Abstract:Objective: We tried to understand the prevalence of depression comorbidity with anxiety disorders and the effect of depression comorbidity with anxiety disorder on the clinical severity.Methods: This was a prospective,multi-center collaborating study.Subjects meeting the diagnostic criteria of DSM-IV were enrolled.Scales including Hamilton Depression Scale with 17 items(HAMD-17),Hamilton Anxiety Scale(HAMA),Social Disability Screening Schedule(SDSS) and Clinical Global Impression(CGI) were used to evaluate all patients after being passed the entering criteria.Patient's demographic data,anxiety and depressive symptoms,condition of suicide were recorded.The diagnostic list of anxiety disorder of the Schedule for Affective Disorders and Schizophrenia(SADS) was applied for all patients.Results:There were totally 508 patients in the study,269 patients(53.0%) with first episode,294 patients(57.9%)with suicide attempt,and 55 patients(10.8%)with suicide behaviors.45 patients(8.9%)patients with psychotic symptoms.The average scores of HAMD and HAMA scales were 32.56±7.07 and 21.01±7.27 separately.The prevalence of depression comorbidity with anxiety disorders was 68.9% and 16.7% with two or more anxiety disorders.The majority of comorbidity anxiety disorder was generalized anxiety disorder(GAD).There was significant difference of the severity degree between patients with depression only and comorbidity with anxiety disorders,but no significant difference in patients with suicide attempts and behaviors or not.36.0% patients had comorbidity with somatic diseases at the same time.Conclusion:This study suggests that it is common of depression comorbidity with anxiety disorders in clinical practice.GAD is the most common disorder in comorbidity anxiety disorders and about 1/3 patients are accompanied with somatic diseases.Clinical practitioners should pay more attention on comorbidity wity depression and anxiety disorders.

Abstract:Depressive and anxiety disorders often comorbid, which causes more severe impairments. The high comorbidity and shared genetic and psychological factors between the 2 disorders have brought arguments about whether they represent a common construct, and whether the current classification is meaningful. In this editorial, a state-of-the-art overview of recent studies on the underlying mechanism of such comorbidity, and the association between and differentiation of the 2 disorders is provided. Recent studies employing data-driven approaches such as latent class analysis (LCA) and network analysis to investigate the symptomatology of depression and anxiety have indicated unique characteristics and bridging symptoms of their comorbidity. Whereas previous neurobiological and neuroendocrinological studies reported common alterations in prefrontal-limbic pathways, serotonergic projections and the hypothalamic-pituitary-adrenal (HPA) axis, recent research suggests that distinct neural circuits and heterogeneous changes in HPA activity may exist in depression when compared to anxiety. Lastly, both depression and anxiety have been long associated with decision-making deficits; however, emerging evidence from computational psychiatry demonstrate that there may be unique neurocognitive and computational alterations in each disorder. By investigating the common and unique symptomatic characteristics and underlying neurobiological and neurocomputational mechanisms of the 2 disorders as well as their comorbidity, it can be concluded that recent studies have greatly advanced our understanding of the etiology and neuropathophysiology of these disorders.

Abstract:Depression, a highly prevalent disorder affecting over 280 million people worldwide, is comorbid with many neurological disorders, particularly Alzheimer's disease (AD). Depression and AD share overlapping pathophysiology, and the search for accountable biological substrates made it an essential and intriguing field of research. The paper outlines the neurobiological pathways coinciding with depression and AD, including neurotrophin signalling, the hypothalamic–pituitary–adrenal axis (HPA), cellular apoptosis, neuroinflammation, and other aetiological factors. Understanding overlapping pathways is crucial in identifying common pathophysiological substrates that can be targeted for effective management of disease state. Antidepressants, particularly monoaminergic drugs (first-line therapy), are shown to have modest or no clinical benefits. Regardless of the ineffectiveness of conventional antidepressants, these drugs remain the mainstay for treating depressive symptoms in AD. To overcome the ineffectiveness of traditional pharmacological agents in treating comorbid conditions, a novel therapeutic class has been discussed in the paper. This includes neurotransmitter modulators, glutamatergic system modulators, mitochondrial modulators, antioxidant agents, HPA axis targeted therapy, inflammatory system targeted therapy, neurogenesis targeted therapy, repurposed anti-diabetic agents, and others. The primary clinical challenge is the development of therapeutic agents and the effective diagnosis of the comorbid condition for which no specific diagnosable scale is present. Hence, introducing Artificial Intelligence (AI) into the healthcare system is revolutionary. AI implemented with interdisciplinary strategies (neuroimaging, EEG, molecular biomarkers) bound to have accurate clinical interpretation of symptoms. Moreover, AI has the potential to forecast neurodegenerative and psychiatric illness much in advance before visible/observable clinical symptoms get precipitated.

Abstract:Advances in healthcare have led to an extended life expectancy throughout the developed world, but cognitive impairment in later life, and Alzheimer's disease (AD) in particular, remains one of the intractable problems which can blight quality of life as we age. Depression in Alzheimer's disease is an additional factor which has a significant impact on disability, disease progression, and caregiver burden.This book, volume 4 of the Advances in Alzheimer’s Disease book series which is published in coordination with the Journal of Alzheimer’s Disease, presents papers which reflect the progress in recent years of research into depression in AD. This research has focused on several areas, including the improvement of diagnostic criteria and outcome measures for depression and depressive symptoms in AD, genetic and imaging studies to elucidate the neurobiological mechanisms, and clinical trials of antidepressants. The book is divided into sections on phenomenology, epidemiology, neuropsychology, neurobiology and neuropathology, neuroimaging, genetics, and treatment. Providing a stimulus to further research in this challenging area by engaging both basic and clinical researchers, this book will be of interest to all those whose work involves understanding and dealing with depression in those suffering from Alzheimer's disease.

Abstract:Depression and Alzheimer's disease (AD) are substantial public health concerns. In the past decades, a link between two disease entities has received extensive acknowledgment, yet the complex nature of this relationship has demanded further clarification. Some evidence indicates that mid-life depression may be an AD risk factor, while a chronic course of depression in late life may be a precursor to or symptom of dementia. Recently, multiple pathophysiological mechanisms have been proposed to underlie the bidirectional relationship between depression and AD, including genetic predisposition, immune dysregulation, the accumulation of AD-related biomarkers (e.g., amyloid beta and tau), and alterations in brain structure. That being the case, numerous therapeutic approaches, such as pharmacology treatments, psychotherapy, and lifestyle interventions, have been suggested as potential means of interfering with these pathways. However, the current literature on this topic remains fragmented and lacks a comprehensive review characterizing the association between depression and AD. In this review, we aim to address these gaps by providing an overview of the co-occurrence and temporal relationship between depression and AD, as well as exploring their underlying mechanisms. We also examine the current therapeutic regimens for depression and their implications for AD management, and outline key challenges facing the field.

Abstract:consensus on the involvement of non-neuronal cells in the pathological progression. Recently, glial cells are getting attention as a key players of non-cell autonomous neurodegeneration in neurological disorders. Especially, glial crosstalk and its action on neurons are highlighted. It is demonstrated that microglia convert astrocytes to neurotoxic reactive A1 astrocytes (Liddelow, et al., 2017). In contrast, it is also reported that astrocytes activate microglia to produce neuroinflammation (Rohl and Sievers, 2005). Besides, astrocyte-microglia crosstalk contributes to degradation of protein aggregates (Rostami, et al., 2021). Furthermore, recent studies indicated microglia-oligodendrocyte or astrocyte-oligodendrocyte interaction promoted neuronal dysfunction and neurodegeneration (Lohrberg, et al., 2020, Papazian, et al., 2021). Thus, glial communication could be a main target to understand pathological mechanism and develop neuroprotective therapeutic approach. This series includes five articles covering different aspect of the topic outlined above. I hope that this Special Issue can produce discussion about therapeutic strategies that glial crosstalk as a target for the development of disease-modifying therapies for neurological disorders in the future. Finally, I would like to take this opportunity to express my gratitude to the all authors and co-authors for their excellent contributions to this special issue.

Abstract:Emerging behavioral and neuroimaging studies in schizophrenia (SCZ) and major depressive disorder (MD) are mapping mechanisms of co-occurring and distinct affective disturbances across these disorders. This constitutes a critical goal towards developing rationally guided therapies for upstream neural pathways that contribute to comorbid symptoms across disorders. We highlight the current state of the art in our understanding of emotional dysregulation in SCZ versus MD by focusing on broad domains of behavioral function that can map onto underlying neural systems, namely deficits in hedonics, anticipatory behaviors, computations underlying value and effort, and effortful goal-directed behaviors needed to pursue rewarding outcomes. We highlight unique disturbances in each disorder that may involve dissociable neural systems, but also possible interactions between affect and cognition in MD versus SCZ. Finally, we review computational and translational approaches that offer mechanistic insight into how cellular-level disruptions can lead to complex affective disturbances, informing development of therapies across MD and SCZ.

Abstract:Depression and Alzheimer's disease (AD) are prevalent neuropsychiatric disorders with intriguing epidemiological overlaps. Their interrelation has recently garnered widespread attention. Empirical evidence indicates that depressive disorders significantly contribute to AD risk, and approximately a quarter of AD patients have comorbid major depressive disorder, which underscores the bidirectional link between AD and depression. A growing body of evidence substantiates pervasive sex differences in both AD and depression: both conditions exhibit a higher incidence among women than among men. However, the available literature on this topic is somewhat fragmented, with no comprehensive review that delineates sex disparities in the depression–AD correlation. In this review, we bridge these gaps by summarizing recent progress in understanding sex-based differences in mechanisms, genetics, and therapeutic prospects for depression and AD. Additionally, we outline key challenges in the field, holding potential for improving treatment precision and efficacy tailored to male and female patients' distinct needs.

Lankton, for recognizing this gap and encouraging the development of two special issues highlighting the relevance of hypnosis in treating this pervasive and debilitating disorder. The first Special Issue was released in January (2024) as this Journal transitioned from hard copy to being published digitally online. This is the second of the two-issue series that I am honored to have guest edited. I earnestly hope those of us in this field will continue to expand our knowledge of ways hypnosis can help provide relief to those who are suffering. New insights and approaches will be critically important to develop and include in future issues.

With a keen focus on Alzheimer's disease (AD), anxiety disorders, epilepsy, schizophrenia, and autism spectrum disorder (ASD), this article delves into their underlying mechanisms, clinical presentations, diagnostic challenges, and therapeutic interventions. Highlighting the considerable disability and societal costs that these conditions impose, it reflects on the over six million individuals grappling with Alzheimer's, the 19 million American adults living with anxiety disorders, the three million with epilepsy, and the global reach of schizophrenia affecting approximately 20 million people. Furthermore, it examines the emerging landscape of ASD, noting the escalating diagnosis rates and the pressing need for innovative treatments and equitable healthcare access. Abstract:The number of people living with diabetes continues to rise. Therefore neurologists or other health care practitioners may be increasingly faced with comorbid neuropsychiatric disorders commonly presented by diabetic patients. More recently there has been an increasing research interest not only in the interactions between diabetes and the nervous system, the fine structure and functional changes of the brain, but also in the cognitive aspects of antidiabetic treatments. Patients with both types of diabetes mellitus may show signs of cognitive decline, and depression. Comorbid insomnia, anxiety, and distress may also occur. The bidirectional relationships between all these phenomena as well as their connection with diabetes can lead to further health and quality of life deterioration. Therefore it is important that all practitioners involved in the care of diabetic patients recognize the presence of comorbid neuropsychiatric disturbances early on during the healthcare process. Identifying higher risk patients and early screening could improve the prognosis of diabetes and may prevent complications.

Abstract:Depression occurs with a high prevalence of up to 50% in patients with Alzheimer's disease (AD) and increases the caregivers' burden. Depression symptoms can precede clinical diagnosis of AD for years or occurs around the onset of AD, although the etiology and pathologic mechanism of depression in AD pathogenesis remain unclear. Here, we provide an overview on recent studies, indicating that genetic factors, neuroanatomic changes, vascular risk factors, and the imbalance of neurotransmitters might contribute to depressive symptoms in AD. Tau pathology and amyloid-beta accumulation also correlate with depression in AD. In addition, the alteration of hypothalamic-pituitary-adrenal axis, inflammatory pathway, and neurotrophin deficiency are the possible biological mechanisms linking depression and AD, and might become the potential targets for AD treatment. Current data support that antidepressants are promising to alleviate the symptom, though the efficacy is controversial. Moreover, antidementia medication and non-pharmacological interventions can be potential choices. In this review, we describe the prevalence and clinical course of depression in AD, analyze the underlying mechanisms, and discuss the possible management strategies for depression in patients with AD.
