Abstract:Introduction Eosinophilia poses a unique diagnostic challenge due to association with a wide range of clinical presentations. We describe a case of smoldering eosinophilia with acute presentation for new-onset heart failure secondary to eosinophilic granulomatosis with polyangiitis (EGPA). Case Description A 61-year-old female established care for adult-onset allergic rhinoconjunctivitis, sinusitis with anosmia, and severe persistent asthma with limited response to maximal therapy. Initial evaluation was notable for CT sinus with diffuse opacification, chest xray with bilateral opacities and normal cardiomediastinal silhouette, absolute eosinophil count 1.5K/μL, elevated ESR, negative ANA, normal immunoglobulins, negative fungal and parasitic serologies, and normal troponin. Approximately 6 months later, she presented to the emergency department for generalized malaise and nausea/vomiting, where evaluation was largely unchanged excepting for WBC 18.8K/μL (absolute eosinophil count 8.8K/μL), and subsequent bone marrow biopsy noted eosinophilia without concerning cytogenetics. Shortly after biopsy, she was admitted to the hospital for edema, creatinemia, and BNP 1578pg/mL consistent with new-onset heart failure (EF 25%), as well as pericardial effusion and left ventricular thrombus strongly suggestive of eosinophilic myocarditis secondary to EGPA despite two non-diagnostic nasal biopsies. She received high-dose steroid and cyclophosphamide with normalization of eosinophil count, and was discharged with LifeVest for persistently reduced cardiac function. Discussion This case illustrates the importance of including EGPA among the differential for adult-onset atopic conditions with eosinophilia. It also highlights the role for early, aggressive disease-modifying agents in cases complicated by severe end-organ involvement, including those without pathologic confirmation.
