Endoscopic treatment for hepaticojejunostomy anastomotic stricture: Advantages and disadvantages of balloon dilatation with stenting

Abstract:Hepaticojejunostomy anastomotic stricture (HJAS) after hepatic or pancreaticobiliary surgery is an important issue that needs to be solved and eliminated through the cooperation of surgeons, radiologists, and endoscopists. To prevent HJAS, surgeons should improve the suturing method and devices. At the same time, radiologists and endoscopists should develop effective and secure procedures to achieve a high clinical success rate and decrease recurrence. According to a recent meta-analysis on risk factors for anastomotic stricture after hepaticojejunostomy for bile duct injury,1 concomitant vascular injury, post-repair bile leak, and repair by a non-specialist surgeon increased the rate of HJAS. Thus, surgeons need to undergo sufficient training with an expert as a mentor before surgery. In addition, new preventive methods such as drug injection at the anastomotic site, intraoperative dilation, or biodegradable stent placement should be developed and established, although some devices have been tested to date.
