Abstract:Blood cyst is a rare primary cardiac tumor usually affecting the cardiac valves. Associated with severe occlusion-related complications, it is generally removed surgically. With a brief review of the literature, we present a case of pedunculated tumor in the left ventricular outflow tract identified by echocardiography. CASE REPORT A 29-year-old man experienced chest discomfort for nine months. The patient was admitted to our hospital on March 28, 2005 after a left ventricular mass was suspected on echocardiography. Physical examination on admission revealed nothing abnormal in blood pressure, heart sounds, respiratory sounds or neurological findings; His height was 172 cm and weight was 63 kg. General hematological and biochemical tests were normal. Electrocardiography showed sinus rhythm, a heart rate of 55 beats per minute, left ventricular high-voltage, and no evident ST-T changes. Chest X-ray demonstrated no abnormalities. A 3-cm circular, slightly strong echo mass in the left ventricular outflow tract was identified by surface echocardiography (Fig. 1). The mass was pedunculated, movable in the left ventricular outflow tract at heart beating, and attached to the submitral valve apparatus. The mass was diagnosed as a left ventricular tumor, which was subjected to surgery.Fig. 1.: Surface echocardiography showing a 3-cm circular slightly strong echo tumor in the left ventricular outflow tract. Attached to the submitral valve apparatus, a pedunculated, movable tumor was observed in the left ventricular outflow tract at the time of heart beating.On March 31, after a median sternotomy, extracorporeal circulation was initiated between the ascending aorta and the right atrium. Under cardiac arrest, an incision was made in the left atrium to examine the mitral valve and the left ventricle. A cystic mass was found to be attached tightly to the anterior leaf of the mitral valve and submitral valve apparatus. After resection of the mass, we tried to repair the mitral valve because of partial defect of the anterior leaf of the mitral valve and submitral valve apparatus, but failed. The patient was subjected to mitral replacement with a 27-mm mitral bi-leaflet prosthesis (Carbomedics Inc., Texas, USA). The operation was successful. The mass about 3 cm in diameter was characterized by the white cystic wall with hematic content. Extracorporeal circulation lasted for 77 minutes and aortic clamping 62 minutes. Postoperative pathological analysis confirmed that the mass was a blood cyst, with the wall consisting of thick valvular tissues with collagen and elastin fibers (Fig. 2).Fig. 2.: Histologically, the cyst is thin-walled and normally lined by cobblestone-shaped endothelial cells delimiting from the endocardium with a basement membrane (HE, original magnification ×20).The postoperative course was uneventful. The patient was discharged on the 11th postoperative day. Warfarin was given as anticoagulation therapy after the operation. The patient was quite well at 16 months of follow-up. DISCUSSION Blood cyst in the heart is rare and was first reported by Elasser in 1844.1 It should be differentiated from primary cystic tumor such as hemangioma or myxoma. The cyst is an incidental autopsy of cardiac valves in approximately 50% of infants under 2 months of age. It is rarely seen after 2 years of age but the reason is not clear.2 The collapse of the cyst caused by adhersion and fibrosis of opposite surfaces of the endothelium may lead to its disappearance. Blood cyst is congenital and located in the endocardium, particularly along the lines of closure of heart valves. The cyst is lined by flattened endothelium and filled with non-organized blood. Histologically, it is thin-walled and normally lined by cobblestone-shaped endothelial cells and does not contain any tumorous cells.3 Small deposits of hemosiderin, fibrosis, and lymphocytes are adjacent to the blood cyst, indicating a tissue reaction that is possibly related to cystic resolution. The cyst is usually small and asymptomatic.4–6 In our case the mass was fully compatible with blood cyst histologically, being lined by normal endothelial cells delimiting from the endocardium with a basement membrane, and it contained blood. The cyst was attached tightly to the anterior leaf of the mitral valve and grew into the left ventricular outflow tract. The left ventricular outflow tract or the opening of the mitral valve was obstructed by the giant mass, which might cause sudden death.
