Abstract:Although young children may participate in education and intervention programs that take place in classrooms or groups, there is little information about how toddlers with special needs, and specifically toddlers with autism, are engaging with their peers. This study takes place in a public center-based early intervention program for toddlers with autism. Classrooms of toddlers were randomly assigned to an individual social communication intervention or the same intervention adapted to include a peer. Children in both groups made gains in social communication and play skills. Children who had the peer intervention were more engaged with peers when an adult was present, but not when the children were unsupported. This article adds information about early skills that may be important for children to master so that they have more success when trying to interact with their peers. These skills include understanding language (referred to as "receptive language" at 12 months or more) and play skills including building and stacking (referred to as "combination play"-for example, building with blocks or completing a puzzle) and extending familiar actions to themselves, others, and figures (referred to as "presymbolic play"-for example, putting a bottle to the doll or to themselves). Understanding which skills to target can help practitioners focus their instruction to build children's skills toward connecting with peers through play.

Abstract:Over the past 15 years, empirical studies have described how the psychological aspects of autism differ from other developmental disorders. Studies of general development describe a pattern of specific relative strengths and weaknesses involving cognitive, emotional, linguistic, and motoric capacities. Because much of the syndrome-specific pattern of autism is present at the preschool period, this has implications for the development of curriculum and educational strategies early on. Early intervention programs that specifically target autism include a variety of educational strategies that address the unique learning profile of autism. As community preschool programs offer enhanced services for this group of young children, they can draw on a body of work generated both from the scientific work in autism and the effective educational practices developed by model preschool programs.

Abstract:Autism, Ahead of Print. Caregiver coaching is an evidence-based practice for autistic children that is poorly implemented in community-based early intervention. Previous studies have identified factors that influence implementation of caregiver coaching in early intervention; however, multi-informant (e.g., caregiver, provider, leadership) qualitative research is lacking. As such, the purpose of this study was to simultaneously examine the perspectives of caregivers and early intervention providers and agency leaders for triangulation of perceived barriers and facilitators to caregiver coaching in community-based early intervention. Interviews with 36 providers, 6 agency leaders, and 20 caregivers of autistic children were conducted. We used qualitative thematic analysis to identify barriers and facilitators, which we then mapped onto the Consolidated Framework for Implementation Research post hoc. Several themes emerged, including at the outer setting level (i.e., COVID-19 pandemic), inner setting level (i.e., caregiver views toward telehealth), individual characteristic level (i.e., caregiver attitudes toward coaching, caregivers concerns preventing progress, caregiver expectations regarding services, caregiver stress), and implementation process level (i.e., caregiver and provider collaboration, caregiver and provider relationship, caregivers coordinating services, caregiver involvement, provider flexibility). We offer suggestions for implementation strategies that will increase adoption of caregiver coaching in early intervention for autistic children and their families.Lay Caregiver coaching is an evidence-based practice for young autistic children, but it is not widely used in community-based early intervention services. Previous research has explored why caregiver coaching is not widespread in early intervention, but only from the perspective of early intervention providers. Caregivers, providers, and administrators are all involved in the decision of whether to use caregiver coaching in early intervention. Therefore, it is important to include all perspectives in research regarding this practice. In this study we interviewed 20 caregivers of autistic children, 36 early intervention providers, and 6 administrators from early intervention agencies and asked questions about their perspectives regarding the use of caregiver coaching in early intervention. We did this to figure out what factors help and hinder the use of caregiver coaching in this setting and to see how caregivers, providers, and administrators agreed or disagreed on these factors. All participants agreed that caregivers' attitudes and expectations can influence whether caregiver coaching is used. In addition, all participants agreed that when caregivers and providers collaborate and have a strong working relationship, it can facilitate the use of caregiver coaching in early intervention. Other factors, such as caregiver stress and provider flexibility were also discussed. Based on these findings, we suggest strategies that can be used to possibly increase the use of caregiver coaching in early intervention for autistic children and their families.

Abstract:Journal of Positive Behavior Interventions, Ahead of Print. Children with developmental delays are more likely to experience difficulties in social-emotional skills and challenging behavior, which can lead to poor long-term outcomes if left untreated. The purpose of this literature review was to synthesize the literature related to home-based interventions to address social–emotional skills and challenging behavior. A systematic review of the literature resulted in the identification of 26 single-case design studies and 31 group design studies that evaluated a home-based intervention to improve social–emotional skills or challenging behavior for young children with a disability or delay. The most common interventions implemented were differential reinforcement, functional communication training, and antecedent-based interventions. Data based on the quality of experiment, characteristics of participants, and characteristics of interventions were analyzed. Fewer than half of the studies (41%) met design standards with or without reservations. However, the majority of studies that met design standards demonstrated strong or moderate evidence for the efficacy of the intervention (67%). Overall, the literature indicates caregivers can accurately implement established evidence-based practices with their young children to improve social–emotional skills and challenging behavior. Implications for practice and future research directions are discussed.

The effectiveness of early parental coaching in the autism spectrum disorder

Abstract:Objective: Analysis of the effectiveness of early Parental Coaching in the Autism Spectrum Disorder. Method: Randomized, controlled and blinded clinical trial to analyze parent-child interaction videos. Results: The sample consisted of 18 children being followed up at the Autism Outpatient Clinic of a Neuropediatric Center in southern Brazil diagnosed with Autism Spectrum Disorder, between 29 and 42 months of age, randomly allocated to two groups: the Study Group (SG; n=9), which received Parental Coaching performed by a professional certified by the Early Start Denver Model; and the Control Group (CG; n=9), which was in a routine follow-up, without treatment and training of parents by a trained professional. The parents of the SG were willing to attend weekly meetings and to apply the instructional techniques at home with their children. It took 12 weeks and an average of 2h per meeting. Conclusions: The learning rate for comprehensive development skills in the Early Start Denver Model checklist, such as receptive communication, expressive communication, social capacity, imitation, cognition, games, fine motor skills, gross motor skills, behavior, and personal independence was significantly higher in the SG, as well as the strategies and the quality of interaction between parents and children. Thus, Parental Coaching presents as a possibility of early intervention in children with Autism Spectrum Disorder.

Abstract:The purpose of this study was to increase the social interaction skills of four preschool children with autism. Four teachers in integrated preschool classrooms participated. Experimental sessions occurred during daily 10-minute activities in which all children in the class were free to select from six to eight different activities. Prior to beginning the study, all four teachers were introduced to a variety of naturalistic teaching tactics designed to stimulate children's play and interaction with others. A multiple baseline design was employed to examine three different conditions. In baseline, teachers used these tactics with no assistance from research staff. In a second phase, teachers received daily feedback and technical assistance. Assistance was provided on only one occasion and then withdrawn during a maintenance phase. Results indicated that all four children increased their social interactions during the technical assistance phase. Each boy exhibited his skills in a range of different play activities, and two continued to display high levels of interaction during a maintenance phase.

Abstract:Journal of Early Intervention, Ahead of Print. Although early intervention is known to be effective in promoting positive child and family outcomes, not all families equally benefit from these services due to issues of access. Disparities are known to occur in the publicly funded early intervention system, which are exacerbated among marginalized families with diverse backgrounds, including cultural or linguistic differences. To examine determinants of culturally responsive early intervention, we conducted a qualitative study with 15 early intervention providers who work with marginalized families and their young child. We used a set of interview questions based on the Exploration, Preparation, Implementation, Sustainment (EPIS) framework to better understand the barriers, facilitators, and suggestions from providers in the field. Through thematic coding, we identified facilitators, barriers, and suggestions at the provider, organization, and systemic levels to provide culturally responsive early intervention services for marginalized families. Implications for policy and practice are discussed.

Over the past decade, a newer class of interventions has emerged specifically designed for young children with or at high likelihood of autism, which are called Naturalistic Developmental Behavioral Interventions (NDBIs). NDBIs are particularly well-suited as a discipline agnostic, primary intervention for young children with autism and other social communication challenges; however, community providers from multiple disciplinary backgrounds who serve these children typically do not develop competencies in areas crucial for effectively implementing NDBI. The goal of this commentary is to: (a) describe the interdisciplinary competencies needed to deliver NDBIs effectively and (b) provide recommendations for moving the field of early intervention for autism and social communication delays forward. Resources for building these competencies for providers at the pre- and in-service level are also provided.

Abstract:Children with autism spectrum disorder (ASD) are often delayed in acquiring social initiation skills utilized during common early childhood activities. A multiple probe design was utilized to assess the efficacy of a shared book reading intervention to increase the independent commenting of three young children with ASD. The intervention resulted in participant’s acquisition of social commenting in the form of independent pointing and verbal commenting to gain social attention from an adult. The results suggest that young children with ASD can independently initiate social interactions during shared book reading when these behaviors are systematically taught and reinforced. Implications for practice and directions for future research aimed toward generalization and improving the implementation of shared book reading interventions are discussed.

Abstract:Focus on Autism and Other Developmental Disabilities, Ahead of Print. The establishment of reciprocal friendships is a challenge for many individuals with autism spectrum disorder (ASD). Social interaction difficulties emerge early, persist throughout development, and have widespread functional impact. As such, interventions focused on social functioning are needed in early intervention programs. This two-part study evaluated the outcomes of a parent-assisted social skills training (SST) program for young children with ASD. The intervention is a downward extension of the Program for the Education and Enrichment of Relational Skills (PEERS(R)), which is one of the few evidence-based SST programs for adolescents and young adults with ASD. Modifications included developmentally appropriate social skills, greater parent involvement, and behavioral strategies embedded in play-based activities. Results reveal improvements in parent ratings of social responsiveness, social cognition, and social motivation, as well as decreased restricted/repetitive behaviors and interests and problem behaviors. Results are promising and encourage further evaluation of this SST curriculum.

Abstract:Focus on Autism and Other Developmental Disabilities, Ahead of Print. The publicly funded early intervention program for preschoolers with autism spectrum disorder (ASD) in Nova Scotia, Canada, uses the pivotal response treatment (PRT) model. Parents are coached in use of PRT strategies during their children's first week of treatment. We explored whether parents of older preschoolers with limited verbal ability learned to implement PRT strategies with fidelity after brief coaching and whether PRT strategy use was associated with gains in children's communication skills. We coded precoaching and postcoaching parent–child play episodes of 39 children (4.4–5.5 years) with limited verbal skills for parents' PRT fidelity and children's correct responding to language opportunities and frequency of initiations. Parents' use of PRT strategies significantly improved, as did children's responsiveness to parent-provided language opportunities. This study provides evidence for the utility of brief parent coaching in PRT for older preschoolers with ASD whose verbal skills are limited.

Abstract:The purpose of the study was to evaluate the effects of cooperative physical activities on social interactions of children with autism spectrum disorder (ASD) in China. Cooperative physical activities include procedures such as peer selection, peer practice, group task completion, and an interdependent group contingency. The intervention took place during inclusive physical education (PE) classes. Generalization of interactions with peers was evaluated during free play. Three preschool boys (aged 4–5) with ASD participated in the study. Using a multiple baseline design across three participants, results indicated that the procedure effectively increased the frequency of appropriate peer interactions for all three children in the PE and free play settings. Although the frequency of inappropriate interactions increased after the intervention in both settings, the proportion of inappropriate interactions relative to appropriate interactions decreased for two children in the PE setting and for all three children in the free play setting.

Abstract:Early intervention (EI) based on behavioral science is one of the most promising treatment options for children with autism. Ever since Ivar Lovaas study from 1987, researchers are aware of the benefits of intensive early intervention in improving the outcomes of children with autism. Children who receive EI often show remarkable improvements in their social, cognitive, and adaptive skills. These interventions typically involve structured teaching methods, intensive behavioral therapies, and a variety of support services tailored to the individual needs of each child. By addressing the core symptoms of autism early in a child's development, these programs aim to foster better long-term outcomes. In this paper, we presented an overview of the main topics in the field of EI for children with autism published in major scientific journals. For this purpose, we performed a search of the Web of Science citation base and identified 91 articles published in the period 2019-2023 that had the words “early intervention” and “autism” OR “ASD” in their titles. The selected articles dealt with several interesting topics ranging from improving social communication and joint attention to topics dealing with quality of life and parent mediated EI programs. We concluded the paper with a discussion on the future directions of EI research. Despite the significant progress made, there is still much to learn about optimizing these early behavior interventions. Future research should aim to personalize intervention strategies to meet the unique needs of each child with autism and their families. Additionally, more longitudinal studies are needed to understand the long-term impacts of EI on various life outcomes.

Abstract:Purpose: We investigated effects of an adaptive telehealth coaching model on caregiver implementation of enhanced milieu teaching (EMT) with newly diagnosed toddlers with autism. Method: Three caregiver–child dyads participated in a multiple-baseline-across-behaviors, single-case design. Caregivers were taught EMT via telehealth using the teach–model–coach–review approach. EMT strategies were taught sequentially in four components corresponding to design tiers. Caregivers reported their needs for support and adaptation via weekly surveys. Primary outcomes were measures of caregiver's implementation, including (a) a fidelity checklist for wholistic use of EMT and (b) a percentage of correct use of a subset of key EMT strategies (e.g., matched turns, target talk, expansions, play actions, milieu episodes). Generalization and maintenance of caregiver strategy use in uncoached home activities were measured. The number of different words used by children was measured as a secondary, descriptive outcome. Social validity data were collected through ratings and interviews at the end of the study. Results: There was a functional relation between the intervention and caregiver's implementation of EMT for all dyads. Caregiver's use of EMT strategies often generalized and maintained post-intervention. Child response to intervention was variable. Social validity data indicated that the model was beneficial to caregivers and children. Conclusions: An adaptive telehealth coaching model is effective for teaching caregivers of toddlers with autism to implement EMT and potentially helps to bridge the gap between diagnosis and comprehensive intervention. Further exploration of the relation between caregiver fidelity and dosage of active ingredients and child spoken language outcomes is needed. Abstract:Research in developmental and educational psychology points to the importance of sensitive interactions between adult and children in order to promote children's social, emotional, and academic learning. Despite this evidence, there are many conceptual and programmatic challenges that have prevented this emphasis in early childhood special education (ECSE). This article provides a definition of and theoretical framework for teacher sensitivity and responsiveness; describes the evidence that supports this focus; and delineates the research, interventions, and challenges that bear upon the enhancement of teacher—child sensitivity and responsivity in ECSE. This article also discusses and critiques a pilot program for facilitating the development of sensitive teacher—child interactions.

Abstract:Objective Children with autism spectrum disorder(ASD) who participated in the " Play-based Communication and Behavior Intervention(PCBI) " were studied in order to evaluate the impact of caregiver factors on the efficacy of the PCBI very early intervention model systematically.Methods Seventy-six children with ASD aged 8-30 months were recruited in this study at Brain Hospital Affiliated to Nanjing Medical University form December 2017 to June 2018,and they were trained for PCBI very early intervention.The Autism Treatment Evaluation Checklist (ATEC) and the Gesell Developmental Schedules were used to evaluate the efficacy of the PCBI very early intervention.Parenting Stress Index Short Form,General Self-Efficacy Scale and the self-developed Caregiver Training Course Evaluation Scale and the Homework Completion Level Scale were used to assess the level of parental stress,self-efficacy,curriculum satisfaction and acceptance.A pair of sample t-test was used to analyze the ASD symptoms,developmental level and caregiver factors of ASD children before and after intervention.Multivariate regression analysis was used to analyze the caregiver factors and the effects of PCBI very early intervention.Results (1) At the end of 12 weeks of PCBI very early intervention,the score of the ATEC (17.36 ± 15.98) was lower than that before intervention (78.43± 22.84),and the difference was statistically significant (P ＜ 0.05);the adaptive developmental quotient (5.90 ± 15.45),the fine action developmental quotient (5.13 ± 19.89),the language developmental quotient (12.43 ± 20.60) and the personal social developmental quotient (4.79 ± 16.02) scores of the Gesell Developmental Schedules were higher than those before intervention (73.08 ± 20.54,77.33 ± 23.63,52.24 ± 19.12,71.79 ± 20.81),and the differences were statistically significant (all P ＜ 0.05).(2) At the end of PCBI very early intervention,there was a significant decrease in the scores of Parenting Stress Index Short Form,and the differences were statistically significant (4.84 ± 14.69 vs.94.05 ± 29.67,P ＜ 0.05).(3) Acceptance of the intervention model,and mastery of the skills in the caregiver factors had a positive predictive effect on the effect of PCBI very early intervention (t =3.068,10.468,all P ＜ 0.05).Conclusions PCBI very early intervention can effectively reduce the parenting pressure of the caregivers,and in the PCBI very early intervention,the better the mastery of skills and the completion of homework,the better the efficacy of intervention.
Examining US Public Early Intervention for Toddlers With Autism: Characterizing Services and Readiness for Evidence-Based Practice Implementation

Abstract:As the rates of Autism Spectrum Disorder (ASD) increase and early screening efforts intensify, more toddlers with high likelihood of ASD are entering the United States' (US') publicly funded early intervention system. Early intervention service delivery for toddlers with ASD varies greatly based on state resources and regulations. Research recommends beginning ASD-specific evidence-based practices (EBP), especially caregiver-implemented intervention, as early as possible to facilitate the development of social-communication skills and general learning. Translating EBP into practice has been challenging, especially in low-resourced areas. The main goal of this study was to obtain a more comprehensive understanding of public early intervention system structure, service delivery practices, and factors influencing EBP use for children with ASD in the US. Participants ( N = 133) included 8 early intervention state coordinators in 7 states, 29 agency administrators in those states, 57 early intervention providers from those agencies, and 39 caregivers of children with ASD receiving services from those providers. Online surveys gathered stakeholder and caregiver perspectives on early intervention services as well as organizational factors related to EBP implementation climate and culture. Stakeholders identified key intervention needs for young children with ASD. In general, both agency administrators and direct providers reported feeling somewhat effective or very effective in addressing most needs of children with ASD. They reported the most difficulty addressing eating, sleeping, family stress, and stereotyped behaviors. Data indicate that children from families with higher income received significantly higher service intensity. While administrators and providers reported high rates of high-quality caregiver coaching (>60%), caregivers reported low rates (23%). Direct providers with more favorable attitudes toward EBP had greater EBP use. In turn, provider attitudes toward EBP were significantly associated with implementation leadership and culture at their agency. Results suggest that publicly funded early intervention programs in the US require additional resources and training for providers and leaders to support improved implementation climate and attitudes toward ASD EBPs. Results also suggest that more state system support is needed to increase use of ASD-specific EBP use, including high-quality caregiver coaching, to better serve toddlers with ASD. Recommendations for implementation strategies are addressed.
