Abstract:End Stage Renal Disease (ESRD) represents the terminal phase of several chronic kidney diseases, characterized by gastrointestinal symptoms such as nausea and vomiting, cardiovascular symptoms such as heart failure and shortness of breath, as well as neurological symptoms such as irritability and depression, which can pose a serious threat to the patient's life. [ 1–3 ] As of 2022, the number of ESRD patients in China has reached 3.592 million, and the number of patients is constantly increasing year by year. Hemodialysis and kidney transplantation are the main treatment methods for these patients. [ 4 ] ESRD patients mainly use maintenance hemodialysis (MH) as kidney replacement therapy, which not only changes their lifestyle but also impacts their life quality. However, 90% of patients in China still choose hemodialysis for treatment. [ 5 ] Hemodialysis is a process that entails routing blood from the body to an external dialyzer consisting of numerous hollow fibers. Within these fibers, there occurs an exchange of blood and electrolyte solutions, which possess comparable concentrations to those present in the body. This exchange, facilitated through diffusion and convection, effectively eliminates metabolic waste, while ensuring the maintenance of electrolyte and acid–base balance and finally removing excess water from the body. Purified blood is then transfused back. [ 6 , 7 ] Although hemodialysis can effectively prolong the life of patients, MH can have a significant impact on their physical and mental health. In long-term treatment, it not only causes great pain to the patient's body, but also increases the burden on the family. Pu T et al found through retrospective analysis that 2 patients with rheumatoid arthritis on hemodialysis experienced severe pancytopenia complications after receiving methotrexate treatment. After receiving various support and hemodialysis treatments, the patients were able to recover. The results showed that caution should be exercised when using methotrexate in the absence of alternative options. [ 8 ] Spinowitz et al found through dose-effect analysis of circulating sodium zirconate in hemodialysis patients that adjusting the circulating sodium zirconate from 5 g to 15 g on nondialysis days can increase the serum potassium concentration before dialysis to 4.0 to 5.0 mmol/L, without affecting the safety of fluid balance in the short term. [ 9 ] To help patients better treat diseases, the study proposes an integrated health belief model (IHBM) intervention method to explore the impact of self-care ability and related indicators on MH.
