Abstract:Objective To explore the cut-off values of D-dimer for prediction of calf intermuscular deep vein thrombosis (DVT) after total hip arthoplasty (THA) and evaluate its clinical significance.Methods A total of 108 patients undergone primary THA in the WestChina hospital were included in the current study.The inclusion criteria for THA were patients diagnosed as osteoarthritis or osteonecrosis of the femoral head, without history DVT, without diseases of blood system, coagulation mechanism, liver and kidney.Color duplex ultrasonography (US) was performed preoperatively and 5 d postoperatively in order to detect any evidence of DVT of the lower limb.All the preoperative US results were negative.Those who didn`t meet the inclusion criteria were excluded.The patients were divided into the thrombosis group(n=28) and the non-thrombosis group (n=80) according to the 5 d US results.Plasma D-dimer was measured preoperatively and 1 d, 3 d, 5 d postoperatively.Receiver operating characteristics (ROC) analysis was performed using SPSS 19.0 software according to the results.The area under the curve (AUC) at different time point was easured,and the value got from the most left part of the curve according to the biggest AUC area.Comparing the basic information, like the age, body mass index, blood loss and operation time of both groups with t test, and using chi-square test for sex and American Society of Anesthesiologists staging.Results There was no statistical difference between the basic information of two groups.Comparing with the non-thrombosis group, postoperative D-dimer value of the thrombosis group increased significantly at each time point, especially at day 1 increased for 81.4% (t=4.602,P<0.01), and the peak value presented postoperative at day 1 too, the non-thrombosis group increased to(4.94±1.27)g/L,thrombosis group inceased to(8.96±1.67)g/L (t=10.616,P<0.01),and gradually got down after that.ROC curve showed that D-dimer was less than 5.9 g/L at day 1 after the surgery when the AUC was 0.963, which excluded DVT, but when D-dimer was over 9.8 g/L, the specificity for DVT detection reached 100%.Conclusions Using 5.9 g/L as the cut-off value of D-dimer to exclude calf intermuscular DVT at day 1 after the surgery was appropriate.For the prevention of DVT, it is important to administer anticoagulant treatment within three days after THA for high risk patients.
