Abstract:The National Institute on Aging (NIA) conducts and funds research aimed at extending the healthy, active years of life. 1 This mission distinguishes the institute from others that focus primarily on the study of specific organs and diseases. Over its 50 years, the NIA has supported clinical and behavioral research across a wide range of issues. In this Viewpoint we highlight multicomplexity and dementia, 2 areas where the NIA has made integral and distinctive contributions. We also discuss the NIA's role in applying principles from behavioral economics to research designed to improve the health of older persons.

Abstract:The National Institute on Aging (NIA), part of the National Institutes of Health (NIH), was founded in 1974 to support and conduct research on aging and the health and well‐being of older adults. Fifty years ago, the concept of studying aging generated much skepticism. Early NIA‐funded research findings helped establish the great value of aging research and provided the foundation for significant science advances that have improved our understanding of the aging process, diseases and conditions associated with aging, and the effects of health inequities, as well as the need to promote healthy aging lifestyles. Today, we celebrate the many important contributions to aging research made possible by NIA, as well as opportunities to continue to make meaningful progress. NIA emphasizes that the broad aging research community must continue to increase and expand our collective efforts to recruit and train a diverse next generation of aging researchers.

Abstract:The founding mission of the National Institute on Aging (NIA) was visionary and broad—from understanding the basic biology of aging to enhancing care, quality, and outcomes for older adults. Their investment and vision have led to innovative research in biological sciences exemplified by identification of about a dozen cellular and molecular mechanisms that drive both aging and several chronic diseases. 1 These advances have given rise to the geroscience hypothesis and the field of geroscience. The geroscience hypothesis states that interventions modifying biological mechanisms of aging (gerotherapeutics) might prevent, delay, or alleviate several age-related diseases simultaneously, lengthening the lifespan and, more importantly, increasing the health span, defined as the length of life without illness or disability. 1 ,2 Thus, geroscience focuses on translational applications that hold the potential to alter the course of aging and chronic diseases (Box).

Innovation in Aging

Abstract:Abstract Modern improvements in cancer detection and treatment coupled with the implementation of population-based cancer prevention and control strategies have contributed to a sustained decline in overall cancer mortality rates. Although this trend is promising, challenges at the nexus of cancer and aging are, in turn, becoming more prominent. Older adults (age 65 years and older) are the largest growing segment of the U.S. population, and aging into older adulthood is disproportionally associated with the incidence of common cancers. Many survivors of childhood cancer will live for decades after cancer treatment and mature into older age. Strategic investments in aging research will contribute to population health by preserving or improving healthspan and ensuring equitable access to – and benefit from – advances in cancer prevention, control, and population science. This presentation will describe ongoing cancer and aging efforts at the National Cancer Institute, including programmatic priorities and current funding opportunities.
The arts, health, and aging in america: 2005-2015

Abstract:In advance of the White House Conference on Aging (WHCoA) in 1981, 1995, and 2005, the arts and aging communities held mini-conferences to ensure that arts, culture, and livability were part of larger public policy discussions. This article takes a historical look at recommendations from the 2005 WHCoA Mini-Conference on Creativity and Aging in America, including arts in health care, lifelong learning, and livability through universal design. Overarching recommendations in 2005 requested investments in research, including cost-benefit analyses; identification of best practices and model programs; program dissemination to broaden the availability of arts programs. The "Arts" is a broad term encompassing all forms of arts including music, theater, dance, visual arts, literature, multimedia and design, folk, and traditional arts to engage the participation of all older Americans; promotion of innovative public and private partnerships to support arts program development, including workforce development (e.g., artists, social workers, and health care providers); and public awareness of the importance of arts participation to healthy aging. Through the leadership of the National Endowment for the Arts and U.S. Department of Health and Human Services, thinking about the arts and aging has broadened to include greater emphasis on a whole-person approach to the health and well-being of older adults. This approach engages older adults in arts participation not only as audience members, but as vital members of their community through creative expression focusing on life stories for intergenerational as well as interprofessional collaboration. This article reviews progress made to date and identifies critical gaps in services for future consideration at a 2015 Mini-Conference on Creativity and Aging related to the WCHoA area of emphasis on healthy aging.

Abstract:As the population ages, risks for cognitive decline threaten independence and quality of life for older adults and present challenges to the health care system. Nurses are in a unique position to advise older adults about cognitive health promotion and to develop interventions that optimize cognition in older adults. A literature review was conducted to provide nurses in mental health and geriatric care with an overview of research related to the promotion of successful cognitive aging for older adults. Research evaluating cognitively stimulating lifestyles and the effects on cognitive function in older adults of interventions targeting cognitive training, physical activity, social engagement, and nutrition were reviewed. Overall research findings support positive effects of cognitive and physical activity, social engagement, and therapeutic nutrition in optimizing cognitive aging. However, the strength of the evidence is limited by research designs. Applications for health promotion to optimize cognitive aging and future directions for research are discussed.

Identification and assessment of effective services and interventions: the nursing home perspective

Abstract:In the past decade, researchers across the world have published a great number of studies on the care of nursing home residents with dementia. Investigations have concentrated on such important subjects as special care units, staff training, functional performance, and problem behaviors. However, most of the studies have lacked the rigor needed to establish the treatments that they tested as standards of care, according to criteria from the American Academy of Neurology. Without a solid scientific foundation, researchers in the USA have found it difficult to offer authoritative advice to policy makers and advocacy groups. This article will discuss some theoretical and methodological issues of research that require attention to fortify the scientific foundation. Topics include theoretical and definitional inconsistencies, sampling issues, limitations related to data collectors, the need for greater specificity of interventions, and outcome measures. The article will continue with an overview of areas that need further refinement and development. It will focus on combining basic and clinical sciences, combining behavioral and pharmacological research, combining clinical and health services research, reconceptualizing meaningful activity, palliative care in advanced dementia, certified nursing assistant training issues, and family training issues. The article will conclude with recommendations to establish collaborative research enterprises among existing organizations in the USA.

Abstract:In concert with 6 decennial White House Conferences on Aging, psychologists have considered how developments in psychological science can contribute to the well-being of older Americans. We suggest 5 illustrative areas of psychological research: Advances in neuroscience elucidate ways to promote healthy cognitive aging; associated developments in neuropsychological assessment can help in protecting older Americans with cognitive losses from financial exploitation, abuse, and neglect. Psychological research on decision making and behavioral economics has much to offer to planning for retirement security and reducing vulnerability to financial abuse. Psychological research on self-management and behavior change can contribute importantly to enhancing good health behaviors among older adults; similarly the power of context on behavior can be harnessed in long-term care settings. Psychological research on attitudes and stereotypes gives insight into age bias that can be detrimental to healthy aging. Adaptive technologies and information technologies are beginning to transform assessment in research and clinical settings; technology also holds the promise of improving long-term support for older adults in both institutional and community-based settings. Finally, with 1 in 7 Americans now ages 65 and older, compared with 1 in 11 50 years ago, the psychology workforce-including health services providers and faculty to train those providers-is insufficient to meet the challenge of the aging population. The role of the Alzheimer's Association in the genesis of Alzheimer's Disease Neuroimaging Initiative

Abstract:Here we highlight the Alzheimer's Association's role since its inception, as a strategic collaborator with National Institutes of Health-National Institute on Aging in the development of the modern era of the Alzheimer's Movement and in making Alzheimer's disease (AD) a national priority in the United States by developing several initiatives to advance knowledge about the cause, diagnosis, and treatment of dementia. Among these collaborative undertakings, the Alzheimer's Disease Neuroimaging Initiative (ADNI) is an exemplary case, launched with groundwork by the Neuroimaging Working Group sponsored by the Association's Ronald and Nancy Reagan Research Institute on AD. The unique contribution of the Association to the development of ADNI includes participation as a member of ADNI's Private Partner Scientific Board and involvement in developing an AD biomarker standardization and validation subproject, which has led to a conceptual shift in the field to define AD based on its underlying biology. Furthermore, the creation of Worldwide ADNI (WW-ADNI) is highlighted, underscoring the global impact of these efforts. HIGHLIGHTS: The Alzheimer's Disease Neuroimaging Initiative (ADNI) is a keystone undertaking in the evolving landscape of Alzheimer's disease (AD) research, and is now in its fourth iteration. The Alzheimer's Association has partnered with ADNI since its inception. ADNI 4 and the Association continue to collaborate, ensuring representation within the study population.

Abstract:As the world becomes more globalized and socially connected, challenges associated with diversity and disparities are taking center stage in the field of aging. Aging is no longer viewed as a biological monolith but one that is underpinned and buttressed by constructs like equity, accountability, access, and inclusion. The need for research to examine diversity and disparities among healthy and clinical populations is reflected in the recent literature. Brewster et al. (2019) discussed the importance of adopting a systems-level approach (e.g., National Institute on Aging [NIA] Health Disparities Research Framework) to identify mechanisms and pathways that reinforce health disparities. Babulal et al. (2019) proposed recommendations for prioritizing and advancing recruitment of racial and ethnic minorities in Alzheimer's disease and related dementia (ADRD) research. Fredriksen-Goldsen and colleagues (2018) elaborated on core competencies required to care for older LGBT (lesbian, gay, bisexual, and transgender) older adults living with cognitive impairment and ADRD. These perspectives and position papers originating from national conferences and working groups call for aging studies to better inform diversity and disparities research. This Special Issue of Journal of Applied Gerontology includes 12 articles that employ a spectrum of quantitative, qualitative, and mixed methodologies to critically examine the intersectionality of nationality, race, ethnicity, and LGBTQ orientation (lesbian, gay, bisexual, transgender, and queer) surrounding diversity and disparities among older adults.

Perceptions of cognitive decline among American Indian and Alaska Native elders

Abstract:Introduction: American Indian and Alaska Native elders aged ≥ 65 years are experiencing increased life expectancy. Elders are critical to intergenerational knowledge, yet limited data exist on the health challenges faced by this group. Methods: This study engaged individuals attending the National Indian Council on Aging 2021 Annual Meeting in Reno, Nevada. A 19-question survey, designed to examine perceptions about cognitive decline and to identify comfort with potential risk and protective factors, was disseminated to 50 participants. Results: Participants indicated that they are concerned about cognitive decline, are willing to plan for their future care and cognitive testing, and articulated a desire for Tribally led long-term support services. Discussion: This study found similar results to studies on White individuals, which include a lack of knowledge, stigma around the aging process, and gaps in services available. More work is necessary to address the gap in literature and policy. Highlights: American Indian and Alaska Natives (AI/ANs) are underrepresented in literature on Alzheimer's disease and related dementia (ADRD).AI/ANs believe that they will experience cognitive decline as they age.AI/ANs indicate a willingness to plan for future care and participate in future research on ADRD.
Public health imperative of the 21st century: innovations in palliative care systems, services, and supports to improve health and well-being of older americans

Abstract:A primary aim of federal aging and health policy must be promoting innovations in palliative care systems, services, and supports that improve the experience of growing old in America. Older adults must contend today with increasing burden over the life course often as the result of life-limiting chronic pain and chronic illnesses as well as social and economic factors beyond their control. These burdens are frequently shared with unpaid family caregivers who provide significant uncompensated medical care and social support to their loved ones. Enjoyment of the highest attainable standard of physical and mental health, recognized as a fundamental human right under international law, remains a goal for all older adults and encompasses the right to palliative care. For many older Americans, especially vulnerable subgroups who face health and pain disparities, however, this goal remains elusive. A public health strategy for implementing palliative care policy interventions will help to build age-friendly environments, assure the availability and accessibility of palliative systems of care, essential medicines, and an adequate generalist-level workforce, and sustain diffusion of innovation across all levels of health and social provision. The 2015 White House Conference on Aging must make these realignments a policy priority in order to foster social and economic development for all older Americans.

Abstract:This perspective outlines the Artificial Intelligence and Technology Collaboratories (AITC) at Johns Hopkins University, University of Pennsylvania, and University of Massachusetts, highlighting their roles in developing AI‐based technologies for older adult care, particularly targeting Alzheimer's disease (AD). These National Institute on Aging (NIA) centers foster collaboration among clinicians, gerontologists, ethicists, business professionals, and engineers to create AI solutions. Key activities include identifying technology needs, stakeholder engagement, training, mentoring, data integration, and navigating ethical challenges. The objective is to apply these innovations effectively in real‐world scenarios, including in rural settings. In addition, the AITC focuses on developing best practices for AI application in the care of older adults, facilitating pilot studies, and addressing ethical concerns related to technology development for older adults with cognitive impairment, with the ultimate aim of improving the lives of older adults and their caregivers. Highlights Addressing the complex needs of older adults with Alzheimer's disease (AD) requires a comprehensive approach, integrating medical and social support. Current gaps in training, techniques, tools, and expertise hinder uniform access across communities and health care settings. Artificial intelligence (AI) and digital technologies hold promise in transforming care for this demographic. Yet, transitioning these innovations from concept to marketable products presents significant challenges, often stalling promising advancements in the developmental phase. The Artificial Intelligence and Technology Collaboratories (AITC) program, funded by the National Institute on Aging (NIA), presents a viable model. These Collaboratories foster the development and implementation of AI methods and technologies through projects aimed at improving care for older Americans, particularly those with AD, and promote the sharing of best practices in AI and technology integration. Why Does This Matter? The National Institute on Aging (NIA) Artificial Intelligence and Technology Collaboratories (AITC) program's mission is to accelerate the adoption of artificial intelligence (AI) and new technologies for the betterment of older adults, especially those with dementia. By bridging scientific and technological expertise, fostering clinical and industry partnerships, and enhancing the sharing of best practices, this program can significantly improve the health and quality of life for older adults with Alzheimer's disease (AD).

Abstract:The first baby boomer born in 1946 turned 65 in 2011 and the last baby boomer will turn 65 in 2029. By then, the total U.S. population over 65 is projected to be 71.5 million (compared with 44.6 million in 2013). The current (2014) median cost of a nursing home is $226 a day ($82,490 per year), while assisted living is $3500 a month ($42,000 per year). While the elderly population continues to get larger and costs will continue to rise, nearly ninety percent (90%) of people want to grow old in their own home and community and remain out of the hospital, nursing home or other institutional setting. New technologies could potentially allow older adults and people with disabilities to remain in their homes longer, reduce health care costs and enhance the quality of life. In summary, there is a need for a new generation of research that addresses the complexity of supporting the quality of life and independence of a vast, diverse, and aging population. While there are common themes and needs in this research that we describe shortly, we must start by recognizing that there is more than one needed path and approach to meet these diverse needs. One path includes the tight integration of chronic disease management in the home with existing acute healthcare systems. Another path embraces comprehensive home health for improving nutrition and social connectedness while combating physical, cognitive and psychological ailments. Yet another path emphasizes wellness, consumer technologies and removing basic barriers to meaningful community participation. These paths will intersect in interesting ways for individuals, families, healthcare providers, and communities. However research is critically needed to illuminate these paths and to make measurable strides in our care and support for over 15% of our nation's citizens.

Abstract:Objectives: This analysis examines the implications of new Alzheimer disease drugs in the era of the Inflation Reduction Act (IRA). It focuses on balancing innovation in Alzheimer disease treatment with affordability and access, assessing the impact on Medicare's budget, patient cost, and health care system readiness. Study design: A comprehensive review was conducted, synthesizing information from recent FDA drug approvals, drug pricing models, Medicare coverage policies, and the updated regulations under the IRA. This analysis reflects on the broader clinical and economic consequences of introducing new Alzheimer disease treatments. Methods: The study employs a qualitative review of existing literature, policy documents, and economic data. It explores the implications of Alzheimer disease drugs on health care policy, analyzing the economic and clinical impacts within the current health care landscape in the US. Results: The study highlights the economic challenges posed by the high costs of new Alzheimer disease drugs, contrasting with their moderate clinical benefits and potential risks. It discusses the limitations of the IRA in regulating drug prices and the resulting implications for Medicare's budget. Additionally, it examines disparities in health care access and system preparedness for these new treatments. Conclusions: The study findings underscore the need for a comprehensive approach to ensure fair pricing and equitable access to Alzheimer disease treatments. It suggests the application of frameworks such as the ISPOR Value Flower, focusing on diversity, equity, and comprehensive economic evaluations, to navigate the evolving landscape of Alzheimer disease treatment in the context of the IRA.
Families and elder care in the twenty-first century

Abstract:Although most Americans know that the U.S. population is aging, they are far less informed about the reality of providing elders with personal care, health care, and social support. Families-particularly women-have always been critical in providing elder care, but the entry of so many women into the paid labor force has made elder care increasingly difficult. Ann Bookman and Delia Kimbrel show how changes in both work and family life are complicating families' efforts to care for elderly relatives. Because almost 60 percent of elder caregivers today are employed, many forms of caregiving must now be "outsourced" to nonfamily members. And because elders are widely diverse by race and socioeconomic status, their families attach differing cultural meanings to care and have widely different resources with which to accomplish their care goals. Although the poorest elders have access to some subsidized services, and the wealthiest can pay for services, many middle-class families cannot afford services that allow elders to age in their homes and avoid even more costly institutional care. Six key groups--health care providers, nongovernmental community-based service providers, employers, government, families, and elders themselves--are engaged in elder care, but their efforts are often fragmented and uncoordinated. All six groups must be able to work in concert and to receive the resources they need. Both employer and government policies must be improved. Although large businesses have taken up the elder care challenge, most small and mid-sized firms still do not offer flexible work arrangements. Social Security and Medicare have provided critical support to families caring for elders, yet both face significant financial shortfalls. The Older American Act and the National Family Caregiver Support Program have broadened access to elder services, but need updating to address the needs of today's employed caregivers and elders who want to "age in place." And just over half of the nation's workforce is eligible for the unpaid leave benefits provided by the Family and Medical Leave Act. The authors close by reflecting on the need for a coordinated, cross-sector movement to create an "aging-friendly" society in the United States-a society that values well-being across the life span and supports citizens from diverse cultures and income levels as they age.
The Role of Nutrients in Reducing the Risk for Noncommunicable Diseases during Aging

Abstract:An increasing aging population worldwide accounts for a growing share of noncommunicable diseases (NCDs) of the overall social and economic burden. Dietary and nutritional approaches are of paramount importance in the management of NCDs. As a result, nutrition programs are increasingly integrated into public health policies. At present, programs aimed at reducing the burden of NCDs have focused mostly on the excess of unhealthy nutrient intakes whereas the importance of optimizing adequate essential and semi-essential nutrient intakes and nutrient-rich diets has received less attention. Surveys indicate that nutrient intakes of the aging population are insufficient to optimally support healthy aging. Vitamin and mineral deficiencies in older adults are related to increased risk of NCDs including fatigue, cardiovascular disease, and cognitive and neuromuscular function impairments. Reviewed literature demonstrates that improving intake for certain nutrients may be important in reducing progress of NCDs such as musculoskeletal disorders, dementia, loss of vision, and cardiometabolic diseases during aging. Current knowledge concerning improving individual nutrient intakes to reduce progression of chronic disease is still emerging with varying effect sizes and levels of evidence. Most pronounced benefits of nutrients were found in participants who had low nutrient intake or status at baseline or who had increased genetic and metabolic needs for that nutrient. Authorities should implement ways to optimize essential nutrient intake as an integral part of their strategies to address NCDs.

Non-invasive brain stimulation of the aging brain: State of the art and future perspectives

Abstract:Favored by increased life expectancy and reduced birth rate, worldwide demography is rapidly shifting to older ages. The golden age of aging is not only an achievement but also a big challenge because of the load of the elderly on social and medical health care systems. Moreover, the impact of age-related decline of attention, memory, reasoning and executive functions on self-sufficiency emphasizes the need of interventions to maintain cognitive abilities at a useful degree in old age. Recently, neuroscientific research explored the chance to apply Non-Invasive Brain Stimulation (NiBS) techniques (as transcranial electrical and magnetic stimulation) to healthy aging population to preserve or enhance physiologically-declining cognitive functions. The present review will update and address the current state of the art on NiBS in healthy aging. Feasibility of NiBS techniques will be discussed in light of recent neuroimaging (either structural or functional) and neurophysiological models proposed to explain neural substrates of the physiologically aging brain. Further, the chance to design multidisciplinary interventions to maximize the efficacy of NiBS techniques will be introduced as a necessary future direction.

Abstract:Objective: The purpose of this article is to introduce a special issue dedicated to research at the intersection of aging and disability. Method: We provide some context for the importance of cross-disciplinary collaboration among aging and disability researchers and summarize the nine articles in this issue. Results: Articles in the special issue are centered around several overarching themes. These include meaningful social and community participation, goals and values in the context of disability, and the reach and effectiveness of programs and policies on rehabilitation and service utilization. Conclusion: As care models continue to merge aging and disability services, collaboration among traditional aging and disability research networks can lead to improved outcomes for adults aging with long-term disability.

Aging, experienced nurses: their value and needs

Abstract:The nursing workforce mirrors the trend in population; that is, it is aging. Subsequently, older nurses experiencing some aging effects themselves are caring for more elderly patients needing more assistance with illness and wellness problems. To meet the growing demand for care in this era of nursing shortage, predicted to last beyond 2020, these nurses are needed to remain in the workforce longer. Lack of nurses in the workplace compromises patient care and increases job stress. Therefore, retention incentives need to be implemented to assist aging, experienced nurses to delay retirement or prevent them from leaving the profession early, as well as encouraging younger and future nurses to work longer. This article focuses on aging nurses, describing their demographics and needs, explicating their value and listing the resources and benefits needed to prolong their vital services in the workforce. (For this article, aging, older and mature refer to experienced nurses in their 40s, 50s and 60s.).
