Abstract:Thymomas are rare, and information regarding their surgical outcomes and possible prognostic factors is limited. In this study, we aimed to determine the clinicopathological characteristics of thymoma and estimate independent predictors of both overall and disease-free survival in thymoma patients. We carried out a retrospective review of the clinicopathological characteristics and prognostic factors in 761 consecutive patients with pathologically confirmed thymoma treated in Shanghai Chest Hospital between January 2001 and December 2011. Survival was calculated using the Kaplan-Meier method and evaluated with log-rank tests. Multivariable analysis was performed using the Cox regression model. Complete follow-up information was available for 544 patients. The overall survival rate was 92.8% at 5 years and 90.5% at 10 years. The 5- and 10-year disease-free survival was 87.9 and 82.1%, respectively. On multiple Cox regression analysis, the Masaoka-Koga clinical stage [odds ratio (OR), 2.057; 95% confidence interval (CI), 1.454-2.911; P < 0.01] and sex (OR, 2.244; 95% CI, 1.115-4.519; P = 0.02) were found to be independent predictors of overall survival. The Masaoka-Koga clinical stage (OR, 2.127; 95% CI, 1.487-3.042; P < 0.01) and completeness of resection (OR, 2.935; 95% CI, 1.410-6.109; P < 0.01) predicted disease-free survival. The four-tiered Masaoka-Koga clinical stage is the most important prognostic factor, predicting not only overall survival but also disease-free survival after thymoma resection. Completeness of resection predicts disease-free survival, and the World Health Organization histological classification may not have significant prognostic implications.

Abstract:Thymoma is the most common tumor in the anterior mediastinum, the preferred treatment for thymoma is surgical resection. The current standard operation is extended thymectomy, which includes the complete removal of the thymus and mediastinal fat tissue. Commonly used surgical method including the sternum split, video-assisted thoracoscope surgery and robot assisted thoracoscope surgery. As the split in the middle of their chest surgery because of its large traumatic, now has become less in clinical application. With the development of minimally invasive technology of thoracoscope, by the side of the chest and xiphoid process down into road thoracoscope surgeries are more used clinically, while the robotic surgery is only part of the hospital. However, controversies arose regarding which surgical method is more advantageous.

Abstract:Objective To explore the results of surgical treatment for thymoma,and to investigate the influence of pathological predictors on long-term survival.Methods The clinical and pathologic features of 104 cases from 2000 to 2009 underwent surgical treatment for thymoma were reviewed.The prognostic factors against postoperative survival were analyzed.Results The rate of complete resection was 92.3%.The morbidity was 3.8% and no death happened perioperatively.Thymomas were staged according to the Masaoka system as 26.9% stage Ⅰ,40.4% stage Ⅱ,28.8% stage Ⅲ,3.8% stage Ⅳa and classified according to the World Health Organization system as 3.8% type A,8.6% typeAB,22.1% type B1,43.3% type B2,17.3% type B3,5.8% type C.Mean followup was 62.3±18.5 months.The overall 1-,3- and 5-yearsurvival rates were 98.1%,88.5 and 82.3%,respectively.The Masaoka staging and the World Health Organization classification significantly influenced the long-term survival.Stage Ⅲ,Ⅳa,type B3 and type C were related with worse prognosis.Conclusions Surgical method is safe and effective in the treatment of thymoma.The clinical staging and the histologic classification influence long-term survival.

Abstract:The efficacy and safety of subxiphoid thoracoscopic thymectomy (SVATS) for early thymoma are unknown. The purposes of this meta-analysis were to evaluate the effectiveness and safety of SVATS for early thymoma, to compare it with unilateral intercostal approach video thoracoscopic surgery (IVATS) thymectomy, and to investigate the clinical efficacy of modified subxiphoid thoracoscopic thymectomy (MSVATS) for early anterior mediastinal thymoma.

Abstract:OBJECTIVES: Surgeons at different institutions worldwide choose different types of operations for thymic tumours. It is not known whether these differences affect the outcomes of the patients.METHODS: A total of 1430 patients with Masaoka-Koga pathological Stage I-II thymic tumours without myasthenia gravis or pre-treatment were identified from the International Thymic Malignancy Interest Group retrospective database. Outcomes of patients from 3 major continents (Europe, North America and Asia) were compared.RESULTS: Patients from the 3 continents were comparable in gender and performance status. More European patients had more paraneoplastic syndromes; North American patients had the smallest tumour sizes and less adjuvant therapy; and Asian patients were younger and had more Stage I disease but higher grade tumours. Partial thymectomy was performed more often in Asian patients (31.7%) than in European (2.4%) and North American (5.4%; P < 0.001) patients. The median approach (sternotomy/clamshell) was the major approach in Europe (75.3%) and North America (76.6%). In contrast, the median approach was applied significantly less frequently in Asia (45.6%, P < 0.001); unilateral open (thoracotomy/hemi-clamshell, 23.3%) and minimally invasive approaches (video-assisted thoracoscopic surgery/robot, 31.1%) were used more often with similar rates of complete resection. The 10-year overall survival rate was 82% for Europe, 78% for North America and 90% for Asia (P = 0.005), respectively. The 10-year cumulative recurrence rates were similar among the geographic groups (European 0.08, North American 0.07, and Asian 0.06, P = 0.61). Age was the only independent predictive factor for overall survival (P < 0.001, HR = 1.089, 95% CI 1.056-1.123) in multivariable analysis. Types B3 and thymic carcinoma (P = 0.003, HR = 3.932, 95% CI 1.615-9.576) were independent risk factors for increased recurrence.CONCLUSIONS: This study shows that the selection of the surgical approach and the extent of resection for Stage I and II thymic tumours differ by geographic region. However, these differences seem to have little impact on outcomes.

Abstract:Background. Thymic epithelial tumors consist of a series of neoplasm that differ morphologically and biologically. Management strategy for these tumors remains controversial. Methods. We retrospectively reviewed 204 thymic epithelial tumors surgically treated during the period of 1970 to 1995, and we reexamined the histologic specimens using the current World Health Organization classification. Results. One hundred eighty patients (88.2%) underwent complete resections, 17 (8.3%) received partial resections, and 7 (3.4%) received biopsies only. The complete resection rate of stages I and II tumors was significantly higher than stages III and IV tumors (98.2% vs 76.6%; p < 0.001). Twenty-four patients (11.8%) experienced 29 events of complications postoperatively, including 11 events of myasthenia gravis crisis. Myasthenia gravis, partial resection or biopsy, and stages III and IV were independent risk factors for postoperative complications. Fifteen patients (7.4%) died during hospital stays, including 6 patients from myasthenia crisis. Partial resection or biopsy and myasthenia gravis were independent risk factors for postoperative mortality. There were significantly more stage I and stage II cases in histologic types A, AB, and B1 tumors than in B2, B3, and C tumors (87.6% vs 26.4%; p < 0.001), and their complete resection rate was significantly higher than the latter group (98.9% vs 78.3%; p < 0.001). The 5-year and 10-year survival rates were 63.2% and 50.4%, respectively. Masaoka stages III and IV, histologic types B2, B3, and C, and incomplete resection were independent risk factors for poor prognosis. Conclusions. Complete resection remains the hope of cure for thymic epithelial tumors. The treatment strategy should be based on the current World Health Organization histologic classification and the Masaoka staging system.

Abstract:Background. Thymic squamous cell carcinomas are rare malignancies with limited information regarding results of surgery therapy. The objective of the current study was to evaluate their clinical and pathologic features and possible prognostic factors. Methods. From February 2003 to December 2010, 105 consecutive surgical patients were enrolled in this study with pathologic confirmed thymic squamous cell carcinoma in Shanghai Chest Hospital. Clinical and pathologic data were retrospectively reviewed. Survival analysis was performed using the Kaplan-Meier and log rank tests. Multivariate analysis was performed using the Cox regression model. Results. The squamous cell subtype made up 73.4% of the thymic carcinomas and 16.1% of total thymic tumors. Four patients (3.8%) presented with myasthenia gravis. The 5-year overall survival rate and the disease-free survival rate were 59.5% and 57.6%, respectively. On multivariate analysis, only the completeness of resection (hazard ratio 3.692; 95% confidence interval: 1.827 to 7.464; p = 0.001) was found to have significant impact on overall survival. Completeness of resection (hazard ratio 2.361; 95% confidence interval: 1.226 to 4.547; p = 0.010) and chemotherapy (hazard ratio 0.307; 95% confidence interval: 0.134 to 0.704; p = 0.005) were associated with disease-free survival. Conclusions. Complete resection not only predicts better overall survival but also extends disease-free interval after resection of thymic squamous cell carcinoma. Our results provide additional evidence supporting an alternative clinical stage specific for thymic carcinoma. (C) 2013 by The Society of Thoracic Surgeons
Surgical Approach for Stage Ⅳ A Thymoma-a Clinical Experience with 7 Cases

Abstract:Objective To discuss the safety and the outcome of surgical treatment for patient with ⅣA thymoma. Methods Seven patients with stage ⅣA thymoma were treated surgically between Jan. 2007 and Dec. 2014. Clinical data and follow-up information were collected and analyzed. Results The median age of the patients was 38 years old (range 27-57 years). There were four male patients. Tumor was located in the left thoracic cavity in four patients. In 5 patients it was synchronous primary thymoma (Stage ⅣA), and in 2 patients it was a recurrence after initial thymoma resection. Mean operation time was 10.4±2.8 hours, median blood loss was 1000(300-8200)ml, median postoperative stay in hospital was 14 (10-54) days. There was no postoperative death. Major postoperative complication occurred in 3 patients (42.9%). Minor postoperative complication occurred in 5 patients (71.4%). A mean of twelve lymph nodes (range 4-47) were resected, and lymph node metastasis was found in one patient. Median follow-up time was 24(5-81) months, and all patients except one survived without evidence of recurrence. One patient died of recurrence of tumor 81 months after surgery. Conclusions The recommended therapy for ⅣA stage thymoma is multidisciplinary, including radiation, chemotherapy, and surgery. In selected patient with stage ⅣA thymoma, surgical resection is a safe and effective procedure with a long survival time. Abstract:Thymoma and thymic carcinoma are the most common tumors of the anterior mediastinum and a relatively rare type of thoracic cancer. The prerequisite for surgery is clinical staging and operative evaluation, both of which are based on medical imaging. The best strategy for treating a thymic epithelial tumor is surgical resection of the organ and surrounding tissue. Thymectomy modalities vary, including open surgery and minimally invasive surgery, and surgeons have used various innovations to better meet the needs of the procedure; therefore, it is critical to select the appropriate procedure based on the patient's characteristics. Evaluation of resectability is the first step of surgical resection for thymic tumors without distant metastasis. The decision regarding unresectability should be made carefully. During subsequent chemotherapy or chemoradiotherapy, reevaluation of whether an area is resectable or not remains essential. Despite numerous technological advances in the surgical treatment of thymic tumors, several contentious issues remain, including the selection of surgical approaches for difficult cases, the selection of video-assisted thoracoscopic approaches, the evaluation of resectability, minimally invasive surgery for locally advanced thymic tumors, lymphadenectomy in thymic tumors, neoadjuvant therapy for thymic tumors, debulking surgery, and salvage surgery. In solving these problems, the surgeon's judgment, surgical experience, and surgical skills are especially important.

Trans-sternotomy Versus Video-Assisted Thoracic Surgery for Early-Stage Thymoma Patients: a Meta-Analysis.

Abstract:Background Over the years, video-assisted thoracic surgery (VATS) thymectomy has progressively replaced trans-sternotomy (TS) in early-stage thymoma (Masaoka stage I and stage II). This meta-analysis aimed to confirm the differences in the efficacies of VATS and TS approaches in early-stage thymoma patients. Methods A thorough literature search of the following online databases was performed: PubMed, Cochrane Library, Web of Science, and EMBASE. Appropriate search terms, such as "thymoma or thymus neoplasms or Thymic Carcinoma" and "Video-Assisted Thoracic Surgeries or Video-Assisted Thoracoscopic", were used with MeSH search methods. Heterogeneity was assessed first with the Q-test and inconsistency index and sensitivity analysis and subgroup analysis were then used to find the source of heterogeneity. Results We retrieved 1,228 articles, 11 articles were selected as the subjects of our research, and 1,222 patients were included in the research (666 VATS cases versus 556 TS cases). VATS caused less blood loss (P=0.02), and required shorter hospital stay (P<0.001), shorter duration of chest tube drainage (P=0.03) than TS. No obvious difference was found in operative time (P=0.14), postoperative recurrence (OR =0.81, 95% CI: 0.35-1.85, P=0.613), postoperative complications (OR =0.60, 95% CI: 0.31-1.16, P=0.129) and R0 resection (OR =0.35, 95% CI: 0.12-1.04, P=0.06), but the trend showed that more patients in the TS group achieved R0 resection. Conclusions For early-stage thymoma patients, VATS thymectomy seems to provide many advantages to be considered as a legitimate alternative to TS; however, when performing VATS, surgeons should pay special attention to ensure that R0 resection is achieved.
Diagnosis and surgical treatment of 135 cases of thymoma

Abstract:Objective:To summarize the experience in the diagnosis and surgical treatment of thymoma.Methods:135 patients with thymoma,surgically intervened and pathologically confirmed in Renji Hospital from Jan 2001 to Dec 2008,were retrospectively analyzed.Complete resection of the tumor was performed in 123 patients(91%),partial or incomplete resection in 12(9%).Results:One patient died after operation,123 cases were followed up,and 8 cases recurred.The 3-and 5-year survival rates were 89% and 75% respectively.By Masaoka's stage system,the 3-and 5-year survival rates were 70% and 46% respectively for stage Ⅲ and stage Ⅳ.Conclusion:The imaging examination,such as chest CT,is the main diagnostic method for thymoma.Myasthenia gravis is a common concomitant disease in patients with thymoma.Regardless of clinical staging and histology,complete surgical resection is certainly the gold standard to achieve cure,which helps to reduce the recurrence.Combination with other methods(including chemotherapy and radiotherapy)after operation may improve symptoms and prolong life span in the patients with thymomas,including stages Ⅱ,Ⅲ and Ⅳ.
A Single-Center Analysis of the Treatment and Prognosis of Patients with Thymic Carcinoma

Abstract:Background. The low incidence of thymic carcinoma has precluded the development of randomized clinical trials, and present knowledge is based on small retrospective studies. We performed this single-center retrospective analysis to evaluate the clinical characteristics, treatments, and prognosis in patients with pathologically confirmed thymic carcinoma.& para;& para;Methods. Data regarding clinicopathologic characteristics, treatment protocols, toxicities, and survival were collected from 135 patients who attended our institution between January 1980 and January 2010. Survival was assessed using the Kaplan-Meier method. Univariate and multivariate analyses were performed using the log-rank test and Cox proportional hazards model.& para;& para;Results. The 135 patients (88 men) were with a median age of 48 years, and 123 patients were diagnosed with Masaoka stage III to IV disease. R0 resection was performed in 35 patients. Treatment comprised radiotherapy in 121 patients and chemotherapy in 60. The median follow-up time was 12.5 years. At 5 and 10 years, local-regional relapse free survivals were 81.4% and 54.4%, overall survivals were 42.2% and 15.4%, progression-free survivals were 29.7% and 8.0%, and distant metastasis-free survivals were 35.9% and 25.6%, respectively. R0 resection was the only independent prognosticator of overall survival, progression-free survival, and distant metastasis-free survival in univariate and multivariate analyses.& para;& para;Conclusions. Thymic carcinoma was frequently diagnosed at Masaoka stage III to IV with a poor prognosis. Surgical resection is still the predominant treatment. Radiotherapy may increase local-regional relapse free survival with mild toxicities in advanced-stage patients. (C) 2017 by The Society of Thoracic Surgeons

Abstract:BACKGROUND To evaluate the surgical outcomes of tumor resection with or without total thymectomy for thymic epithelial tumors (TETs) using the Chinese Alliance for Research in Thymomas (ChART) retrospective database. METHODS Patients without preoperative therapy, who underwent surgery for early-stage (Masaoka-Koga stage I and II) tumors, were enrolled for the study. They were divided into thymectomy and thymomectomy groups according to the resection extent of the thymus. Demographic and surgical outcomes were compared between the two patients groups. RESULTS A total of 1,047 patients were enrolled, with 796 cases in the thymectomy group and 251 cases in the thymomectomy group. Improvement rate of myasthenia gravis (MG) was higher after thymectomy than after thymomectomy (91.6% vs. 50.0%, P<0.001). Ten-year overall survival was similar between the two groups (90.9% after thymectomy and 89.4% after thymomectomy, P=0.732). Overall, recurrence rate was 3.1% after thymectomy and 5.4% after thymomectomy, with no significant difference between the two groups (P=0.149). Stratified analysis revealed no significant difference in recurrence rates in Masaoka-Koga stage I tumors (3.2% vs. 1.4%, P=0.259). However in patients with Masaoka-Koga stage II tumors, recurrence was significantly less after thymectomy group than after thymomectomy (2.9% vs. 14.5%, P=0.001). CONCLUSIONS Thymectomy, instead of tumor resection alone, should still be recommended as the surgical standard for thymic malignancies, especially for stage II tumors and those with concomitant MG.
Comparative study of the clinical efficacy of subcostal thoracoscopy and median sternotomy in treating thymoma: a propensity score-matching analysis

Abstract:Journal of International Medical Research, Volume 52, Issue 1, January 2024. ObjectiveThis study was performed to evaluate the clinical efficacy of subcostal thoracoscopy and median sternotomy as surgical approaches for thymoma resection and lymph node dissection. The feasibility, safety, and clinical outcomes of subcostal thoracoscopy were compared with those of median sternotomy.MethodsThe clinical data of 335 patients with thymoma were retrospectively analyzed. The patients were divided into the subcostal thoracoscopy group and the median sternotomy group. Propensity score matching was performed to obtain comparable subsets of 50 patients in each group. A comparative analysis was conducted on various parameters.ResultsAll surgeries were successful, and no conversions to open thoracotomy were required in the subcostal thoracoscopy group. Significant differences in the operative time, intraoperative blood loss, chest tube drainage duration, postoperative hospital stay, patient satisfaction scores, pain assessment, and postoperative complications were observed between the two groups. However, there was no significant difference in the number of lymph nodes or lymph node stations dissected intraoperatively between the two groups.ConclusionSubcostal thoracoscopy is not inferior to median sternotomy as a surgical approach for thymoma resection and lymph node dissection. Our research provides important new comparative data on minimally invasive thymoma resection.

Abstract:Background: Thymoma is a primary tumor of the thymus, commonly located in the anterior mediastinum. Most thymomas are benign or low-grade malignant, but they can invade surrounding organs or metastasize. The primary treatment for thymoma is surgical resection. Traditional methods involve open thoracotomy, but it is traumatic, with slow recovery and many complications. In recent years, with the development of thoracoscopic techniques, thoracoscopic total thymectomy has gradually become the preferred method for small size thymomas due to its minimally invasive, safe, and effective. Methods: This paper introduces a thoracoscopic extend thymectomy technique, the subxiphoid video-assisted thoracoscopic extend thymectomy with sternal suspension. This method involves placing hooks at the upper and lower ends of the sternum to suspend the sternum upward, increasing the thoracic cavity space and facilitating thoracoscopic operations. This research reviews the clinical data of 59 patients with early-stage thymomas treated with this technique at our center since 2020 and analyzes the perioperative therapeutic efficacy and safety. It also compares the outcomes with those of 17 patients who underwent thoracoscopic approaches. Results: The results show that subxiphoid video-assisted thoracoscopic total thymectomy with sternal suspension is an innovative and effective surgical method, achieving the same tumor eradication as other thoracic surgeries. The flexible switching of observation ports provides a more comprehensive surgical field, reduces surgical trauma and complications, and improves the surgical outcomes and quality of life for patients.

Abstract:The literature describing multimodal treatment of thymomas consists almost exclusively of retrospective case series. We have used a systematic review to investigate the role of surgery in the management of thymomas. The use of surgery as the sole therapeutic maneuver in thymoma depends on the stage considered. Subtotal resection followed by adjuvant treatment may prolong survival, but studies are equivocal. Some data supports re-resection of recurrent thymoma in the belief that survival will be prolonged. Approaches to thymectomy other than sternotomy in early stage thymoma are technically sound, but long-term outcome data are lacking.

Abstract:Background: Conventionally, minimally invasive thymectomy for early-stage thymoma is performed via unilateral thoracoscopic approach. However, it is sometimes criticized for bad exposure of anterior mediastinum and contralateral area which may be disadvantageous for the surgery. Thus, we attempted a modified subxiphoid thoracoscopic approach with innovative sternal-elevating technique to reach better surgical exposure and fewer traumas.

Abstract:Objective To accumulate the experience on the diagnosis and surgical treatment of thymic tumor.Methods Seventy-three patients with thymic tumor,treated by operation in our department from 1995 to 2006,were retrospectively analyzed,which included simple thymoma resection and extended malignant thymoma and thymic carcinoma resection.Results The imaging examination,such as chest X-ray and chest-CT,was the main diagnosis method for thymic tumor.Myasthenia gravis was common concomitant disease in the patients with thymic tumor.The therapeutic effect of the operation for simple thymoma was superior to that for malignant thymoma and thymic carcinoma.Conclusion Thymic tumor is a low potential malignancy tumor,and the main treatment for thymic tumor is to resect it as far as possible.The clinical stage before operation and the combination with other methods(including chemotherapy and radiotherapy) after operation may improve symptoms and prolong life span in the patients with thymic tumor.

Abstract:Background Thymic carcinoma is a type of rare and highly malignant tumor that originates from the thymic epithelium. Treatment and prognosis of thymic carcinoma remain controversial. We retrospectively analyzed survival data from a large-sample multicenter database in China. Methods The Chinese Alliance for Research of Thymoma constructed a retrospective database of patients with thymic epithelial tumors, which enrolled 1930 patients from January 1996 to August 2013, including 329 with thymic carcinomas. In this study, we analyzed clinical, pathologic, and treatment information, measured long-term survival rates, and identified relevant prognostic factors. Results Of 329 patients, R0 resection was performed in 211 (57.7 %), R1 in 34 (9.2 %), and R2 in 84 (22.5 %). The 3-, 5-, and 10-year survival rates were 78.3, 67.1, and 47.9 %, respectively. In univariate analysis, early Masaoka-Koga stage, R0 resection, and postoperative radiotherapy were associated with better overall survival. Early Masaoka-Koga stage and postoperative radiotherapy were also associated with disease-free survival. In multivariate analyses, R0 resection, Masaoka-Koga stage, and postoperative radiotherapy were significant prognostic factors of survival. Conclusions Complete resection is the preferred primary treatment for thymic carcinoma. R0 resection, early Masaoka-Koga stage, and postoperative radiotherapy are significant predictors of improved survival.

Abstract:Objective To review the presentation, diagnosis, staging and treatment of thymoma. Data sources Data were obtained from papers on thymoma published in English within the last 30 years. No formal systematic review was conducted, but an effort was made to be comprehensive. Study selection Studies were selected if they contained data relevant to the topic addressed in the particular section. In particular, standards adopted by the International Thymic Malignancies Interest Group through a formal process of achieving worldwide consensus are featured. Because of the limited length of this article, we have frequently referenced recent reviews that contain a comprehensive amalgamation of literature rather than the actual source papers. Results Thymomas are rare malignant tumors. They account for about half (47%) of anterior mediastinal tumors. About one third of these are associated with myasthenia gravis. Computed tomography with intravenous contrast is the standard diagnostic modality. Thymomas appear as round or oval masses in early stages but irregular shapes with calcifications occurring in later stages. They can invade surrounding structures including mediastinal fat, pleura, major blood vessels and nerves. Fine needle aspiration, core needle biopsy or open biopsy is used to obtain tissue diagnosis. Masaoka-Koga classification is currently used to stage thymomas. All thymomas should be considered for resection due to their malignant potential. A complete resection is a major prognostic factor and every effort should be made to achieve this even if this means resection and reconstruction of a major thoracic structure. Median sternotomy is the standard approach for thymoma resection. A number of minimally invasive techniques are used in selective centers. While stage I and II tumors undergo primary surgery, preoperative chemotherapy appears to increase the chances of complete resection for stage III and IVa tumors. Postoperative radiation could be considered for patients with residual disease. Excellent 5 and 10-year survival rates are noted for completely resected early stage thymomas. Conclusions Thymic malignancies are rare tumors. Standards have recently been achieved to allow better communication and promote collaborative research. Surgical resection is the mainstay of treatment, but a multimodality approach is useful for many patients.
