Abstract:End stage renal disease (ESRD) is defined as treatment of renal dysfunction dependent on maintenance renal replacement therapy. It is estimated that the number of ESRD patients is about one million in China and is still fast growing. Patients with chronic kidney diseases are susceptible to cardiovascular disease because impaired glomerular filtration rate contributes independently to atherosclerosis in addition to the other usual risk factors. For patients of ESRD, cardiovascular disease is the leading cause of death, accounting for about 50% of mortalities.1 With the increasing prevalence of ESRD, more and more patients dependent on renal replacement therapy will need intervention, either surgical or catheter-based, for coronary artery disease. With regard to surgical revascularization, such patients undoubtedly have higher procedure-related mortality and morbidity. However, numerous worldwide centres have confirmed that good outcome can still be achieved through meticulous perioperative management. Interestingly, similar reports from China are very scant, with each containing few cases mixed with nondialysis patients. We published successful cardiovascular operations for dialysis patients from China.2 Here, we present the results of isolated coronary artery bypass grafting (CABG) surgery in patients dependent on maintenance dialysis at our institution. METHODS Between December 2004 and April 2011, five consecutive patients (three men and two women) dependent on maintenance dialysis underwent isolated coronary bypass grafting at Peking Union Medical College Hospital. The median age of these patients was 59 years (51 to 65). The causes of renal failure were diabetic nephropathy in four patients and chronic glomerolonephritis in one patient. Median preoperative duration of dialysis was 15 months (9 to 28 months). There were three patients on hemodialysis and two patients on peritoneal dialysis. At the time of operation, left ventricular function was normal in all patients and New York Heart Association status was II in three and III in two patients. The timing of operation was urgent in one patient due to intractable angina pectoris following failed percutaneous coronary intervention and elective in the other four patients. Preoperative patient characteristics are summarized in Table 1.Table 1: Preoperative data of patientsAll patients underwent coronary artery bypass surgery through standard midline sternotomy and with use of cardiopulmonary bypass (CPB). Proactive use of intra-aortic balloon pump was initiated before induction of anaesthesia for the case of urgent operation. Left internal mammary artery and saphenous vein (harvested via endoscopy) were used for all patients. After systemic heparinization, conventional CPB was instituted via two-stage cannulation in the right atrium and arterial perfusion through ascending aorta. Cardiac arrest was achieved through initial antegrade administration of high potassium cold blood cardioplegia, followed by intermittent retrograde perfusion. Patients were cooled to systemic temperature of 30°C to 32°C. After the grafts were anastomosed with target coronary arteries and ascending aorta, the aortic crossclamp was removed and CPB weaned after rewarming. Protamine was administrated according to the heparin dosage. All patients received dialysis as usual 12 to 24 hours prior to surgery and intraoperative hyperfiltration during CPB. Bedside continuous venovenous haemofiltration was routinely started on the first postoperative day, or later without presence of hyperkalaemia or volume overload. RESULTS There were three distal anastomoses in three patients and four in two patients (Tables 2 and 3). New onset of atrial fibrillation was observed in three of five patients and was converted to sinus rhythm with amidarone. One patient underwent re-exploration due to mediastinal bleeding 76 hours after the operation. The location of bleeding was found to be a stump of thin communicating artery in the bed of left internal mammary artery. For the patient requiring urgent operation, intraaortic balloon pump was discontinued 20 hours after the operation.Table 2: Operative variablesTable 3: Details of postoperative and follow-up characteristicsPatients were ventilated for 36 to 92 hours after the operation. Previous fashion of dialysis was resumed on the 5th to 7th postoperative day. Total volume of transfusion varied from 900 ml to 1600 ml. Moderate ileus was observed in one patient and was successfully managed by conservative treatment. One patient suffered transient cognitive disorder. No wound complication was seen. All patients were discharged (9th to 16th postoperative day) and were systemically followed up for 20 to 96 months. One of two patients previously on peritoneal dialysis switched to hemodialysis 14 months later due to peritonitis. At the end of the follow-up, all patients were in NYHA functional class II, free of angina pectoris and presented no major adverse cardiovascular events. No late death occurred. Computed tomographic angiogram was used to evaluate graft patency at 12 months and 36 months after the operation. All mammary grafts were patent, but one and three venous grafts were found to be occluded at 12 and 36 months respectively. DISCUSSION An optimal approach for coronary revascularization in the setting of ESRD remains undetermined. ESRD patients usually exhibit diffuse and severe atherosclerotic lesions of both coronary and peripheral arteries. Both surgical and interventional coronary revascularization treatment confront technical challenges. Previous studies suggest surgical revascularization conveys survival advantages over percutaneous coronary intervention. However, these patients usually have comorbidities predisposing them to increased operative morbidity and mortality. Documented in-hospital mortality of conventional CABG ranges between 3.4% to 25% in ESRD patients, which is roughly threefold to fivefold higher than nonESRD patients.3 Moreover, mortality was reported to have negative correlation with case volume. Although we have small number of cases, our ESRD patients undergoing isolated CABG exhibited good early and late outcomes. We believe that the key factors to success are adequate diameter of target coronary arteries and meticulous perioperative management. In addition, our perseverance in use of internal mammary artery also has substantial contribution, because previous studies have proven it to be protective against early and late death when compared with venous grafts in patients of impaired renal function.4 Off-pump CABG has become another appealing option for ESRD patients. Many studies reported off-pump CABG is associated with lower rates of mortality and morbidity in dialysis dependent patients. Without using CPB, off-pump CABG results in smaller volume shifts, less blood loss and less production of inflammatory mediators. Consequently, patients benefit from less cognitive disorders, shorter ICU stay and less transfusion. On the other hand, coronary arteries tend to be more diffusely calcified compared to the patients without ESRD, making off-pump surgical revascularization much more technically demanding. Therefore the possibility of incomplete revascularization might increase in ERSD patients if off-pump technique is used. This may subsequently undermine the survival advantage of surgical revasculerization. In a study published by Dewey et al,5 it was confirmed that patients had fewer bypass grafts and worse long-term survival after off-pump CABG than did patients undergoing on-pump CABG. We believe that success of off-pump coronary surgery in dialysis dependent patients can only be achieved by experienced surgeons and stringent patient selection. None of our patients was assigned to off-pump technique because their lesions were deemed as unsuitable for complete revascularization on a beating heart. In conclusion, surgical revascularization was successfully performed under cardiopulmonary bypass in five ESRD patients, early and late outcomes are good. Our experience suggests that CABG is a safe and effective procedure for ERSD patients.
