Abstract:Objective: To assess the effectiveness of early bedside cycling training in enhancing recovery among patients following liver transplantation. Methods: A total of 64 patients who underwent liver transplantation were randomly assigned to either an experimental group or a control group, with 32 patients in each group. Patients in the control group received routine rehabilitation training after postoperative consciousness recovery and tracheal extubation. The experimental group received supplemental lower limb bedside cycling training in addition to the routine rehabilitation. Differences in physical function, length of intensive care unit, and hospital stays were compared between the two groups. Results: The experimental group showed significantly greater improvements in physical function, such as activities of daily living scores and Berg Balance Scale scores, compared to the control group (P < .05). While the experimental group also exhibited decreased length of intensive care unit and hospital stays compared to the control group, these differences lacked statistical significance. Conclusion: Early bedside cycling training contributes to enhanced physical function, such as balance function and activities of daily living, among patients undergoing liver transplantation.

Abstract:Thoracoscopic lobectomy is the main type of surgical treatment for lung cancer. Postoperative patients have complications and decreased pulmonary function, which affects their discharge time and quality of life. Lung ventilator training has been shown to promote the postoperative recovery of patients; however, no specific treatment plan has been approved to enhance lung rehabilitation. Therefore, it is necessary to explore methods to promote the postoperative rehabilitation of patients with lung cancer. The patients with lung cancer who were admitted to Banan Hospital Affiliated to Chongqing Medical University (Chongqing, China) between January 2022 and January 2023, and who planned to undergo a thoracoscopic lobectomy, were randomly categorized into two groups. The experimental group began lung rehabilitation training 2 weeks before the operation and received individualized nutrition programs. The control group did not receive lung rehabilitation training and nutrition programs. The quality of life, lung function, 6‐min walking distance (6MWD), nutritional status, postoperative complications, hospital expenses and hospital stay between the two groups were compared. Finally, 86 and 83 patients were included in the test and control groups, respectively. Regarding the postoperative indicators, the patients in the test group scored higher in all areas of quality of life, exhibited higher lung function and 6MWD, and had significantly higher serum total protein, albumin and hemoglobin levels, and body mass index, compared with the control group. Furthermore, the incidence of postoperative pulmonary complications, the duration of hospitalization and the hospitalization costs were lower in the experimental group. In conclusion, lung rehabilitation training combined with nutritional intervention can promote the postoperative rehabilitation of patients with lung cancer. The research has been duly registered in the Chinese Clinical Trial Register platform (registration no. ChiCTR2300078681; registered Dec 15, 2023).

Abstract:BACKGROUND: For patients in intensive care unit (ICU), mechanical ventilation is an effective treatment to survive from acute illness and improve survival rates. However, long periods of bed rest and restricted physical activity can result in side effects. This study aimed to investigate the feasibility of early rehabilitation therapy in patients with mechanical ventilation.METHODS: A randomized controlled trial was carried out. Sixty patients, with tracheal intubation or tracheostomy more than 48 hours and less than 72 hours, were admitted to the ICU of the Affiliated Hospital of Medical College, Qingdao University, from May 2010 to May 2012. These patients were randomly divided into a rehabilitation group and a control group. In the rehabilitation group, rehabilitation therapy was performed twice daily, and the training time and intensity were adjusted according to the condition of the patients. Early rehabilitation therapy included heading up actively, transferring from the supine position to sitting position, sitting at the edge of the bed, sitting in chair, transferring from sitting to standing, and ambulating bedside. The patient's body mass index, days to first out of bed, duration of mechanical ventilation, length of ICU stay, APACHE II score, highest FiO2, lowest PaO2/FiO2 and hospital mortality of patients were all compared between the rehabilitation group and the control group. The differences between the two groups were compared using Student's t test.RESULTS: There was no significant difference in body mass index, APACHE II score, highest FiO2, lowest PaO2/FiO2 and hospital mortality between the rehabilitation group and the control group (P>0.05). Patients in the rehabilitation group had shorter days to first out of bed (3.8±1.2 d vs. 7.3±2.8 d; P=0.00), duration of mechanical ventilation (5.6±2.1 d vs. 12.7±4.1 d; P=0.005) and length of ICU stay (12.7±4.1 d vs. 15.2±4.5 d; P=0.01) compared with the control group.CONCLUSION: Early rehabilitation therapy was feasible and effective in improving the outcomes of patients with mechanical ventilation.

Abstract:Abstract Background Physical exercise training is the central component of pulmonary rehabilitation. This study aimed to further investigate the rehabilitative effects of pulmonary-based Qigong exercise (PQE) in stable patients with chronic obstructive pulmonary disease (COPD). Methods In this randomized, assessor-blinded clinical trial, 44 participants with stable COPD were randomly assigned to 2 groups in a 1:1 ratio. Participants in the control group received usual care for 3 months. Participants in the intervention group received usual care combined with PQE (60 min each time, 2 times per day, 7 days per week, for 3 months). The outcome included exercise capacity, lung function test, skeletal muscle strength, dyspnea, and quality of life were measured before and after intervention. Results A total of 37 participants completed the trial. Compared to the control group, after 3 months of PQE, the mean change in exercise capacity, skeletal muscle strength, and quality of life were statistically significant ( P < 0.05, for each), but no significant differences were observed in lung function (except for the forced expiratory volume in one second) and dyspnea ( P > 0.05, for each). Conclusion The findings of study suggest that the proposed program of 3 months of PQE intervention has significant improvement in exercise capacity, skeletal muscle strength, and quality of life of COPD-stable patients. Abstract:Objective This study aimed to evaluate the impact of exercise and respiratory training for chronic obstructive pulmonary disease (COPD) patients on specific COPD-related outcomes, such as lung function, exercise capacity, and patient prognosis. Methods This work had a retrospective design. The subjects of this study were 90 patients with COPD who were treated in our hospital from May 2020 to May 2021. The patients were divided into two groups according to whether they performed exercise combined with respiratory training based on conventional treatment. After two weeks of intervention, the difference in rehabilitation outcomes between the two groups was detected at the first and third months after discharge. Results The rehabilitation compliance of the observation group was significantly higher than that of the control group (95.56% vs. 77.78%, P < .05). The forced expiratory volume in one second (FEV1) (2.66±0.71 vs. 2.28±0.48, P = .004), forced vital capacity (FVC) (3.30±0.70 vs. 3.00±0.63, P = .035), FEV1/FVC (80.61±8.01% vs. 76.77±7.34%, P = .020), maximal ventilation volume per minute (MVV) (96.14±3.50 vs. 91.20±4.97, P < .001), partial pressure of blood oxygen (PaO2) (89.52±6.87 vs. 82.65±6.54, P < .001), and oxygen saturation of blood (SaO2) (98.05±1.27 vs. 95.90±1.42, P < .001) in the observation group were significantly higher than those in the control group. In contrast, arterial partial pressure of carbon dioxide (PaCO2) (39.52±1.66 vs. 43.21±2.01, P < .001) was significantly lower than that in the control group. The observation group's 6-minute walking distance (6MWD) (401.05±65.66 vs. 360.25±54.94, P = .002) after treatment was significantly longer than those of the control group. The acute exacerbation rate (6.67% vs. 20.00%, P = .036), rehospitalization rate (4.44% vs. 17.78%, P = .044), and mortality rate (0.00% vs. 8.89%, P = .041) of the observation group were lower than those of the control group. Conclusion The application of exercise training combined with respiratory training in the rehabilitation treatment of COPD patients can more effectively promote the improvement of lung function, blood gas indexes, exercise endurance, and respiratory function and can improve the symptoms of dyspnea, with a good prognosis. This study confirmed the additional health benefits of exercise/respiratory training for COPD patients and provides evidence for the application of exercise/respiratory training in clinical practice. Therefore, it is recommended that in clinical practice, the necessity of exercise/respiratory training in COPD patients needs to be considered. However, the duration of intervention and follow-up period in this study were short, and longer intervention and follow-up are still needed to further confirm the results of this study.

Abstract:Introduction: Chronic Obstructive Pulmonary Disease (COPD) brought a heavy risk of morbidity, hospital admission, and death. Respiratory training as an important part of pulmonary rehabilitation has not been validated in home-based elderly patients with stable COPD. This study aims to demonstrate the efficacy of a standardized respiratory training in home-based elderly patients with stable COPD. Methods: The prospective, randomized controlled trial will be designed and conduced in the Second Affiliated Hospital of Chengdu Medical College (China National Nuclear Corporation 416 Hospital),China. Eligible patients with stable COPD will be randomly divided into experimental group and control group, both of the 2 groups will carry out regular home-based pulmonary rehabilitation for 3 months, additionally experimental group will receive a standardized respiratory training for 3 months. The primary outcome is the pulmonary function test before and after 3 months exercises. Discussion: The study will provide a objective basis for the use of respiratory training in home-based pulmonary rehabilitation for elderly patients with stable COPD.

Abstract:To explore the therapeutic effect of aerobic exercise nursing plans based on target heart rate in cardiopulmonary exercise tests on male patients with chronic obstructive pulmonary disease. This study recruited 90 male patients with chronic obstructive pulmonary disease who met specific screening criteria and were evenly divided into a control group and an experimental group based on a random number table. The control group received respiratory training based on the target heart rate in the cardiopulmonary exercise test, while the experimental group received aerobic exercise based on the target heart rate in the cardiopulmonary exercise test. Both groups received 12 weeks of exercise each. Cardiopulmonary function tests showed that peak oxygen consumption (peak VO2), anaerobic threshold (AT), forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), and the FEV1/FVC ratio in the test group were significantly higher than those in the control group (p < 0.05); furthermore, the carbon dioxide ventilation equivalent (VE/VCO2) slope was significantly lower than that in the control group (p < 0.05). The 6MWT of the test group was significantly higher than that in the control group (p < 0.05). The CAT score, Borg score and scores related to the symptoms, activities, and impacts related to quality-of-life were significantly reduced after intervention (p < 0.05). Aerobic exercise based on cardiopulmonary exercise testing can improve the health status, quality of life and prognosis of COPD patients.

Abstract:Objective To explore the effects of integrated respiratory function training for elderly patients with lung cancer on the incidence rate of postoperative pulmonary complications,duration of chest tubes' retention and hospitalization stay length.Methods Sixty elderly paitents with lung cancer admittted in thoracic surgery department were recruited in this study by convenience sampling and divided into experimental group and control group evenly with table of random digit.Routine nursing was provided to all patients by primary nurses and one-to-one integrated respiratory training was given in experimental group.Results There was significant difference in incidence rate of postoperative pulmonary complications(P<0.01),duration of chest tubes' retention(P<0.01)and hospitalization stay length(P<0.05)between two groups.Conclusion Integrated respiratory function training could reduce the incidence rate of postoperative pulmonary complications and shorten the duration of chest tubes' retention and hospitalization stay length of elderly patients with lung cancer.Integrated respiratory function training comprised of effective coughing,abdominal breathing,pursed-lip breathing and staged breathing exercises is suitalbe for mastery and use by elderly patients with lung cancer,and can effectively improve the rehablitation outcome.
Early comprehensive pulmonary rehabilitation for hospitalized patients with acute ex-acerbation of chronic obstructive pulmonary disease: a randomized controlled trial

Abstract:Objective: To investigate whether an early comprehensive pulmonary rehabilitation intervention initiated during hospital admission is safe and effective for patients with acute exacerbation of chronic obstructive pulmonary disease. Design: Prospective randomized controlled study. Subjects/patients: Patients with acute exacerbation of chronic obstructive pulmonary disease. Methods: In total, 108 patients were randomized to the early comprehensive pulmonary rehabilitation and usual care groups within 48 hours. The 6-min walking distance, quality of life, breathlessness, and inspiratory muscle strength were measured on admission and discharge. Any adverse events of pulmonary rehabilitation were recorded. Results: On discharge, the patients in the early comprehensive pulmonary rehabilitation group had a more significant improvement in the 6-min walking distance (47.5 vs 23.0, p = 0.04). There was no significant difference in quality of life and breathlessness between the 2 groups. In the early comprehensive pulmonary rehabilitation group, inspiratory muscle strength and peak inspiratory flow were significantly improved, and the changes were much more pronounced than in the usual care group. There were no adverse events. Conclusion: Early comprehensive pulmonary rehabilitation is safe and effective for hospitalized patients with acute exacerbation of chronic obstructive pulmonary disease, and should be performed during the early stage of hospitalization.

Abstract:BACKGROUND: The functional status and outcomes in patients with prolonged mechanical ventilation (PMV) are often limited by poor endurance and pulmonary mechanics, which result from the primary diseases or prolonged time bedridden. We evaluate the impact of exercise training on pulmonary mechanics, physical functional status, and hospitalization outcomes in PMV patients.METHODS: Twenty-seven subjects with PMV in our respiratory care center (RCC) were divided randomly into an exercise training group (n = 12) and a control group (n = 15). The exercise program comprised 10 sessions of exercise training. The measurement of pulmonary mechanics and physical functional status (Functional Independence Measurement and Barthel index) were performed pre-study and post-study. The hospitalization outcomes included: days of mechanical ventilation, hospitalization days, and weaning and mortality rates during RCC stay.RESULTS: The training group had significant improvement in tidal volume (143.6 mL vs 192.5 mL, P = .02) and rapid shallow breathing index after training (162.2 vs 110.6, P = .009). No significant change was found in the control group except respiratory rate. Both groups had significant improvement in functional status during the study. However, the training group had greater changes in FIM score than the control group (44.6 vs 34.2, P = .024). The training group also had shorter RCC stay and higher weaning and survival rates than the control group, although no statistical difference was found.CONCLUSIONS: Subjects with PMV in our RCC demonstrated significant improvement in pulmonary mechanics and functional status after exercise training. The application of exercise training may be helpful for PMV patients to improve hospitalization outcomes.

Abstract:This article conducts an in-depth study on the recovery of lung function in patients with congenital heart disease after cardiopulmonary bypass through early rehabilitation training, and discusses the effectiveness of early activities after cardiopulmonary bypass in children with congenital heart disease. The children with congenital heart disease who met the criteria were divided into an intervention group and a control group according to the random number table method, and prospective randomized controlled clinical trials were conducted. The control group was given routine clinical care. In addition to routine care, the experimental group increased early active intervention. The intervention content mainly includes preoperative early activity education and postoperative child vital signs to provide early activity guide. The rehabilitation was used as indicators to evaluate the postoperative recovery of the children. Data were collected at the time of discharge and 3 months after discharge, and statistics were performed using SPSS18.0 software analysis. To evaluate the effect of early activity on postoperative recovery in children with congenital heart disease, it improved the heart and lung function in children, improve their quality of life, promotes postoperative recovery of children, and provide a scientific basis for early active intervention in clinical work.

Abstract:Objective: This study aimed to evaluate the clinical effectiveness of early pulmonary rehabilitation (PR) treatment methods for stroke-associated pneumonia (SAP). Methods: This is a prospective, randomized controlled intervention study. Eighty SAP patients admitted to the rehabilitation department of Liuzhou People's Hospital from June 2020 to December 2021 were selected and divided into an intervention group (40 cases) and a control group (40 cases) using the random number table approach. Patients in both groups received conventional treatments. Patients in the intervention group received early PR interventions, while patients in the control group received conventional rehabilitation treatment. The clinical effectiveness and pulmonary function indexes were compared. Results: The intervention group showed shorter durations of stay in the intensive care unit, relief from symptoms, and mechanical ventilation compared to the control group following treatment (P < 0.05). The total clinical efficacy after intervention in the treatment group was 82.50%, significantly higher than the control group's rate of 47.50% (P < 0.05). After 21 days of treatment, the forced expiratory volume in one second (FEV1)/predicted value in the intervention group was 64.92 ± 8.53, while it was 52.69 ± 7.08 in the other group. The FEV1/forced vital capacity in the intervention group was 59.73 ± 7.18, compared to 50.36 ± 6.54 in the control group. The intervention group had a clinical pulmonary infection score of 2.62 ± 1.13, while the control group had a score of 4.17 ± 2.08. The intervention group showed a significant improvement in lung function, with statistically significant differences compared to the other group (P < 0.05). Moreover, the intervention group had significantly lower levels of interleukin-4 and interleukin-10 compared to the control group, with statistical significance (P < 0.05). The average National Institute of Health Stroke Scale score of the intervention group was significantly lower than that of the control group in the second and third weeks after treatment (P < 0.05). Conclusion: Early comprehensive pulmonary rehabilitation can significantly enhance the pulmonary function and respiratory status of SAP patients and facilitate the early recovery of brain function. Abstract:Chronic obstructive pulmonary disease (COPD) is an important comorbidity of lung cancer. COPD has been found to be an independent risk factor for lung cancer and is estimated to affect approximately 55% of lung cancer patients worldwide. [ 1 ] The prognosis of lung cancer is heavily affected by this comorbidity. Higher rates of postoperative pulmonary complications and poorer recovery from surgery in lung cancer patients are associated with chronic pulmonary obstructive disease. [ 2 ] Surgical resection remains the best treatment option for lung cancer patients in the early stage. [ 3 ] Surgical operation for lung cancer comorbid with chronic pulmonary obstructive disease can be challenging, as chronic pulmonary obstructive disease may increase postoperative morbidities and decrease the survival rate. [ 4 ] Even in patients with early-stage chronic pulmonary obstructive disease, the prevalence of postoperative pulmonary complications is higher than that in patients with normal spirometry. [ 5 ]

We aimed to explore the effect of a hybrid structured pulmonary rehabilitation education program (SPREP) on patients with lung cancer at high risk of PPCs. Methods: A quasi-experimental trial with a pre-post test design was conducted. The control group (n = 53) adopted routine perioperative pulmonary rehabilitation, while the intervention group (n = 53) received SPREP. Respiratory function, 6-min walk distance, Borg dyspnea scale, quality of life, anxiety-depression scores at admission, discharge, 2 weeks and 3 months post-discharge, and incidence of PPCs were compared between the two groups. Results: There were no significant differences on the 6-min walk distance and Borg Dyspnoea Scale at discharge between the two groups (P > 0.05), whereas the intervention group showed improved performance at the remaining time points (P < 0.05). In addition, the intervention group had improved exercise capacity, pulmonary function and quality of life, reduced levels of anxiety and depression at discharge, 2 weeks post-discharge and 3 months post-discharge (P < 0.05). In addition, incidence of PPCs was significantly reduced in the intervention group, especially postoperative pneumonia. Conclusions: The SPREP could show significant benefits in enhancing exercise capacity, lung function, and quality of life, while diminishing the occurrence of PPCs and mitigating the levels of anxiety and depression, future large RCT need to further explore the efficacy. Abstract:BACKGROUND: Anesthetic practices such as early tracheal extubation facilitate postoperative recovery. Early extubation after liver transplantation has been adopted by some centers in the recent two decades. No universal clinical guidelines are used and questions remain. This review aimed to address the current status of early extubation after liver transplantation.DATA SOURCES: A literature search of MEDLINE and IS! Web of Knowledge databases was performed using terms such as liver transplantation, early extubation, immediate tracheal extubation, fast tracking or fast track anesthesia and postoperative tracheal extubation. Additional papers were identified by a manual search of the references in the key articles.RESULTS: Review of the available literature provided an insight into the definition, evolution, advantages and risks of early extubation, and anesthetic techniques that prompt early extubation in liver transplant patients. Early extubation has proved to be feasible and safe in these patients, but the outcomes are still uncertain.CONCLUSIONS: Early extubation after liver transplantation is feasible, safe and cost-effective in the majority of patients and has been increasingly accepted as an option for conventional postoperative ventilation. Comprehensive and individualized evaluation of the patient's condition before extubation by an experienced anesthesiologist is the cornerstone of success. Understanding of its effect on the outcome remains incomplete. In the future, additional trials are required to establish universal early extubation guidelines and to determine its benefits for patients and practitioners. Physical Rehabilitation Before and After Lung Transplantation for COVID-19 ARDS: A Case Report

Abstract:Purpose: To describe the functional trajectory and physical rehabilitation of an individual who underwent lung transplantation for COVID-19 acute respiratory distress syndrome (ARDS). Client description: A previously healthy 60-year-old man admitted to critical care pre-transplantation and followed six months post-transplant. Intervention: Physical rehabilitation in the critical care, acute ward and in-patient rehabilitation settings. Measures and outcome: Despite a successful surgery, a long and complex acute care admission contributed to a slow and variable functional recovery. Significant functional limitations and physical frailty were present in the early post-transplant period. Implications: Little is known of the effects of COVID-19 superimposed upon lung transplantation on muscle function, exercise capacity, and physical activity. Future research should include case series to further understand the functional deficits and trajectory of recovery in this emerging clinical population. Standard core outcome measures should be identified for this population to enable synthesis of findings and inform short- and long-term rehabilitation strategies.

Impact and effect of preoperative short term preoperative pulmonary-related training on gastric cancer patients: a randomized controlled single center trial

Abstract:Purpose: The purpose of this study is to evaluate the impact of pre- and postoperative pulmonary-related training, including respiratory and aerobic training, on postoperative pulmonary complications (PPCs) after radical resection of gastric cancer (GC). Methods: We conducted a randomized controlled trial of 121 participants who received a systematic, comprehensive, high-intensity pulmonary exercise regimen for 5 days preoperatively and at least 5 days postoperatively. The control group received standard preoperative care. We analyzed the occurrence of PPCs in both groups as the primary outcome RESULTS: 43 out of 121 participants (35.5%) were diagnosed with PPCs, and the incidence of PPCs was markedly lower in the training group than in the control group (26.2%, 16/61 vs. 45.0%, 27/60, P= 0.031). Moreover, the duration of the postoperative hospital stay was shorter in the training group (8.69 ± 1.92 days) compared to the control group (9.57 ± 2.16 days, P=0.020) as well as reduced hospitalization costs, amounting to €9605.1 ± 2556.26 in the training group and €10594.6 ± 2560.7 in the control group (P=0.035). Conclusion: Our study established that a perioperative pulmonary-related training notably diminishes the incidence of PPCs, curtails the duration of hospitalization, and mitigates hospitalization expenses for patients undergoing gastric cancer surgery.
