Abstract:ObjectiveThe aim of this study is to report the clinical application of noninvasive prenatal testing (NIPT) to detect chromosomal aneuploidies, especially trisomies 21, 18, and 13 in Chinese singleton pregnancies. MethodsPilot validation between NIPT with full karyotyping was conducted blindly on 306 cases. Subsequently, 7705 pregnancies were offered with NIPT. Follow-up data was obtained in all cases. ResultsIn the validation stage, a total of five NIPT positive cases were observed for trisomies 21 and 18, and results were confirmed by karyotyping; there were no cases of trisomy 13. Thus, the overall sensitivity and specificity in the validation stage was 100%. In 7705 cases, NIPT results were obtained in 7701 cases; 66 cases were classified as positive, including 48 cases of trisomy 21, 14 cases of trisomy 18, and 4 cases of trisomy 13. Subsequent karyotyping documented two false positive diagnoses for trisomies 21, 18, and 13, respectively. Sensitivity and specificity for detection of trisomies 21 and 18 and 13 were 100% and 99.9%, respectively. Additionally, prenatal chromosomal detection for pregnancies with NIPT has shown a gradual increase since its implementation. ConclusionNoninvasive prenatal testing allows a more suitable and efficient workflow for our patients' needs, together with invasive procedures allows a higher prenatal detection of chromosomal aneuploidies. Ultrasound screening for trisomy 21 syndrome at 11-13+6 gestational weeks

Abstract:Objective:To evaluate ultrasound examination(fetal nasal bone and NT) in screening for trisomy 21 syndrome at 11-13+6 gestational weeks.Methods:2342 pregnant women registered at Mother and Children's Health Care Center in our hospital were examined at 11-13+6 gestational weeks.Both fetal nasal bone and NT were evaluated by two qualified experts.The diagnosis of trisomy 21 syndrome was confirmed by cytogenetic laboratory in our hospital.Results:From Nov,2006 to Dec,2007,fetal nasal bones and NTs had been successfully examined in 2342 pregnancies with 48 pregnant women older than 35 years old(2%).Among 2342 fetuses,110 NT were moreathen >3.0 mm,2 without nasal bone.Based on Lab results,2 were diagnosed as trisomy 21 syndromes,another 2 were trisomy 18 sydrome,1 was trisomy 13 syndrome,and another 4 were congenital cardiac malformation.Among these fetuses,only one normal fetus had no nasal bone,while its NT>3.0 mm.Therefore,using "absent nasal bone" to evaluate trisomy 21 syndrome,the detection rate was 50%,and using NT>3.0 mm was 1.8%.Significant difference existed between these two methods(P<0.01).However,NT hads higher clinic significance in screening for other fetal abnormalities,with its sensitivity of 64.3%,specificity of 95.7%,PPV of 8.2%,NPV of 99.8%,and accuracy of 95.5%.Conclusion:According to our study,absent nasal bone is an important ultrasound maker in screening for trisomy 21 syndrome.Even though,NT still has higher clinic significance in screening for other abnormalities.So,when we are screening for trisomy 21 syndrome and other abnormalities,both nasal bone and NT should be taken into consideration.

Abstract:Background Absent or hypoplastic nasal bone (AHNB) on first or second‐trimester ultrasonography (USG) is an important soft marker of Down syndrome. However, due to its varied incidence in euploid and aneuploid fetuses, there is always a dilemma of whether to go for invasive fetal testing for isolated AHNB. This study aims to assess outcomes specifically within the context of Indian ethnicity women. Materials and Methods This was a prospective observational study. All patients who reported with AHNB in the first‐ or second‐trimester USG were included. Genetic counseling was done, and noninvasive and invasive testing was offered. Chromosomal anomalies were meticulously recorded, and pregnancy was monitored. Results The incidence of AHNB in our study was 1.16% (47/4051). Out of 47 women with AHNB, the isolated condition was seen in 32 (0.78%) cases, while AHNB with structural anomalies was seen in nine cases (0.22%). Thirty‐nine women opted for invasive testing. Six out of 47 had aneuploidy (12.7%), while two euploid cases (4.25%) developed nonimmune hydrops. The prevalence of Down syndrome in fetuses with AHNB was 8.5% (4/47) and 0.42% (17/4004) in fetuses with nasal bone present. This difference was statistically significant (p = .001). Conclusion The results indicate that isolated AHNB cases should be followed by a comprehensive anomaly scan rather than immediately recommending invasive testing. However, invasive testing is required when AHNB is associated with other soft markers or abnormalities. As chromosomal microarray is more sensitive than standard karyotype in detecting chromosomal aberrations, it should be chosen over karyotype.

Abstract:OBJECTIVE:To assess the clinical application of single nucleotide polymorphism microarray (SNP array) in prenatal genetic diagnosis for fetuses with absent nasal bone.METHODS:Seventy four fetuses with absent nasal bone detected by prenatal ultrasound scanning were recruited from Women's Hospital, Zhejiang University School of Medicine during June 2015 and October 2018. The chromosome karyotypes analysis and SNP array were performed. The correlation between absent fetal nasal bone and chromosome copy number variants was analyzed.RESULTS:Among 74 fetuses, 19 were detected to have chromosomal abnormalities, including 16 cases of trisomy-21, 1 case of trisomy-18 and two cases of micro-deletion/duplication. Among 46 cases with isolated absence of nasal bone, 3 had trisomy-21, and 1 had a micro-duplication. Absence of nasal bone in association with nuchal translucency thickening had a higher rate of abnormal karyotypes compared with isolated absence of nasal bone (χ2=32.27,P<0.01).CONCLUSIONS:Fetuses with absent nasal bone and nuchal translucency thickening are likely to have chromosome abnormalities, and SNP array testing is recommended to exclude the chromosome abnormalities.

Abstract:ABSTRACTObjectiveThe objective of this study is to assess the performance of noninvasive prenatal testing for trisomies 21 and 18 on the basis of massively parallel sequencing of cell‐free DNA from maternal plasma in twin pregnancies.MethodA double‐blind study was performed over 12 months. A total of 189 pregnant women carrying twins were recruited from seven hospitals. Maternal plasma DNA sequencing was performed to detect trisomies 21 and 18. The fetal karyotype was used as gold standard to estimate the sensitivity and specificity of sequencing‐based noninvasive prenatal test.ResultsThere were nine cases of trisomy 21 and two cases of trisomy 18 confirmed by karyotyping. Plasma DNA sequencing correctly identified nine cases of trisomy 21 and one case of trisomy 18. The discordant case of trisomy 18 was an unusual case of monozygotic twin with discordant fetal karyotype (one normal and the other trisomy 18). The sensitivity and specificity of maternal plasma DNA sequencing for fetal trisomy 21 were both 100% and for fetal trisomy 18 were 50% and 100%, respectively.ConclusionOur study further supported that sequencing‐based noninvasive prenatal testing of trisomy 21 in twin pregnancies could be achieved with a high accuracy, which could effectively avoid almost 95% of invasive prenatal diagnosis procedures. © 2013 John Wiley & Sons, Ltd.

Abstract:To evaluate the value of absent fetal nasal bone in the prediction of fetal chromosomal abnormalities, according to whether it was associated with other soft markers or structural abnormalities in a prescreened population of Chinese pregnant women.In this retrospective cohort study, women whose fetuses had absent nasal bone detected during the second trimester ultrasound scan were followed. Fetal karyotyping was performed and pregnancy outcomes were recorded. The association between absent fetal nasal bone with abnormal karyotype was evaluated according to whether soft markers or structural abnormalities were also observed.Fetal nasal bone was assessed in a total of 56 707 singleton pregnancies. After exclusion of unqualified cases, a total of 71 (71/56 707, 0.13%) fetuses were included in the final analyses, of which 16 (16/71, 22.54%) were detected to have chromosomal abnormalities, including 12 cases of trisomy-21, three trisomy-18, and one case of micro-deletion (in 7q). Among the 42 cases with isolated absence of nasal bone, two had trisomy-21 and one had micro-deletion. Absence of nasal bone in association with other structural abnormalities had a higher rate of abnormal karyotypes compared with isolated absence of nasal bone [83.33% (10/12) vs. 7.14% (3/42), Fisher's exact test χ(2) =25.620,p<0.001].Absent fetal nasal bone is a highly specific ultrasonographic soft marker that should be included in the routine second trimester ultrasound scan. Abstract:Objective It is important to detect fetal chromosomal abnormality in prenatal examination,some ultrasound indications were found to reflect chromosomal abnormality.This study is to investigate the clinic value of ultrasonographic fetal nasal bone(NB) examination for chromosomal defects risk assessment in the second trimester.Methods A total of 2519 women with a singleton pregnancy who registered from Oct 2007 to May 2009 was reviewed.Ultrasound measurements of nasal bone lenth(NBL) were performed at strict sagittal plan of the fetal head.Other fetal biometry profiles were also conducted and chromosome kasyotype analysis was taken in one with abnormal nasal bone.Results NBL and fetal biometry profiles were measured successfully in 100% of the fetuses.There were linear correlation between NBL and gestation,biparietal diameter(BPD),respectively(P0.05).Fifteen fetals were abnormal nasal bone with other malformations,among which 14 were trisomy fetuses detected by chromosome kasyotype analysis.Conclusion Evaluating fetal nasal bone with ultrasound in the second trimester scan is valuable for determining chromosomal abnormality.

Abstract:Objective We present low-level mosaic trisomy 21 at amniocentesis and cordocentesis in a pregnancy associated with a favorable fetal outcome. Case Report A 26-year-old, primigravid woman underwent amniocentesis at 17 weeks of gestation because of positive non-invasive prenatal testing (NIPT) for trisomy 21 at 16 weeks of gestation. Amniocentesis revealed a karyotype of 47,XX,+21[3]/46,XX[17], and multiplex ligation-dependent probe amplification (MLPA) on uncultured amniocytes revealed rsa X(P095) × 2, (13, 18, 21) × 2. She underwent cordocentesis (cord blood sampling) at 21 weeks of gestation which revealed a karyotype of 47,XX,+21[2]/46,XX[48]. At 27 weeks of gestation, she was referred to our hospital for genetic counseling, and repeat amniocentesis revealed a karyotype of 46,XX in 20/20 colonies. Quantitative fluorescent polymerase chain reaction (QF-PCR) analysis on the DNA extracted from uncultured amniocytes and parental bloods excluded uniparental disomy (UPD) 21. Array comparative genomic hybridization (aCGH) analysis on the DNA extracted from uncultured amniocytes revealed arr (1–22,X) × 2, Y × 0 with no genomic imbalance. Interphase fluorescence in situ hybridization (FISH) analysis on 104 uncultured amniocytes detected one cell (1/104 = 0.9%) with trisomy 21, while the rest cells were disomy 21, compared with 0% (0/100) in the normal control. The woman was encouraged to continue the pregnancy. The pregnancy was carried to 38 weeks of gestation, and a 2771-g female baby was delivered no phenotypic abnormality. aCGH analysis on the cord blood showed arr (1–22,X) × 2, Y × 0 with no genomic imbalance. The umbilical cord had a karyotype of 47,XX,+21[3]/46,XX[37]. The placenta had a karyotype of 46,XX. When follow-up at age 31⁄2 months, the neonate was phenotypically normal and had normal development. The peripheral blood had a karyotype of 46,XX in 40/40 cells. Interphase FISH analysis on buccal mucosal cells detected normal disomy 21 cells in 100/100 cells. Conclusion Low-level mosaic trisomy 21 at amniocentesis and cordocentesis in the second trimester can be associated with perinatal progressive decrease of the trisomy 21 cell line and a favorable fetal outcome.

Abstract:Background Pathogenic copy number variants (pCNVs) are associated with fetal ultrasound anomalies, which can be efficiently identified through chromosomal microarray analysis (CMA). The primary objective of the present study was to enhance understanding of the genotype–phenotype correlation in fetuses exhibiting absent or hypoplastic nasal bones using CMA. Methods Enrolled in the present study were 94 cases of fetuses with absent/hypoplastic nasal bone, which were divided into an isolated absent/hypoplastic nasal bone group (n = 49) and a non‐isolated group (n = 45). All pregnant women enrolled in the study underwent karyotype analysis and CMA to assess chromosomal abnormalities in the fetuses. Results Karyotype analysis and CMA detection were successfully performed in all cases. The results of karyotype and CMA indicate the presence of 11 cases of chromosome aneuploidy, with trisomy 21 being the most prevalent among them. A small supernumerary marker chromosome (sSMC) detected by karyotype analysis was further interpreted as a pCNV by CMA. Additionally, CMA detection elicited three cases of pCNVs, despite normal findings in their karyotype analysis results. Among them, one case of Roche translocation was identified to be a UPD in chromosome 15 with a low proportion of trisomy 15. Abstract:The objective of this paper was to determine the rate of prenatal detection of ultrasonographic abnormalities in fetuses with trisomy 18 during the early second trimester. Our prenatal diagnosis database (encompassing January 1987 to June 1996) was searched for all patients referred for prenatal genetic evaluation between 14 and 22 weeks of gestation and who were found to have a fetus with a trisomy 18 karyotype. The sonographic reports and films were evaluated for the presence or absence of fetal anatomic abnormalities. Thirty-five fetuses were identified with a mean age of 17.3+/-2.0 (standard deviation) weeks. Thirty of the 35 (86%) had at least one detected abnormality. Most fetuses had more than one abnormality, with the mean number of abnormalities per fetus being three (range, 0 to 6). The most common abnormalities noted were persistent abnormal position of fetal fingers (89%); choroid plexus cysts (43%); abnormally shaped fetal head (strawberry or lemon) (43%); two-vessel umbilical cord (40%); cardiac defects (37%); intrauterine growth restriction (29%); omphalocele (20%); neural tube defects (9%); and cystic hygroma or lymphangiectasia (14%). Abnormalities of amniotic fluid volume (12%) and renal defects (9%) were seen less frequently. These data suggest that in the early second trimester, the time of most routine screening ultrasonographic examinations, most but not all fetuses with trisomy 18 have sonographically detectable anatomic abnormalities. The fetal hand appears to be abnormal in most early second trimester fetuses with trisomy 18, but the abnormality may be subtle and or unilateral.
