Abstract:We thank Shah S et al for their interest in our study about a promising natural orifice transluminal endoscopic surgery (NOTES) technique, transcolonic endoscopic appendicectomy (TEA). We are happy to respond to their comments. With the development of flexible endoscope, endoscopic surgery is opening a door for NOTES. Similarly, TEA eliminates abdominal incisions and has several advantages. However, TEA has a major challenge with complex interventions relates to the technical difficulty in extraction without the assistance of laparoscope. In the procedures of our early cases, the average duration exceeded 100 min. But, with the increasing experience and proficiency, especially in the development of traction techniques and new endoscopic instruments with both electric cutting and coagulation functions, the procedure time was significantly reduced in recent cases, around 50 min. Meanwhile, the postoperative care was also improved and average hospitalisation was reduced. Traditionally, endoscopic treatment of sessile serrated lesions (SSLs) deeply involving the appendiceal orifice is challenging. In cases where the lesion greatly extends away from the appendix on the cecum, ileocecal resection or a right colectomy could be optional. Combined endoscopic mucosal resection and extended laparoscopic appendectomy could remove the lesion and retain the ileocecal valve function, but is timeconsuming, laborious and expensive. Additionally, the steps are relatively complex. Thus, TEA could be a good idea: (1) with the advantages of magnifying endoscopy, the margin of SSLs could be clearly identified and the caecal resection range is more accurate to decrease residual risks (figure 1B–C) and (2) combined endoscopic resection and TEA could be optional for large lesions. With the advancement in endoscopic technology, especially inverted observation in the abdominal cavity, endoscopy can observe the appendix lesions and surrounding conditions as clearly as laparoscopy, and perform tissue resection under direct vision (figure 1D–F). Besides, the traction technique is improved by dentalfloss and internal traction instruments, which not only reduces the procedure time, but also greatly improves the success rate. Additionally, the defect in the gastrointestinal wall can be closed by endoloopclips through a doublechannel endoscope (figure 2A–D).
