Abstract:We propose that doping be legalized under medical supervision. First, we discuss two motivations for allowing medically supervised doping. We reject the 'compromised choice/harm minimization' motivation as unlikely to win the support of athletes. We agree that it could lead to an arms race. Instead, we favor full acceptance of doping under medical supervision and answer Reid's spirit of sport objection to medical manipulation. After presenting a set of guiding principles, we use them to answer the arms race objection and rebut one of the most prominent objections in the literature about the safety of medically supervised doping, the game-theoretic objection.

Abstract:The use of performance enhancing substances and methods (known as "doping") in sport is an intractable issue, with current anti-doping strategies predominantly focused on the personal responsibility and strict liability of individual athletes. This is despite an emerging understanding that athletes exist as part of a broader complex sports system that includes governance, policymakers, media, sponsors, clubs, team members, and athlete support staff, to name a few. As such, there is a need to examine the broader systemic factors that influence doping in sport. The aim of this systematic review was to identify and synthesise the factors contributing to doping and doping behaviours, attitudes, and beliefs and the extent to which this knowledge extends beyond the athlete to consider broader sports systems. The review followed PRISMA guidelines with risk of bias and study quality assessed by the Mixed Methods Appraisal Tool, and identified contributory factors synthesised and mapped onto a systems thinking-based framework. Overall, the included studies were determined to be of high quality. Support personnel, the coach, and the coach-athlete relationship represent key influences on the athletes' decisions to dope. From the evidence presented, doping is an emergent property of sport systems and represents a complex systemic problem that will require whole-of-system interventions. The implications for this and the focus of future research are discussed.

Abstract:The illicit utilization of performance-enhancing substances, commonly referred to as doping, not only infringes upon the principles of fair competition within athletic pursuits but also poses significant health hazards to athletes. Doping control analysis has emerged as a conventional approach to ensuring equity and integrity in sports. Over the past few decades, extensive advancements have been made in doping control analysis methods, catering to the escalating need for qualitative and quantitative analysis of numerous banned substances exhibiting diverse chemical and biological characteristics. Progress in science, technology, and instrumentation has facilitated the proliferation of varied techniques for detecting doping. In this comprehensive review, we present a succinct overview of recent research developments within the last ten years pertaining to these doping detection methodologies. We undertake a comparative analysis, evaluating the merits and limitations of each technique, and offer insights into the prospective future advancements in doping detection methods. It is noteworthy that the continual design and synthesis of novel synthetic doping agents have compelled researchers to constantly refine and innovate doping detection methods in order to address the ever-expanding range of covertly employed doping agents. Overall, we remain in a passive position for doping detection and are always on the road to doping control.

Efficient Ways to Combat Doping in a Sports Education Context!? A Systematic Review on Doping Prevention Measures Focusing on Young Age Groups

Abstract:Youth is characterized by testing and crossing natural boundaries, sometimes with the help of performance-enhancing substances. In this context, doping prevention measures play a crucial role to protect individuals both within and outside the context of elite sport. Based on the PRISMA guidelines, a systematic literature search was conducted in the databases ProQuest (ERIC), Scopus, PSYNDEX/PsychInfo, PubMed, and Web of Science Core Collection to provide an overview of the impact of doping prevention measures, with particular attention to the underlying understanding of learning. As a result of the screening process, 30 of the initial 5,591 articles met the previously defined and recorded eligibility criteria. The analysis led to heterogeneous results regarding content, implementation, target group, or outcome variables considered relevant. Two-thirds of the studies related to the competitive sports context. Nevertheless, there has been a growing interest in studying doping prevention and its effects on non-elite athlete target groups in recent years. In terms of effectiveness, many measures did not achieve long-term changes or did not collect any follow-up data. This contrasts with understanding learning as sustained change and reduces the intended long-term protection of prevention measures, especially for adolescent target groups. Even young age groups from 10 years upwards benefited from doping prevention measures, and almost all doping prevention measures enabled their participants to increase their physical and health literacy. No conclusion can be drawn as to whether doping prevention measures based on constructivist ideas are superior to cognitivist approaches or a combination of both. Nevertheless, programs that actively engage their participants appear superior to lecture-based knowledge transfer. Most of the prevention measures offered a benefit-orientation so that participants can achieve added value, besides trying to initiate health-promoting change through rejection. Because of the lack of sustained changes, a further modification in doping prevention seems necessary. The review results support the value of primary prevention. Doping prevention measures should enable tailored learning and development options in the sense of more meaningful differentiation to individual needs. The implementation in a school context or an online setting is promising and sees doping as a problem for society. The review highlights the importance of accompanying evaluation measures to identify efficient prevention components that promote health and protect young people.
How to justify a ban on doping?

Abstract:BACKGROUND: This article deals with arguments that challenge the possibility of an ethical justification for a doping ban.HYPOTHESIS/PURPOSE: It shows that a <i>justification</i> for the doping ban is only possible if its <i>implementation</i> can be safeguarded.STUDY DESIGN: Systematic review.METHODS: Based on the proposition of the game theory, this article examines the scope of the arguments of naturalness, health, equal opportunity and fairness used in scientific literature.RESULTS/CONCLUSIONS: Ceteris paribus, athletes will always prefer a situation that presents no health risk to a situation in which they face a threat to their health. They will therefore consent to a doping ban on the condition that it is ensured that all parties are bound to this rule, so that anyone complying with the rules will not be afraid of losing the competition as a result. For even if we condoned self-harm, it could still be argued plausibly that the individual should not suffer more disadvantages than absolutely necessary for the sake of gaining an advantage over others. Of course, it is possible to plead for a restricted approval of doping measures with acceptable risk. But even taking minor risks would not seem sensible under the condition that all participants without exception adhere to the same conditions when there is the option to renounce the (avoidable) risks. So as far as the use of performance-enhancing substances or methods is concerned, we can maintain that even if minor health risks are to be expected, a ban on doping can be justified from an ethical point of view.

New strategies for doping control

Abstract:Doping in sports is commonplace. The prevention of harm to the athlete, the guarantee of fair play, and a level playing field for all competitors are the basis of the current anti-doping strategies. As healthcare systems are forced to allocate increasing resources to prevent and treat the prevailing pathologies worldwide, funding for anti-doping campaigns will necessarily be restricted. Ideally, additional resources should be devoted to increasing the number of athletes tested, the panels of tests used, and the frequency of out-of-competition controls. Since doping prevention cannot be considered a priority for most healthcare systems, such an approach is unaffordable and an alternative framework should be devised, focused primarily on harm reduction rather than fair play. The identification of abnormal deviations from reference values, regardless of pathological or artificial causes, would allow the athlete to be followed and tested using conventional laboratory tests, which are affordable to governments and healthcare systems and available to clinical laboratories. Although this strategy would not detect cheating, it would safeguard athletes' health.
Barriers and enablers in doping, anti-doping, and clean sport: A qualitative meta-synthesis informed by the theoretical domains framework and COM-B model

Abstract:To protect the integrity of sport, and the health of athletes, global anti-doping programmes seek to prevent doping behaviours, and elicit anti-doping and clean sport behaviours, through education, deterrence, detection, enforcement, and rules. To guide programme development, this meta-synthesis of qualitative research applied a behavioural science framework to identify barriers and enablers to doping, anti-doping, and clean sport. A systematic search of electronic databases, followed by critical appraisal, resulted in 73 included articles. Fifty-two articles reported the athlete perspective, thirteen included athletes, athlete support personnel (ASP), and other experts, and eight focused on ASP only. Rigorous methods of thematic synthesis were drawn upon to construct analytical themes in line with the theoretical domains framework (TDF) and the capability, opportunity, and motivation model of behaviour (COM-B). A wide range of barriers and enablers were identified which influenced capability, opportunity, and motivation to participate in a clean sport environment. The weight of evidence pointed to limitations in the current anti-doping education system in providing athletes and ASP with the knowledge and skills to protect against doping, as well as the significant influence of social and cultural norms in shaping doping and clean sport behaviours through a shared social identity, and risky contexts leading to moments of vulnerability to doping. We identified a need for anti-doping programmes to move beyond the current focus on athlete capability, and address the opportunity and motivation components of clean sport behaviours through a targeted and tailored focus on education , training , persuasion , modelling and environmental restructuring interventions.

Abstract:The use of performance‐enhancing substances in sports undermines values and risks athletes' health. This study analyzes doping literature, identifying influential documents and research domains. While prevalent doping classes are well‐studied, emerging trends like contaminated supplements receive less attention. Technological advancements drive sophisticated detection techniques. The study emphasizes the need for awareness campaigns among athletes. The use of performance‐enhancing substances not only undermines the core values of sports but also poses significant health risks to athletes. In a fast‐evolving doping environment, where sport professionals are constantly seeking novel and illegal means to bypass doping tests, and new substances are regularly detected on the drug market, it is crucial to inform authorities with updated evidence emerging from scientific research. The current study aims to (i) outline the structure of knowledge in the literature on performance enhancers in sports (i.e., most active countries, main sources, most productive authors, and most frequently used keywords); (ii) identify the most impactful documents in the field; and (iii) uncover the main domains of research in the literature. To do so, we conducted a comprehensive scientometric analysis of the literature on doping, sourcing our data from Scopus. Our research involved a document co‐citation analysis of 193,076 references, leading to the identification of the 51 most influential documents and seven key thematic areas within the doping literature. Our results indicate that the scientific community has extensively studied the most prevalent doping classes, such as anabolic agents and peptide hormones, and little is still known about the use of contaminated supplements or other types of enhancers identified as emergent trends. Concurrently, technological advancements contributed to the development of more sophisticated doping detection techniques, using blood or urine samples. More recently, the focus has shifted towards the athlete biological passport, with research efforts aimed at identifying biomarkers indicative of doping. The dynamic nature of doping methods underlines the necessity for more robust educational campaigns, aiming at raising awareness among sports professionals and their entourage about the dangers of doping and the intricacies of its control mechanisms.

Annual banned-substance review: analytical approaches in human sports drug testing.

Abstract:Monitoring the misuse of drugs and the abuse of substances and methods potentially or evidently improving athletic performance by analytical chemistry strategies is one of the main pillars of modern anti-doping efforts. Owing to the continuously growing knowledge in medicine, pharmacology, and (bio)chemistry, new chemical entities are frequently established and developed, various of which present a temptation for sportsmen and women due to assumed/attributed beneficial effects of such substances and preparations on, for example, endurance, strength, and regeneration. By means of new technologies, expanded existing test protocols, new insights into metabolism, distribution, and elimination of compounds prohibited by the World Anti-Doping Agency (WADA), analytical assays have been further improved in agreement with the content of the 2013 Prohibited List. In this annual banned-substance review, literature concerning human sports drug testing that was published between October 2012 and September 2013 is summarized and reviewed with particular emphasis on analytical approaches and their contribution to enhanced doping controls.

Prevalence of doping use in elite sports: a review of numbers and methods

Abstract:The prevalence of doping in elite sports is relevant for all those involved in sports, particularly for evaluating anti-doping policy measures. Remarkably, few scientific articles have addressed this subject so far, and the last review dates back to 1997. As a consequence, the true prevalence of doping in elite sports is unknown. Even though it is virtually impossible to uncover the exact prevalence of a prohibited activity such as doping, various methods are available to uncover parts of this particular problem, which enables the circumvention (to a certain degree) of the issues of truthfulness, definition problems and the limits of pharmacological evidence. This review outlines the various methods that exist and presents the scarce data available in this area. It is concluded that a combination of questionnaires using the Randomised Response Technique and models of biological parameters is able to provide the statistical possibilities to reveal accurate estimates of this often undisclosed practice. Data gathered in this way yield an estimation of 14-39% of current adult elite athletes who intentionally used doping. These period prevalences have been found in specific sub-groups of elite athletes, and the available data suggest that the prevalence of doping is considerably different between sub-groups with varying types of sport, levels and nationalities. The above-mentioned figure of 14-39% is likely to be a more accurate reflection of the prevalence of intentional doping in elite sports than that provided by doping control test results (estimate of doping: 1-2% annually) or questionnaire-based research (estimations between 1 and 70% depending on sport, level and exact definitions of intent and doping). In the future, analytical science may play a more important role in this topic if it may become feasible to detect very low concentrations of prohibited substances in sewage systems downstream of major sporting events. However, it is clear that current doping control test results show a distinct underestimation of true doping prevalence. It does not seem feasible to distil better estimates of the prevalence of doping based on performance indicators or ego documents because of the various existing effects that influence athletic performance. Such information can only be used as extra information to augment the accuracy of prevalence rates that have been found by using other techniques. True doping prevalence studies have been scarce in elite sports so far. With the correct application of the available scientific methods, preferably using harmonised definitions of the terms 'doping' and 'elite sports', more information on this topic may be gathered in a relatively short time. This would assist anti-doping professionals in the future in order to evaluate the effects of possible anti-doping measures, and better anti-doping policies would serve athletes who compete without doping. The existing anti-doping measures seriously impact the lives of elite athletes and their immediate entourage, which imposes a moral burden to evaluate these measures in the best possible way.
Coping with Doping: Performance-Enhancing Drugs in the Athletic Culture

Abstract:Performance-enhancing drug (PED) use is a known and increasingly recognized maladaptive behavior among athletes of all ages and levels of competition. The underlying reasons for use are diverse and may include attempting to gain a selective advantage, to recover from injury, or to optimize cosmetic appearance. The veil of secrecy and stigma regarding PED use challenges the careful study of use behavior patterns, but there is an emerging body of evidence linking PED use to the presence and development of mental health disorders. In order to counteract PED use in the interest of athlete safety and fair play, most governing sports bodies have imposed escalating penalties, including performance bans, for multiple-time PED offenders. It is important for the treating provider to become familiar with PED use and use patterns in order to have an educated discussion with the at-risk athlete.
A steroidomic approach for biomarkers discovery in doping control

Abstract:Anti-doping authorities have high expectations of the athlete steroidal passport (ASP) for anabolic-androgenic steroids misuse detection. However, it is still limited to the monitoring of known well-established compounds and might greatly benefit from the discovery of new relevant biomarkers candidates. In this context, steroidomics opens the way to the untargeted simultaneous evaluation of a high number of compounds. Analytical platforms associating the performance of ultra-high pressure liquid chromatography (UHPLC) and the high mass-resolving power of quadrupole time-of-flight (QTOF) mass spectrometers are particularly adapted for such purpose. An untargeted steroidomic approach was proposed to analyse urine samples from a clinical trial for the discovery of relevant biomarkers of testosterone undecanoate oral intake. Automatic peak detection was performed and a filter of reference steroid metabolites mass-to-charge ratio (m/z) values was applied to the raw data to ensure the selection of a subset of steroid-related features. Chemometric tools were applied for the filtering and the analysis of UHPLC-QTOF-MS(E) data. Time kinetics could be assessed with N-way projections to latent structures discriminant analysis (N-PLS-DA) and a detection window was confirmed. Orthogonal projections to latent structures discriminant analysis (O-PLS-DA) classification models were evaluated in a second step to assess the predictive power of both known metabolites and unknown compounds. A shared and unique structure plot (SUS-plot) analysis was performed to select the most promising unknown candidates and receiver operating characteristic (ROC) curves were computed to assess specificity criteria applied in routine doping control. This approach underlined the pertinence to monitor both glucuronide and sulphate steroid conjugates and include them in the athletes passport, while promising biomarkers were also highlighted.
Ethics of a relaxed antidoping rule accompanied by harm-reduction measures

Abstract:Harm-reduction approaches are used to reduce the burden of risky human behaviour without necessarily aiming to stop the behaviour. We discuss what an introduction of harm reduction for doping in sports would mean in parallel with a relaxation of the antidoping rule. We analyse what is ethically at stake in the following five levels: (1) What would it mean for the athlete (the self)? (2) How would it impact other athletes (the other)? (3) How would it affect the phenomenon of sport as a game and its fair play basis (the play)? (4) What would be the consequences for the spectator and the role of sports in society (the display)? and (5) What would it mean for what some consider as essential to being human (humanity)? For each level, we present arguments for and against doping and then discuss what a harm-reduction approach, within a dynamic regime of a partially relaxed antidoping rule, could imply. We find that a harm-reduction approach is morally defensible and potentially provides a viable escape out of the impasse resulting from the impossibility of attaining the eradication of doping. The following question remains to be answered: Would a more relaxed position, when combined with harm-reduction measures, indeed have less negative consequences for society than today's all-out antidoping efforts that aim for abstinence? We provide an outline of an alternative policy, allowing a cautious step-wise change to answer this question and then discuss the ethical aspects of such a policy change.

Abstract:The systemic effect of glucocorticoids (GCs) following injectable routes of administration presents a potential risk to both improving performance and causing harm to health in athletes. This review evaluates the current GC antidoping regulations defined by the World Anti-Doping Agency and presents a novel approach for defining permitted and prohibited use of glucocorticoids in sport based on the pharmacological potential for performance enhancement (PE) and risk of adverse effects on health. Known performance-enhancing doses of glucocorticoids are expressed in terms of cortisol-equivalent doses and thereby the dose associated with a high potential for PE for any GC and route of administration can be derived. Consequently, revised and substance-specific laboratory reporting values are presented to better distinguish between prohibited and permitted use in sport. In addition, washout periods are presented to enable clinicians to prescribe glucocorticoids safely and to avoid the risk of athletes testing positive for a doping test.

Abstract:Background: Over the last years, various revelations demonstrated that the doping problem is far from being solved. These included the American cyclist Lance Armstrong's disclosure and subsequent conviction for doping abuse over a period of many years. Furthermore, these revelations underlined the importance of strong and independent national antidoping agencies (NADA). During the current revision process of the World Anti-Doping Programme (WADP), Antidoping Switzerland is campaigning for national anti-doping agencies to have the same rights, the same authority and the same degree of responsibility as international sports associations. Further, the newly revised Federal Act on the Promotion of Sport and Exercise (Sport Promotion Act), which entered into force on 1 October 2012, establishes the framework for cooperation with customs officers when suspected doping substances are seized. By the end of 2012 Antidoping Switzerland received about 50 reports from the customs authorities, and in 24 cases an administrative ruling for the seizure and destruction of these doping substances was issued. In addition, there was also a greater cooperation between customs and the Swiss Agency for Therapeutic Products Swissmedic. Two athletes have already been sanctioned under private law for importing doping substances. Controls: Antidoping Switzerland carried out 2'551 controls in 2012. Of these, 1'752 were urine tests, of which 1'089 were conducted out of competition and 663 in competition. The majority of the 799 blood controls were conducted out of competition. In 2012 Antidoping Switzerland lodged about 20 applications on violations of the anti-doping provisions with Swiss Olympic's Disciplinary Chamber for Doping Cases (DC). In numbers, four athletes were banned for two years for using anabolic steroids. A trainer was also suspended for two years for having given an athlete a stimulant before a competition. 2012 was the first year in which two athletes were convicted of import of doping substances (EPO and an anabolic drug respectively) on information provided by customs officers. Both athletes were banned from competition for two years. Legal basis: Of importance to all the physicians looking after athletes is to know the actual legal basis: The international law basis of the Swiss fight against doping are the Council of Europe Convention against Doping of 16 November 1989 and the International Convention against Doping in Sport of the General Conference of the United Nations Educational, Scientific and Cultural Organization (UNESCO) of 19 October 2005. The basis in public law of the Swiss fight against doping is the Federal Act on the Promotion of Gymnastics and Sport of 17 March 1972. Article 11e, paragraph 1 states that "national sport organisations, the relevant umbrella bodies and bodies responsible for sporting events that are supported in the framework of this act […] are required to ensure that the necessary doping controls are carried out in their area of responsibility". Article 11 f, paragraph 1 of the Federal Act also states that "whoever produces, introduces, transfers, sells, prescribes or provides substances for doping purposes or who uses methods for doping purposes for third persons will be liable to a prison sentence or to a fine of up to 100'000 francs". The civil law basis of the Swiss fight against doping consists of the norms established by various actors in the sporting world. These actors are in most cases clubs, associations and foundations in accordance with the stipulations of the Swiss civil code. The Swiss Olympic Association's revised Doping Statute was approved by the Sports Parliament on 15 November 2008. The statute implements the code of the World Anti-Doping Agency (WADA) in Switzerland. In the introduction it defines the anti-doping agencies in our country: Antidoping Switzerland and the Disciplinary Chamber for Doping Offences of the Swiss Olympic Association. The revised Statute entered into force on 1 January 2009. Finally, it is well known that the use of anabolic steroids and other doping substances is a phenomenon not restricted to professional sport only. Also in popular sport, in particular in fitness, the use of anabolic steroids to increase physical performance is very common. To summarize, doping is a widespread phenomenon not only in athletes. In these situations physicians involved have to know not only the medical but also the legal side of prescribing, applying substances to persons actively involved in sport activities. Abstract:This article is part of a larger investigation looking into recent changes in the demographics of fitness doping and the possible consequences of such changes. Contesting the historical alliance between masculinity and fitness doping, the article focuses on women’s narratives and experiences of fitness doping in a male-dominated open online community called Flashback. The article builds upon a qualitative and netnographic approach to the research. Employing the lens of the potential emergence of a woman-based ethnopharmacological culture, this article investigates the ways in which women talk about and rationalise their use of performance and image enhancing drugs (PEIDs), their potency and potential gendered side-effects. The results show that although fitness doping can be largely understood in terms of hegemonic patterns, women have gained ground in the context of online fitness doping, heralding a changing doping demography and a movement towards a ‘sis science’ ethnopharmacology. Although critiqued by men, the context enables women to freely discuss harm reduction, risks and the potential potencies of various drugs, and to share knowledge that is relevant to female biology and discuss their own experiences, an activity that also makes visible the negotiation of new gender positions.

Abstract:Analytical chemistry represents a central aspect of doping controls. Routine sports drug testing approaches are primarily designed to address the question whether a prohibited substance is present in a doping control sample and whether prohibited methods (for example, blood transfusion or sample manipulation) have been conducted by an athlete. As some athletes have availed themselves of the substantial breadth of research and development in the pharmaceutical arena, proactive and preventive measures are required such as the early implementation of new drug candidates and corresponding metabolites into routine doping control assays, even though these drug candidates are to date not approved for human use. Beyond this, analytical data are also cornerstones of investigations into atypical or adverse analytical findings, where the overall picture provides ample reason for follow-up studies. Such studies have been of most diverse nature, and tailored approaches have been required to probe hypotheses and scenarios reported by the involved parties concerning the plausibility and consistency of statements and (analytical) facts. In order to outline the variety of challenges that doping control laboratories are facing besides providing optimal detection capabilities and analytical comprehensiveness, selected case vignettes involving the follow-up of unconventional adverse analytical findings, urine sample manipulation, drug/food contamination issues, and unexpected biotransformation reactions are thematized.

Abstract:Sports and exercise training research is constantly evolving to maintain, improve, or regain psychophysical, social, and emotional performance. Exercise training research requires a balance between the benefits and the potential risks. There is an inherent risk of scientific misconduct and adverse events in most sports; however, there is a need to minimize it. We aim to provide a comprehensive overview of the clinical and ethical challenges in sports and exercise research. We also enlist solutions to improve method design in clinical trials and provide checklists to minimize the chances of scientific misconduct. At the outset, historical milestones of exercise science literature are summarized. It is followed by details about the currently available regulations that help to reduce the risk of violating good scientific practices. We also outline the unique characteristics of sports-related research with a narrative of the major differences between sports and drug-based trials. An emphasis is then placed on the importance of well-designed studies to improve the interpretability of results and generalizability of the findings. This review finally suggests that sports researchers should comply with the available guidelines to improve the planning and conduct of future research thereby reducing the risk of harm to research participants. The authors suggest creating an oath to prevent malpractice, thereby improving the knowledge standards in sports research. This will also aid in deriving more meaningful implications for future research based on high-quality, ethically sound evidence.

Abstract:The effects of some widely abused doping substances such as anabolic androgenic steroids (AAS) on performance are well-documented, particularly in the short-term, and the use of these substances is banned by various sporting authorities, with athletes sanctioned from competing for up to 4 years. However, controversy exists on whether residual physiological effects of some doping practices could persist even years after discontinuation, granting unfair advantages to athletes long after sanctions have been served. Particularly, in support of the so-called muscle memory theory, growing evidence in both animals and humans suggest that AAS administration could exert long-term effects at the muscle level, notably a higher number of myonuclei. This effect could enhance retraining/muscle remodelling capacity long after AAS cessation, thus supposing an advantage for doped athletes even +4 years after doping practices have been discontinued. If confirmed, the persistence of physiological improvements resulting from past doping practices raises serious ethical concerns in the sports field and opens the door to lifelong sanctions.

Abstract:The analytical approaches taken by laboratories to implement robust and efficient regulation of horseracing medication and doping control are complex and constantly evolving. Each laboratory's approach will be dictated by differences in regulatory, economic and scientific drivers specific to their local environment. However, in general, laboratories will all be undertaking developments and improvements to their screening strategies in order to meet new and emerging threats as well as provide improved service to their customers. In this paper, the published analytical advances in horseracing medication and doping control since the 22nd International Conference of Racing Analysts and Veterinarians will be reviewed. Due to the unprecedented impact of COVID-19 on the worldwide economy, the normal 2-year period of this review was extended to over 5 years. As such, there was considerable ground to cover, resulting in an increase in the number of relevant publications included from 107 to 307. Major trends in publications will be summarised and possible future directions highlighted. This will cover developments in the detection of 'small' and 'large' molecule drugs, sample preparation procedures and the use of alternative matrices, instrumental advances/applications, drug metabolism and pharmacokinetics, the detection and prevalence of 'endogenous' compounds and biomarker and OMICs approaches. Particular emphasis will be given to research into the potential threat of gene doping, which is a significant area of new and continued research for many laboratories. Furthermore, developments in analytical instrumentation relevant to equine medication and doping control will be discussed.
