Abstract:Abstract A clinical practice guideline on telerehabilitation was developed by an American Physical Therapy Association volunteer guideline development group consisting of international physical therapists and physiotherapists, a physician, and a consumer. The guideline was based on systematic reviews of current scientific literature, clinical information, and accepted approaches to telerehabilitation in physical therapist practice. Seven recommendations address the impact of, preparation for, and implementation of telerehabilitation in physical therapist practice. Research recommendations identify current gaps in knowledge. Overall, with shared decision-making between clinicians and patients to inform patients of service delivery options, direct and indirect costs, barriers, and facilitators of telerehabilitation, the evidence supports the use of telerehabilitation by physical therapists for both examination and intervention.

Abstract:Medical practitioners involved in the treatment of diseases, disorders, and injuries of the musculoskeletal system will appreciate the very - portant role of rehabilitation. Following the publication of two books Orthopedic Principles – A Resident’s Guide and Orthopedic Traumatology – A Resident’s Guide, Dr. Ip has expended further time and effort to p- duce a third book, which contains a comprehensive account of orthop- dic rehabilitation. The material is easy to grasp with succinct points. There are 19 chapters, arranged in a logical sequence. The first ch- ter gives a holistic view of what the rehabilitation process should include and emphasises that the unique feature of a rehabilitation service is thinking about a patient and his problems, not simply doing something to the patient, and the focus should be on trying to help each patient achieve his own goals. Chapters 2 to 8 are about the rehabilitation tools for assessing, inv- tigating and treating patients. Chapters 9 to 17 are about rehabilitation for specific types of musculoskeletal disorders, each of which has its own complex problems, requiring practitioners to gain further in-depth training and experience to give effective treatment. The format is similar to the previous two books, containing brief, yet comprehensive, core information on the subject, which can be enhanced by more detailed literature review if necessary. This unique feature r- ders the book very readable.

Abstract:Many individuals with disabilities and/or chronic conditions (da/cc) experience symptoms that may require intermittent or on-going medical care. However, healthcare is an often-overlooked domain for accessibility work, where access needs associated with temporary and long-term disability must be addressed to increase the utility of physical and digital interactions with healthcare workers and spaces. Our work focuses on a specific domain of healthcare often used by individuals with da/cc: physical therapy (PT). Through a twelve-person interview study, we examined how people's access to PT for their da/cc is hampered by social (e.g., physically visiting a PT clinic) and physiological (e.g., chronic pain) barriers, and how technology could improve PT access. In-person PT is often inaccessible to our participants due to lack of transportation and insufficient insurance coverage. As such, many of our participants relied on at-home PT to manage their da/cc symptoms and work towards PT goals. Participants felt that PT barriers, such as having particularly bad symptoms or feeling short on time, could be addressed with well-designed technology that flexibly adapts to the person's dynamically changing needs while supporting their PT goals. We introduce core design principles (adaptability, movement tracking, community building) and tensions (insurance) to consider when developing technology to support PT access. Rethinking da/cc access to PT from a lens that includes social and physiological barriers presents opportunities to integrate accessibility and adaptability into PT technology.

Abstract:Background: The demand for interprofessional collaboration continues to grow due to changing priorities of the health care system. The benefits of interprofessional collaboration are well documented; however, the methods of collaboration are often unclear and often difficult to put into practice. While there is a growing number of studies on the impact of interprofessional collaboration in settings such as inpatient, intensive care units, and acute care, there are limited documented cases regarding the interprofessional management of a physical therapy patient in the outpatient orthopedic private practice setting. Purpose: The purpose of this paper is to describe the reflections of a physical therapist and an athletic trainer in their experience with interprofessional collaboration, describe the barriers they have experienced that make interprofessional collaboration challenging, and to offer solutions to these barriers. The barriers discussed in the paper include limited knowledge of and respect for other professionals’ skill set, high-productivity work environments, medical hierarchy, overlapping bodies of knowledge, discrepancy between professional reasoning, territorial behavior, and ineffective communication. Discussion: It appears that contextual factors, such as community in which the professionals practice, the healthcare setting, and the practice environment, have a far less important impact to successful collaboration than the professionals’ attitudes and investment in the collaborative efforts. Conclusion: Collaboration between a PT and an AT can lead to power struggles and suboptimal patient care if these barriers are not overcome and collaboration may be necessary to provide the highest quality of patient care.

This field endeavors to enhance well-being and independence levels by adopting a comprehensive approach that addresses physical, mental, psychological, and social aspects, while incorporating advancements in medical research. Grounded in the International Classification of Functioning, Disability, and Health (ICF) model by the World Health Organization, rehabilitation targets diverse levels of functional impairment. A collaborative effort among professionals from various disciplines characterizes the rehabilitative process, guided by an Individual Rehabilitation Project, and oversight by a specialist in physical medicine and rehabilitation. Through initiated prompt post-injury, rehabilitation progresses through hospital-based interventions and extends to community-based therapies upon the patient's return home. In this special edition of "Harefuah" readers will find profound scientific articles and literature reviews spanning a breadth of topics within the realm of physical medicine and rehabilitation. Abstract:Abstract Introduction Burn survivors often have ongoing therapy needs after injury and skin grafting to address the multi-system impact of burns on their functional outcomes. A challenge to follow-up care can be related to social and political determinants of health. These determinants can negatively impact a patient’s recovery including the opportunity to receive medical services after their hospitalization. We sought to bridge this gap by locating and utilizing pro bono physical and occupational therapy services that have not been previously used by our burn center. Methods We referred a patient to a local University Doctor of Physical Therapy and Occupational Therapy pro bono clinic who is undocumented, uninsured and has limited English-speaking proficiency. The patient sustained 58% total body surface area full-thickness flame burns involving the head, face, neck, chest, abdomen, back, buttocks, bilateral lower extremities, bilateral upper extremities, and hands after a motor vehicle collision. He underwent seven excision and skin grafting interventions over his 89-day hospital admission. His inpatient rehabilitation stay was 20 days. We collected data from the Doctor of Physical Therapy and Occupational Therapy students who treat this patient to understand their experience. We also had dialogue with the patient on his return to the burn clinic and with one of the physical therapy professors for anecdotal perspectives of the experience. It is noted that the professors are actively involved in the treatment of the patient in the clinic. Results We collected 10 student surveys at a 76% completion rate. 75% of the students state knowledge of treatment strategies are the most challenging aspect of treating a burn survivor. This is followed by 12.50% stating pain management is most challenging. 55.56% of the students report limited confidence, 33.33% are not confident and 11.11% are fully confident with treating burn survivors. 70% of the students state treating a burn survivor during school impacts decision-making for their future career path. 90% report a pro bono clinic is a positive resource for the burn survivor population while 10% are undecided. Within the open-ended questions students request more robust education as well as guidance from the referring provider for treating patients referred to the pro bono clinic. Conclusions Doctor of Physical and Occupational Therapy program pro bono clinics have the opportunity to fill two needs in the burn community. One is the exposure of students to burn survivors to gain experience in treatment implementation following their didactic burn lecture. The second is to address long-term therapy needs of burn survivors in the setting of limited resources. Applicability of Research to Practice These survey results demonstrate there is potential for building relationships with pro bono clinics connected to universities. However, there is a need for increased communication and education to improve the patient and student experience.

Abstract:Background: Despite strides in early detection and management of breast cancer, the primary treatments for this disease continue to result in physical impairments for some of the nearly 3 million people diagnosed annually. Over the past decade, evidence-based clinical practice guidelines (CPGs) have been developed with goals of preventing and ameliorating these impairments. However, translation of these guidelines into clinical practice needs to be accelerated. Methods: Relevant health science databases (2001-2011) were searched to identify CPGs on breast cancer rehabilitation for the following impairments: upper extremity restrictions, lymphedema, pain, fatigue, chemotherapy-induced peripheral neuropathy, treatment-related cardiotoxicity, bone health, and weight management. Results: Recommendations from 19 relevant CPGs were first summarized by impairment within tables; commonalities across guidelines, within each impairment, were then synthesized within the article. The CPGs were rated using the Appraisal of Guidelines for Research and Evaluation II (AGREE II); wide variability was noted in rigor of development, clarity of presentation, and stakeholder involvement. The most rigorous and comprehensive of those rated was the adult cancer pain guideline from the Scottish Intercollegiate Guidelines Network. Conclusions: Based on a large body of evidence published in recent years, including randomized trials and systematic reviews, there is an urgent need for updating the guidelines on upper extremity musculoskeletal impairments and lymphedema. Furthermore, additional research is needed to provide an evidence base for developing rehabilitation guidelines on management of other impairments identified in the prospective surveillance model, eg, arthralgia.
Evaluating the Uptake and Acceptability of Standardized Postoperative Rehabilitation Guidelines Using an Online Knowledge Translation Approach

Abstract:Background: Providing physical therapists with evidence-based and consensus-derived guidelines to manage postoperative shoulder patients is essential; these guidelines should be readily available and provide clinically applicable information. Knowledge translation (KT) initiatives that encourage interaction between clinicians and researchers, that have multifaceted components and use a variety of strategies, can significantly change practice. Objective: The objective of this study was to determine the uptake and acceptability of standardized postoperative shoulder guidelines with an accompanying online KT resource through evaluation of website analytics and a quantitative survey. Design: A multi-pronged approach was used to assess uptake and acceptability of the guidelines and online KT resource. Methods: Website analytics of usage and geographical location of users was measured as were physical therapist survey responses. Results: Website analytics revealed that 5406 individuals used the online resource between October 2012 and September 2013 with the average visit lasting 8 minutes; only 47% of users were within the guideline developers' surgical referral region. Physical therapists who used the new shoulder guidelines were very satisfied or satisfied (96%) with the guidelines, reporting they promoted patient-specific clinical decision-making extremely or very well (68%). They viewed the online KT resource positively, with 79% rating it as "very useful" or "quite useful." Physical therapists from regions beyond those expected to use the new shoulder guidelines were also aware of the website and also rated it as very useful. Limitations: The survey sample was relatively small and did not directly assess patient outcomes. Conclusions: An online KT web resource developed in conjunction with standardized postoperative shoulder guidelines was perceived as useful based on website analytics and survey responses. Active KT strategies such as this can improve uptake and dissemination of best practice in physical therapy.
Household Items-Based Personalized Video Rehabilitation amidst the COVID-19 Pandemic: Handouts We All Wish We Had

Abstract:Category: Ankle; Sports Introduction/Purpose: Rehabilitation is vital in the recovery of countless foot and ankle injuries and operations.1With our attention directed to providing care on the frontlines of the COVID-19 pandemic, rehabilitation has fallen by the wayside with poor consequences. Access to outpatient rehabilitation services and their specialized exercise equipment has significantly decreased,2 leaving patients feeling abandoned and surgeons looking for help on how to guide patients’ postoperative rehabilitation. This poster serves to introduce household items-based personalized video rehabilitation (PVR). Methods: Rehabilitation exercise videos were produced by a board-certified athletic trainer and board-certified orthopaedic foot and ankle surgeon. The videos showed how to perform rehabilitation exercises with common household items (Figure 3).The videos were produced in a structured order, explaining the purpose of the exercise, showing what household items are needed, and demonstrating the proper technique of the exercise. Following recording of the videos, they were assembled into injury/scenario-specific infographics (Figure 1). Results: 34 rehabilitation exercise videos were recorded, with an average length of two minutes, 11 seconds.11 injury/scenario- specific infographics were assembled, with each page of an infographic containing seven exercises (Figure 1), with specific information and access for each exercise (Figure 2).Patients can access the videos by clicking on the hyperlink, or by scanning the QR code. When giving infographics, healthcare professionals can guide patients on which exercises to perform. Conclusion: Household items-based PVR provides patients with an interactive infographic that gives access to concise videos of rehabilitation exercises, especially during a time of reduced rehabilitation accessibility. Healthcare professionals can assign patient- specific exercises on the infographic that are appropriate for a patients’ phase of rehabilitation. Not meant to replace the current rehabilitation model, household item-based PVR is a paradigm shift that will allow physicians to bridge gaps in care.
Barriers, Facilitators and Interventions to Support Virtual Reality Implementation in Rehabilitation: A Scoping Review

Abstract:Virtual reality and active video games (VR/AVGs) are promising rehabilitation tools because of their potential to facilitate abundant, motivating, and feedback-rich practice. However, clinical adoption remains low despite a growing evidence base and the recent development of clinically accessible and rehabilitation-specific VR/AVG systems. Given clinicians' eagerness for resources to support VR/AVG use, a critical need exists for knowledge translation (KT) interventions to facilitate VR/AVG integration into clinical practice. KT interventions have the potential to support adoption by targeting known barriers to, and facilitators of, change. This scoping review of the VR/AVG literature uses the Theoretical Domains Framework (TDF) to (1) structure an overview of known barriers and facilitators to clinical uptake of VR/AVGs for rehabilitation; (2) identify KT strategies to target these factors to facilitate adoption; and (3) report the results of these strategies. Barriers/facilitators and evaluated or proposed KT interventions spanned all but 1 and 2 TDF domains, respectively. Most frequently cited barriers/facilitators were found in the TDF domains of Knowledge, Skills, Beliefs About Capabilities, Beliefs About Consequences, Intentions, Goals, Environmental Context and Resources, and Social Influences. Few studies empirically evaluated KT interventions to support adoption; measured change in VR/AVG use did not accompany improvements in self-reported skills, attitudes, and knowledge. Recommendations to target frequently identified barriers include technology development to meet end-user needs more effectively, competency development for end-users, and facilitated VR/AVG implementation in clinical settings. Subsequent research can address knowledge gaps in both clinical and VR/AVG implementation research, including on KT intervention effectiveness and unexamined TDF domain barriers. Abstract:Medical rehabilitation is a complex, long-term and financially costly process of restoring the physiological functions of the body after injuries and the consequences of past diseases. The need for an individual approach to each life situation necessitates the search and development of new technologies in the method of providing rehabilitation assistance. One of the principles of a successful rehabilitation process is continuity, which, in most cases, is disrupted after the transfer of patients to the outpatient stage. The 21st century, in contrast to the previous one, is distinguished by more accessible technologies for everyday and individual use. A separate category of patients are patients with the consequences of brain damage, socialization and the return of the ability to self-serve for whom is one of the most difficult in rehabilitation practice. One of these technologies is distance rehabilitation, which provides the principle of continuity, social and informational support for relatives who provide care for patients on an outpatient basis.
Understanding User Needs for Injury Recovery with Augmented Reality

Abstract:Physical therapy (PT) plays a crucial role in muscle injury recovery, but people struggle to adhere to and perform PT exercises correctly from home. To support challenges faced with in-home PT, augmented reality (AR) holds promise in enhancing patient's engagement and accuracy through immersive interactive visualizations. However, effectively leveraging AR requires a better understanding of patient needs during injury recovery. Through interviews with six individuals undergoing physical therapy, this paper introduces user-centered design considerations integrating AR and body motion data to enhance in-home PT for injury recovery. Our findings identify key challenges and propose design variables for future body-based visualizations of body motion data for PT.

Abstract:Remote rehabilitation is an innovative approach that allows patients to receive medical support and rehabilitation services in a convenient and safe way in their own home or remotely from a medical facility. Remote rehabilitation can be used for a wide range of medical conditions and problems, including physical therapy, rehabilitation after injuries or surgeries, pain management, rehabilitation after stroke, Parkinson's disease and other neurological diseases, pulmonary rehabilitation for patients with chronic obstructive pulmonary disease, as well as psychological support and counseling. This approach may include the use of video communication, monitoring of physiological parameters, mobile applications, and sensors to measure progress and monitor the patient. Remote rehabilitation is especially useful in situations where it is difficult or impossible for patients to physically visit a medical facility, for example, due to physical activity limitations, remote geographical locations, or epidemiological circumstances. Remote rehabilitation has both positive and negative aspects. Positive aspects: accessibility – allows patients to get the help they need regardless of location; convenience – to receive rehabilitation services at a time and place convenient for them; cost – can be more cost-effective, as it reduces travel and accommodation costs for patients. Negative aspects: lack of direct contact; limited capabilities – some types of therapy that require guidance or correction from a specialist may be less effective in a remote format; technological problems. It is important to consider these positive and negative aspects of remote rehabilitation when deciding whether to use it. The specific circumstances and needs of the patient may affect the effectiveness of this approach to rehabilitation. Remote rehabilitation is a promising area in medicine, as it allows for increased accessibility and convenience of rehabilitation services for patients. It can also help reduce the burden on healthcare facilities and reduce the cost of organizing rehabilitation programs. Rehabilitation in Oncology Care Guidelines: A Gap Analysis

Abstract:Background: Cancer survivors experience a high prevalence of functional impairments. Rehabilitation interventions include an expansive array of services that can help optimize function, address pain, decrease symptom burden, and improve quality of life. Nonetheless, rehabilitation services remain underutilized. Thus, it is important to enhance the understanding of and establish guidelines for specific rehabilitation disciplines and interventions. Methods: This is a gap analysis of rehabilitation recommendations in published oncology guidelines from selected nationally recognized organizations. Symptom-specific guidelines and cancer type-specific guidelines were analyzed for inclusion of common functional impairments (fatigue, pain, peripheral neuropathy, cognitive dysfunction, and lymphedema) and the rehabilitation discipline recommendations. Results: The prevalence of recommendations for rehabilitation in cancer type-specific guidelines was 29%, and was higher in symptom-specific guidelines at 60%. However, the frequency of specific rehabilitation disciplines (physiatry, physical therapy, occupational therapy, speech-language pathology, and rehabilitation psychology/neuropsychology) was notably lower. Overall rehabilitation was mentioned in 33% and physiatry in 18%. Nonrehabilitation specialties were recommended in 18% of the guidelines. No specialty referral was endorsed in 53% of guidelines in which 1 of 5 symptoms were discussed. This highlights the relative paucity of recommendations for specific rehabilitation disciplines in oncology guidelines. The more general term "rehabilitation" was included more frequently but lacks critical guidance for oncology providers. Other crucial rehabilitation services may be underrecognized and underutilized. Rehabilitation specialists must work to improve patient access and the presence of indicated specific rehabilitation disciplines and goals within guidelines. Conclusions: Most oncology guidelines do not include specific recommendations for rehabilitation disciplines. However, including specific rehabilitation disciplines is more common in symptom-specific guidelines. With a stronger evidence base and increased involvement of rehabilitation specialists in guideline development, rehabilitation recommendations in oncologic guidelines may be more precise, leading to improved utilization of rehabilitation services to optimize function and quality of life.

Abstract:BACKGROUND: Rehabilitation medicine is a multidisciplinary field aimed at improving patients' quality of life by improving function. Patients receiving palliative care frequently share common symptoms including fatigue, decreased functional independence, mood disorders, pain, and breathlessness. Many rehabilitation interventions can improve these symptoms.OBJECTIVE: To evaluate the scope and effectiveness of rehabilitation interventions and exercise programs in improving quality of life and distressing symptoms in patients receiving palliative care.METHODS: We conducted a literature review of cancer rehabilitation topics and techniques specifically applied to patients with life-limiting conditions. Exercise and other rehabilitation interventions were analyzed for their effects on common symptoms and disabilities experienced by this patient population.CONCLUSION: Current available literature supports the use of exercise programs and rehabilitation interventions to improve fatigue, mood, functional independence, breathlessness, and pain. Rehabilitation and palliative care practitioners share many goals in their approach to patient care and augment one another well. Palliative care providers should consider referral to physiatry (physical medicine and rehabilitation) to help optimize patients' quality of life.
Approaches to worker rehabilitation by occupational and physical therapists in the United States: Factors impacting practice

Abstract:Work-related rehabilitation services have changed in nature and scope since their inception in the late 1970s. A review of the literature reveals a large body of published data concerning the various approaches used by therapists in this practice area, but a limited number of comparison studies documenting the value of one approach over the other. This national survey of physical and occupational therapists in the US was conducted in 2002, and examined the prevailing trends in service provision, and factors that determine the nature of services provided to clients. Results indicate that services continue the move to onsite service provision and an emphasis on prevention, but that select services, such as onsite job analysis and comprehensive, inter-disciplinary programs are being used to a limited degree. Research that examines the relative contributions of selected work-related services to successful and efficient return to work outcomes is necessary to identify best practice approaches. Abstract:Background: Low-intensity treatments (LITs), such as bibliotherapy or online self-help, have the potential to reach more individuals than traditional face-to-face care by circumventing many of the common barriers to mental health treatment. Despite substantial research evidence supporting their usability and efficacy across several clinical presentations, prior work suggests that mental health providers rarely recommend LITs for patients waiting for treatment. Methods: The present study analyzed provider open responses to a prompt asking about perceived barriers, thoughts, and comments related to additional treatment resources for patients on treatment waiting lists. We surveyed 141 practicing mental health providers, 65 of whom responded to an open text box with additional thoughts on using LITs for patients on treatment waiting lists. Responses were qualitatively coded using a thematic coding process. Results: Qualitative outcomes yielded 11 codes: patient appropriateness, research evidence, feasibility, patient barriers, liability, patient personal contact, additional resources, positive attitudes, trust in programs, systemic problems, and downplaying distress. Conclusions: Results suggest providers are predominantly concerned about the potential of suggesting a LIT that would be ultimately inappropriate for their patient due to a lack of assessment of the patient's needs. Furthermore, providers noted ambiguity around the legal and ethical liability of recommending a LIT to someone who may not yet be a patient. Guidelines and standards for recommending LITs to patients on treatment waiting lists may help address ambiguity regarding their use in routine care.

Abstract:PURPOSE: Knowledge translation (KT) has emerged as a concept that can lead to a greater utilization of evidence-based research in systems of care. Despite a rise in KT research, the literature on KT in relation to physical therapy practice is scarce. This article provides physical therapists (PTs) with recommendations that can support the effective implementation of new knowledge and scientific evidence in clinical practice.METHOD: Recommendations are grounded in the Ottawa Model of Research Use and in the literature in KT in the health professions.RESULTS: A well-established KT process, which is supported by a planning model, is essential to guide the implementation of scientific evidence. Consensus among all stakeholders about what evidence will be implemented must be reached. Context-related barriers and facilitators should be assessed and tailored active and multi-component interventions should be considered. Participation from individuals in intermediary positions (e.g. opinion leaders) supports implementation of KT interventions. Monitoring of the process and assessment of intended outcomes should be performed in order to assess the success of the implementation.CONCLUSION: Five major recommendations grounded in the Ottawa model are provided that can assist PTs with the complex task of implementing new knowledge in their clinical practice.IMPLICATIONS FOR REHABILITATION: In order to support EBP, knowledge translation interventions can be used to support best practice. Implementation of new knowledge should be guided by a framework or a conceptual model. Consensus on the evidence must be reached and assessment of context-related factors should be done prior to the implementation of any KT intervention. Intervention strategies should be active, multi-component and include individuals with intermediary positions that can facilitate the KT process.

Abstract:Psychiatric rehabilitation is a therapeutic approach that encourages people with mental illness and intellectual disabilities to develop their inherent capacities through learning and environmental support. Psychiatric treatment (pharmacological and psychological) controls psychiatric symptoms; psychiatric rehabilitation focuses on functioning and role outcomes. The review aimed to explore the end-user's perception of facilitators and barriers in accessing psychiatric tele-rehabilitation services. Various electronic databases like Web of Science, Scopus, PubMed, Research Gate, Science Direct, ProQuest, Springer, Wolters-Taylors, Elsevier, PsycINFO, and Wiley online library were searched using Google scholar. Inclusion criteria were studies including psychiatric rehabilitation, web-based interventions, and facilitators and barriers in accessing psychiatric tele-rehabilitation services. A systematic search identified 13 studies, including quantitative, qualitative, and mixed-method approaches. The results identified were based on the facilitators and barriers in accessing telerehabilitation. The common themes identified in this review are (1) facilitators of telerehabilitation, (2) barriers of tele-rehabilitation, and (3) expectations in tele-rehabilitation. The facilitating factors include an internet-enabled device, cost/financial benefits, ehealthcare knowledge, technology as a valuable and accessible tool, motivational factors, satisfaction and willingness. The barriers include affordability to the internetenabled device, network connectivity, lack of technical skills, and digital literacy. Expectations include some modifications needed to carry out effective practices of psychiatric tele-rehabilitation. Effective tele-rehabilitation helps people with mental illness and intellectual developmental disorders for their optimal functioning/quality of life.
