Abstract:Dental wear arises from mechanical (attrition or abrasion) and chemical (erosion) factors. Despite its prevalence and clinical significance, accurately measuring and understanding its causes remain challenging in everyday practice. This one-year study with 39 participants involved comprehensive examinations and full-arch intraoral scans at the start and after 12 months. Volume loss exceeding 100 μ on each tooth's surfaces (buccal, lingual/palatine and incisal/occlusal) was measured by comparing three-dimensional scans from both time points. This study also assessed factors such as abrasion and erosion through clinical exams and questionnaires. There were no significant differences in dental wear in participants with sleep bruxism. However, noticeable wear occurred in the front teeth of those with waking bruxism and joint-related symptoms. Increased wear was associated with frequent consumption of acidic drinks, regular swimming, dry mouth, nocturnal drooling and heartburn, while no significant wear was found in patients with reflux. The used methodology proved effective in accurately assessing the progression of dental wear, which is important as many patients may initially be asymptomatic. The variability observed in dental wear patterns underscores the need to develop specific software applications that allow immediate and efficient comparison of wear areas based on extensive analysis of patient databases.

Automated monitoring of tooth wear progression using AI on intraoral scans

Abstract:Objectives: This study aimed to develop and evaluate a fully automated method for visualizing and measuring tooth wear progression using pairs of intraoral scans (IOSs) in comparison with a manual protocol. Methods: Eight patients with severe tooth wear progression were retrospectively included, with IOSs taken at baseline and 1-year, 3-year, and 5-year follow-ups. For alignment, the automated method segmented the arch into separate teeth in the IOSs. Tooth pair registration selected tooth surfaces that were likely unaffected by tooth wear and performed point set registration on the selected surfaces. Maximum tooth profile losses from baseline to each follow-up were determined based on signed distances using the manual 3D Wear Analysis (3DWA) protocol and the automated method. The automated method was evaluated against the 3DWA protocol by comparing tooth segmentations with the Dice-Sørensen coefficient (DSC) and intersection over union (IoU). The tooth profile loss measurements were compared with regression and Bland-Altman plots. Additionally, the relationship between the time interval and the measurement differences between the two methods was shown. Results: The automated method completed within two minutes. It was very effective for tooth instance segmentation (826 teeth, DSC = 0.947, IoU = 0.907), and a correlation of 0.932 was observed for agreement on tooth profile loss measurements (516 tooth pairs, mean difference = 0.021mm, 95% confidence interval = [-0.085, 0.138]). The variability in measurement differences increased for larger time intervals. Conclusions: The proposed automated method for monitoring tooth wear progression was faster and not clinically significantly different in accuracy compared to a manual protocol for full-arch IOSs. Clinical significance: General practitioners and patients can benefit from the visualization of tooth wear, allowing quantifiable and standardized decisions concerning therapy requirements of worn teeth. The proposed method for tooth wear monitoring decreased the time required to less than two minutes compared with the manual approach, which took at least two hours.

Abstract:Objectives: To evaluate the efficacy of the True Definition®intraoral scanner in quantifying the wear of the glass ionomer restorative material (Ketac TM Universal and Ketac TM Molar) over 1 year. We also studied the correlation between visual and digital assessments of restoration wear. Materials and Methods: This was a clinical follow-up study of a post-marketed material with a prospective, controlled, randomised, split-mouth, and blinded assessment design. Intraoral optical impression and visual assessment were carried out over three appointments over 12 months, starting with 36 patients.Results: According to the visual indices, all restorations in this study were clinically healthy. However, in the digital measurement of wear, 94.74% and 94.44% of the restorations during the T0-T6 and T6-T12 observation periods, respectively, showed deterioration greater than 41 microns. Moreover, in the analysis of agreement between measurement techniques, no agreement was obtained in the two analysed time periods: T0-T6 yielded a kappa (k) value of 0.000, and T6-T12 yielded k=0.0030. Discordant results were obtained in the correlation analysis. In T0-T6, the results were not considered statistically significant (p=0.838); however, the results obtained during T6-T12 showed a correlation (p-value <0.001 ).Conclusions: The wear of dental materials as observed by the human eye did not agree with that observed by intraoral scanning. The scanner effectively measures wear, detecting details that are beyond the capability of the human eye and conventional photographs. The surface deterioration of the restorations at both observation times can be considered non-physiological, potentially leading to premature occlusal alterations and accelerated physiological ageing.Clinical Relevance: Early diagnosis is crucial for avoiding alterations in the function of the stomatognathic system due to the wear of dental restorations. Additionally, since most of the tools applied are qualitative in nature, such as visual inspection, it is essential to find a standardised and precise tool that offers diagnosis, monitoring, and records of the evolution of tooth wear. Abstract:In recent years, there has been an increase in the incidence of dental wear; thus, an early diagnosis is important. Conventional methods of diagnosis are based primarily on the visual abilities of the dentist, and therefore the use of new technologies for the detection of dental wear may be very useful. The aim of the study was to analyze the sensitivity and specificity of the intraoral scanner for measuring dental wear, as well as to evaluate patients’ satisfaction with the use of the scanner. The study was conducted with 46 volunteers who underwent three intraoral analyses: a first baseline scanning, a second scanning after 6 months and a final scanning after one year performed by four operators divided into two groups. One of the operators performed the visual analysis of dental wear, and the other performed the analysis using the intraoral scanner 3MTM True Definition intraoral scanner (ESPE, Seefeld, Germany). The data obtained from the intraoral scanner showed levels of specificity and sensitivity that enable the intraoral scanner to be used as a diagnostic tool in the assessment of tooth wear. The participants also showed a high degree of satisfaction with the scanner as a communication tool.

Abstract:In 30 patients (average age 38 ± 8 years, 77% male, 23% female) with intra-oral scans made at intake and after 3 years, tooth wear progression was measured. With the aid of GeoMagic to superimpose the scans, the maximum difference in height of 64 surfaces was measured per surface. A large variation was found in progression rates between patients, between various teeth in a single mouth, and between surfaces on a single tooth. Tooth wear progression rates are therefore highly individual and can even be very localized. Treatment must therefore be individualized, with an essential role for measuring tooth wear when deciding on the need for restorative treatment.

Abstract:Intraoral scanners are widely used in a clinical setting for orthodontic treatments and tooth restorations, and are also useful for assessing dental wear and pathology progression. In this study, we assess the utility of using an intraoral scanner and associated software for quantifying dental tissue loss in non-human primates. An upper and lower second molar for 31 captive hamadryas baboons (Papio hamadryas) were assessed for dental tissue loss progression, giving a total sample of 62 teeth. The animals are part of the Southwest National Primate Research Center and were all fed the same monkey-chow diet over their lifetimes. Two molds of each dentition were taken at either two- or three-year intervals, and the associated casts scanned using an intraoral scanner (Medit i700). Tissue loss was calculated in WearCompare by superimposition of the two scans followed by subtraction analysis. Four individuals had dental caries, and were assessed separately. The results demonstrate the reliability of these techniques in capturing tissue loss data, evidenced by the alignment consistency between scans, lack of erroneous tissue gain between scans, and uniformity of tissue loss patterns among individuals (e.g., functional cusps showing the highest degree of wear). The average loss per mm2 per year for all samples combined was 0.05 mm3 (0.04 mm3 for females and 0.08 mm3 for males). There was no significant difference in wear progression between upper and lower molars. Substantial variation in the amount of tissue loss among individuals was found, despite their uniform diet. These findings foster multiple avenues for future research, including the exploration of wear progression across dental crowns and arcades, correlation between different types of tissue loss (e.g., attrition, erosion, fractures, caries), interplay between tissue loss and microwear/topographic analysis, and the genetic underpinnings of tissue loss variation.

Twenty-four months in vivo wear of enamel antagonists to lithium disilicate implant crowns - a pilot study

Abstract:Objectives: To evaluate whether there is a significant difference between the ratio of wear of lithium disilicate implant crowns and their enamel antagonists and adjacent enamel/enamel antagonist contacts. Additionally, the movement of the adjacent teeth was determined. Materials and methods: Intraoral scans of the occlusal surfaces of 41 patients were performed after insertion of the implant restoration (baseline), after 12 and 24 months. From the initial study cohort, eleven implant crown/enamel antagonist pairs with adjacent enamel antagonist pairs could be analyzed in nine patients after 12 months. After 24 months, ten implant crowns of eight patients were available due to one decementation of an implant crown. A semiautomatic specifically developed iterative closest point algorithm was used to superimpose the surfaces of the baseline scan with the follow-up scans to measure the maximum height loss of each identified wear side. The mean maximum tooth wear and the standard deviation (SD) were calculated per unit. The ratio of wear of enamel/enamel antagonist teeth and the wear ratio of adjacent lithium disilicate implant crowns and their enamel antagonist surfaces were evaluated. Tooth movements were described by translations and rotations using a separate measurement procedure. The surfaces of the scans were superimposed setting the implant as a reference structure for registration. Results: After 12 months the wear ratio between the enamel antagonist pairs was 0.95 ± 0.27 µm and the ratio between lithium disilicate/enamel was 0.73 ± 0.49 µm. After 24 months the ratio of enamel antagonist pairs was 1.04 ± 0.28 µm and the ratio of lithium disilicate/enamel was 0.73 ± 0.51 µm. The ratios did not differ significantly. Conclusions: Intraoral scanning and computer analysis showed that the two-years wear ratios between enamel/enamel and enamel/lithium disilicate implant crowns did not differ significantly. Statement of clinical relevance: The methodology described in this study could measure tooth wear and detect long-term wear performance. In the future, digital monitoring of prosthetic restorations should be integrated into clinical workflow to identify potential factors affecting longevity.
Occlusal caries detection and monitoring using a 3D intraoral scanner system. An in vivo assessment

Abstract:Objective To assess the agreement in detecting and monitoring occlusal caries over thirty months using conventional visual and radiographic assessment and an intraoral scanner system which supports automated caries scoring. Methods Ninety-one young participants aged 12-19 years were included in the study. All occlusal surfaces were examined visually, radiographically (when indicated), and scanned with the TRIOS 4 intraoral scanner. TRIOS Patient Monitoring software (vers. 2.3, 3Shape TRIOS A/S, Denmark) was used for automated caries detection on the 3D digital models. Results Fifty-five of the study participants were re-examined after 30-months. Significant differences regarding caries detection were found between the conventional methods and the automated caries scoring system (p<0.01), with moderate positive (49-61%), and high negative agreements (87-98%). All methods reported significant caries progression over the follow-up period (p<0.01). However, the automated system showed significantly more caries progression than the other methods (p<0.01). Conclusions The software for automated caries detection and classification showed moderate positive and strong negative agreements with the conventional methods considering baseline and follow-up. The automated system detected significantly fewer caries lesions and tended to underestimate the caries severity. All methods indicated significant caries progression over the follow-up period, while the automated system detected more caries progression. Clinical Significance The TRIOS system supporting automated occlusal caries detection and classification can assist in detecting and monitoring occlusal caries on permanent teeth as a complementary tool to the conventional methods. However, the operator should be aware that the automated system showed a tendency to underestimate the caries presence and lesion severity.

Abstract:Dental wear can be differentiated into different types on the basis of morphological and etiological factors. The present research was carried out on twelve extracted human teeth with dental wear (three teeth showing each type of wear: erosion, attrition, abrasion, and abfraction) studied by scanning electron microscopy (SEM). The study aimed, through analysis of the macro- and micromorphological features of the lesions (considering the enamel, dentin, enamel prisms, dentinal tubules, and pulp), to clarify the different clinical and diagnostic presentations of dental wear and their possible significance. Our results, which confirm current knowledge, provide a complete overview of the distinctive morphology of each lesion type. It is important to identify the type of dental wear lesion in order to recognize the contributing etiological factors and, consequently, identify other more complex, nondental disorders (such as gastroesophageal reflux, eating disorders). It is clear that each type of lesion has a specific morphology and mechanism, and further clinical studies are needed to clarify the etiological processes, particularly those underlying the onset of abfraction.

Abstract:Background Three-dimensional scans are increasingly used to quantify biological topographical changes and clinical health outcomes. Traditionally, the use of 3D scans has been limited to specialized centers owing to the high cost of the scanning equipment and the necessity for complex analysis software. Technological advances have made cheaper, more accessible methods of data capture and analysis available in the field of dentistry, potentially facilitating a primary care system to quantify disease progression. However, this system has yet to be compared with previous high-precision methods in university hospital settings. Objective The aim of this study was to compare a dental primary care method of data capture (intraoral scanner) with a precision hospital-based method (laser profilometer) in addition to comparing open source and commercial software available for data analysis. Methods Longitudinal dental wear data from 30 patients were analyzed using a two-factor factorial experimental design. Bimaxillary intraoral digital scans (TrueDefinition, 3M, UK) and conventional silicone impressions, poured in type-4 dental stone, were made at both baseline and follow-up appointments (mean 36 months, SD 10.9). Stone models were scanned using precision laser profilometry (Taicaan, Southampton, UK). Three-dimensional changes in both forms of digital scans of the first molars (n=76) were quantitatively analyzed using the engineering software Geomagic Control (3D Systems, Germany) and freeware WearCompare (Leeds Digital Dentistry, UK). Volume change (mm3) was the primary measurement outcome. The maximum point loss (μm) and the average profile loss (μm) were also recorded. Data were paired and skewed, and were therefore compared using Wilcoxon signed-rank tests with Bonferroni correction. Results The median (IQR) volume change for Geomagic using profilometry and using the intraoral scan was –0.37 mm3 (–3.75-2.30) and +0.51 mm3 (–2.17-4.26), respectively (P<.001). Using WearCompare, the median (IQR) volume change for profilometry and intraoral scanning was –1.21 mm3 (–3.48-0.56) and –0.39 mm3 (–3.96-2.76), respectively (P=.04). WearCompare detected significantly greater volume loss than Geomagic regardless of scanner type. No differences were observed between groups with respect to the maximum point loss or average profile loss. Conclusions As expected, the method of data capture, software used, and measurement metric all significantly influenced the measurement outcome. However, when appropriate analysis was used, the primary care system was able to quantify the degree of change and can be recommended depending on the accuracy needed to diagnose a condition. Lower-resolution scanners may underestimate complex changes when measuring at the micron level.

[Preliminary Study of Tooth Measured by a Noncontact 3D Image Survey System].

Abstract:OBJECTIVE:To perform the quantitative study of tooth occlusal surface wear by a noncontact 3D image survey system.METHODS:Thirty-two old people's complete dental casts in five years which had normal occlusion were studied. The same way was used to determine the individual reference surface, and the range of the maxillary first molar occlusal surface was delimited by the unite regulation. Then the quantity of wear in five years and wear rate of maxillary first molar occlusal surface were measured by the noncontact 3D image survey system.RESULTS:Wear of maxillary first molar occlusal surfaces was as follows: the mean height loss was 0.293 mm in five years and the mean wear rate was 0.059 mm/year; the mean volume loss was 97.881 mm(3) in five years and the mean wear rate was 19.576 mm(3)/year.CONCLUSIONS:Under the necessary conditions, the noncontact 3D image survey system is an effective method to measure the wear of dental occlusal surface, but some set-up conditions need to be improved further.
Cervical wear pathobiology by robot-simulated 3-year toothbrushing – New methodological approach

Abstract:OBJECTIVES An ex-vivo study was aimed at (i) programming clinically validated robot three-year random toothbrushing, (ii) evaluating cervical macro- and microwear patterns on all tooth groups of different functional age, (iii) documenting and codificating wear related morphological features at the cemento-enamel junction in young teeth and on roots in older teeth. DESIGN Following ethical approval random toothbrushing (44 strokes per tooth horizontally, rotating, vertically; 2x/d) with manual toothbrushes and low-abrasive dentifrice was performed in an artificial oral cavity with brushing-force 3.5 N on 14 extracted human teeth. Morphological features were examined by SEM at baseline and after simulated 3 years using the replication technique. 3D-SEM analyses were carried out with a four-quadrant back scattered electron detector. Wilcoxon-Mann-Whitney-test was used for statistical analyses. RESULTS 3-year random toothbrushing with horizontal, rotating and vertical brushing movements revealed morphological features classified as four enamel patterns, one dentin pattern and three cervical patterns. Negative impacts were enamel, cementum and dentin loss. Positive impact on oral health was removing dental calculus and straightening cervical traumatic and iatrogenic damages. The volume loss varied from x̅=34.25nl to x̅=87.75nl. Wear extended apically from 100 to 1500 micrometres. CONCLUSION Robot simulated toothbrushing in an artificial oral cavity, with subsequent SEM and 3D-SEM assessment, elucidated both negative and oral health-contributing micromorphology patterns of cervical wear after simulated 3-year random toothbrushing. Cervical macro- and microwear of cementum revealed, for the first time, what we describe as overhanging enamel peninsulas and enamel islands on roots in young teeth, but no enamel islands on roots from older teeth after root cementum loss. In contrast, many older teeth exhibited enamel peninsulas.

Minimally Invasive Adhesive Rehabilitation for a Patient With Tooth Erosion: Seven-year Follow-up

Abstract:Tooth wear is a multifactorial condition of growing concern. In clinical practice, it is often a challenge for prevention and treatment since many etiological factors may be involved. This case report describes an esthetic rehabilitation of a young patient presenting tooth wear due to erosion. The etiological factor of this case was the patient sucking on lemons, an acidic fruit. The main complaint of the patient was the appearance of short maxillary incisors. The treatment involved orthodontic and restorative dentistry techniques. First of all, an orthodontic device was used to increase the vertical dimension of occlusion and create an adequate space for the direct restorations. The posterior teeth were restored with two direct composite resin techniques; the anterior teeth were then restored using a balanced occlusion. The seven-year follow-up of the case is presented. Replicas of the restorations were made and visualized under scanning electron microscopy up to the 12-month evaluation. In the clinical follow-up at seven years, maintenance of the results and restorations can be clearly seen.
In vivo evaluation of the enamel wear of primary molar against four types of crowns using the intra-oral scanner

3D Method for Occlusal Tooth Wear Assessment in Presence of Substantial Changes on Other Tooth Surfaces

Abstract:Early diagnosis and timely management of tooth or dental material wear is imperative to avoid extensive restorations. Previous studies suggested different methods for tooth wear assessment, but no study has developed a three-dimensional (3D) superimposition technique applicable in cases where tooth surfaces, other than the occlusal, undergo extensive morphological changes. Here, we manually grinded plaster incisors and canines to simulate occlusal tooth wear of varying severity in teeth that received a wire retainer bonded on their lingual surfaces, during the assessment period. The corresponding dental casts were scanned using a surface scanner. The modified tooth crowns were best-fit approximated to the original crowns using seven 3D superimposition techniques (two reference areas with varying settings) and the gold standard technique (GS: intact adjacent teeth and alveolar processes as superimposition reference), which provided the true value. Only a specific technique (complete crown with 20% estimated overlap of meshes), which is applicable in actual clinical data, showed perfect agreement with the GS technique in all cases (median difference: −0.002, max absolute difference: 0.178 mm3). The outcomes of the suggested and the GS technique were highly reproducible (max difference < 0.040 mm3). The presented technique offers low cost, convenient, accurate, and risk-free tooth wear assessment.

Effects of remote digital monitoring on oral hygiene of orthodontic patients: a prospective study

Abstract:Abstract Background Remote digital monitoring during orthodontic treatment can help patients in improving their oral hygiene performance and reducing the number of appointments due to emergency reasons, especially in time of COVID-19 pandemic where non-urgent appointments might be discouraged. Methods Thirty patients scheduled to start an orthodontic treatment were divided into two groups of fifteen. Compared to controls, study group patients were provided with scan box and cheek retractor (Dental Monitoring®) and were instructed to take monthly intra-oral scans. Plaque Index (PI), Gingival Index (GI), and White Spot Lesions (WSL) were recorded for both groups at baseline (t 0 ), every month for the first 3 months (t 1 , t 2 , t 3 ), and at 6 months (t 4 ). Carious Lesions Onset (CLO) and Emergency Appointments (EA) were also recorded during the observation period. Inter-group differences were assessed with Student's t test and Chi-square test, intra-group differences were assessed with Cochran’s Q-test (significance α = 0.05). Results Study group patients showed a significant improvement in plaque control at t 3 ( p = 0.010) and t 4 ( p = 0.039), compared to control group. No significant difference was observed in the number of WSL between the two groups. No cavities were detected in the study group, while five CLO were diagnosed in the control group ( p = 0.049). A decreased number of EA was observed in the study group, but the difference was not significant. Conclusions Integration of a remote monitoring system during orthodontic treatment was effective in improving plaque control and reducing carious lesions onset. The present findings encourage orthodontists to consider this technology to help maintaining optimal oral health of patients, especially in times of health emergency crisis.

Five-year study of mandibular overdentures on stud abutments: Clinical outcome, patient satisfaction and prosthetic maintenance-Influence of bone resorption and implant position

Abstract:Objectives: This prospective study analyses the 5-year clinical outcome, patient satisfaction and the technical outcome of a 2-implant overdenture on stud abutments. In addition, the influence of mandibular resorption and implant position on these outcomes was assessed. Material and methods: Patients received two implants in a one-stage delayed protocol. Final prosthesis on stud abutments was finalized after 3 months. Implant survival, bone loss, plaque and sulcus bleeding index, OHIP-14, technical outcome and costs were assessed after 5 years. The Cawood-Howell classification defined the mandibular resorption. Inter-implant distances and angles (frontal, sagittal) were digitally measured. The significance level was p < .05. Results: Fifty-six patients, 23 female and 33 males (mean age = 66.3, range 41-82), completed the follow-up. No implants were lost. Radiographical crestal bone loss was on average 1.25 mm (standard deviation SD 1.06), bleeding index was 0.75 (SD 0.73), and plaque index was 1.15 (SD 0.75) and higher for low mandibles (p = .001). The OHIP-14 reduction post-connection and after 5 years was significant (p < .001). Five-year maintenance required on average 6.7 (SD 4.8, range 0-25) interventions per patient. A larger inter-implant distance was associated with fewer replacement of retention inserts (p = .034) and less interventions (p = .006). Larger frontal (p = .023) and sagittal (p = .046) inter-implant angles (non-parallelism) required more inserts. The maintenance cost negatively influences the patient satisfaction (p = .004). 83% of the stud abutments showed wear. Conclusion: The implant overdenture on cylindrical stud abutments is a good and stable solution, irrespective of the resorption profile of the mandible. OHIP-14 improved permanently up to 5 years but is negatively influenced by complication cost. A small inter-implant distance, more frontal, sagittal inter-implant divergence increased maintenance.
