy in patients with Stage III Tuberculous Empyema, specifically focusing on non-Multi-Drug Resistant (MDR)/Extensively Drug-Resistant (XDR) cases. Methods: The study included 122 patients with non-MDR/XDR tuberculous empyema who were initially scheduled for VATS decortication. Patients were divided into two groups: the Thoracoscopy group (n = 64), who successfully underwent VATS decortication, and the Conversion group (n = 58), who required intraoperative conversion to open thoracotomy. Complex cases were excluded from the study. The analysis focused solely on factors leading to conversion, rather than overall treatment outcomes. Results: A notable difference was observed in the rate of regular preoperative glucocorticoid utilization between the two cohorts, with the Conversion group exhibiting a lower percentage (46.5 %) in comparison to the Thoracoscopy group (75.0 %). Furthermore, the Thoracoscopy group displayed a significantly reduced frequency of ipsilateral lung abnormalities prior to the surgery (37.5 %), as opposed to that of the Conversion group (65.5 %). Multivariate logistic regression analysis revealed that the regular preoperative glucocorticoid use (odds ratio (OR) = 3.444, 95 % confidence interval (CI): 1.602-7.407) and pre-existing pulmonary lesions (OR = 0.31, 95%CI: 0.150-0.663) were potential influential factors. Conclusion: Inconsistent preoperative glucocorticoid administration and ipsilateral lung lesions were identified as exacerbating factors leading to the complexity of VATS decortication by causing intraoperative pulmonary tissue contusion or hemorrhage, thus hindering the successful completion of VATS decortication and necessitating a conversion to thoracotomy. Awareness of these factors can aid surgeons in making well-informed decisions regarding the preoperative surgical approach.
Electronic Endoscope Insertion into a Thoracic Drainage Tube is a New Technique in the Treatment and Diagnosis of Pleural Diseases

Abstract:BACKGROUND:Pleural disease remains a commonly encountered clinical problem for both physician and surgeon. This study describes a new way to better diagnose and treat pleural diseases (hemothorax, empyema, and pleural effusion) using an electronic endoscope (gastroscope or bronchoscope).METHODS:We conducted a retrospective study of the use of an electronic endoscope in the treatment and diagnosis of pleural diseases. From November 2006 to February 2008, a total of 17 patients (3 women, 14 men; mean age = 41.8 years; range = 18-62 years) underwent procedures for thoracic empyema (13 patients), traumatic clotted hemothorax (3 patients), and undiagnosed pleural effusion (1 patient). The electronic endoscope was inserted via the thoracic drainage tube for the treatment or diagnosis of pleural diseases after regular treatments, including thoracentesis, tube thoracostomy, and biopsy, failed.RESULTS:All patients were cured and discharged from hospital and were followed up for 6 months. The patients recovered well and there was no recurrence.CONCLUSION:The technique of inserting an electronic endoscope into the thoracic drainage tube for diagnosis and treatment of pleural diseases is simple, effective, minimally invasive, and cost-effective.

Abstract:To explore the influence of CT findings on the optimal timing of open decorticationin patients with stage III tuberculous empyema. A total of 80 patients with stage III tuberculous empyema who had undergone open decortications were recruited; 44 patients had chest CT findings indicating low-density lines, while 36 patients did not show this imaging finding. Demographic data, perioperative data and preoperative and postoperative chest CT images were collected. In the low-density line group, the duration of disease ( P = 0.0030) and the preoperative anti-tuberculosis time ( P = 0.0016) were longer than those of the group without low-density lines, and the ESR ( P = 0.0218), CRP ( P = 0.0027) and leukocyte count ( P = 0.0339) were lower in the low-density line group. Additionally, in the median operative time ( P = 0.0003), intraoperative blood loss ( P < 0.0001), volume of catheter drainage during 48 h after operation ( P = 0.0067), chest tube duration ( P < 0.0001), and length of hospital stay ( P = 0.0154) were significantly lower in the low-density line group than in the group without low-density lines. A total of 88.64% of participants in the low-density line group showed hyperplasia with hyaline degeneration in pathological examination, which was observed only in 41.67% of patients without low-density lines. In addition, gaseous necrosis was considerably higher in patients without a low-density line ( P = 0.004), while the low-density line group had a higher rate of treatment success ( P < 0.05). Patients with stage III tuberculous empyema presenting with low-density lines around the thickened fibrous pleural rind on preoperative CT imaging may be good candidates for open decortication.

Abstract:Background: Mediastinal mature teratoma is the most common histological type of primary extragonadal germ cell tumor. In this report, we describe a rare case of giant mature teratoma located primarily in the anterior mediastinum and causing partial atelectasis of the upper and middle lobes of the right lung, as well as extrinsic compression of the right atrium. Case summary: A 31-year-old male with a giant mediastinal mature teratoma presented with progressive exertional dyspnea and chest pain for 1 mo. Computed tomography of the chest indicated the diagnosis of anterior mediastinal teratoma. The patient underwent right uniportal anterior approach video-assisted thoracoscopic surgery (VATS). En bloc resection of the giant teratoma, wedge resection of the upper and middle lobes of the right lung, resection of the thymus and partial excision of the pericardium were successfully performed. The pathological diagnosis revealed a mature cystic teratoma with foreign-body reaction that was closely related to the right lung, atrium dextrum, superior vena cava and ascending aorta. An atrophic thymic tissue was also discovered at the external teratoma surface. The patient was discharged on postoperative day 7. Conclusion: This is the first report of the use of uniportal VATS for complete resection of a teratoma in combination with wedge resection of the right upper and middle lung lobes and partial resection of the pericardium.
Application of three-dimensional thoracoscope system in uniportal video-assisted thoracoscopic surgery

Abstract:Objective To evaluate the feasibility of 3D thoracoscopic system in uniportal video-assisted thoracoscopic surgery (uniportal VATS) for the treatment of thoracic diseases. Methods 7 cases of thoracic diseases were accomplished with 3D thoracoscope from Nov 2015 to Dec 2015, including 3 cases of lobectomy, 2 cases of wedge resection of pulmonary tumor, 2 cases of mediastinal mass. Results All the operations were completed successfully, without conversion to open surgery. All the patient recovery successfully. The data of operative time, bleeding volume, postoperative chest tube drainage and hospital time and postoperative complications were counted. No complications and tumor recurrence or metastasis were observed during the followed 3 months. Conclusion 3D thoracoscopic system can be used in uniportal video-assisted thoracoscopic surgery (uniportal VATS) safely and provide high-definition and better sense of depth which facilitate the operation more precise and safer.
Clinical characteristics and decortication outcomes of bacterial, tuberculous and fungal pleural infection

Abstract:<sec><title>BACKGROUND</title>Pleural infection leading to empyema is a severe condition marked by accumulated infected fluid in the pleural space. Pneumonia with parapneumonic effusion is its most common precursor. The global incidence of pleural infections has increased significantly, with existing literature mainly focusing on bacterial empyema, leaving a gap in comparative analyses.</sec><sec><title>METHOD</title>A retrospective review was conducted on 561 cases of bacterial, tuberculous, and fungal empyema over a 10-year period. The study compared and analysed overall survival rates, 30-day mortality rates after surgery, and clinical characteristics.</sec><sec><title>RESULTS</title>The three empyema groups displayed distinct clinical characteristics. Fungal empyema had the worst overall survival compared to bacterial and tuberculous empyema, which had similar survival rates based on 30-day and 2-year mortality. Fungal empyema, advanced age, and high Charlson Comorbidity Index (CCI) score were independent predictors of poor prognosis.</sec><sec><title>CONCLUSION</title>Fungal empyema has the highest mortality rate post-decortication surgery. Advanced age and high CCI score are independent predictors of poor prognosis.</sec>.
Video-Assisted Thoracoscopic Surgery for Posterior Mediastinal Tumors

Abstract:Objective To discuss the security,effectiveness and risk factors of video-assisted thoracoscopic surgery for posterior mediastinal tumors.Methods We retrospectively analyzed the data of 59 patients including 36 men and 23 women who underwent thoracoscopic resection of posterior mediastinal tumors in People's Hospital of Peking University from May 2001 to July 2009.Their age ranged from 6 to 73 years old with an average age of 40.6 years old.The average maximum diameter of the tumors was 4.86 cm.All procedures were performed under general anesthesia and tumors were cut out with three ports.The anterior port was extended to 6 to 10 cm when conversion to thoracotomy was needed.After mediastinal pleura were opened,the tumor was stripped out along the outside of peplos and the vascular pedicle nerves were managed respectively.Results All surgeries were carried out successfully.The surgical duration,perioperative blood loss,postoperative chest tube duration and postoperative stay in hospital were respectively 45-300 min(125.80±57.40 min),10-1 000 ml(168.10±157.70 ml),1-10 d(2.50±1.74 d),and 2-14 d(5.24±2.24 d).There were 6 cases of conversion to open thoracotomy with a conversion rate of 10.2%.Postoperative pathology showed that there were 46 cases of neurogenic tumors,10 cases of cyst,2 cases of teratoma,and 1 case of lipoma.Follow-up was done on 51 cases for a period of 7-108 months(55.0±24.0 months) and 8(13.6%) cases were missed out during the period.No recurrence or death occurred during the follow-up.Logistic multi-variable analysis showed that maximum diameter of the tumor ≥6 cm was the independent risk factor for extending operative time(OR=1.932,P=0.004),increasing perioperative blood loss(OR=2.267,P=0.002),increasing conversion rate to thoracotomy(OR=3.123,P=0.004) and increasing postoperative complication rate(OR=1.778,P=0.013).Conclusion Video-assisted thoracoscopic surgery for posterior mediastinal tumor is safe and effective.Maximum diameter of the tumor ≥6 cm is an independent risk factor for increasing operation difficulty and risk.
Angioplasty With Stent Implantation for Portal Venous Stenosis Caused by Abdominal Tuberculosis: A Case Report and Literature Review

Abstract:Abdominal tuberculosis is one of common forms of extra-pulmonary tuberculosis. However, portal vein involvement leading to portal venous stenosis and portal hypertension is a rare complication in abdominal tuberculosis. Because of the non-specific presentations and insensitive response to anti-tuberculosis therapy of the lesions involving portal vein, it continues to be both a diagnostic and treatment challenge. We have reported a 22-year-old woman presented with massive ascites and pleural effusion, which was proved to be TB infection by pleural biopsy. After standard anti-tuberculosis therapy, her systemic symptoms completely resolved while ascites worsened with serum-ascites albumin gradient >11 g/L. Contrast-enhanced computed tomography and portal venography showed severe main portal vein stenosis from compression by multiple calcified hilar lymph nodes. Finally, the patient was diagnosed with portal venous stenosis due to lymphadenopathy after abdominal tuberculosis infection. Portal venous angioplasty by balloon dilation with stent implantation was performed and continued anti-tuberculosis therapy were administrated after discharge. The ascites resolved promptly with no recurrence occurred during the six-month follow-up. Refractory ascites due to portal venous stenosis is an uncommon vascular complication of abdominal tuberculosis. Portal venous angioplasty with stent placement could be a safe and effective treatment for irreversible vascular lesions after anti-tuberculosis therapy.

Abstract:Objective To discuss the feasibility of uniportal video-assisted thoracic surgery ( VATS) technique applied to pulmonary bullae resection for the treatment of spontaneous pneumothorax .Methods Three hundred and twenty-seven cases undergoing video-assisted pulmonary bullae resction and pleurodesis were divided into uniportal VATS technique group ( n=194) and traditional two-port VATS technique group ( n=133) .The clinical data including the operation time , postoperative thoracic cavity drainage volume , postoperative leakage time , postoperative extubation time,postoperative recurrence rate and perioperative staple-using number were compared between two groups .Results There were no significant differences in operation time ,bleeding ,postoperative recurrence ,postoperative hospital stay and perioperative staple-using number between two groups (P>0.05).The postoperative pain and wound numbness in the uniportal VATS technique group improved significantly compared with two -port VATS technique group ( P <0.05 ) . Conclusion The uniportal VATS technique has similar effect with two-port VATS technique on spontaneous pneumothorax patients undergoing pulmonary bullae resction ,but the patients using the uniportal technique have less pains and wound numbness after operation .
Uniportal Video-Assisted Thoracoscopic Right Upper Posterior Segmentectomy with Systematic Mediastinal Lymphadenectomy

Abstract:Uniportal video-assisted thoracoscopic surgery (VATS) has now evolved into a sophisticated technique that can be used in some of the most complex thoracic procedures; however, this approach to segmentectomy is not standardized, and the surgical procedure varies between surgeons. Here, we describe the use of our uniportal VATS procedure during right upper posterior segmentectomy in a patient with a nodule in the right upper lobe. Subsequent mediastinal lymphadenectomy was performed. The patient has recovered well after surgery. We believe that uniportal VATS segmentectomy is a technically safe and feasible alternative approach to conventional thoracoscopic techniques for treating lung cancer.

Abstract:In the past 20 years, video-assisted thoracic surgery has made a great progress, from 4-ports to 2-ports, and eventually to this revolutionary approach-uniportal video-assisted thoracic surgery (VATS). It can share the same instruments, the same surgical principles, the same strategies of trouble-shooting and the same postoperative short-term outcomes with conventional VATS via the improvement of instruments and surgical skills. And it has already been safe to adopt uniportal VATS in complicated pulmonary resections. In this study, we shared five video clips about uniportal VATS for complicated pulmonary resections: sleeve resection, segmentectomy, pneumonectomy and angioplasty. We hope these video clips can be instrumental for young surgeons.
Perioperative Management Challenges for Post-Tuberculous Stage-III Empyema with Massive Pneumothorax Mimicking Vanishing Lung Syndrome: A Case Report

Abstract:Surgical resection is frequently the intervention required for post-tuberculous empyema or other sequels. However, pneumonectomy may not be feasible in some situations, and video-assisted thoracoscopic surgery (VATS) plays a role in such a scenario. Whether a patient undergoes open resection of VATS, isolation of infected lung is integral to one-lung ventilation and better access to the surgical field, and a double-lumen tube (DLT) remains the preferred choice. Difficulties in DLT placement after pneumonectomy are reported; however, failure to isolate a lung by appropriately placed DLT is scarce or absent. A 28-year cachectic gentleman with poor preoperative lung function was suffering from endobronchial tuberculosis. He also had one episode of tuberculosis twelve-year back. At presentation, he had a massive pneumothorax and stage-III empyema as a sequel, including a rare finding of plastered mediastinum mimicking vanishing lung syndrome. He underwent uniportal-VATS under general anesthesia using one-lung ventilation. Complete lung destruction from active tuberculosis and its sequel leading to the plastered mediastinum and deformed airway pose a significant lung isolation challenge. U-VATS can be considered for therapeutic purpose where standard thoracotomy and pneumonectomy is contra-indicated. However, lung isolation in such patients is tricky and poses a risk. The present case highlights the challenges faced with lung isolation using a DLT and discusses the probable remedy to these problems.

Abstract:Background: Uniportal video-assisted thoracoscopic surgery (VATS) is an emerging technique in the field of thoracic surgery. In this article, we aimed to describe uniportal VATS lobectomy that was conventionally performed in an Eastern center, which might help facilitate a good beginning in practice this technique. Methods: The indications of uniportal VATS lobectomy are merely equal, if not the same, to multiportal VATS lobectomy. All patients would receive a combination of epidural and general anesthesia, and were provided with patient-controlled analgesia (PCA) postoperatively. Generally, the patient was kept in a folding knife gesture (with the cranial side slightly raised up and caudal side pushed down) in lateral decubitus position. Results: In uniportal VATS lobectomy, the geographical view differed from multiportal VATS, while the thoracic cavity required carefully exploration to identify any unexpected invasion or metastasis. Since the incision was located at the anterior axillary line close to the anterior part of the hilar, the posterior part of the hilar, and diaphragm were far from the surgical incision. Conclusions: Uniportal VATS, is not the happy ending in thoracic surgery. Beside its rapid progression, there remained a lot of questions to answer. Its uncovered benefits and drawbacks over traditional VATS, its possible improvements, and less invasive peri-operative managements could be the future topics. A comparison between uniportal versus multiportal VATS using high-level methodology is therefore required. With the development of surgical minds and instruments, uniportal VATS would have done more to the patients.
