Abstract:Ethmoid sinus mucoceies are benign,expansile and cyst-like lesions, when sufficiently large, may causing compression of the optic nerve and nearby structures. We report an extremely rare case of ethmoid sinus mucocele causing orbital apex syndrome. A 59-year-old female presented with over one month history of left-side headache that worsened with left-side ophthalmodynia for six days, accompanied by left-side sudden ptosis and vision loss for half a day. Clinical findings were proved with that of a combined CN II, III, IV and VI paralysis. Computed tomographic scan demonstrated a dense homogeneous mass expanding the left ethmoid sinus and rarefaction of the lateral wall of the left ethmoid sinus with the contents compressing the optic nerve. She underwent a prompt endoscopic sinus surgery. Three days after the operation, the movement and vision of the left eye returned to normal, the left eye pain and headache had also resolved.

Abstract:We report a case of first-episode sphenoid mucocele successfully treated via transnasal endoscopic drainage and marsupialization of the mucocele. A 55 year-old female presented with persistent right-side facial numbness (in the areas of the first and second branches of the trigeminal nerve) and right-side ptosis. Computed tomography (CT) imaging and Magnetic resonance imaging (MRI) revealed opacification and expansion of the right-side sphenoid sinus lesion. The lesion was diagnosed as right-side sphenoid mucocele affecting the functions of the trigeminal (first and second branches), and oculomotor nerves. Transnasal endoscopic drainage and marsupialization of the mucocele result in rapid regression of these symptoms.

Abstract:Objective: To describe an infrequent instance of sphenoid sinus mucocele presenting as retrobulbar optic neuritis and highlight the value of imaging in the diagnosis and treatment plans. Methods: A woman aged 26 noted a sub-acute decrease in vision in the right eye, which mimicked optic neuritis. Magnetic resonance imaging (MRI) indicated a large mass in the sphenoid sinus, which was in favor of mucocele. Results: Endoscopic sphenoidotomy and marsupialization of the mucocele were carried out, and the diagnosis was confirmed by pathology. The patient was also administered a high dose of corticosteroid, upon which progression of the disease was halted. Unfortunately, no significant improvement in vision was achieved. Conclusions: This report emphasized the important role of imaging in differentiating between different causes of optic neuropathies. A high level of clinical skepticism along with appropriate imaging studies can help diagnose rare causes. With timely management, gratifying results may be achieved.

Abstract:We report a challenging and uncommon case involving a 53-year-old Japanese man with cerebrospinal fluid (CSF) leakage caused by a meningocele in the lateral recess of the sphenoid sinus. Our innovative treatment approach involved a combination of transpterygoid and endoscopic modified medial maxillectomy techniques, with special emphasis on the preservation of the sphenopalatine artery. This strategic preservation was pivotal to the successful use of the ipsilateral nasoseptal flap for reconstruction, which played a crucial role in the prevention of postoperative CSF leakage. Otolaryngologists and neurosurgeons collaborated to perform the bath-plugging technique; effective collaboration was instrumental to the success of the procedure. Abstract:Because fungi usually spread from the paranasal sinuses to the orbital apex in invasive fungal sinusitis (IFS), IFS often presents as an orbital apex syndrome (OAS) characterized by dysfunction of cranial nerves II, III, IV, V1, and VI. We report a case of sphenoid sinus mucormycosis that presented as isolated retrobulbar optic neuropathy. A 94-year-old woman presented with acute blindness in the right eye. Examination revealed the absence of light perception and pupillary reflex in the right eye. Head MRI showed a mass in the right sphenoid sinus, which was contiguous with the right optic nerve. She underwent endoscopic surgery, and a histopathological diagnosis of mucormycosis was established. Treatment with intravenous liposomal amphotericin B reduced the size of the mass. She has survived for more than 1 year without recurrence. Clinicians should consider that IFS can present as isolated retrobulbar optic neuropathy.

Abstract:To improve optic nerve function in a patient with progressive visual dysfunction, performing early decompressive and debulking surgery for a metastatic tumor located in the optic canal is essential. The endoscopic endonasal approach could be a practical and effective alternative for lesions in the inferomedial part of the optic canal. A 66-year-old man with a right visual eye field deficit had multiple lesions in the pineal gland, occipital lobe, and right inferomedial optic canal. The optic nerve was distorted by a tumor compressing against the falciform ligament. Although a systemic examination suggested the presence of primary lung cancer, the patient only complained of progressive visual impairment in the right eye. We planned surgery with endoscopic transethmoidal and transsphenoidal approaches to restore visual function and make a pathological diagnosis. During the procedure, we drilled the sella floor, tuberculum sellae, and optic canal and successfully removed the tumor underneath the dura mater. The patient's visual function improved rapidly following surgery, and no complications were observed, such as cerebrospinal fluid leakage. After confirming the pathological diagnosis, the patient subsequently received whole-brain radiotherapy. The endoscopic endonasal skull base approach to the optic canal region could be a practical alternative for treating symptomatic metastatic tumors.
[Rhinogenic neuropathia of the optic nerve due to muco- and pyoceles of the sphenoid sinus and posterior ethmoid cells]

Abstract:16 cases (18 eyes) of rhinogenous optic neuropathy due to muco-pyoceles in the sphenoid sinus and posterior ethmoid cells are reported. The clinical symptomes of this condition are characterized as follows: 1) acute or chronic progressive visual disturbance, 2) ophthalmoscopically normal or oedematous disc in the acute stage and optic atrophy in the chronic stage, 3) central scotoma or inferior hemianopsia in the visual field, 4) good recovery of vision in the cases with mucoceles, occasionally no recovery of vision in the cases with pyoceles, 5) oculomotor as well as abducent palsy and exophthalmos; occasionally, 6) past history of optic neuropathy and nasal operation, 7) few rhinologic complaints, 8) rhinoscopically: swelling, obstruction and/or polyp in the middle meatus, and 9) CT scan showing homogeneous mass in the sphenoid sinus and/or posterior ethmoid cells. The causative factor of the visual disturbance due to pyoceles might be caused not only by compression but also by inflammatory infiltration and vascular disturbance. Recurrence of rhinogenous optic neuropathy is frequent.
Endoscopic Surgical Treatment of Ossifying Fibroma of Sphenoidal Sinus

Abstract:OBJECTIVE To explore the microtrauma's method of surgical treatment of ossifying fibroma of sphenoidal sinuses involving sella turcica and clivus blumenbachii and optic canal and internal carotid artery.METHODS The clinical data of surgical treatment of ossifying fibroma of sphenoidal sinuses involving sella turcica and clivus blumenbachii and optic canal and internal carotid artery with nosal endoscope in 8 cases in 2003.06-2007.07 were retrospectively analyzed.RESULTS Six cases was whole resected and 2 cases was bulk resected.There was no recurrence whole resected cases and no obvious augmentation bulk resected cases followed up six months to 2 years.Symptom of all the patients was obvious improvement.There was no intracranial infeion of cases.Three skull bases defect was repaired by artificial meninges, 2 exposure optic nerve was coverage by nasal mucosa,1 nasal leakage of cerebrospinal fluid was repaired by fascia.CONCLUSION It is minimally invasive and efficiency of transnasal endoscopic technique for ossifying fibroma of sphenoidal sinuses. The satisfactory endoscope and operated tool and abundance knowledge in base of skull microdissection and masterly endoscopic technology are the key for successful operation.

Abstract:OBJECTIVETo explore the feasibility and clinical value of the endoscopic operations around the skull base.METHODSA retrospective study was made of 44 cases treated by endoscopic operations. Twenty-five cases underwent intranasal endoscopic operations.RESULTSForty of 44 cases were cured by one-stage surgery. One case of cartridge foreign body clamped in clivus was not successfully removed; only window operation and puncture biopsy were done in another case of pituitary adenoma; high cranial pressure has not been completely reduced after operation in the third case with relapsing craniopharyngioma a companied with obstructive hydrocephalus. One case of sphenoidal malignant adenoma involving saddle based and extended laterally was partly resected. Complications were follows: cerebrospinal fluid rhinorrhea in 2 cases, diabetes insipidus in 1 case, all of them were cured by conservative treatment; nasal septum perforation in 1 case, without any treatment.CONCLUSIONSThe endoscopic skull base surgery by intranasal approach is feasibility. Intranasal approach has direct path, slight wound and no wound in face. Combined with operation microscope, sinus endoscope can make up for its limits.

Abstract:Invasive fungal sinusitis (IFS) is rare but potentially fatal in immunocompromised patients. Orbital extension of IFS can cause visual loss or ophthalmoplegia, and the rate of recovery of visual acuity can be low even after treatment with antifungal agents and sinus surgery. In this case, optic nerve decompression with the endoscopic endonasal approach was successful for the treatment of visual loss in a patient with IFS with orbital apex extension. The authors describe the recovery of visual acuity after optic nerve decompression in a patient with IFS who had exhibited visual loss caused by invasion into the orbital apex. The authors also provide an intraoperative video and literature review.

Background:Paranasal sinus mucoceles, by virtue of extension into the orbit or cranial vault, can lead to significant ocular, nasal, or neural complications. There is limited comprehensive data on the presentation, management, and outcomes of cases with intraorbital or intracranial extension. A thorough analysis is essential to enhance clinical practices and improve patient outcomes.Objective:This retrospective case series investigates the clinical presentations, radiological features, and surgical outcomes of 10 patients with paranasal sinus mucoceles exhibiting intraorbital or intracranial extensions.Methods:Ten diagnosed cases of paranasal sinus mucoceles with intraorbital or intracranial extensions, as identified on CT or MRI imaging, were selected and retrieved from the electronic database of an academic-affiliated hospital from July 2013 to December 2023. Data were analyzed using descriptive statistics with IBM SPSS version 23.0 software.Results:The study revealed a mean patient age of 65 years, with females (60%) affected more than males (40%). The ethmoidal and frontal sinuses were predominantly involved, with varied clinical symptoms ranging from periorbital pain to visual disturbances. Radiologically, CT and MRI scans depicted extensive sinus involvement, often with intraorbital (50%), intracranial (20%), or combined (30%) extensions. Surgical interventions, primarily endoscopic sinus surgery, resulted in favorable outcomes, including symptom resolution, minimal complications, and no recurrence.Conclusion:This case series emphasizes the importance of thorough preoperative evaluation, individualized surgical approaches, and vigilant postoperative care in managing mucoceles with orbital and cranial involvement. Further research with larger cohorts and extended follow-up periods is essential to refine treatment strategies and enhance patient outcomes for this complex pathology.

Transsphenoidal Optic Nerve Decompression

Abstract:The objective was to study the relationships among the optic nerve, posterior paranasal sinuses, and ophthalmic artery. The identification of reliable anatomic landmarks would provide an endoscopic anatomic basis for optic nerve localization and effective prevention of ophthalmic artery injury during transsphenoidal optic nerve decompression surgery. The Messerklinger technique was performed to open the posterior ethmoid sinus and sphenoid sinus in 8 adult-head specimens. The opticocarotid recess and optic canal were identified under endoscopy, and the relationships between the optic nerve and posterior paranasal sinuses were observed. The relationship between the optic nerve and ophthalmic artery was examined. In all specimens, the opticocarotid recess could be identified; however, the optic nerve protuberance was present in only 10 sides (62%). Three types of relationships were found to exist among the optic nerve, sphenoid sinus, and posterior ethmoid sinus. The posterior ethmoid sinus was anterior and the sphenoid sinus was posterior in 8 sides (50%), the sphenoid sinus was both anterior and posterior in 5 sides (31%), and the ethmoid sinus was both anterior and posterior in 3 sides (19%). At the intracranial opening of the optic canal, the ophthalmic artery was inferomedial to the optic nerve in 9 sides (56%), inferior to the optic nerve in 4 sides (25%), and inferolateral to the optic nerve in 3 sides (19%). The ophthalmic artery coursed laterally from inferior to the optic nerve, and at the intraorbital opening of the optic canal, the ophthalmic artery was inferolateral to the optic nerve in 13 sides (81%) and inferior to the optic nerve in 3 sides (19%). The opticocarotid recess is always present; the imaging characteristics of endoscopy allow this recess to be more reliably identified than the optic nerve protuberance. It can be used as the anatomic landmark of choice in endoscopic optic nerve decompression. The origin of the ophthalmic artery is inferomedial to the optic nerve, and thereafter, it courses inferolaterally. An intersecting relationship exists along its course, a factor that must be taken into account during surgery to avoid injuring the ophthalmic artery.
Anatomy and preliminary clinical applications of endoscopic endonasal approach to optic canal and orbit

Abstract:Objective To provide the endoscopic anatomic basis and anatomic parameters for endoscopic surgical therapy on orbital lesions , and to analyze the advantages and key points of this surgical approach .Methods Five fresh adult heads were used in this experiment .Endonasal thanslamina approach and endoscopic technique were applied to observe important anatomic marks while intraoperative medicalization of the medical rectus muscle was applied to observe the exposure and positioning of important structures and trends of the optic canal and intra orbit .Results Uncinate process was at the lower front corner of middle nasal concha; ethmoidalis bulla was behind the uncinate process , and ethmoidei sinus was reachable after an incision was made on ethmoidalis bulla;anterior ethmoidal artery and posterior ethmoidal artery were the important anatomic landmarks of the inner ethmoidei sinus;optic canal prominence , carotid artery prominence and OCR were the important anatomic landmarks of the inner sphenoid sinus; lamina papyracea was at the lateral wall of ethmoidei sinus , and orbital contents were approacchable after lamina papyracea was cut off ;inside orbit , the optic nerve was approachable through the gap between the medial rectus muscle and inferior rectus muscle .The ophthalmic artery of 9 out of 10 sides of the specimens was originated from the supraclinoidal segment of the internal carotid artery while the remaining one was originated from the cavernous segment of internal carotid artery .There were 7 sides in which ophthalmic artery was located at the inferior lateral of the optic nerve;there were 2 sides in which ophthalmic artery was located at the inferior of the optic nerve; the remaining one was located at the inferior medial of the optic nerve .Conclusion The endoscopic endonasal thanslamina approach can sufficiently expose the optic nerve and the structures in the medical space of the orbit.Uncinate process, ethmoid bulla, anterior ethmoidal artery, posterior ethmoidal artery and posterior ethmoid sinus are the important landmarks of the endoscopic endonasal thanslamina approach .Optic canal prominence , internal carotid artery prominence and OCR are the important landmarks for optic canal decompression .Ophthalmic artery , orbital branches , anterior ethmoidal artery , posterior ethmoidal artery , internal carotid artery are the important vessels . Medialization of the medial rectus muscle is effective to approach the orbital anatomical structures .
Orbital apex syndrome secondary to acute invasive fungal rhinosinusitis diagnosed by transnasal endoscopic biopsy of the optic canal:A case report

Abstract:Orbital apex syndrome (OAS) is a complex condition characterized by visual loss, diplopia, and eye pain that occurs secondary to several pathological processes involving the orbital apex. We report a case of acute invasive fungal rhinosinusitis (AIFRS) associated with OAS. A 76-year-old man with left-sided visual loss, diplopia, palpebral ptosis, and headache was diagnosed with OAS secondary to Tolosa-Hunt syndrome and received systemic corticosteroid therapy from his neurologist. Owing to persistent symptoms, we opened the optic canal using a transnasal endoscopic approach for a surgical biopsy of the orbital apex lesions. Histopathological evaluation revealed numerous Aspergillus organisms in the biopsied granuloma. After surgical debridement, he received a 12-month course of voriconazole, and no recurrence of AIFRS occurred during 8-year follow-up. Patients with OAS may occasionally be prescribed corticosteroids because the clinical manifestations of AIFRS-induced OAS are similar to those observed in OAS secondary to Tolosa-Hunt syndrome, especially no nasal symptoms which is known to respond to corticosteroid therapy. Because both AIFRS-induced OAS and OAS secondary to Tolosa-Hunt syndrome induce ophthalmoplegia, proptosis, eye pain, it is sometimes difficult to differentiate these two diseases in early stage. However, corticosteroid therapy causes exacerbation of fungal infection in patients with AIFRS-induced OAS resulting in delayed accurate diagnosis and poor prognosis. AIFRS is associated with a high mortality rate ; therefore, transnasal endoscopic biopsy of orbital apex lesions before corticosteroid administration is recommended in patients with OAS. J. Med. Abstract:Background: Congenital optic canal stenosis causing compressive optic neuropathy is a rare disorder that presents unique diagnostic and treatment challenges. Endoscopic endonasal optic nerve decompression (EOND) has been described for optic nerve compression in adults and adolescents but has never been reported for young children without pneumatized sphenoid sinuses. The authors describe preoperative and intraoperative considerations for three patients younger than 2 years of age with congenital optic canal stenosis due to genetically confirmed osteopetrosis or chondrodysplasia. Observations: Serial ophthalmological examinations, with a particular focus on object tracking ability, fundoscopic examination, and visual evoked potential trends in preverbal children, are important for detecting progressive optic neuropathy. The lack of pneumatization of the sphenoid sinus presents unique challenges and requires the surgical creation of a sphenoid sinus with the use of neuronavigation to determine the limits of bony exposure given the lack of easily identifiable anatomical landmarks such as the opticocarotid recess. There were no perioperative complications. Lessons: EOND for congenital optic canal stenosis is safe and technically feasible even given the lack of pneumatization of the sphenoid sinus in young patients. Abstract:OBJECTIVE:To evaluate endoscopic surgery for sphenoidal and ethmoidal mucoceles.METHOD:Seventeen patients diagnosed as having paranasal sinus mucoceles underwent attempted endoscopic marsupialization between 1989 and 1995. Ten of the mucoceles were in the sphenoid sinus, seven in the ethmoid sinus. Postoperative endoscopic follow-up period varied from 3 to 9 years.RESULT:All the sphenoidal and ethmoidal mucoceles were satisfactorily approached endoscopically. Postoperative endoscopic examination manifested all the operating cavity was satisfactorily marsupialized. The symptoms were improved or relieved in 15 patients. None of the patients undergoing endoscopic surgery had significant complications.CONCLUSION:Our results show that endoscopic surgery seems to be a safe, valuable way for management of sphenoidal and ethmoidal mucoceles.

Abstract:PurposeTo describe the clinical presentation, imaging characteristics and management of a rare case of Onodi cell sinusitis associated with optic neuropathy.ObservationsA 46-year-old male presented to the emergency department with progressive left eye vision loss over the course of ten days. The constellation of findings from this patient's history, physical, and fundoscopic exam, as well as CT and MR imaging led to the diagnosis of Onodi air cell sinusitis complicated by optic neuropathy. The patient's symptoms resolved fully and vision returned to baseline with oral antibiotics.Conclusionsand Importance: The sphenoethmoidal air cell, also known as the Onodi cell, is an anatomic variant of the paranasal sinuses whose spatial relationship with important neurovascular structures carries significant clinical implications when it becomes inflamed or infected. Our case of Onodi cell sinusitis complicated by optic neuropathy demonstrates how vision loss secondary to sinusitis may resolve with oral antibiotic treatment. We additionally review the relevant anatomy, clinical course and treatment of Onodi cell pathologies.

Ethmoid Osteoma As a Culprit of Orbital Emphysema

Abstract:Orbital emphysema is generally recognized as a complication of orbital fractures involving any paranasal sinuses. The recognition about its etiology has extended beyond sole trauma, but few articles mentioned tumors to be a possible cause.In this case report, we present a patient with orbital emphysema associated with ethmoid osteoma without orbital cellulitis or trauma history. The patient developed sudden proptosis, eyelid swelling, and movement limitation of the left eye, peripheral diplopia, and left periorbital crepitus after a vigorous nose blowing.Complete surgical resection of ethmoid osteoma followed by repair of the orbital medial wall was performed with assistance of combined endoscopy and navigational techniques. Twelve-month follow-up showed no residual lesion or recurrence; the orbital medial wall was accurately repaired with good visual function and facial symmetry.Tumors should be considered for differential diagnosis of orbital emphysema, and combined endoscopy and navigational techniques may improve safety, accuracy, and effectiveness of orbital surgeries.
Intraorbital Surgery by Transnasal Endoscopic Ethmoidal-Lamina Papyracea Approach

Abstract:OBJECTIVE To explore the feasibility of transnasal endoscopic surgery to treatment of intraorbital space occupying by ethmoidal-lamina papy-racea approach. METHODS The clinical data of 10 cases who underwent intraorbital surgery via transnasal endoscopic ethmoidal-lamina papyracea approach from June 2003 and Aug. 2006 were retrospectively studied. The patients included foreign body 1 case, cavernous hemangioma 2 case, lipoma 2 cases, inflammatory pseudotumor 2cases and hematoma 3cases. All intraorbital space occupying were operated by transnasal endoscopic ethmoidal-lamina papyracea approach but for one cavernous hemangioma which was dealt with through extra orbitectomy. The inflammatory pseudotumors were prescribed with glucocorticoid after operation.RESULTS Foreign body was removed successfully. One case of cavernous haemangioma was removed completely by transnasal endoscopic surgery and the other by extroorbital operation. All symptoms of proptosis and diplopia disappeared. One vision was enhanced from hand moving to 0.3 and another from light to hand moving. However, there was one hematoma which had been hindered about 14 days after damaged lost eyesight after operation. All cases were cured but one pseudotumor which recurred and was operated again. All cases were followed up 3 to 24 months. CONCLUSION Intraorbital surgery via transnasal endoscopic ethmoidal-lamina papyracea approach was available and feasible for the advantages of clear operation field, direct vision operation, less damage, rapid recovery and avoiding of facial scar, and intranasal endoscopic surgery can treat the rhino-orbital related diseases and nasal diseases at the same time.
