Abstract:Background & Aims: Patient safety is an important component and one of the main elements of quality in health care systems. The patient safety status among the pre-hospital Emergency Medical Staff (EMS) in Iran is unclear and there is almost no information about adverse complications. Assessing the current patient safety culture can be a starting point for improving safety attitudes among health care workers, including pre-hospital EMS who are at the forefront of managing patients in critical conditions. This study aims to survey the attitude towards patient safety among pre-hospital EMS in Mashhad, Iran. Materials & Methods: This descriptive cross-sectional study was conducted on 438 full-time pre-hospital EMS with at least 6 months of operational work experience in Mashhad city in 2019, who were selected by a census method. The assessment tool was the Emergency Medical Services Safety Attitudes Questionnaire (EMS-SAQ). The collected data were analyzed in SPSS v. 16 using descriptive statistics (frequency, percentage, mean, and standard deviation) and inferential statistics (independent t test, analysis of variance, and Pearson correlation test). The significance level was set at 0.05. Results: It was reported that 13.75% had a positive attitude towards patient safety. The mean total score of EMS-SAQ was 53.55±10.60. The most positive attitude was related to stress recognition (34.2%) and the lowest positive attitude was related to safety climate and perception of management (5.7%). There was no statistically significant relationship between any demographic characteristics and overall attitude towards patient safety (P>0.05). Conclusion: The patient safety culture of the pre-hospital EMS in Mashhad is not positive. This requires further research and also taking measures such as holding briefing sessions for managers and staff, increasing the number of staff, setting up an error reporting system, and including topics related to patient safety in the in-service training courses for the staff.

Abstract:Background and aims: to explore the psychometric properties of the Persian version of the Patient Satisfaction Questionnaire (PSQ). Methods: In this cross-sectional study in 2015, the psychometric properties of the Persian version of the PSQ were determined by participation of 10 patients, 14 experts and 488 residents. Impact score, content validity index, content validity ratio of the items were calculated. Construct validity and reliability of the scale were examined conducting exploratory factor analysis and Cronbach's Alpha and Inter class correlation coefficient (ICC), respectively, using STATA, version 13. This research project was approved by the Ethics Committee of Tabriz University of Medical Sciences. Results: Based on the findings, questionnaire had: CVI=0.94, CVR=0.84, Impact Score=4.61, Cronbach's Alpha=0.93 and ICC=0.513 (Sig.0.00). A single factor was identified according to the eigenvalue distribution of PSQ. That factor could predict about 93% of the variance. Conclusion: Persian version of PSQ had satisfactory reliability and validity, as well as adequate feasibility based on the field results. Medical teachers and managers at every Persian-speaking country can reliably evaluate residents' communication skills, applying the Persian version of PSQ.

Abstract:Background: Participation and initiative of workers are effective in promoting safety in the workplace. Proactive-Safety Role Orientation questionnaire (PRO-SAFE) is a proper tool to evaluate the psychological drivers that support the proactive orientation of individuals toward workplace safety. This study was conducted to translate and measure the psychometric properties of the Persian version of PRO-SAFE. Methods: The PRO-SAFE was translated into Persian using procedures for translation and cross-cultural adaptation. To collect data, 252 employees of a steel complex were selected. To measure the validity of the questionnaire, face, content, convergent, and construct validity was utilized. The questionnaire's reliability was evaluated by assessing its internal consistency. Results: The mean of the content validity index and content validity ratio was equal to 0.83 and 0.85, respectively. We found a positive correlation between PRO-SAFE and safety behavior dimensions (r = 0.372 to 0.792, P < 0.001). Confirmatory factor analysis showed the Persian version of the PRO-SAFE questionnaire had an excellent six-factor model consistent with the original questionnaire. Cronbach's alpha of the Persian version of the PRO-SAFE questionnaire was obtained between 0.717 to 0.880. Conclusions: The Persian version of the PRO-SAFE questionnaire was found to have appropriate psychometric properties, indicating that it can be confidently used as a valid tool for assessing proactive role orientation toward safety management among Iranian employees.

Abstract:Background: A valid and reliable tool compatible with the culture is needed to evaluate the safety culture as one of the vital and promotional components in improving the quality of safety and health care. This study aimed to investigate the psychometric properties of the Persian version of the "Hospital Survey on Patient Safety Culture (HSOPSC)" in physicians and nurses working in Neonatal Intensive Care Units. Methods: In this methodological research, the qualitative face, content validity, and construct validity were performed by Confirmatory Factor Analysis to the psychometric evaluation of the HSOPSC questionnaire. Based on convenience sampling and the inclusion criteria, 360 individuals completed the questionnaire. Internal consistency and stability were measured. Data analysis was performed using SPSS 21 and LISREL. Results: In examining the construct validity, fit indices were not appropriate for the 12-dimension model of the Persian version. According to T-value, six heterogeneous items and a dimension were omitted. The 11-dimension model with 36 items showed an appropriate fit with the data. Cronbach's alpha was evaluated at 0.79, and the stability was 0.82 (p˂0.001). Conclusion: The Persian version of HSOPSC with 11 dimensions and 36 items has favorable validity and reliability and can be used in NICUs.

Abstract:Abstract Background To translate and cross-culturally adapt the Extended Version of the Nordic Musculoskeletal Questionnaire (NMQ-E) into Persian (NMQ-E-P) and evaluate the psychometric properties in a general population with different occupational tasks across nine body regions. Methods This cross-sectional study was designed according to the standard guidelines and the COSMIN checklist. The NMQ-E-P was achieved through forward and backward translation methods and consensus to produce the final draft. A Persian-speaking population ( n = 571, age 38.24 ± 7.65 years, female = 46.2%) was recruited from industries and office workers with three occupational task inclusion criteria: assembly, office, and lifting. Psychometric properties included validity for face (from confirmed clarity, simplicity, and readability), content (via the content validity index); and construct (through known group validity); additionally, the properties of internal consistency (Cronbach’s α); and test-retest reliability (Kappa coefficient of agreement) were considered. Results No significant issues during the translation process were found. The NMQ-E-P showed adequate internal consistency for all regions (α ≥ 0.87). The test-retest reliability was examined with Kappa agreement correlation coefficient and all items, except ankle regions, showed very good agreements (Kappa coefficient = 0.87-1.0). Excellent ICC values were obtained for quantitative variables (ICC > 0.88) and good construct validity was revealed ( p < 0.001). Conclusion The Persian version of the NMQ-E has very good validity and reliability and can be used by researchers and professionals to evaluate the prevalence of MSDs in nine body regions simultaneously.

Abstract:Introduction: One of the most important steps to save patients is to perform resuscitation. The aim of this study was to investigate the knowledge and practice of emergency personnel in Tehran regarding the baseline level of resuscitation operations. Methods: This descriptive cross-sectional study was performed on 200 emergency personnel working in Tehran. The level of awareness questionnaire with 45 qualifications and the base of BLS indicators was prepared and collected. Results: According to Chi-square test, there was a significant correlation between age and practical score. Also, there was a significant relationship between the ALS courses and the acquired theoretical score. Conclusion: The results of the increase in awareness of this study in relation to previous studies in Iran and especially in the region show that during the last few years, the level of knowledge of medical emergencies personnel has had good improvement, while on the other hand, although there has been improvement in the performance compared with last few years.
Development and psychometric evaluation of a 360-degree evaluation instrument to assess medical students’ performance in clinical settings at the emergency medicine department in Iran: a methodological study

Abstract:Background: In the Iranian context, no 360-degree evaluation tool has been developed to assess the performance of prehospital medical emergency students in clinical settings. This article describes the development of a 360-degree evaluation tool and presents its first psychometric evaluation. Methods: There were 2 steps in this study: step 1 involved developing the instrument (i.e., generating the items) and step 2 constituted the psychometric evaluation of the instrument. We performed exploratory and confirmatory factor analyses and also evaluated the instrument’s face, content, and convergent validity and reliability. Results: The instrument contains 55 items across 6 domains, including leadership, management, and teamwork (19 items), consciousness and responsiveness (14 items), clinical and interpersonal communication skills (8 items), integrity (7 items), knowledge and accountability (4 items), and loyalty and transparency (3 items). The instrument was confirmed to be a valid measure, as the 6 domains had eigenvalues over Kaiser’s criterion of 1 and in combination explained 60.1% of the variance (Bartlett’s test of sphericity [1,485]=19,867.99, P<0.01). Furthermore, this study provided evidence for the instrument’s convergent validity and internal consistency (α=0.98), suggesting its suitability for assessing student performance. Conclusion: We found good evidence for the validity and reliability of the instrument. Our instrument can be used to make future evaluations of student performance in the clinical setting more structured, transparent, informative, and comparable.

Abstract:Background and aims: Pre-hospital emergency technicians face many problems in the workplace daily, so the ability to solve or overcome them in the workplace is essential. This article aimed to assess the predictors of problem-solving skills among emergency medical services staff in Iran. Methods: This study was cross-sectional correlational research. Using convenience sampling methods, 140 emergency medical services (EMS) staff were enrolled in the study. Response time was assessed using ASAYAR software, problem-solving skills (PSS) were measured using the Hepner Petersen Problem Solving Questionnaire (PSI), and cognitive emotion regulation strategies were assessed using the Garnfsky Cognitive Emotion Regulation Questionnaire. Descriptive statistics, t-test, one-way analysis of variance (ANOVA), Pearson's r correlation coefficient, and multiple linear regression analysis were applied using SPSS 14.0. Results: The results of our study showed that the total mean score for problem-solving skills was 136.84 (14.65) (range, 175-107 points). Multiple linear regression indicated that refocusing on planning, positive evaluation, stress management courses, delays and their causes, positive refocusing, catastrophizing, and acceptance were effective predictors of problem-solving skills in emergency personnel, accounting for 54% of the total variances. Conclusion: This study is one of the first studies in this field. Based on our findings, individuals who consider their emotions and feelings when solving problems are better able to use the process of thinking and problem-solving skills. Therefore, by training people in the field of emotional regulation skills, the skills to solve problems technicians can be increased.

Abstract:Purpose: The present study was an attempt to investigate cross-cultural adaptability and evaluate the psychometric properties of the Persian version of the Manchester respiratory activities of daily living questionnaire ((MRADLQ-P). Patients and methods: In a cross-sectional study, we selected 260 patients with severe respiratory diseases who needed to be admitted to the respiratory wards of this city hospital. The process of cultural localization of the questionnaire was performed based on a standard and valid process. Psychometric properties of the instrument were confirmed based on face and content validity assessments, convergent validity, discriminative validity and internal consistency. Data collected by demographic questionnaire, MRADL questionnaire and work ability index. Data were analyzed by SPSS 22 using descriptive statistics (mean and standard deviation), Spearman correlation coefficient, Cronbach's alpha coefficient, and Mann-Whitney test. Results: The mean age of participants was 48.8 ± 20.1 years. 176 (71.5%) were male. face content validity including content validity index (CVI) was 0.82 and content validity ratio and it was good. The questionnaire was measured at the same time as the work ability index, which Mann-Whitney test showed that the questionnaire has good differential power. Cronbach's alpha coefficient of 0.9 indicates a very good reliability of the questionnaire. Conclusion: The results show that intercultural psychometrics of MRADL questionnaire has good validity, reliability, and differential power that can be a good tool for use in future studies. Also, the translation of this checklist included translation into the target language, backward translation of the Persian versions into the original language, and comparisons and ambiguities to obtain a final and acceptable version.

Abstract:Introduction: Theories and numerous empirical studies indicate teaching performance and students' learning progress are affected by teaching self-efficacy. Therefore, the present study examines the psychometric properties of the Persian version of the physician teaching self-efficacy questionnaire. Methods: The 16-item physician teaching self-efficacy questionnaire was translated from English to Persian and back-translated to English and then administered to 242 medical teachers from six medical universities. To assess construct validity, researchers made use of confirmatory factor analysis. To check the reliability and validity of the physician teaching self-efficacy questionnaire, we used internal consistency, discriminant, convergent, and criterion validity. Results: PLS-SEM results substantiated the original three factor structure of the questionnaire which is dyadic, triadic, and self-regulation. For all sub-scales, internal consistency- measured by Cronbach's alpha and composite reliability, convergent validity- measured by factor loading and AVE, and discriminant validity- measured by cross-loading, Fornell-Larcker, and HTMT metrics- confirmed the construct reliability and validity of the questionnaire. A positive correlation was, also, fund between teaching motivation and experience with the physician teaching self-efficacy questionnaire scales, proving the criterion validity of the questionnaire. Conclusion: The Persian version of physician teaching self-efficacy questionnaire is a valid, highly reliable, and multidimensional tool to measure physicians' clinical teaching self-efficacy working in medical universities.
Assessment of the Emotional Reactivity Through the Positive and Negative Emotions: The Psychometric Properties of the Persian Version of the Perth Emotional Reactivity Scale

Abstract:Background: Emotional reactivity is a key construct for understanding psychopathology. Examining the psychometric properties of the instruments in societies with different cultures can help with their external validity. Objectives: The current study aimed at standardization and validation of the Perth emotional reactivity scale (PERS) in Iranian university students. Methods: The Persian version of the PERS was produced through forward translation, reconciliation, and back-translation. A total of 302 (169 males and 133 females) nonclinical students were selected by convenience sampling method, and completed a set of questionnaires, including the PERS, weight efficacy lifestyle questionnaire-short form (WEL-SF), eating attitude test-16 (EAT-16), self-esteem scale (SES), difficulties in emotion regulation scale-16 (DERS-16), and self-compassion scale (SCS) short-form. The construct validity of the PERS was assessed using confirmatory factor analysis and divergent and convergent validity. Internal consistency and test-retest reliability (a 2-weeks interval) were used to evaluate the reliability. Data analysis was performed using LISREL (version 8.8) and SSPS (version 22) software. Results: All PERS scales and subscales were found with good internal consistency and test-retest reliability in a nonclinical population. The convergent validity and divergent validity were also found good. The results of this study provides the support for the applicability of the six-factor (RMSEA = 0.06, NFI = 0.94, NNFI = 0.96, and CFI = 0.97) and two-factor (RMSEA = 0.07, NFI = 0.94, NNFI = 0.96, and CFI = 0.96) models of the PERS. Conclusions: The PERS showed good validity and reliability and can be useful in assessing emotional reactivity in the Iranian population. The PERS can be promising as a measure to use in emotional-based studies and clinical settings.
Psychometric Properties of the Persian Version of Social Anxiety Scale for Social Media Users (SAS-SMU)

Abstract:Objective: Social anxiety refers to an excessive concern or fear about social situations. It seems that social media, which has become one of the most popular and effective tools for communication today, can be one of the contexts of social anxiety. Due to the lack of a Persian instrument to assess social anxiety in the context of social networks, the current study was undertaken to analyze the psychometric criteria of the Iranian version of the Social Anxiety Scale for Social Media Users (SAS-SMU). The SAS-SMU is a 21-item questionnaire designed by Alkis and colleagues (2017) to measure social anxiety emerging from the social media platforms. Method : In this study, a total of 842 participants within the age range of 11 to 82 years old (mean age 33.11 ± 12.134), 59% female) answered the questions in an online survey. The original version of the scale was translated into Persian using the back translation procedure. All participants completed a Demographic Questionnaire, the SAS-SMU, and the Beck Anxiety Inventory. In order to analyze the collected data, internal and external consistency, factor analysis, construct validity, and confirmatory factor analysis (CFA) were examined. A significance level of less than 0.05 was considered to determine statistical significance. Results: Four subscales were obtained from the exploratory factor analysis (SCA, PCA, IA, and SEA), which were confirmed by the confirmatory factor analysis. Cronbach's alpha for internal consistency was found to be 0.931 for the total scale and 0.920, 0.846, 0.901, 0.828 for SCA, PCA, IA, and SEA, respectively. In addition, the test-retest scores of 30 participants (interval: between 2 to 3 weeks) for all four subscales (SCA = 0.641, PCA = 0.773, IA = 0.688, SEA = 0.727) indicated acceptable stability of the questionnaire over time. Conclusion: This study validates the Persian version of the SAS-SMU for use in studies in the field of psychological problems related to social media and online communications.

Abstract:The Ryerson Social Anxiety Scales (RSAS) are a new measure to assess the severity of social anxiety symptoms. The purpose of the present study was to develop the Persian version of the Ryerson Social Anxiety Scales (RSAS-P) and examine its factor structure, measurement invariance, reliability and validity from different dimensions in both the general population and individuals with social anxiety disorder (SAD) as well as its treatment sensitivity. This research involved 1023 participants from various normal and pathological groups. The RSAS was translated to Farsi and administered to all participants. Participants also completed the Fear of Negative Evaluation Scale (BFNE-S) and the Emotion Regulation Questionnaire (CERQ-P-Short). Confirmatory factor analysis supported the unidimensional structure of the RSAS-P. The RSAS-P showed excellent internal consistency. Correlations between RSAS-P and other related and distinct conceptual measures supported the discriminant validity of RSAS. The reliability test results were satisfactory. The RSAS was also sensitive to changes in the severity and status of social anxiety following treatment. Findings suggest that the Persian version of the RSAS is a valid and reliable instrument for measuring the severity of social anxiety symptoms in Iranian samples. However, further psychometric evaluation of the questionnaire is recommended.

Abstract:Introduction: So far, the psychometric properties of the Persian version of the Pain Anxiety Symptom Scale have not been assessed in Iran. Therefore, this study was conducted to determine the validity and reliability of the Persian version of the Pain Anxiety Symptom Scale among a group of Iranian patients in Amol. Methods: This methodological study was conducted in 2023 with a sample of 400 postoperative patients from Amol, Iran, selected through convenience sampling. The dataset was divided into two groups of 200 for exploratory and confirmatory factor analyses. Construct validity was assessed using maximum likelihood exploratory factor analysis with Promax rotation, supported by Horn's parallel analysis and network analysis to visualize item relationships. Confirmatory factor analysis, convergent and discriminant validity was performed on the second dataset. Reliability was evaluated through various statistical measures, including Cronbach's alpha, McDonald's omega, average inter-item correlation coefficient, composite reliability, and maximal reliability (MaxR). Results: Among the 400 participants, the mean age was 44.38 years (SD = 13.49), with 152 (46.1%) being women and 178 (53.9%) men. Most participants (n = 268, 81.2%) had an education level lower than a diploma, and 93 individuals (28.2%) reported a history of surgery. The results of exploratory factor analysis with Promax rotation developed two factors accounting for 66.29% of the variance comprising 15 items. Also, after necessary modifications during confirmatory factor analysis, the final model was approved. As for reliability, the Cronbach's alpha, composite reliability, and MaxR for all constructs were greater than 0.7, demonstrating good internal consistency and construct reliability. Conclusion: According to the results, the Persian version of the Pain Anxiety Symptom Scale has a valid structure and acceptable reliability. This scale can be used by health professionals in many ways.

Abstract:Background Suboptimal Health Status Question-naire-25 (SHSQ-25) is an established tool for measuring a precision health state between health and illness. The present study aims to assess the valid-ity and reliability of a Persian version of SHSQ-25 (P-SHSQ-25) in a university staff Iranian population.Methods A sample of 316 academic and supporting staff (163 males, age range from 23 to 64 years old) from Hamadan University of Medical Sciences, Hamadan, Iran was recruited in this population-based cross-sectional study with a ques-tionnaire validation from Apri1 to October 2022. Forward-backward translation method was per-formed for the SHSQ-25 translation from English to Persian. Internal reliability, content, conver-gence, discriminative and construct validity of the P-SHSQ-25 were examined. The factorial structure of the P-SHSQ-25 across groups was examined us-ing measurement invariant test.Results In the translation process, the conceptual equivalence of the P-SHSQ-25 with the English ver-sion was confirmed. The item-content validity index and content validity ratio of all P-SHSQ-25 items were higher than the cut-off values of 0.70 and 0.62, respectively. Cronbach's alpha was higher than 0.70 for all P-SHSQ-25 domains. The confirmatory factor analysis (CFA) showed the fitness of five factors on the data set (comparative fit index = 0.88, and root mean square error of approximation = 0.07). The CFA model fit did not change substantially across sex, age, occupation, economic status, and body mass index (Delta comparative fit index (CFI)<0.01).Conclusions The P-SHSQ-25 can be used as a re-liable and valid tool to measure health status for screening pre-chronic disease conditions in a primary care setting among Iranian population.
