Abstract:Coverage of the left subclavian artery is often necessary to establish an adequate landing zone in patients undergoing thoracic endovascular aortic repair,which can avoids endoleaks that contribute the important aspect of technical success of thoracic endovascular aortic repair.However,there have been controversial whether the routine revascularization of left subclavian artery benefits in preventing the complications in perioperative,like cerebrovascular accident,and which approach of revascularization should be taken for more advantages.In this review,the disputes have been discussed between the routine revascularization and selective revascularization of left subclavian artery based on clinic research published in recent years,and also introduce the progress in open surgery and endovascular technique of left subclavian artery revascularization.
A Single Center Experience of in Situ Needle Fenestration of Supra-Aortic Branches During Thoracic Endovascular Aortic Repair.

Abstract:Background: To report the early results and technical details of in situ needle fenestration during thoracic endovascular aortic repair (TEVAR). Materials and Methods: Thirty-seven patients with thoracic aortic disease who underwent TEVAR with in situ needle fenestration in our department between February 2016 and April 2017 were included in this study. Their demographic information, clinical manifestations, periprocedural outcomes, and follow-up results were analyzed. Results: In total, 96.4% of attempted fenestrations were successful (53/55). Fenestration of the left subclavian artery (LSA) was performed in 26 patients, fenestration of either the LSA or brachiocephalic trunk (BCT) plus the left common carotid artery (LCCA) was performed in six patients, and fenestration of the three supra-aortic branches was performed in five patients. In the three-fenestration group with five patients, the procedure was under the protection of a cardiopulmonary bypass. External iliac artery rupture occurred in one case. Cerebrovascular events occurred in three cases, and myocardial infarction occurred in one. One case had a branch stent graft stuck in the brachial artery. During a median follow-up of 21 months, two patients developed retrograde type A dissection, and one was subsequently treated with a stent graft and BCT fenestration. One case presented with numbness in the fingertips of the left hand, and computed tomography angiography (CTA) indicated stenosis of the branch stent in the LSA. CTA examination revealed no endoleaks or expansion of the aortic aneurysms in any of the patients. Conclusions: In situ needle fenestration of endografts in the aortic arch is technically feasible, yielding acceptable short-term results in a selected patient cohort. Further studies are needed to determine mid- and long-term outcomes before more widespread adoption.
Endovascular Coil Embolization and Stenting for the Treatment of Iatrogenic Right Internal Mammary Artery Injury: A Case Report

Abstract:A 54-year-old Chinese woman presented with a 10-year history of repeated paroxysmal palpitations. She was diagnosed with paroxysmal supraventricular tachycardia by 12-lead electrocardiogram and was advised to undergo catheter-based radiofrequency ablation. During the procedure, a rare complication occurred that was diagnosed as a right internal mammary artery penetrating injury. After appropriate emergency treatment with arterial embolization and membrane-covered stent implantation, the patient was out of immediate danger of haemorrhaging. Follow-up computed tomography angiography of the subclavian artery at 3 months after she was discharged from hospital revealed stent-graft patency with no evidence of in-stent thrombosis or stent stenosis. No problems were observed at the 6-month follow-up visit.
In situ fenestration to preserve supra-aortic branch during thoracic endovascular aortic repair

Abstract:Objective To summarize experience of in situ fenestration techniques during thoracic endovascular aortic repair (TEVAR) preserving supra-aortic branch vessels. Methods A total of 15 patients underwent TEVAR with an in situ stent graft fenestration technique from July 2016 to February 2017 in our hospital. Results All of the TEVAR procedures were completed successfully. Of the 23 target vessels, 21 vessels were successfully reconstructed using in situ fenestration with the success rate 91.3%. Three cases underwent in situ fenestration of brachiocephalic artery, left common carotid artery and left subclavian artery simultaneously. The operative time was (550.0 ±155.6) mins. Two patients had in situ fenestration of left common carotid artery and left subclavian artery during (405.0 ±275.8) mins. Ten cases were done only left subclavian artery with (175.0±43.5) mins. One case of external iliac artery injury was found during operation and 1 case with cerebral infarction postoperatively. There was no peri-operative death. The length of stay was(15.3±4.3) (8-26) days. No symptom was found for all patients during the follow-up of (3.1±2.0) (1-7) months. Thirteen patients performed CTA examination and disease progression was not present. All the branch stent grafts were patent without endoleak or any other stent-associated complications. Conclusions In situ fenestration techniques could be used effectively and safely so as to enlarge the indication of TEVAR in the treatment of aortic disease with minimal invasion.
Management of Brachiocephalic Vein Injury During Tubeless Subxiphoid Thoracoscopic Thymectomy

Abstract:We have reported the usefulness of the subxiphoid video-assisted thoracoscopic surgery approach in thymectomy. However, such a new method may have unknown complications that rarely occur. The brachiocephalic vein has been considered to be the vessel that is most frequently injured when performing thymectomy because of the skeletal anatomy. We herein report a case of intraoperative injury of brachiocephalic vein using tubeless subxiphoid thoracoscopic thymectomy. No additional complications have been found in the 3 months since the operation. Though subxiphoid video-assisted thoracoscopic surgery thymectomy is a safe and less invasive operation, intraoperative complications are possible, and surgeons should express caution.
Right subclavian artery injury during catheter insertion into the right internal jugular vein treated with endovascular stent graft placement after balloon occlusion test: A case report

Abstract:Subclavian artery injuries during internal jugular vein puncture when attempting central venous catheter insertion are rare. A 60-year-old man undergoing treatment for neuromyelitis optica with paralysis and sensory loss developed a complication during catheter placement into his right internal jugular vein for plasmapheresis. His previous physician felt resistance and discontinued the procedure. The patient later developed mild dyspnea and dysphagia. Computed tomography scans indicated thrombus formation and tracheal deviation. Contrast-enhanced computed tomography scans showed right subclavian artery injury with extravasation and a large pseudoaneurysm. Following transferal to our hospital, he was stable and asymptomatic; however, contrast-enhanced computed tomography scans showed a pseudoaneurysm located proximal to the right subclavian artery. Considering challenges with compression hemostasis and the invasiveness of open surgery, endovascular treatment was selected using a VIABAHN stent graft. A balloon occlusion test of the right vertebral artery was performed to assess stroke risk. Prophylactic embolization of the right vertebral artery, internal thoracic artery, and thyrocervical trunk were performed to prevent a type 2 endoleak. On hospital day 5, our patient showed no postoperative complications and was transferred to the referring hospital. Follow-up imaging showed the graft was intact with no pseudoaneurysm, confirming successful treatment. Endovascular treatment with a stent graft is highly effective for peripheral artery injuries. Using a balloon occlusion test to assess collateral blood flow and stroke risk is essential pretreatment, especially when a graft might occlude the vertebral artery. Balloon occlusion tests are recommended when planning treatment for iatrogenic and other types of subclavian artery injuries.
A real-time ultrasound-guided supraclavicular approach to the brachiocephalic vein to prevent Hickman catheter bending and occlusion

Abstract:The infraclavicular approach (subclavian approach) to the central vein is most often selected for implantation of a tunnelled Hickman/Broviac catheter because central venous catheter (CVC) insertion into the upper body is optimal considering patient comfort as well as the risk of infection [1] and intravenous thrombosis [2, 3]. However, the actual puncture site for a real-time ultrasound-guided infraclavicular axillary vein approach is at the level of the axillary vein in a much more lateral location than that for the traditional landmark infraclavicular approach to the subclavian vein. This is because an optimal view of the subclavian vein is difficult to obtain with a real-time ultrasound-guided technique, as the ultrasound beam is restricted by the clavicle [4]; the younger the patient, the greater would be its influence because of the width of the ultrasound probe. As a result, Hickman/Broviac catheter bending due to a sharply curved angle in the subcutaneous tunnel may occur when the subcutaneous tunnel exit needs to be created inside the patient’s nipple to obtain sufficient tunnel length, especially in young paediatric patients. When the internal jugular vein is chosen as an alternative puncture site, adverse events of catheter bending and occlusion may occur because the puncture direction to the target vein and the direction of the subcutaneous tunnel become very steep [5].

Abstract:Aortic injuries are devastating complications after spinal surgery. We here would like to share our experience with a successful endovascular treatment of an iatrogenic thoracic aortic injury caused by misplaced pedicle screw after spinal surgery. A patient underwent posterior spinal surgery for L1 burst fracture was transferred to our department for instrumentation removal. An iatrogenic thoracic aortic injury at T11 level caused by a pedicle screw was recognized after routine CT scans. Complete screw extraction and instrumentation removal were performed after the placement of a thoracic aortic stent graft covering the injured region. The patient had an uneventful postoperative period and no complications were observed in the 1 and 12-month follow-up by contrast-enhanced CT scans. For the delayed thoracic aortic injuries which usually were usually shown on CT scans as pseudoaneurysm or penetration of the aorta, stent graft implantation would provide a preferred solution with high-effectiveness, low-invasiveness and fewer complications compared with conventional open surgery.
Anatomic Basis of Safe Percutaneous Subclavian Venous Catheterization

Abstract:BACKGROUND: The technique of percutaneous catheterization of the subclavian vein by the infraclavicular approach is dependent on the location of the subclavian vein in relation to the clavicle. The purpose of this study was to analyze the anatomic relationship between these two structures and how it is influenced by changes in shoulder positioning.METHODS: Dissections of the infraclavicular region were performed in seven fresh cadavers and linear measurements made to determine the extent of overlap between the vein and the clavicle in different shoulder positions.RESULTS: When the shoulder was in neutral position, the subclavian vein was overlapped by the medial third or more of the clavicle and this segment of bone was able to serve as a landmark for the vein. However, shoulder elevation displaced the clavicle cephalad and reduced the degree of overlap. Mild shoulder retraction increased the area of contact between the vein and the undersurface of the clavicle, whereas protraction lifted the clavicle off the vein.CONCLUSION: Infraclavicular subclavian venipuncture should be performed with shoulders in a neutral position and also in slight retraction. An appreciation of the anatomic relationship between the clavicle and the subclavian vein is the key to successful execution of this technique.
Supraclavicular vein approach to overcoming ipsilateral chronic subclavian vein obstruction during pacemaker-ICD lead revision or upgrading

Abstract:Aims: We report our experience with the supraclavicular vein approach of subclavian vein puncture to overcome ipsilateral chronic obstruction when implanting pacemaker or implantable cardioverter defibrillator leads. Methods and results: The subclavian vein obstruction was documented by venography. The skin was punctured with an 18-gauge needle, 1 cm lateral to the lateral head of the sternocleidomastoid muscle and 1 cm cranial to the clavicle. The needle was directed under and close to the clavicle pointing to the sternal notch. Once the vein was successfully punctured, medial to the obstruction, a 0.38 in. guidewire was inserted into the venous bed. A peel-away sheath was indwelled using the Seldinger technique. The leads were placed in the standard fashion; they were secured by suture to the subcutaneous tissue of the fossa supraclavicularis major using a protective sleeve. The proximal portion of the lead was tunnelled over the clavicle down to the device's prepectoral pocket. Lead insertion was performed in four patients (twice in one patient) with total left subclavian vein obstruction; the site of the obstruction was in the mid-segment of the left subclavian vein in two patients, in the axillary and distal segment of the subclavian vein in one patient, and in the distal segment of the subclavian vein in one patient. There were no complications with the surgical wound and the lead parameters remained stable. Conclusion: The supraclavicular approach of the subclavian vein puncture to overcome ipsilateral total occlusion is feasible and safe.

Abstract:Superior vena cava syndrome (SVCS) is a clinical condition characterized by signs and symptoms resulting from the blockage or narrowing of the thin-walled superior vena cava (SVC). This obstruction can lead to significant morbidity and mortality. In this case, we report a 58-year-old patient who was diagnosed with SVCS due to a massive compressing anterior mediastinal mass leading to signs and symptoms of SVCS, including shortness of breath, dizziness, palpitations, and neck swelling, which was managed surgically by excision of the mass and reconstruction of the brachiocephalic vein using a synthetic graft.

Abstract:Abstract Background Ultrasound-guided central venous catheterization has become a standard procedure. However, mechanical complications are still reported. Case presentation An 85-year-old woman presented with coagulopathic bladder tamponade. Ultrasound-guided right internal jugular venous catheterization was planned because of difficult peripheral venous access. A guidewire was advanced through a needle inserted at the midpoint of the right carotid triangle. The guidewire was identified in the short axis, but not in the long-axis ultrasound view, leading to inadvertent insertion of the catheter into the right subclavian artery through the internal jugular vein. Stent graft insertion was performed for perforation closure. The patient exhibited no symptoms of cerebral ischemia following stent graft insertion. Discussion This case demonstrated that the needle-sticking site should not be placed close to the clavicle for ultrasound-guided internal jugular venous catheterization, as it may not confirm the position of guidewire in the long-axis ultrasound view.

Abstract:Background Central venous catheterization is an important device that is frequently performed for various purposes in operating theatres and intensive care units, there are many different sites that are commonly used for central venous catheterization, and each site has its own advantages, disadvantages, and possible complications. To avoid catheterization failure and mechanical complications, ultrasound guidance is currently recommended in several guidelines and is widely used for central venous catheterization. It has been shown to reduce the incidence of arterial puncture, hematoma formation, pneumothorax, hemothorax, and catheterization failure. The subclavian vein (SCV) catheterization is more comfortable for patients than other sites but has a higher risk of some complication as pneumothorax. The aim of this study was to compare the Supraclavicular and Infraclavicular approaches for ultrasonographic guided right subclavian venous catheterization as regard the accessibility and the safety of both approaches. Methods This randomized controlled study was conducted in Sohag University Hospital after local ethical committee approval, 210 patients had a right subclavian vein catheterization by either the supraclavicular or the infraclavicular approach (105 for each group), using a two dimensional high-frequency ultrasound guide were enrolled in the study regarding the accessibility (time to first puncture and number of punctures) and the complications like arterial puncture, hematoma formation, arrhythmia, and catheter misplacement. Results The infraclavicular approach is more accessible, regarding the number of punctures, for the Supraclavicular approach there were 56 (53.3%) cases needed only one puncture to achieve successful venous puncture and 27 (25.7%) cases needed 2 punctures and 5 (4.8%) needed 3 punctures and 17 (16.2%) failed in catheterization, for the Infraclavicular approach there were 79 (75.2%) of cases in these group needed only one puncture to achieve successful venous puncture and 17 (16.2%) cases needed 2 punctures and 9 (8.6%) failed in catheterization with a significant difference between both groups P value was 0.003. As regard the puncture time, for the Supraclavicular approach 83 (79%) cases successes in less than 3 min, and 22 (21%) cases needed more than 3 min, in the infraclavicular approach 94 (89.5%) cases succeeded in less than 3 min and 11 (10.5%) cases needed more than 3 min at P value 0.037 and had a lower rate of complications than the supraclavicular approach, like arrhythmia, arterial puncture, hematoma formation, catheter misplacement, and postcatheterization infection, but a higher rate of pneumothorax formation however in a nonsignificant way. Conclusion The infraclavicular approach for ultrasound-guided right subclavian venous catheterization is better than the supraclavicular approach due to a higher chance of first pass success rate and a lower rate of complications.
ProGlide entrapment of the occlusive balloon during repair of an iatrogenic subclavian artery injury

Abstract:The insertion of any central venous catheter (CVC) is associated with a risk of damage to neurovascular structures, pneumothorax, cardiac arrhythmias, and infection1. Unintentional arterial puncture remains rare, occurring in 6.3-9.4% of attempted internal jugular vein (IJV) catheterisation and 3.1-4.9% of attempted subclavian vein catheterisation2. We present a previously undocumented complication encountered while utilising the Perclose ProGlide device in the case of a 59-year-old male who underwent right subclavian artery closure following the accidental insertion of a 14Fr Vascath into the right subclavian artery. This was performed using two ProGlide devices and one Angio-Seal device. Following deployment of the ProGlide devices, an uninflated balloon passed into the subclavian artery as a precaution, but not used, was removed. One of the ProGlide devices became dislodged having been deployed into the balloon, threatening haemostasis.

Abstract:This report describes the case of a frail 36-year-old patient who underwent an endovascular treatment of a right subclavian artery pseudoaneurysm (SAP) associated with an arteriovenous fistula secondary to a traumatic central venous catheter insertion. The deployment of a covered stent from the innominate to the right common carotid artery combined with coiling of the SAP and the internal mammary artery was performed. Two additional covered stents were deployed from the vertebral artery to the distal subclavian artery to preserve right upper extremity circulation. This case highlights the feasibility of an endovascular treatment of a complex SAP in a candidate unsuitable for open surgery.

Abstract:A fibrin sheath with central venous occlusion is a common complication after central venous catheterization, and these patients often experience catheter dysfunction. A calcified fibrin sheath can cause a catheter to be stuck, and typically necessitates catheter removal or replacement. From another point of view, a calcified fibrin sheath can be seen in ultrasound and computed tomography, and the original fibrin sheath channel between the internal jugular vein and the atrium is unusually strong. When central vein occlusion occurs, the remnant calcified fibrin sheath of the internal jugular vein can be punctured under ultrasound guidance, allowing the guidewire to enter the atrium directly through the fibrin sheath. Here, we report a case in which we achieved easy recanalization of a long segment occluded superior vena cava by puncturing the remnant calcified fibrin sheath of the internal jugular vein.
Ultrasound-guided, long-axis, in-plane, infraclavicular axillary vein cannulation: A 6-year experience

Abstract:Background: Despite its potential advantages, ultrasound-guided cannulation of the axillary vein in the infraclavicular area is still rarely used as an alternative to other techniques. There are few large series demonstrating the safety and feasibility of this approach. Methods: Retrospective analysis of data on patients undergoing ultrasound-guided, long-axis, in-plane infraclavicular axillary vein cannulation for the incidence of complications and the failure rate from two secondary-care hospitals. Results: The analysis included 710 successful attempts of axillary vein long-axis, in-plane, US-guided cannulation, and 24 (3.3%) failed attempts. We recorded a 96.7% success rate with an overall incidence of complications of 13%, mainly malposition (8.1%). There was one case of pneumothorax (0.14%), five cases of arterial puncture (0.7%), and two cases of brachial plexus injury. Conclusions: The US-guided axillary central venous cannulation (CVC) access technique can be undertaken successfully in patients, even in challenging circumstances. Taken together with existing work on the utility and safety of this technique, we suggest that it should be adopted more widely in clinical practice.

Various patient characteristics, including age, gender, body mass index (BMI), preoperative hemoglobin, preoperative hematocrit, preoperative mean corpuscular hemoglobin concentration (MCHC), preoperative albumin, preoperative serum creatinine, puncture needles from different manufacturers and previous history of subclavian vein catheterization were assessed via univariate and multivariate analyses. Results: For the included patients, landmark-guided subclavian vein puncture was successful in 1476 cases and unsuccessful in 86 cases (success rate of 94.5%). Successful subclavian vein catheterization was achieved via right and left subclavian vein puncture in 1392 and 84 cases, respectively. In univariate analyses, age and preoperative hemoglobin were associated with failure of subclavian vein catheterization. In a multivariate analysis, aged more than 60 years was a risk factor while the central venous access with Certofix (R) was associated with an increased rate of success (p-values of 0.001 and 0.015, respectively). Conclusions: This study has demonstrated that patient aged more than 60 years was a risk factor for failure of subclavian vein catheterization while the central venous access with Certofix (R) was associated with an increased rate of success. Exsanguination Following Iatrogenic Subclavian Artery Injury: A Surgical Rescue

Abstract:Iatrogenic injuries can result from a variety of medical interventions and procedures, ranging from surgical errors to adverse reactions to medications, but what is more important is that it has to be timely managed. This case report highlights the complication of subclavian artery injury following multiple attempts of central venous catheterization for total parenteral nutrition, where intensive monitoring, early evaluation with imaging, and timely intervention by multidisciplinary team approach rescued the patient from the catastrophic exsanguination at the level I trauma center. An 18-year-old gentleman, a follow-up case of low velocity penetrating torso trauma with right-sided cardiac, diaphragmatic, liver, and transverse colonic injury with class IV hemorrhagic shock, and undergone damage control surgery with primary repair of cardiac, diaphragmatic, and transverse colonic injury with perihepatic packing, was re-admitted with the clinical features of acute intestinal obstruction. He underwent emergency laparotomy and adhesiolysis. In the postoperative period, total parenteral nutrition supplementation was planned. Ultrasound-guided central line insertion via the right subclavian vein failed despite multiple attempts. Immediate post-procedure, his vital parameters were normal. But later, he developed clinical signs of hemorrhagic shock. Resuscitative measures started immediately as per the advanced trauma life support protocol. His contrast-enhanced computed tomography chest and angiographic studies showed the right subclavian artery injury with right-sided massive hemothorax with underlying lung collapse. Emergent endovascular stenting of the right subclavian artery, followed by emergency thoracotomy with evacuation of hemothorax, was done. The post-procedure course was uneventful.
